Utility of Bone SPECT in Temporomandibular Joint Pain (악관절 동통 환자에서 Bone SPECT의 유용성)

  • Temporomandibular (TM) joint Pain results from many etiologic factors. The aim of this study was to evaluate the utility of Bone SPECT in patients with TM joint pain. The subjects were 34 patients with TM joint pain. All patients underwent plain radiography, planar bone scan, and Bone SPECT The intensity of radioisotope uptake at TM joint was graded into three; no increased uptake above the background activity as grade 0, uptake similar to occipital bone as grade I, and uptake similar to maxillary sinus as grade II. Clinical findings and therapeutic methods were reviewed. Twenty-seven patients (80%) out of 34 patients with TM joint pain had increased uptake in bone SPECT. Twenty-one (78%) out of 27 patients had increased uptake in the mandibular condyle and remaining six patients (22%) had uptake in the mandibular and maxillary arch, which proved to be dental problem. Seven patients (21%) out of 34 were grade as 0, four (12%) were grade I, 23 (68%) were grade II. Four patients with grade I had clicking sound and symptoms which were subsided with medication in all cases. Among 23 patients with grade II, 7 patients had clicking sound and 14 patients underwent medication and decompression therapy. With Planar bone scan, 11 cases (32%) had increased uptake in TM joint area. Plain radiography revealed narrowing, distension, erosion and limitation of TM joint in 16 cases (47%). Bone SPECT can be valuable for screening and managing the patients with TM joint pain. Patients with grade II needed intensive treatment such as joint aspiration. However degree of the radioisotope uptake did not well correlated with clinical symptoms.

Treatments for the Plantar Pain of the First Metatarsal Head (제 1 중족골 두 족저부 동통의 치료)

  • Purpose: To validate the major bony causes and postoperative results of the first metatarsal head plantar pain. Materials and Methods: We experienced one case of the fracture and non-union of the medial sesamoid treated by autogenous calcaneal bone graft, one case of the fracture and non-union and two cases of the hypoplasia of the medial sesamoid treated by excision of medial sesamoid, one case of the arthrosis between the first metatarsal head and the medial sesamoid treated by plantar 1/2 partial excision of the medial sesamoid, and two cases of the metatarsus primus nexus treated by basal metatarsal closing wedge osteotomy between October 1995 and September 1999. The mean follow-up period was 28 months. We evaluated the results by using of the clinical rating systems for the hallux and the radiographic findings. Results: An excellent results were achieved in all cases except one which was preoperatively diagnosed as metatarsus primus nexus. But, clinically this one case also satisfied with the postoperative result. Radiologically, We did not find the malunion or nonunion of the medial sesamoid treated by bone graft and of the metatarsus primus flex us treated by basal metatarsal closing wedge osteotomy. And also we did not find the postoperative fracture of the medial sesamoid treated by plantar 1/2 partial excision. There were no postoperative complications in all cases. Conclusion: We think that the good results may be achieved from the patients with the plantar pain of the first metatarsal head by the exact diagnosis and aggressive treatments.

Quantitative sensory Testing on Edema of Upper Extremity for Stroke Patients (뇌졸중 환자의 상지부종에 대한 정량적 감각 평가)

  • This study is conducted to provide basic data through the Quantitative Sensory Testing(QST) about edema of the upper extremity with hemiplegia for subacute and chronic patients in management and treatment. For the purposes of the study group I, subacute stroke patients(n = 15) and group II, chronic stroke patients(n = 17), was targeted, it was performed difference between the unaffected side and affected side of the upper extremity volume, MPT, CST, WST, CPT, WPT and QST of VST. As the results, all measurement items showed significant differences when comparing each group of all(p<.001), group I and group II of patients affected side, there was a significant difference in the measurements, with the exception of items MPT sensory test(p<.05). In subacute than chronic numbness due to increased sensory threshold appears in the upper extremity edema was able to confirm. It was focused on the difference in sensory properties according to the edema of stroke patients through quantitative test.

  • 일반적으로 견관절의 전방안정력은 하관절와상완인대에서 주역할을 하며, 불안정성으로 인한 병변은 관절순과 골관절와사이의 분리(Bankart lesion)로 나타난다고 한다. 중관절와상완인대는 특히 중등도의 외전위치에서 견관절의 전방안정성에 중요한 역할을 하는 정적 안정물로써, 다른 관절와상완인대의 동반손상없이 단독으로 파열된 례는 문헌고찰상 보고된 바가 없었다. 본 증례의 경우 특별한 외상력없이 내원 1년전부터 우측 견관절의 전방 불안정성과 동통성운동제한의 소견을 보였으며 상기 증상은 3개월 전부터 점차 악화되었다. 관절경 소견상 관절와순 부착부위에서 파열된 중관절와상완인대를 확인할 수 있었으며 동반된 관절와순의 부분파열 및 관절와관절면의 미란을 확인할 수 있었다. 파열된 중관절와상완인대와 비후된 활막을 변연절제 후 8개월 정도의 단계적 재활운동을 시행하였으며 동통과 관절운동의 회복소견을 볼 수 있었다. 본저자들은 관절경검사상 중관절와상완인대의 단독파열소견을 보인 39세의 여교사를 치료하였으며 그 결과를 문헌고찰과 함께 보고하는 바이다.

  • The Incidence of groin injury was about 10% in football player These injuries occur most commonly in athletes participating in sports Involving cutting, quick accelerations and decelerations and sudden directional changes. Symptoms may range from intermittent episodes of mild discomfort to severe and chronic career-ending pain. Groin injuries may result from a variety of causes. We must keep in mind that many other conditions may also affect the groin. Because of the anatomy of the region is so complex. Ligament/muscle strains, nerve entrapment syndromes, sportsman's hernia, symphysitis, stress fractures and various hip pathologies are reviewed as causes of groin pain are reviewed as common causes of groin pain.

Suprascapular Nerve Entrapment Syndrome Caused by Ganglion (결절종에 의한 견갑상신경 포착 증후군)

  • Purpose : To review the operative results and find out better method of a treatment of suprascapular nerve entrapment syndrome caused by ganglion. Materials and Methods : From March of 1996 to May of 2001, thirteen cases of suprascapular nerve entrapment syndrome caused by ganglion were reviewed which were diagnosed using physical examination, EMG and MRI. All of them were treated with open exision of ganglion, nerve decompression and arthroscopic examination. The clinical results were evaluated using subjective recovery of muscle power and ASES (American Shoulder and Elbow Surgeons) score. Results : Nine cases (69$\%$) showed residual symptom such as intermittent pain, insufficient recovery of muscle power, persistent muscle atrophy. Complete recovery in four cases (31$\%$). Mean value of ASES score was 86.4 (70-99.8) points. Postopertively, pain was relieved in 84$\%$ of patients and muscle power was improved in 53$\%$ of the patients but the atrophy persisted. Conclusions : Excision of ganglion is insuffient to relieve the nerve entrapment. Not only excision of ganglion but also treatment of intraarticular lesion, decompression of suprascapular and spinoglenoid notch which attributable to entrapment should be considered for the better clinical results.

  • Leukocyte adhesion deficiency(LAD) is a rare autorecessive defect of phagocytic function resulting from a lack of leukocyte cell surface expression of ${\beta}_2$ integrin molecules(CD 18) that are essential for leukocyte adhesion to endothelial cells and chemotaxis. As a results, patients with LAD suffer from severe bacterial infections and impaired wound healing. A small number of patients with leukocyte adhesion deficiency-1 have a milder defect, with residual expression of CD18. These patients tend to survive beyond infancy; they manifest progressive severe periodontitis, alveolar bone loss, periodontal pocket formation, and partial or total premature loss of the primary and permanent dentitions. In this report, we report on a 7 year old girl with severe oral involvement. The most import focus should be to control infections to reduce the risk for future infection.

  • Pain disorder is the chronic syndrome that onset and change of pain is associated with mood change. But the patient of pain disorder can't perceive their inner conflict. We experienced a 58 year-old woman who had general pain, and whose condition was improved through oriental medical treatment. In this case, we assessed the pain disorder patient at MMPI. So we learned out the patient's inner conflict and dynamic. The applicability of understanding with MMPI has positive effects on the patient with Panic disorder.

Iatrogenic Subclavian Artery Aneurysm: Report of a Case (의인성 쇄골하 동맥류 수술 치험 1례)

  • Iatrogenic suclavian artery aneurysm is a rare disease which requires surgery because of the dangers of a rupture. We report a case of an aneurysm of the right subclavian artery developed by an iatrogenic trauma in a 43-year-old male. The preoperative diagnosis was made by an angiography and Doppler ultrasonogram. After the resection of a 6${\times}$7 cm sized aneurysm, an end to end anastomosis was done with a 6 mm Gore-Tex vascular graft. The post-operative course was uneventful and has been followed up from 3 months after discharge.

The Effects of Thermotherapy on Abdominal Distension and Pain during Colonoscopy (복부 온열요법이 대장내시경검사 대상자의 복부 팽만감 및 통증에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of thermotherapy on abdominal distension and pain during colonoscopy. Methods: This study used a nonequivalent control group and a non-synchronize design. Twenty-nine participants were assigned in an experimental group and 29 in the control group. For the experimental group, electronic heating pad was applied before test throughout the entire procedure. Results: Abdominal distension and total time required for colonoscopy significantly were lower in the experimental group than those in the control group. Conclusion: The results suggest that thermotherapy can be effective to improve abdominal distension and colonoscopy time in patients with colonoscopy. Therefore, thermotherapy can be recommended for those patients undergoing of colonoscopy.
