A 13-year-old girl with psoriasis of the elbow, trunk, and face suddenly developed a severe headache followed by left hemiparesis and facial palsy. Brain magnetic resonance imaging showed an acute infarction of the right temporofrontal lobe and basal ganglia on the T2- and diffusion-weighted images. Cerebral angiography showed pre-occlusive irregular scalloped stenosis (99%) in the proximal M1 segment of the right middle cerebral artery and a web-like stenosis at the supraclinoid portion of the right internal carotid artery (ICA) suggestive of a spontaneous intracranial ICA dissection. The patient was administered a low dose of dipyridamole, and a rehabilitation program was initiated. Headache, left motor weakness, and facial droop improved within a week. However, mild left facial palsy and reduced fine motor function of the left hand were still present after 3 weeks. We report a rare case of spontaneous intracranial ICA dissection in a child with psoriasis.

This study investigated which recumbent posture can give rise to the highest vagal modulation in patients with coronary artery disease(CAD), among three recumbent postures; namely, the supine, left lateral, and right lateral postures. For this purpose, 43 patients as CAD group and 31 patients as control group were studied. Heart rate variability(HRV) was measured on these patients for three recumbent postures in random order Normalized high-frequency power was the highest, whereas normalized low-frequency power was the lowest in the right lateral postures, among the three recumbent postures.

A rupture of an innominate artery caused by blunt trauma is relatively rare because this artery is short and protected by the chest bony cage. This report describes a 25-year-old man who suffered a traffic accident, that resulted in an innominate artery rupture, which was detected by a chest computed tomogram and angiogram. This patient underwent urgent surgery through a right clavicular incision and median sternotomy without a cardiopulmonary bypass due to multiple injuries. An approximately 3 cm sized injury was found from the innominate artery to the proximal right subclavian artery and the origin of the common carotid artery. The injured lesion was repaired with a saphenous vein patch. After surgery, he was discharged from hospital without complications.

The aortic inclusion technique is frequently used in the original Bentall's procedure for the control of excessive postoperative bleeding. Although this procedure has improved the outcome of patients with aortic root disease, there is a high incidence of both early and late complications, including coronary artery stenosis, kinking or pseudoaneurysm formation at the coronary suture lines. Pseudoaneurysm of the ascending aorta is a relatively rare, but fatal complication, which occurs after wrap-inclusion composite graft replacement. Herein, the case of a 45-year-old female, who developed a perigraft aortic pseudoaneurysm 10 years after a Bentall's procedure (wrap-inclusion technique), but was successfully managed using the Cabrol's method, is reported.

GRAS probiotics can be used to modulate intestinal microbiota and to alleviate various gastrointestinal disorders. In several recent studies, researchers have explored the potential expansion and usability of probiotics to reduce the risk factors associated with diseases, including obesity, hypercholesterolemia, arterial hypertension, hyperhomocysteinemia, and oxidative stress. In this review, our aim was to clarify the mechanism underlying interactions between hosts (animal or human) and probiotics and the beneficial effects of probiotics on human health.

The purpose of the present study was intended to assess psycosocial stress of male researchers working in a Research Institute, and to reveal the relationship between psycosocial stress and coronary risk factors. Data were obtained from 356 male researchers who underwent health package check-up at the health promotion center of a university hospital from May to June 2007, using by self-administered questionnaire & health check-up. For psycosocial stress, we used the questionnaire with 18 items developed as Psychosocial Well-bing Index(PWI). For coronary risk factors, systolic and diastolic blood pressure(SBP, DBP), triglyceride(TG), total cholesterol(TC), high and low density lipoprotein cholesterol(HDL, LDL), fasting blood sugar(FBS) and % fat rate were measured. As a results, the level of psycosocial stress is influenced by socio-demographic characteristics, job-related characteristics and health-related behaviors. Especially, it was found to have the inter-relational effects that the higher the blood pressure, the higher the level of psycosocial stress.

Aneurysmal dilatation of the ductus arteriosis has been considered a rare but potentially fatal abnormality. The mechanism of ductal aneurysmal formation remains uncertain. Plain chest radiography has proven helpful in the diagnosis of ductus arteriosus aneurysm (DAA), before the application of transthoracic echocardiography. The transthoracic echocardiography is an important tool for the diagnosis and follow-up of DAA. We present a case of congenital ductus arteriosus aneurysm in a newborn, that was an incidental discovery. The diagnosis was made by echocardiography, three-dimensional surface rendering computed tomography (CT), and spontaneous regression after four weeks of follow-up.

이차성 대동맥장루는 아주 드물며 대동맥-장골동맥의 재건수술의 중대한 합병증으로 발생할 수 있다. 이 질환은 진성 대동맥장루와 paraprosthetic enteric fistula 로 분류할 수 있다. 이러한 합병증의 예방, 진단 및 치료는 추구해야할 문제로 되어 있다. 염증성 가성동맥류로 복부대동맥 및 장골동맥을 인조혈관으로 치환한 후 41개월에 내시경으로 Paraprosthetic enteric Fistula로 진단되어 수술을 시행한 39세 남자환자를 치험하였기에 보고한다.

Aortic disease usually recurs after aortic surgery in the form a new aneurysm distal to the previous operation site, and finding a penetrating atherosclerotic ulcer proximal to the previous operation site has rarely been reported. We report here on a case of successful patch repair of a ruptured penetrating ulcer in the distal aortic arch, and this developed late after replacement of the descending thoracic aorta.

Tracheoinnominate artery fistula is a rare complication that can happen after tracheostomy, the mortality rate is high and it reqiures urgent surgical management. The patient had received a left pneumonectomy 30 years ago and post-operative course was in uneventful. And tracheostomy was performed for acute respiratory failure due to trachea stenosis for 2 months in recent. She was improved in general condition and changed to a 11 mm silicone Montgomery T-tube. On the 3rd day after the tube changed, she had cardiac arrest due to the excessive hemorrhaging due to tracheoinnominate artery fistula. We report an successusful experience for control of bleeding by an innominate artery fistula division and the Utley maneuver for the tracheoinnominate artery fistula. We report the operation method of bleeding control.
