A ROENTGENOCEPHALOMETRIC STUDY OF SOFT TISSUE PROFILE FOR THE CHILDREN IN THE DECIDUOUS DENTITION (유치열기(乳齒列期) 아동(兒童)의 악안면연조직(顎顔面軟組織)의 두부방사선(頭部放射線) 계측학적(計測學的) 연구(硏究))

  • This study was made on the facial profile of the normal Korean children using the roentgenographic cephalograms. The subjects consisted 51 males and 47 females children of primary dentition with the normal occlusion and acceptable profile. For this study 13 soft tissue profile landmarks were plotted and 14 liner length, 9 soft tissue thickness, 8 vertical height length, 12 angles of soft tissue profile, and 2 vertical proportion were measured. The mean and standard deviations in the subjects were calculated and compared between male and female. The following results were obtained : 1. By the significant test, total facial convexity angle and soft tissue thickness were no significant difference between both sexes. 2. Lower facial height was greater than upper facial height in both sexes. 3. The vertical length of the upper and lower lips were 21.95 mm, 40.74 mm in male and 21.62 mm, 39.63 mm in female. 4. In the relationship of the upper lip and lower lip to the Ricketts' esthetic line, the male was 1.3 mm, 1.18 mm and the female was 1.16 mm, 1.27 mm front of the esthetic line. 5. Compared with the angulation of flush terminal plane group and mesial step group, the mesial step group was greater than the flush terminal plane group except the chin angle.

ROENTGENOCEPHALOMETRIC STUDY OF CRANIOFACIAL SKELETON ON THE AGE OF PUBERTY WITH NORMAL OCCLUSION (청소년기의 정상교합자에 대한 두부방사선 계측학적 연구)

  • The study was performed to establish the cephalometric standards of Hellman dental age III B, IV A, IV C groups of the age of puberty and to aid for the case analysis and diagnosis of malocclusion. A roentgenocephalometric study was made from 365 subjects, that consist of 162 males, 203 females with normal occlusion, acceptable profile and no history of orthodontic and prosthodontic treatment. The results of this study were obtained as follows: 1. The tables of standards from the measurements by age, sex group were made. 2. All linear measurements of skeletal pattern in male were greater than in females. 3. The Bjork's sum was reduced gradually by aging in group I $396^{\circ}$, group II $395^{\circ}$, and group III $393^{\circ}$. 4. Posterior facial height to anterior facial height was 63% in group I, 64% group II, and 67% in group III. 5. The angulation of SNA and SNB were $81^{\circ}$ & $78^{\circ}$ in group I, $81^{\circ}$ & $78^{\circ}$ in group II, and $82^{\circ}$ & $79^{\circ}$ in group III.

  • A cephalometric radiographic cross sectional comparative study was undertaken to investigate craniofacial growth in cleft lip and palate individuals. The material for this study consisted of 43 subjects with operated cleft lip and palate.(29 males, 14 females). The range of age was from 6 years to 12 years. The roentgenocephalometric values of cleft individuals were compared with values of normal individuals reported by Lee. The following conclusions were obtained; 1) The pattern of cranial base of the cleft subjects was almost the same as that previously reported for the normal individuals. 2) Anterio-posterior length of the mandible did not show any significant difference but in the cleft subjects, that of the males was larger than that of the females. 3) The maxilla of the cleft subjects was very retruded and showed very concave profile. 4) Gonial angle of the cleft subjects was very high, especially in the female clefts. 5) The height of ramus was very poor in the cleft subjects. 6) The facial length was almost the same, but the facial depth of the cleft subjects was smaller than that of the normal individuals. 7) Maxillary and mandibular incisors were severely retroclined. 8) The range of values in the cleft subjects was very variable and the female clefts showed more distured growth than the male clefts.

Relation of the measuring values in cephalometric radiographs and TMJ tomographs (두부규격방사선사진과 측두하악관절 단층방사선사진 계측치간의 상관관계)

  • Purpose : To examine whether the maxillofacial skeletal morphology correlates with the condylar position and the anatomic characteristics of articular eminence using measurements of lateral cephalometric radiographs and individualized sagittal temporomandibular joint (TMJ) tomographs. Materials and Methods : I compared measurements of 202 TMJs of 101 orthodontic patients of Kangnung National University Dental Hospital. 1 used Pearson's correlation for comparison of the measuring values in lateral cephalometric radiographs and individualized sagittal TMJ tomographs. Results : Large occlusal plane angle tendency showed decreased width of posterior eminence slope, decreased depth of articular fossa and decreased posterior slope angle of eminence. Large mandibular plane angle tendency showed decreased superior joint space, decreased depth of articular fossa and decreased posterior slope angle of eminence. Large genial angle tendency showed decreased depth of articular fossa, decreased posterior slope angle of eminence and anterior position of condylar head. Large anterior facial height and large posterior facial height tendency showed increased width of posterior slope of eminence, increased depth of articular fossa, increased posterior slope angle of eminence and posterior position of condylar head. Conclusion : Maxillofacial skeletal morphology has correlation with the anatomic characteristics of TMJ. Individualized sagittal TMJ tomographs can provide useful information for anatomical analysis of TMJ.

  • The Purpose of this study was to investigate the differences in soft tissue characteristics according to the dental or skeletal dysplasia. For this purpose, lateral cephalogram of 153 children (Hellman dental age IIIB: control group 32, Angle CIII. div. 1 malocclusion group 55, Angle Cl III group 66) were traced and measured. For these measurements, following conclusions were made. 1. FH A, FH Sn, FH UL, AA' of the Class III group were thicker than those of the normal and Class II group, but FH B, FH LL, BB' of the Class III group were not significantly different from those of the normal group. 2. FH B, FH LL, BB' of the Class II group were thicker than those of the normal and Class III group, but FH A, FH Sn, FH UL, AA' of the Class II group were not significantly different from those of the noraml group. 3. Ans-Sn, FH P were not significantly different in three groups, while PP' of the Class III group was thicker than those of the other groups. 4. The lower lips of the Class II group were more anterioly everted with respect to the lower incisor inclination than those of the other groups. 5. The severity of skeletal dysplasia was partly camouflaged by the soft tissue.

  • By observing the correlationships between the characteristics of the facial pattern and the methods of establishing the position of the lower central incisor to the five reference lines on the lateral roentgenocephalograms, the author tried to find the most useful method of them, the criteria of which were the independence of a variation according to facial patterns, the close relationships with soft tissue and lip balance, and the simple and consistent usefulness in clinical practice. The subject consisted of forty normal occlusions, forty class II div I malocclusions, forty class III malocclusions, ten treated claas II div 1 cases and ten treated class III cases, all were in 14-17 yrs of age. The findings of this study are as follows: 1. The position of the lower central incisor to EP, OP, MP and NB showed variations according to ANB, FMA, facial convexity and Holdaway angle. 2. The position of the lower central incise. to AP line (A-Pog) was not co..elated with ANB, FMA, facial convexity and Holdaway angle, so it can be used consistently, regardlesss of the facial pattern. 9. The lineal position of the incisal edge of the lower central incisor to AP line has a profound influence on harmonious soft tissue and lip balance.

  • The purposes of the present study were to describe the vortical and anteroposterior facial dysplasia, and to identify morphologic differences associated with various facial patterns. Anteroposterior dysplasia was classified by traditional Angle's malocclusion classification and according to vertical relationships based on the SN-MP angle, facial pattern was subclassified to 3 vertical groups in each malocclusion group. Each vertical group composed of 20 samples and total 180 samples aged from 9 to 14 years. The results were as follows; 1. The skeletal differences that lead to disportionate lower facial height in vortical and anteroposterior facial dysplasia were closely related to mandibular morphology. 2. Hyperdivergent group, compared with hypodivergent group, demonstrated decrease of SNA and SNB and linguoversion of maxillary and mandibular central incisors in all malocclusion groups. 3. Irrespective of rotation of the jaws, Wits appraisal was a useful measurement in evaluation of relative anteroposterior relationship of maxilla and mandible. 4. As SN-MP angle increased, anterior facial height, especially lower anterior facial height, demonstrated significant increase and intermaxillary space also tended to increase. 5. The correlation coefficients of SN-MP angle and PTFH/ATFH demonstrated the highest value in all malocclusion groups.

A Longitudinal Study of the Growth Changes on Nasopharynx and Adenoid of the Korean with Normal Occlusion (정상 교합자의 비인두와 Adenoid 성장에 관한 누년적 연구)

  • The purpose of this study was to evaluate the growth pattern of nasopharyx and adenoid and to obtain useful measurements for evaluating the fuction of the nasal breathing. The biennial serial cephalometric radiographs of 33 samples(19 male, 14 female) with normal occlusion from 8.5 years to 18.5 years of age were used in this study. The distances, areas and ratios on nasopharynx and adenoid were measured and analyzed in each age and sex. The results of this study might be summarized as follows ; 1. The mean value and standard deviation of each measurement was obtained in each age and sex. 2. The nasopharyngeal height and the nasopharyngeal depth increased significantly by 14.5 years of age in male and 12.5 years of age in female(p<0,05), 3. The relative nasal airway area showed the minimal value at 8.5 years of age and showed significant increase from 10.5 years to 12.5 years of age in male and from 12.5 years to 14,5 years of age in female(p<0.05). 4. In the correlation analysis between the measurements on the nasal airway and the measurement of relative nasal airway area, the measurements of Ad2-PNS, Ad2-PNS/Ho'-PNS and Upper pharynx appeared singnificant correlation with the measurement of relative nasal airway area(p<0.01).

A study on correlations between tongue with skeletal pattern and intermaxillary space in Class II malocclusion adult patients (골격성 II급 성인 부정교합자에서 혀와 두개안면골격 및 악간공간과의 상관관계에 관한 연구)

  • This study was designed to evaluate the correlations between tongue with skeletal pattern and intermaxillary space in Class II malocclusion adult patients. Craniofacial skeletal pattern was analyzed on the lateral cephalometric radiograph and the subjects were devided two groups by facial ratio, the 30 subjects of hyperdivergent group and the 30 subjects of hypodivergent group. The size and posture of tongue and intermaxillary space were measured on the lateral cephalometric radiograph. These data were statistically analyzed to examine significant differences between both groups and compared the correlation between tongue with skeletal measurements and intermaxillary space in each group. The results of this study were as follows. 1. In comparison of the tongue and intermaxillary space, the measurement of TS/IS showed significantly larger in hyperdivergent group and PIH and IS showed significantly larger in hypodivergent group. There were no statistically significant differences in the measurements of the height and posture of tongue. 2. In correlation between tongue with craniofacial skeletal measurements, the length and height of tongue showed the highest correlation with AFH(anterior facial height) in both groups(p<0.01). And that measurements showed high correlation with PFH(posterior facial height) in hypodivergent group. 3. In both groups, most measurements of tongue showed high correlation with intermaxillary space and the height and space of tongue showed high correlation with AIH and PIH(p<0.01).

Interrelationships between occlusal plane angle and vertical skeletal patterns of skeletal Class III malocclusion individuals (골격성 III급 부정교합자의 수직적 안면골격형태와 교합평면과의 관계)

  • The purpose of this study was to evaluate the relationships between the occlusal plane angle and craniofacial skeletal pattern in relation to anterior overbite. Methods: Lateral cephalograms of 90 adults with skeletal class III malocclusions were traced and measured to analyze skeletal factors and occlusal plane angles. In terms of anterior overbite, all patients were classified into 3 subgroups of positive overbite, edgebite, and negative overbite groups. All measurements were evaluated statistically by ANOVA and Duncan's Post Hoc, and correlation coefficients were evaluated among measurements. Results: In this study, some skeletal measurements (saddle angle, articular angle, Y axis, AFH, SN-FH, SN-Mn, FH-Mn) showed a significant difference among the 3 groups in relation to overbite changes. Correlation coefficient showed that PFH/AFH, SN-Mn, Mx-Mn, and FH-Mn showed a significant difference with FH-Occ, Mx-Occ, and Mn-Occ. Regression analysis showed that Mx-Mn had a determination coefficient of 0.714, 0.560, and 0.677 in relation to FH-Occ, Mx-Occ, and Mn-Occ, respectively. Conclusion: This study suggests that consideration of the occlusal plane in relation to the maxillomandibular vertical skeletal state enable the establishment of a more predictable orthognathic surgery result.
