A LONGITUDINAL CEPHALOMETRIC STUDY OF THE CRANIOFACIAL GROWTH CHANGES OF KOREANS AGED FROM 8 TO 16 YEARS (한국인 두개 안면골 성장변화에 관한 누년적 연구(8세에서 16세까지))

  • Biennial serial cephalometric radiographs of 47 children (26 boys, 21 girls) were used to analyze the craniofacial growth changes in Korean children from 8 years to 16 years of age. A craniofacial model was designed for this study. It consisted of 72 anatomical points and 98 derived points. The craniofacial changes of these samples during these ages might be summarized as follows: 1. Mandibular growth to cranial base was more forward than maxillary growth. 2. Mandibular growth during this period was found to be a forward-upward (bite closing) rotation of the mandible. 3. Growth changes in total mandibular length (Ar-Gn) showed a pubertal growth spurt at 12-14 years of age in boys and 10-12 years of age in girls. Synchrony of the growth spurts on total mandibular length (Ar-Gn) and standing body height was found. 4. The pubertal spurts occured in the growth of total cranial base (Ba-Na) at 10-12 years of age in boys and 8-10 years of age in girls. The time of the spurts of the cranial base growth was 2 years ahead of that of the total mandibular growth. 5. Synchrony of growth spurts in anterior facial height (Na-Me), posterior facial height (S-Go) and body height was found. 6. The whole craniofacial changes during this period were plotted by using a X-Y plotter and personal computer. A simple profilogram for an diagnostic tool was obtained.

Studies on Involvement of Central GABAergic Mechanism and Central ${\alpha}_{2}-Adrenoceptors$ in Pressor Responses to Raised Intracranial Pressure (두개내압상승에 의한 혈압상승작용과 중추 GABA계 및 중추 ${\alpha}_{2}$-아드레날린 수용체와의 관계)

  • Recent studies have shown that a GABAergic mechanism in the brain modulates arterial blood pressure (BP) through alterations of sympathetic activity in the brain. The purpose of the present study was to determine if this modulation is involved in the pressor response to raised intracranial pressure (ICP). The pressor response to raised ICP was abolished by pretreatment of anesthetized rabbits with intracerebroventricular (icv) muscimol (a GABA agonist) as well as with icv clonidine $(an\;{\alpha}_2-agonist)$. Raising ICP in the hypertensive state after icv yohimbine $(an\;{\alpha}_2-antagonist)$ did not cause an additional increase in the BP, whereas raising ICP in the hypertensive state following icv bicuculline (a GABA antagonist) produced a further increase. Bicuculline produced an increase of the BP which had been lowered by muscimol or by clonidine, whereas it failed to increase the hypertensive state induced by either previous yohimbine or raised ICP. Yohimbine reversed the BP which had been made low by clonidine but was incapable of raising the hypotensive state after muscimol. Yohimbine failed to increase the heightened BP due to raised ICP, whereas bicuculline-induced pressor state was further elevated by yohimbine. Muscimol, besides the bicuculline-antagonizing property, inhibited the pressor response to yohimbine, suggesting participation of a GABAergic mechanism in the pressor action of yohimbine. From these results it was inferred that there were three ways in which BP could be increased via raised ICP: inactivation of the inhibitory sympathetic activity through (1) ${\alpha}_{2}-adrenoceptors$, (2) bicuculline-sensitive GABA receptors, (3) yohimbine-sensitive, clonidine-acting GABAergic sites.

CHARACTERISTICS OF CRANIOFACIAL STRUCTURES IN SEVERE ADULT CLASS III MALOCCLUSION (심한 골격성 III급 부정교합을 가진 성인 두개안면골격의 특성)

  • The purpose of this study is to define the characteristics of the skeleton and soft tissues of severe adult class III malocclusion. The materials selected for this study were lateral cephalograms of 112 adult class III malocclusion patients with ANB difference below -2 degrees. and the mean age was 22.9 years old. The normal control sampler consisted of lateral cephalograms of 50 adults in normal occlusion and the mean age was 22.1 years old. The Horizontal reference line was FH line and the vertical reference line was nasion perpendicular to FH line. The skeletal and soft tissue characteristics of Class III malocclusion are as follows : 1. In the skeletal profile evaluated by vertical reference line (Nasion perpendicular to FH), the forehead and maxilla was similar to normal, but the mandible was protruded significantly. 2. The soft tissue profile is concave. The thickness of soft tissue covering forehead area and nose is within normal range. but the upper lip is thicker and the nasolabial angle is smaller than normal. The lower lip and inferior labial sulcus is thinner than normal. The degree of eversion of lower lip is lesser than normal. 3. The cranial base of class III malocclusion is shorter and saddle angle is smaller than normal. 4. The location of midface evaluated in relations to cranial base is within normal range but, the length of midface is shorter than normal when compared from the deep portion of the facial skeleton. 5. The location of maxilla in reference to cranial base is within normal range but the length of maxilla was shorter in class III malocclusion. 6. The mandible was protruded, ramus height and body length, gonial angle were greater than normal, and the chin angle was smaller. 7. Upper incisor was proclined, lower incisor was retroclined.

The Effect of Low Level Laser Therapy on Pressure Pain Threshold in Patients with Temporomandibular Disorders : A Double-blind Study (저출력레이저 조사가 측두하악장애환자의 압력통각역치에 미치는 영향)

  • 측두하악장애환자와 정상대조군의 두경부 근육에 있어서 실제 또는 위약 치료에 따른 저출력 레이저의 효과와 측두하악장애환자에서 저출력 레이저의 효과적인 치료기간을 평가하기 위하여, 측두하악장애를 가진 부산대학교 치과대학생 19명과 측두하악장애의 병력이나 증상이 없는 부산대학교 치과대학생 20명을 대상으로 치료전, 치료 2주 및 치료 4주에 구외촉진이 가능한 32 근골격 촉진점의 압력통각역치와 각각의 동통정도와 두개하악지수를 측정하여 다음과 갈은 결론을 얻었다, 1. 치료전 환자군의 압력통각역치는 정상대조군 압력통각역치보다 낮았다(p<0.05), 2. 환자군의 압력통각역치는 유의하게 증가하였으며 (p<0.05) 레이저 조사 환자군의 증가된 압력통각역치는 레이저 모의조사-환자군의 증가된 압력통각역치보다 더 크게 나타났다. 3. 대조군에서는 레이저 조사나 모의 조사에 의해 압력통각역치와 변화가 없었다. 4. 레이저 조사-환자군의 압력통각역치가 안면근육에서는 치료2주부터 유의하게 증가하였고 경부근육에서는 치료4주부터 유의하게 증가하였다(p<0.05) 5. 레이저 조사-환자군에서 동통의 정도와 두개하악지수는 유의하게 감소하였다(p<0.05).

  • Aneurysm of the basilar artery trunk are rare and the surgical approach is very difficult because of the complexity of surgical anatomy around the basilar trunk and the vulnerable adjacent neurovascular structures. The development of brain CT and MRI makes the accurate diagnosis and produces the improvement of surgical approaches at the lesion of the skull base. One of the surgical approaches of basilar trunk aneurysms, the retrolabyrinthine presigmoid transtentorial transpetrosal approach to the aneurysm of the basilar trunk has some advantages of minimal retraction of cerebellum and temporal lobe, intact auditory and facial nerve function by the preservation of the vestibulocochlear and facial nerves, a preservation of sigmoid sinus and vein of Labbe and a relatively good operation field. We had a good result with this approach for the patient of basilar trunk aneurysm and reported the case with the review of literatures.

  • Orthodontic patients are individuals that grow and develop ; therefore selection of the proper time for orthodontic treatment is considered to be one of most difficult and yet difficult factor. Since the development of cephalometric X-ray, amount and Pattern of craniofacial growth change with aging could be predicted and be came useful in the process of orthodontic treatment. The relationship between the mean values of cephalometric measurements and body height and weight was studied among the groups(boys and girls) of Korean children from the ages 6-years to 17-years. 409 boys and 437 girls with no abnormality in growth and development and no history of orthodontic treatment from the ages of 6 years to 17 years were chosen as subjects Cephaloment X-ray were taken for 3 years and hard tissue analysis based on Burstone's COGS, which was devided into measurements of 6 parts(Cranial base, Maxillar and Mandible, Dental measurements). The relationship between craniofacial growth and height & weight was studied. The following conclusions were obtained : 1. The maximum growth in the measurements of cranial base, N-Ar(FH), N-Ba(FH) corresponded with the age with the maximum increase in body height & weight in both boys and girls. 2. Genial angle gradually decreased with aging in both boys and girls. 3. N-ANS(L) showed greater amount of growth than ANS-Ne(L), and this had greater influence on facial profile. 4. N-A-$Pog^{\circ}$ decreased with aging, and mandibular growth exceeded maxillary growth in amount and rate. 5. Length of Y-axis Increased, but Y-axis to FH plane remained constant. This show that mandible grows at a constant angulation to cranial base. 6. As permanent teeth erupt, interincisal angle deceased.
