Analysis of Characteristics and Symptoms in Home-Based Hospice-Palliative Care Patients Registered at Local Public Health Centers (일 지역 보건소 등록 호스피스 완화돌봄 대상자의 특성 및 증상 분석)

  • Purpose: This study was aimed at analyzing the characteristics and symptoms in home-based hospice-palliative care (HBHPC) patients registered at local public health centers. Methods: A retrospective study was performed; Data of 144 HBHPC patients registered at six public health centers in Pusan City were analyzed, including their initial visit records (registration cards, initial pain evaluation and symptom evaluation). Results: The average age of the patients was 67.7 years old. Among all, 46.2% of the patient lived alone, and 65% had middle school education or lower. The most popular (36.3%) religion was Buddhism, and 47.5% received medical assistance from the government. The most frequent diagnosis was lung cancer followed by stomach cancer and liver cancer in that order. Of all, 48.9% were functionally too weak to lead a daily life, 39.6% were under cancer treatment when registered at the public health center, and 84.5% were aware of the fact that they have reached the terminal phase. Moreover, 83.6% complained about pain, and the pain level was moderate or severe in 36.5% of them. Besides pain, fatigue was the most complained symptom (84.7%), and 49.3% of them rated their fatigue as moderate or severe. Conclusion: Most of the HBHPC patients were socio-economically underprivileged and complained about moderate or worse pain and symptoms. Therefore, it appears necessary to develop an integrated strategy that is tailored for each patient reflecting their characteristics.

  • This study examined the relationship between target advocacy, work values, and person-centered care among nursing students, and attempted to provide basic data for the development of educational materials to improve fundamental ideology for nursing students. The data collection period was from July 1 to July 31, 2023. The questionnaires of 172 nursing students who were enrolled in universities located in cities D and S and agreed to participate in this study were analyzed. The results of the analysis showed that the influential factors on nursing students' person-centered care were target advocacy and work values, and the explanatory power of these variables was 23.5%. In conclusion, in order to improve nursing students' competence in person-centered care, it is necessary to identify the extent of their work values as nurses, compare and develop programs for the formation of positive work values, and improve their awareness of target advocacy, which is the fundamental philosophy of nursing.

The Relationship of Caregiving Appraisal and Family Function among Senile Dementia Patients' Families using In-home Services for the Long-Term Care Insurance (노인장기요양보험 재가보호서비스를 이용하는 노인성 치매환자 가족의 돌봄평가와 가족기능과의 관계)

  • The aim of this study was to identify the relationship of caregiving appraisal and family function among senile dementia patients' families using in-home services for the Long-Term Care Insurance(LTCI). The participants were 121 family caregivers of senile dementia patients using in-home services for the LTCI. The participants' mean age was $53.80{\pm}10.86years$, all middle-aged women(75.2%). When negative cognition's burden and neutral one's demand in their caregiving appraisal were low and positive cognition's satisfaction and mastery in their caregiving appraisal were high, emotional function in their family function was high. There is a need to develop nursing interventions for strengthening positive cognition's satisfaction and decreasing neutral one's demand in their caregiving appraisal to improve emotional function and communication of senile dementia patients' families.

Death anxiety and Needs of Interpersonal Caring Behaviors of Nurses in Hospice Wards: Focused on Types of Personality (호스피스 병동 간호사의 죽음 불안 및 돌봄 요구: 성격유형을 중심으로)

  • Purpose: The purpose of this study is to investigate Death anxiety and Needs of Interpersonal caring behaviors of nurses in hospice wards focused on types of personality. Methods: The data were collected from 118 nurses working in hospice wards and analyzed SPSS Statistics 26. Results: Death anxeity was 2.41±0.27, and the highest subscale was Dying of Self(2.60±0.38). Needs of Interpersonal caring behaviors was 3.58±0.60, and the highest subscale was forgiving(3.74±0.64). significant static correlation between death anxeity and Needs of Interpersonal caring behaviors was confirmed(r=.265, p=.004). The affect of Death anxiety by personality type on Needs of Interpersonal caring behaviors was confirmed as a factor in which 'fear of others' dying' influences Needs of Interpersonal caring behaviors in Emotion centered type. Conclusion: Death anxiety and Needs of Interpersonal caring behaviors have significant static correlation, Death anxiety influenced Needs of Interpersonal caring behaviors in Emotion centered type.

  • Objectives: This study aimed to find out the effects of hospice care in public health centers by evaluating the quality of life of terminal cancer patients and care-giver burden of their families.Methods: From January to December 2007, 32 terminal cancer patients and their familes were selected as with interviews. Instruments used for this study were C-QOL(Cancer-Quality of Life) that was developed by Le(207) and care-giver burden of families that was developed by Seo et al(193). The data were analyzed using frequency, percentage and paired t-test.

An Analysis of Factors Influencing the Dementia Patient Home Care Intention of Family Caregivers (치매가족의 치매대상자 재가돌봄의사에 대한 영향요인분석)

  • This study was conducted to find a practical implication regarding efficient and smooth service utilization including care burden reduction of family caregivers by empirically analyzing the factors influencing the dementia patient home care intention of family caregivers, based on Anderson's behavior model. For this study, the subjects and characteristics suitable for the study purpose were selected using the source data of "A Survey on the Dementia Recognition and Needs" targeting 26 cities and countries in Gyeonggi-do and then secondary analysis was conducted. This study targeted 539 dementia families, and based on Anderson's behavior model, variables related to predisposing factors, enabling factors and need factors were inserted, and hierarchical regression analysis was applied. The analysis result showed that at a significance level of 5%, the home care intention was high when family caregivers were not living in cities, they were spouses or children and their economic level was high as predisposing factors, and the dementia-related attitude was positive as a enabling factor, and the degree of dementia was mild and care burden was low as need factors. Moreover, need factors were relatively important variables among the three factors mentioned above. These findings demonstrate that for improving the home care intention, there is a need to construct a support strategy, which considers the degree of dementia and economic characteristics and develop diverse counseling and education programs for a positive attitude towards dementia and a differentiated strategy depending on regional and family characteristics.

  • In this study, the problem of child care gaps has arisen due to the expansion of women's entry into the workforce and the increase in working couples, and the care cliff phenomenon after children enter elementary school has been identified as one of the causes of women's career disconnection and low birth rates, and child care services have been initiated to solve care problems and balance work and family. The importance of childcare services to the safety and well-being of children has been highlighted by the restrictions on school attendance and the absence of caregivers during the COVID-19 pandemic. The government has been making policy efforts to reduce the gap in child care, but problems with the effectiveness and efficiency of the child care system have arisen due to unstable target selection and delivery systems by ministries and projects in the implementation of child care services. Therefore, this study examines the child care services implemented by each ministry to reduce the blind spots of after-school care services in the community and prepare efficient operation plans for various delivery systems, and seeks directions for the development of child care services.

  • This study is explorative survey carried out to examine differences in professional self-concepts and emergency patient care perceptions between emergency medical technician trainees (namely, EMT-basic trainees or trainees) and emergency medical technicians basic (namely, EMT-basic or technicians). The subjects are some trainees and technicians. The trainees included 95 fire fighters who had enrolled educational programs for emergency medical technicians basic at K and A Fire Academy while the technicians were 78 fire fighter gathering from all regions of the country who had enrolled a short expertise program for emergency medical technicians basic at S University in C City.

  • This study attempted to develop a scale that Interpersonal Caring Behavior Measurement Tool, based on Kim's Interpersonal caring theory, and confirmed its reliability and validity. The items were used as 50 items of 10 concepts based on the Su-ji Kim's interpersonal care theory in Korea developed by Seon-hee Yun. Subjects were nurses working at a general hospital or higher, who agreed to participate in this study. As a result of the study, 50 questions in 10 concepts of the first were derived from active listening, accepting, complimenting, noticing, and companioning through exploratory and confirmatory factor analysis, and criterion validity and reliability were verified. This tool is meaningful in that it can measure Caring Behavior from the perspective of the subject and family, and can be used as an index to visualize the quality improvement of nursing care by quantifying it.

Care Needs of Hospice Patients in Comparison with Those from the Family Caregivers' Perspective: Q methodology (호스피스 환자의 돌봄 요구와 가족이 인지하는 환자의 돌봄 요구 비교: Q 방법론)

  • Purpose: The purpose of tile study was to identify types of care needs of hospice patients and those from the family caregivers' perspective and to compare these two groups in reporting patients' care needs through Q-methodology. Methods: Twenty three Q-statements concerning care needs were selected through in-depth interviews of hospice patients. Data were collected from 20 hospice patients as well as 20 family caregivers respectively by sorting 23 Q-statements into 9 points standard. Data analysis was performed by using PC QUANL program. Results: Principal component analysis identified four types of care needs of the hospice patients. Overall, the accuracy of family caregiver reports was 48% in all types of care needs. Type 1 was named 'physical care needs type' for those whose greatest need was physical care to be free of pain and comfortable. The accuracy in Type 1 was 62.5%. Type 2 was named 'emotional care needs type' for those who would like to share love and intimacy with their family members. The accuracy in Type 2 was 20%. Type 3 was named 'spiritual care needs type' for those who would like to receive forgiveness from their God and prayers and visitation of clergy. The accuracy in Type 3 was 60%. Type 4 was named 'social care needs type' for those who would like to complete their ongoing work and to give service to others. The accuracy in Type 4 was 50%. Conclusion: There was a great difference between hospice patients and the family caregivers in reporting patients' care needs. Thus, hospice nurses need to educate family caregivers to more accurately assess patients' care needs.
