동작관찰훈련이 만성 뇌졸중 환자의 앉은 자세에서 균형과 몸통조절능력에 미치는 영향 (The Effect of Action on the Balance and the Trunk Control Ability in the Sit Position of Chronic Stroke Patients)

  • Purpose : To find out how action observation training for chronic stroke patients affects their balance and body control abilities in the posture seated in the rehabilitation of stroke. Methods : This study was conducted on 30 subjects who were diagnosed with stroke. The group conducted motion observation training through video clips, while the control group only conducted physical training, and the general physical therapy was performed equally by both counties. The static balance was measured using Biorescue and the dynamic balance was measured using Modified Functional Reach Test (MFRT), Postural Assessment Scale for Stroke, and Trunk Impairment Scale. Results : Static balance showed statistically significant difference in foot pressure (p<.05) as a result of comparison between pre and post exercise training. Dynamic balance was statistically significant (p>.05) as a result of comparing pre and post differences using modified functional reach test. The trunk control ability was statistically significant (p>.001). Comparison between the results of before and after motion observation training showed a statistically significant difference. Conclusion: This study confirmed that exercise training in sitting position was effective for static, dynamic balance ability and trunk control ability of hemiplegic patients due to stroke. These results suggest that the use of motion monitoring in stroke patients may have a positive impact on the diversity and function of rehabilitation.

  • Purpose: The purpose of this study was to determine the effects of action observation training on lower limb muscle activity and dynamic balance in chronic stroke patients. Methods: This study evaluated 20 chronic stroke patients who were divided randomly into a control and an experimental group comprising 10 patients each. Both the experimental group and the control group performed the general exercise therapy provided by the hospital, but the experimental group also performed action observation training. Lower limb muscle activity was measured with a surface electromyograph, and dynamic balance was measured with the Timed Up and Go test and the 10-meter walk test. The paired t-test was used to compare the groups before and after the experiment. Furthermore, the independent t-test was used to assess differences in the degree of change between the two groups before and after the experiment. Results: The within-group comparisons for both the experimental group and the control group showed significant differences in muscle activity and dynamic balance (p<0.05). In a comparison between the groups, the differences in the muscle activity and dynamic balance of the experimental group appeared significant compared with those of the control group (p<0.05). Conclusion: The study results indicate that action observation training is effective in improving the muscle activity and dynamic balance of chronic stroke patients.

  • Purpose: The discovery of mirror neuron system may positively affect functional recovery; therefore, rehabilitation is needed that is practical for use in clinical settings. The purpose of this study was to examine the effect of action observation training on upper motor function in people who had suffered strokes. Methods: Three elderly patients with stroke, aged to years, were recruited from a stroke rehabilitation center. A nonconcurrent, multiple baseline subject approach was taken, with an A-B-A treatment single-subject experimental design, and the experiment was conducted for 3 weeks. The action observation training was repeated 5 times in 5 days during the intervention period. The arm function, including WMFT, BBT, and grip and pinch strength, was evaluated in each subject 5 times during the baseline period, the intervention period, and the baseline regression period. Results: The results of the evaluation for each subject were presented as mean values and video graphs. The WMFT scores of 2 subjects were improved during the intervention period in comparison with the baseline period, and this improvement was maintained even during the regression baseline period. The BBT and the grip and pinch strength were not improved. Conclusion: Based on these results, we suggest that the action observation training for 5 sessions was effective in improving upper limb function of stroke patients but was not effective in improving hand dexterity or grip and pinch strength.

  • PURPOSE: The purpose of this study was to determine the effect of action observation with observation type on the limits of stability and dynamic gait ability in stroke patients. METHODS: The 20 stroke patients who participated in this study were randomly divided into two experimental groups who underwent training three times a week for 4 weeks. Their balance was tested as the limit of stability with Biorescue. Their Dynamic gait ability was tested with the Dynamic Gait Index (DGI) before the intervention, and after 4 weeks. Independent and paired t-tests were used to analyze the results. RESULTS: The results confirmed the limit of stability on the moving areas of the paralyzed and non-paralyzed sides. The limit of stability and dynamic gait index measurements confirmed that the moving area showed a significant difference after the intervention in the whole movement observation group (p<.05), but the partial movement observation group showed no significant difference (p>.05). A significant difference was also noted for the comparison between the both groups after the interventions (p<.05). The functional walking ability showed a significant difference when compared to the ability before the intervention, as determined by the changes in scores obtained for the dynamic gait index (p<.05). CONCLUSION: Interventions utilizing whole movement confirm that training improves stability and functional walking ability in stroke patients with disabilities in balance and walking ability.

유아의 자기표현능력 증진을 위한 오르프 음악활동의 적용 (A Study on Self-Expression Improvement of Children through Orff Activities)

  • 본 연구는 서울시 소재 D어린이집에서 만 4~5세 유아 3명을 대상으로 오르프 음악 활동을 통해 자기표현을 향상을 위한 연구를 실시하였다. 주 3회 총 14회기에 걸쳐 진행된 본 프로그램은 그룹 치료의 형태로 진행되었으며, 각 세션은 40분의 시간으로 이루어졌다. 사용된 기법으로는 오르프 음악활동의 매체를 활용한 말하기, 노래 부르기, 동작, 악기 연주 등의 활동을 시행하였으며 그 내용으로는 대상자들의 자기표현을 향상시키기 위한 신체 이완, 불안감소, 자아 존중감 향상, 자기표현 기술훈련 등의 활동이 제공되었다. 유아의 자기표현성 향상을 살펴보기 위해 단일집단 사전-사후 설계(one group pretest-posttest design)에 기초하여 14회 활동의 사전과 사후에 자기표현 척도 검사를 실시하여 점수를 비교 분석하였다. 또한 오르프 음악활동의 외부환경에서의 변화를 살펴보기 위하여 유아들의 자유놀이를 비디오로 녹화하고 오르프 음악 활동 사전과 음악활동 중간, 음악활동 사후에 교사 3인이 관찰하여 분석하였다. 본 연구에서 얻어진 결과는 다음과 같다. 첫째, 오르프 음악 활동 내의 상황에서 자기표현 언어 척도 점수를 확인한 결과 평균 6.7점 증가함으로 자기표현이 유의미하게 변화되었음을 확인할 수 있었고 둘째, 활동 상황 내의 비언어적 자기표현 행동이 유의미하게 증가하는 것을 관찰할 수 있었으며 셋째, 자유놀이 상황 내에 관찰된 자기표현 언어 척도 점수를 확인한 결과 평균 8점 증가함으로 자기표현이 유의미하게 변화되었음을 확인할 수 있었으며 넷째, 자유놀이 상황 내의 비언어적 자기표현 행동 관찰결과 얼굴표정, 신체접촉, 기타 영역에서 평균점수가 유의미한 증가를 보였다.

  • PURPOSE: The purpose of this study is to evaluate the functional effects of action observation plus functional electrical stimulation (FES) treatment on the weight distribution indexes (heel and toe; right and left), stability indexes, gait velocities, and stride lengths of stroke patients. METHODS: The subjects, who were all more than six months post stroke, were randomly divided into two groups of ten each: an experimental group and a control group. TETRAX (Tetrax Interactive Balance System) and GAITRite (GAITRite$^{TM}$ computerized gait analysis system) were measured at baseline, six weeks after treatment. Participants in both the groups received functional electrical stimulation treatment, but the experimental group was provided with additional action observation. Independent t-tests were used to compare the differences between the groups, and repeated measured two-way ANOVA was used to compare the interaction between the groups. RESULTS: The result of the interactions between the groups and the periods showed significant increases in the weight distribution indexes (heel and toe; right and left), stability indexes, gait velocities and stride lengths (p<0.05). However, a comparison between the groups showed no significance in the weight distribution indexes (heel and toe), stability indexes, and stride lengths (p>0.05). CONCLUSION: Action observation plus functional electrical stimulation treatment should be considered as a therapeutic method for physical therapy for stroke patients to improve the weight distribution indexes, stability indexes, gait velocities, and stride lengths.

이중과제 동작관찰 신체훈련이 만성 뇌졸중 환자의 보행 능력과 일상생활 활동에 미치는 영향 (The Effects of Dual-task Action Observation Physical Training on the Walking Ability and Activities of Daily Living in Chronic Stroke Patients)

  • PURPOSE: The purpose of this study was to determine the efficacy of dual-task action observation training (AOT) and single-task AOT related with daily living task on walking ability and ADL performance in chronic stroke patients. METHODS: Twenty-seven chronic stroke subjects were included in the study. They were randomly assigned to three task categorieds as follows: whole dual-task AOT or partial dual-task AOT or single-task AOT rehabilitation. Whole dual-task AOT observed the movement at once and partial dual-task AOT observed the movement divided into 4 parts related functional gait and activities of daily living task for 2 minutes 30 seconds. Single-task AOT observed the movement related functional gait for 2 minutes 30 seconds. Both groups had physical training session for 12 minutes 30 seconds. The study was conducted for four weeks, with three training sessions a week, for twelve weeks. All subjects were evaluated for their walking ability and activities of daily living through devices, 10m walking test (10MWT), dynamic gait index (DGI), and Korea-Modified Barthel Index (K-MBI). RESULTS: A significant improvement of walking ability and ADL performance happened among dual-task AOT subjects, compared with a single-task AOT subjects, during the 4-weeks course treatment. The results of the study showed statistically significant differences in 10MWT (p<0.05) and DGI (p<0.05), and K-MBI (p<0.05). CONCLUSION: Our results indicated that dual-task AOT has a positive additional impact on recovery of walking ability and ADL performance in chronic stroke patients.

리듬청각자극을 동반한 동작관찰 신체훈련이 만성 뇌졸중 환자의 하지 근활성도와 보행능력에 미치는 영향 (The Effects of Action Observational Physical Training with Rhythmic Auditory Stimulation on Muscle Activity of the Lower Extremity and Gait Ability in Patients with Chronic Stroke)

  • PURPOSE: The purpose of this study was to investigate the effect of action observational physical training with rhythmic auditory stimulation on muscle activity and gait ability in patients with stroke. METHODS: Twenty-six chronic stroke patients participated in this study were assigned into three groups, experimental group 1 (10% faster tempo rhythmic auditory stimulation with action observation training) n=8, experimental group 2 (average tempo rhythmic auditory stimulation with action observation training) n=9, and control group (action observation training) n=9. In this experiment, the corresponding exercise were applied into the subjects of three group for 30 minute a day, 3 time a week during 4 weeks. All participants were measured to muscle activity of lower limb, 10 meter walking test, Figure of 8 walk test, Dynamic gait Index. The collected data were analyzed by using SPSS (version 18.0 for window) and verified that each data was a normal distribution based on Shapiro-Wilk test. Between-group and within-group comparison was analyzed by using One-way ANOVA test, Paired t-test respectively. In all statistical analyses, significance level, ${\alpha}$ was set by .05. RESULTS: The above results revealed that the all experimental group 1 and experimental group 2 and control group were all effective to improve the lower limb muscle activities, gait ability. However more positive effects shown action observational physical training with rhythmic auditory stimulation experimental group. CONCLUSION: This study suggest that action observation physical training with rhythmic auditory stimulation is effective intervention for improvement of muscle activity and walking ability in chronic stroke patients.
