  • Autogenous bone grafting has a broad range of applications and implications, and also limitations, though it is the oldest and most important reconstructive techniques in the oral and maxillofacial surgical field.Further understanding of bone healing mechanisms, bone physiology and bone biology, transplantation immunology, and development of tissue banking procedures had enabled oral and maxillofacial surgeons to reconstruct even the most difficult bony defects successfully with the preserved allogeneic bone implant. Now autogenous bone and allogeneic bone implants present a wide variety of surgical options to surgeons, whether used separately or in combination. The surgeons are able to make judicious and fruitful choices, only with a through knowledge of the above-mentioned biologic principles and skillful techniques. The author evaluated 116 cases where allogeneic bones were transplanted for oral and maxillofacial reconstruction.

  • A female child with unilateral midpalatal cleft was successfully treated by Wardill V-Y pushback palatal flap and superiorly based pharyngeal flap simultaneously. The advantages of this method are to prepare the favorable background of postoperative speech correction and additional nasal lining. We can try this simultaneous operation in delayed cleft palate repair.

  • Skeletal class III malocclusion with aterior deep bite is difficult to manage properly, especially in case of mild mandibular prognathism. We have designed lower anterior segmental osteotomy for improving the lower third of the facial contour. Considerable improvement of esthetic facial contour with correction of cross bite in anterior incisors was observed in patients with mandibular prognathism.

FIBRIN SEALANTS IN MAXILLOFACIAL SURGERY : A INTRODUCTORY REPORT (악안면 외과 영역에서의 FIBRIN SEALANTS 의 이용)

  • The fibrin sealant was first designed as an alternative to surgical suture for the purpose of surface-to-surface union especially in parenchymal organs like the liver, spleen and kidney. The clinical application of currently used fibrin sealant was first introduced in 1972. The fibrin sealant consists of principal two components; lyophilized human fibrinogen and bovine thrombin. The fibrinogen component also contains coagulation factor XIII. A solution of aprotinin, an inhibitor of fibrinolysis is used to dissolve the fibrinogen and to provide the first component, and a solution of calcium chloride is also used to provide the second component. From July to December in 1990, during 6 months, we used fibrin sealant in the 28 patients of 33 various cases, in the following ways; supportive application of fibrin sealant after free autogenouse nerve graft for the repair of inferior alveolar nerve, facial nerve or accessory nerve, treament of hemangioma or lymphangioma to thrombosize and lead to the tumor shrinking, skin grafting to stimulate the adhesion and tissue repair, bone grafting in the patients of cleft alveolus, mandibular reconstruction or orthognathic surgery to facilitate the knitting of bone chips, tissue adhesion after tumor resection, radical neck dissection or flap reconstructions, and supportive adhesion of external auditory cannal after TMJ surgery via postauricular approach. No adverse effects were observed, none of the patients developed hepatitis or other blood transmitted disease, and the wound healing were acceptable.

  • Currently dental laser is used at a variety of dental treatment and many favorable clinical case reports are presented. We performed this study for the presentation of the validity of dental laser which are available and supportable in the oral and maxillofacial surgery. From October, 1998 to March, 1999, dental laser was used in 70 cases. Male patients were 38, female 32. The age ranged from 5 to 77, with a mean of 45.4. Indicated cases were 18 chronic periodontitis, 11 infections, 6 atypical pain, malignant or benign tumor, precancerous lesion, abnormal frenum, TMJ disorder, mucocele, and so forth. $CO_2$ laser(Sharplan 15F, Sharplan Co.) was used in 40 cases, Ho-YAG laser(Duopulse 2000, Excel Co.) 11, Nd-YAG(Duopulse 2000, Excel Co.) 18. We present and discuss about the objectiveness, availability, and future application of laser with case reports.

  • After the Iraq war, the Korean government established an Iraq Peace and Reconstruction Division, 'Zaytun'. Zaytun division has been dispatched in northern Iraq, since August 3, 2004. As one of our missions, 'Zaytun Hospital' was built to support the demand for the insufficient medical facilities and supplies in Iraq, which was opened on November 27, 2004. Zaytun Hospital, equipped with modern operation room and general ward, has been composed of 12 clinical departments. Although we had many problems, such as difficulty of communication with Iraqis, connection with medical facilities of Iraq and risk of terrorism, we overcame these difficulties and were able to achieve our missions successfully. I wish that our special experience in Iraq are will be the guideline to other oral and maxillofacial surgeons, who want to help those individual in unfortunate areas such as Iraq.

  • The bulging of mandibular angle area, frequently, make some difficulties in social life due to its reckless appearence, especially in oriental nations. So, many authors had reported its causes and surgical techniques for correction since late 19th century with diagnosis of masseter hypertrophy. But in spite of its muscular origin, major surgical techniques, in general, should aim complete bony reduction or osteotomy and supplemental myotomy as in hemimandibular hypertrophy or mandibular elongation. We used ultrasonogram for soft tissue depth estimation and expearenced some complications such as incomplete lingual cortical bony reduction, condylar fracture in mandibular angle reduction via intraoral circumvestibular approach. So we reported our surgical technique for angle reduction with possible complications.

Applying Method of Arch Bar in Maxillofacial Surgery without Orthodontic Treatment (교정치료 비적용 악안면수술에서 치열활봉 거치법)

  • Temporomandibular joint(TMJ) and dental occlusion are important in functional aspect as well as in cosmetic aspect for craniofacial surgery such as orthognathic and traumatic surgery. During these operations, appropriate maxillomandibular fixation(MMF) is especially necessary to conserve or reconstruct, dynamic TMJ and functional occlusion. Arch bar is one of the most popular method to gain proper MMF. Seventeen patients including 5 patients with mandibular fracture, 12 patients with orthognathic surgery(6 patients had relatively normal occlusion, however 6 patients had facial anomalies such as hemifacial microsomia with irregular occlusal plane.) were joined in this study. Arch bar was contoured on the dental cast, which was prepared for model surgery, prior to apply it on the facial anomaly patient. On using pre-contoured arch bar, patients felt better during the procedure and surgeons saved time. Moreover, well-contoured arch bar promises precise transmission of force on the bone, which implies surgeons do not need to worry about the occlusion while fixing bony structures. Authors suggest how to apply arch bar to satisfy basic theories about dental row and occlusion based on the experience. In addition, it is proposed to contour arch bar on the dental impression prior to apply it on the patient who has irregular and complex occlusal plane.

  • Several authors have proposed techniques and devices by which the correct position of the proximal segment can be maintained both during fixation and postoperatively. Schendel, Epker, Lake, Worms, Ive and Poulton have been discussed the problem of condylar distration in mandibular orthognathic surgery. This study described have showed the some advantages forward repositioning of the condyle head in vertical ramus osteotomy which used with the Mini-Hoffmann sets in external skeletal pin fixation extraorally in 19 cases.

  • In the treatment of dentofacial deformities, analysis of skeletal pattern, with evaluation of plaster dental cast, is essential procedure. This study was based on 20 patients with dentofacial deformity who were admitted to the department of oral and maxillofacial surgery Kngnam sacred heart Hospital during the period of Jan 1988 through Aug 1989. We studied serial cephalometric radiogarphs of patients (pre-op, postop immediately, postop 6 moth). The obtained results indicates the good resistance of rigid fixation against relapse in orthognathic surgery.
