Purpose: The purpose of this study was the development of a comprehensive nursing intervention program for the client with acute lymph stasis and stage I lymphedema. Method: The Quasi-experimental design using a non-equivalent control group was used. The subjects were 22 stroke patients with lymph stasis in the control group and 23 patients in the experimental group. The complex physical therapy of Casley-Smith was carried out to the control group for 10 hours, and comprehensive nursing intervention for the experimental group was carried out for 60 minutes. The data for this study was gathered from Feb. 2002 until June 2002 and pertains knowledge about lymphedema, self-care for managing lymphedema, and circumferences of affected limbs. Data was analyzed by mean, standard deviation, ${\chi}^2-test$, and t-test. Result: The changes in knowledge about lymphedema, self-care practices, and circumference of affected limbs after nursing intervention did not show significant differences between control group and experiment group. Conclusion: It can be concluded that comprehensive nursing intervention had more efficiency than complex physical therapy in the treatment of edema for stroke patients because of it's simplicity and time saving. Thereby, the comprehensive nursing intervention program developed in this study would be a useful therapy for the clients with lymph stasis and early stage lymphedema.
림프관종은 드물게 발생하는 림프혈관의 양성 과오종으로, 주로 두경부에 호발하는데, 혀나 구저부 또는 심 경부에 발생할 경우 원심성 림프관 흐름의 방해로 이차적 거설증이 발생할 수 있다. 저자들은 출생 시에는 경부 또는 하악 부종 없이 거설증만 보였던 두 명의 환아에서 각각 생후 5개월과 5주에 하악 부종이 보여 MRI로 심 경부 림프관종을 확인한 2례를 경험하였기에 보고하는 바이다.
Purpose: The purpose of this study was to develop and validate of a knowledge scale for lymphedema in patients with breast cancer. Methods: 34 preliminary items were made according to literature review, then verified content validity, construct validity, and reliability of the scale. 28 items among them were confirmed through content analysis by 4 experts. After a preliminary test, a survey for 156 breast cancer patients was performed for confirming construct validity and reliability. The data were analyzed using factor analysis, independent t-test, and KR-20. Results: This scale had 28 items consisting of 2 categories: prevention and self-care of lymphedema. Construct validity was confirmed by known-group technique because there were some categories consisted of just one or two items, and some mismatches between categories and items in factor analysis. Women who were educated about lymphedema scored significantly higher than women who had not (t=-3.92, p<.001). Reliability was appropriate (KR-20=.81). A percentage of correct answers was 74.6%, but it was from 31.8% to 96.1%. Conclusion: The study shows that this scale is reliable and valid to measure the knowledge of lymphedema. This scale can be effective to assess and educate the patients with breast cancer.
Purpose: The purpose of this study was to investigate the emotional state and related factors in patients with lymphedema. Methods: The subjects of this study consisted of 95 patients with lymphedema at 8 hospitals in Busan and Seoul. Data was collected by a self-administered questionnaire between March 2001 to December 2001. Data was analyzed by Pearson correlation coefficient, and stepwise multiple regression using SPSS Win 12.0. Results: The mean score of the emotional state of the subjects was 3.06; of their physical symptoms, 1.84; of their Activities of Daily Living(ADLs), 2.30; and of their social activities, 3.67. The emotional states of lymphedema patients correlated with their physical symptoms, their ADLs, and their social activities. The Factor influencing the emotional state of the subjects was social activities. Conclusion: These results suggest that a negative emotional state is very common in patients with lymphedema, to which appropriate attention should be given. Rehabilitation programs must be implemented to improve lymphedema patients' emotional state, physical symptoms, ADLs, and social activities.
Purpose: The purpose of this study was to examine the relationship between lymphedema self-care management and quality of life in breast cancer patients with mastectomy and lymphedema. Methods: One hundred and eighty-six breast cancer patients with mastectomy and lymphedema (n=186) were recruited at a medical center located in Seoul. The levels of lymphedema self-care management and quality of life were measured by the scale for measurement of practice in lymphedema self-care management, European Organization for Research and Treatment of Cancer-Quality of Life Core 30 (EORTC QLQ-C30) and European Organization for Research and Treatment of Cancer-Quality of Life Questionnaire Breast Cancer Module (EORTC QLQ BR23), respectively. Data were analyzed with SPSS 18.0 program. Results: There were statistically significant correlations between lymphedema self-care management and general health status/quality of life in mastectomy patients with lymphedema (r=.30, p<.001). The physical function score of cancer related function scale (r=-.15, p=.033), fatigue score of cancer related symptom scale (r=.15, p=.036), systemic side effect score of breast cancer related symptom scale (r=.45, p=.034), and upset by hair loss (r=.27, p=.004) were significantly correlated with quality of life. Conclusion: The findings suggest that these significant factors should be considered when caring for lymphedema patients.
Purpose: The purpose of this study were to examine the incidence of lymphedema and to compare to physiological and psychosocial symptoms between breast cancer patients with lymphedema and without lymphedema. Method: Four hundreds fifty women with breast cancer receiving mastectomy were recruited from two out-patient breast clinics of A and S university hospitals in Seoul, Korea from October, 2004 to May, 2005. Lymphedema was defined by circumferential measurement. Differences greater than 2cm between the affected arm and normal arm were considered to be lymphedema. A descriptive design was used for this study and data was collected using a structured questionnaire. The data were analyzed using descriptive statistics and independent t-test. Results: Lymphedema developed in 112(24.9%) of the 450 subjects. Subjects with lymphedema reported more severe sensation change of arm and more depression and confusion than subjects without lymphedema. Range of motion of shoulder as a physical indicator and social support, however, were not significantly different between the two groups. Conclusion: Lymphedema is recognized as an unpleasant and uncomfortable consequence of breast cancer-related treatment. Therefore, more research is needed to evaluate the impact of lymphedema on quality of life in patients with lymphedema and the efficacy of nursing intervention.
Purpose: This qualitative study aimed to understand the experiences of reorganization of everyday lives among the women with chronic health problem. Methods: In this study, the approach and its process of sociology of everyday lives were adopted. Data for the study came from 6 informants and 9 family members of the informants by interview and participant-observation from January, 2004 to May, 2006. Qualitative content analyzing methods were adopted. Results: Informants' everyday lives were reorganized as follows. During the experiences of shock from unpleasant and unclear symptoms of their body, their sight fixed on the body part. Their time also fixed on a point of present. They started to wander from medical care to folk and lay care. After they were informed that the health problem could not be treated completely, they reduced the world of everyday life and protected themselves from the chaotic unfamiliar world by setting -a- side duties as a family member and severing unessential social relationship. As they achieved a skill for managing their health problem, they gained their former pattern of everyday lives as a woman, a family member, and a social member. Finally, they created a new life world. Conclusion: We need more study on the development of an adaptive strategy by the informants, to intervene in the crisis of everyday life.
Burkitt 림프종은 B림프구 기원의 악성종양으로 비호지킨 림프종(non-Hodgkin's lymphoma)의 한 종류이다. Burkitt 림프종은 임상적으로 아프리카에서 나타나는 지역형(endemic), 세계 각지에서 발생하는 산발형(sporadic), 면역결핍상태와 관련되어 나타나는 형태로 나눌 수 있다. 과거에는 그 예후가 매우 불량했으나, 최근에는 집중적인 화학요법으로 인한 치료 성공률이 50-80%에 달한다. 많은 경우 악골을 포함하고 있으며 임상증상으로는 심하게 흔들리는 치아, 치아의 변위 그리고 치은의 발적 및 부종 등이 있다. 본 증례는 진행속도가 상당히 빠른 Burkitt 림프종에 이환된 환아에 대한 내용이다. 악골에 생긴 병소로 내원하였으며, 치과에서 처음 진단하였고 신속하고 적절한 치료를 통해 양호한 결과를 얻었다.
