The purpose of this study was to assess the growth changes of craniofacial complex and to establish Korean norms by McNamara's cephalometric analysis. Lateral cephalograms were obtained from males and females with normal occlusion & acceptable profiles in 9-21 years of age. The lateral cephalograms of males & females in 9-13 years of age were serial cephalometric radiograms which were taken at 2 year interval. 11 landmarks, 10 planes and 3 angles were plotted and measured linearly and angularly. The results were as follows: 1. The author made the tables of means, standard deviations in each item, sex, age. 2. During 9-13 years of age, annual growth increments of effective maxillary length, effective mandibular length and lower anterior facial height were 1.90, 2.89 and 0.99mm. 3. The prediction tabulation was made in an attempt to predict effective mandibular length & lower anterior facial height derived from effective maxillary length.
Objective: The purpose of this study was to compare the cephalometric measurements of obese and non-obese Korean male patients with obstructive sleep apnea syndrome (OSA). Methods: Eighty-seven adults who had visited the Sleep Disorder Clinic Center in Keimyung University, Daegu, Korea were examined and evaluated with polysomnography (PSG) and lateral cephalogram. They were divided into 4 groups (non-obese simple snorers, obese simple snorers, non-obese OSA patients, obese OSA patients) according to AHI (Apnea-Hypopnea Index) and BMI (Body Mass Index). Results: The obese OSA group had the highest AHI among the 4 groups. The non-obese OSA group had a significantly steeper mandibular angle and shorter tongue length than the obese OSA group. The hyoid bone of the obese OSA group was positioned anterior and inferior as compared with the non-obese OSA group. Multiple regression analysis showed that tongue length in the obese OSA group and retroposition of hyoid bone in the non-obese OSA group were significant determinants for the severity of AHI. Conclusions: From a cephalometric point of view, the obese and non-obese pateints with OSA may be characterized by different pathogeneses. Therefore, they have to be managed by individualized treatment. For the obese OSA patients, weight control must be advised as a first choice and for the non-obese OSA patients, oral appliance, nasal CPAP, UPPP and others could be chosen according to the obstructive sites.
The purpose of this study was to determine whether there is an association between disk displacement of the Temporomandibular Joint (TMJ) and dentofacial asymmetry In orthodontic patients. The subjects consisted of 60 female orthodontic patients between the ages of 18 and 38 years (mean age 23.3 years) who had visited the Department of Orthodontics at Seoul National University Dental Hospital from January 2000 to April 2002. On the basis of magnetic resonance imaging (MRI) of their bilateral TMJs, the subjects were divided Into four groups'. bilateral normal group (twenty-one persons); disk displacement of right TMJ group (six persons); disk displacement of left TMJ group (nine persons); and disk displacement of both TMJs group (twenty-four persons) Postero-anterior (PA) cephalograms and diagnostic models which had been taken before orthodontic treatment were measured. In the linear measurements, a line connecting the right and left Latero-Orbitale (Lo) represented the horizontal reference line (H). The vertical reference line (V) was constructed as a line bisecting and running perpendicular to H. One-way analysis of variance (ANOVA) was used to test whether the mean values of measurements between groups were significantly different. In addition, Bonferronil's multiple comparison test was performed at a level of 0.05. The results were as follows; 1 In the diagnostic model analysis, the overjet, nght molar relationship, and left molar relationship were significantly different among the four groups. 2. In the PA cephalometric analysis, differences in the right and left vertical position of the lower first molar and Ag were significantly dissimilar among the four groups. 3. If the disk displacement of TMJ was present on one side, the ipsilateral ramus was shorter, resulting in asymmetry in the vertical position of Ag. This study indicated that dentofacial asymmetry might be related to the disk displacement of TMJ.
The purpose of this study was to determine whether association exists between temporomandibular joint (TMJ) characteristics in transcranial radiographs and TMJ internal derangement and to evaluate the validity of transcranial radiographs in diagnosis of internal derangement. Transcranial radiographs and magnetic resonance imaging (MRI) of 113 TMJs from 76 subjects were used for this study and all TMJs were classified into 3 groups according to the results of MRI: normal disk position, disk displacement with reduction, and disk displacement without reduction. Transcranial analysis included linear measurement of joint spaces and condylar head angle measurement. To determine any relationship between transcranial measurements according to disk displacement, one-way ANOVA was used. The results showed that condyle-fossa relationship in standard transcranial radiographs had no relationships with disk displacement. And, as disk displacement progressed, condylar angle between head and neck increased significantly. This result can be interpreted that condylar head angle reflects structural hard tissue change according to internal derangement progress. But this is insufficient in the determination of internal derangement. Therefore, although still clinically helpful, the validity of standard transcranial radiographs to diagnose TMJ internal derangement was questioned.
It has been known that head posture may influence directly and/or indirectly the growth and development of craniofacial morphology and can also be influenced by the funtional demand of physiologic activity. It was reported that facial morphology has close relationships with hyoid bone position and head posture. In many previous studies, Natural Head Posture(NHP) was guided, and also it was shown that NHP has high degree of reproducibility. Otherwise, There was few study about the relationship of head posture, with routine cephalometric film which is used for clinical orthodontic purpose. In this study, according to the Wits and ANB of initial cephalometric film which was taken with vertical pendulum as representative of true vertical reference line. We classified the subjects which is comprised of 60 adult female patients into Class I, II, III (Cl I, II, III)and we tried to find out the correlation of head posture and hyoid bone position according to the classification of malocclusion. As a result of our research, we found the followigs. 1. In comparison of vertical position of hyoid bone relative to the cranial base. the position of hyoid bone of Cl III was lower than that of Cl II. 2. In comparison of anteriorpostes or position of hyoid bone, relative to the cervical column. The position of hyoid bone of Cl III was more anterior than that of a II 3. in comparison of vertical position of hyoid bone relative to mandible. There was no significant correlation aumoug the groups of malocclusion. 4. ANB and Wits showed no significant correlation with hyoid bone position. 5. The relative extension of head, which was noted in Cl II, showed negative with Sum, ANB. 6. In Cl II and Cl III, Post to Ant facial height showed positive correlation with NSL/VER.

The purpose of this study was to observe the anatomic variation of condylar head and the positional relationship between condylar head and other anatomic structures of head by means of cephalometry using the submentovertex cephalogram from each person in centric occlusion for producing a good quality of temporomandibular joint radiograph. The 100 submentovertex cephalograms of 100 Korean adults consisted of 50 females ranged from 20 to 24 years age and 50 males ranged from 22 to 30 years age, were studied and analyzed statistically. The results were as follows; 1. The mean of the horizontal angulation of condylar head to the transmeatal line (EE-LA) was 18.5° (S.D. 7.9°), and all measurements that indicate the horizontal angulation of condylar head showed considerable differences among individuals. 2. In the comparison of male vs. female in the measurements, statistically significant differences were found in the majority of measurement items, and with exception of R-LA and LA-LA, the rest of these measurements were larger in male than in female. 3. In the comparison of left vs. right in the measurements, statistically significant differences were found in the majority of measurement items, and with exception of PC-CC, the rest of these measurements were larger in right than in left. As above, because the majority of measurements that involve the horizontal angulation of condylar head varied among individuals, between male and female, and between left and right, the condition of various temporomandibular joint radiography should be determined by means of cephalometry using the submentovertex cephalogram for producing the accurate radiographic image of temporomandibular joint.
The purposes of this study were to evaluate the nasolabial angle changes between closed lip position at centric occlusion and relaxed lip position at which the bite is open so that the lips do not touch and to elucidate the significance of the relaxed lip position for dentofacial diagnosis. Methods: The subjects consisted of 60 (35 Males, 25 Females) skeletal Class III malocclusion adult patients (mean age 23.3 years) with anterior crossbite. Results: In Class III malocclusion adult patients, there were significant differences in the nasolabial angle changes between closed lip position and relaxed lip position. Using the cluster analysis, the subjects were divided into three groups according to the pattern of nasolabial angle change: Group 1 (N = 27, 45%, $-8\;{\sim}\;1$), Group 2 (N = 30, 50%, $2^{\circ}\;{\sim}\;17$), and Group 3 (N = 3, 5%, over 18). Conclusion: The results showed that the pattern of the nasolabial angle change between closed lip position and relaxed lip position varies in skeletal Class III malocclusion patients. Thus, relaxed lip position should be taken into account when diagnostic records are obtained and analyzed to accurately to evaluate the facial soft tissues and predict facial esthetics after surgical-orthodontic treatment.
The purposee of this study is to investigate the correlation between the horizontal and vertical discrepancy of facial bones and the horizontal and vertical position of glenoid fossa. For this study, the cephalograms were taken to the adults over 18 of age(96 men and 108women). The cephalograms were divided into three groups according to the ANB angle(below 0.5 degree, 0.5 to 4.0 degree and above 38 degree), and they were divided into three groups according to the SN-MP angle(below 30 degree, 30 to 38 degree and above 38 degree). The following conclusions were obtained : 1. In the horizontal discrepancy of facial bone(ANB), the horiontal position of glenoid fossa(X) was anterior position as the order of Class II, Class I, Cias III. 2. The horizontal position of glenoid fossa(X) showed the significant correlation with ANB and SNB, but not with SNA. 3. In the vertical discrepancy of facial bone(SN-MP), thee less anglee was the greater vertical position of glenoid fossa (X) and was followed by the medium and high angle in order. 4. The vertical position of glenoid fossa(Y) showed thhat the SN-FH was the most significant correlation, and it was followed as the order of SN-FH, SN-MP and SN-OP angle. 5. There was the samllest length of anterior cranial base in the Class III malocclusion.

The aim of this study was to investigate the relationships between bite force, masseter muscle and craniofacial morphology. 141 individuals were included. Bite forces were measured with a bite force transducer(SKT 2004). The facial morphology of the samples was evaluated on the Lateral and PA cephalometric radiographs(Asahi CX-90 SP). Ultrasonographic images were obtained for 36 individuals(Logiq 500). General status of the subjects were also measured for investigating possible relationships. Their relationships were analyzed by means of t-test, Pearson's correlation coefficients and simple regression analysis. In conclusion, bite force of molar was significantly correlated with craniofacial morphology for males and females, but not to masseter muscles. The masseter muscle for males had a significant correlation with craniofacial morphology. The unexpected negative correlations between size of masseter muscle and maxillary width was revealed for males. One possible way to arrive at a plausible explanation for the correlation between masster muscle, bite force and craniofacial morphology of males were the deep layer of masseter muscle had a great influence on males and the superficial layer of masseter muscle had a great influence on females.
Objective: This study examined the craniofacial morphology of young patients in their prepubertal stage showing class I, II malocclusion, by analyzing lateral cephalograms, and analyzed its relationship with tongue position, tongue space, and airway space in order to ascertain the effects of nasopharyngeal airway and tongue morphology on the form of the malocclusion. Methods: Seventy-six patients aging from 9 to 11 were divided into two groups depending on the ANB difference on the lateral cephalogram: Experimental group (CI II malocclusion group) showing $0{\le}ANB$ difference < 4.0; Control group (CI I malocclusion group) showing $0{\le}ANB$ difference < 4.0. The tongue space, space between palate and tongue, nasopharyngeal airway space and craniofacial morphology were compared between the two groups. Results: Tongue space, palate-tongue space, nasopharyngeal airway space showed no significant differences between class I and class II malocclusion groups. Hyperdivergent faces were associated with smaller nasopharyngeal airway space. Longer anterior facial height and posterior facial height were associated with larger tongue space, and greater anterior facial height were associated with lower tongue position. Smaller nasopharyngeal airway space showed smaller tongue space. Conclusions: Tongue space and nasopharyngeal airway space showed no significant differences between class I malocclusion group and class II malocclusion group. Only anterior facial height and posterior facial height had an influence on tongue space and nasopharyngeal airway space.
