A Case of Ascending Colon Diverticulitis with Perforation in a Child (소아 상행 대장 게실염 천공 1예)

  • A diverticulum is a blind pouch communicating with the gut. The term "diverticulitis" indicates inflammation of a diverticulum or diverticula, which is commonly accompanied by gross or microscopic perforation. Acute diverticuitis is a rare disorder in early childhood. Itis difficult to diagnose acute right colon diverticulitis from common causes of RLQ pain. We report a case of acute diverticulitis in the right colon in a 6-year-old girl. She complained of typical RLQ pain mimicking acute appendicitis,but was diagnosed with acute diverticulitis by CT scanning. Conservative treatment failed because of peritonitis due to perforation of an inflamed diverticulum. After the diverticulcetomy, the symptoms resolved.

A Mouse Colon Model to Investigate Stent-Induced Tissue Hyperplasia (쥐 대장에서 스텐트 삽입 후 발생하는 조직 과증식 모델 형성을 위한 연구)

  • The purpose of this study was to investigate the feasibility of stent placement and the evaluation of tissue hyperplasia caused by bare metallic stent placement in a mouse colon model. In a pilot study, C57BL/6 mouse were used to verify diameter of colon. Mean diameter size was 4.05 mm. Twenty C57BL/6 mice were divided into two groups to assess differing stent diameters (Group A, 5 mm diameter; Group B. Group B, 4 mm diameter). Follow-up, 1-week fluoroscopic imaging, 4-week endoscopic imaging were obtained. Mice were sacrificed 4-week after stent placement. Microscopic findings were evaluated. Stent placement was technically successful except one mouse in Group A. Data from the mouse was omitted. During follow-up, five mice in Group A died within 7 days after stent placement, and one stent in Group B was migrated into the rectum. The incidence of stent-related complication was 60% and 10% between group A and Group B, respectively. Gross and Endoscopic findings showed tissue hyperplasia through the mesh, and all the stents had become incorporated into the wall of the colon. Microscopic findings were no statistically significant difference. colonic stent placement was technically feasible, and stent-induced tissue hyperplasia was evident in a mouse model. With stent large size, there was the highest incidence of colon perforation.

  • 내시경을 이용한 진단 및 시술은 빈번하게 이루어지고 있으며, 대장암의 증가와 정부의 암 조기검진사업의 영향으로 시행건수가 많아지면서 천공 및 출혈 등의 의료사고도 늘어나고 있다. 내시경 장비를 이용한 시술은 시술자의 경험이나 감각, 숙련도에 의존하여 시행된다. 내시경 시술시 내시경 선단부의 수평 및 수직, 압력에 대한 실시간 정보를 제공하면 의료사고를 줄일 수 있다. 본 논문에서는 가속도와 자이로센서를 융합하여 각도 및 자세정보를 얻고 압력센서를 이용하여 선단부가 받는 압력을 시술자에게 제공하여 내시경 시술시 천공 및 출혈 등의 의료사고를 줄일 수 있는 방안을 제시한다.

  • Behcet's disease (BD) is a multisystem inflammatory disorder dominated clinically by recurrent oral and genital ulceration, uveitis and erythema nodosum. BD is very rare in children, especially those less than 10 years of age, who account for only an estimated 5% of all cases. Gastrointestinal ulcers, in patients with Behcet's disease with intestinal involvement are rare and have been reported in only 1-2% of all cases. The intestinal ulcers of Behcet's disease are usually multiple and scattered and tend to cause perforations associated with significant morbidity. Patients with BD and abdominal symptoms must be evaluated thoroughly for potential perforation of the gastrointestinal tract. Here we report the case of a 4 year 9 month old child with multiple perforations of the gastrointestinal tract associated with BD.

  • Behest's disease is manifested as ulcers present in oral cavity, on external genital organ, orbital area, and skin. This disease could also cause other symptoms by invading the cardiovascular system, respiratory system, gastrointestinal system, central nervous system, and urogenital system. It is very rare for Behcet's disease to be accompanied by carotid artery aneurysm and intestinal perforation. We report a patient with Behcet's disease, who is diagnosed with symptoms and pathological findings as haying carotid artery aneurysm and intestinal perforation at the same time. The patient underwent operation for the aneurysm and the intestinal perforation.

  • Due to the development of endoscopic technology, endoscopic diagnosis and procedures have been actively performed. However, due to the increase in colorectal cancer and the effect of government's early cancer screening project, the number of endoscopic diagnosis and procedures has increased, thus medical accidents such as perforation and bleeding are increasing. Therefore, efforts to reduce such medical accidents are needed. In this paper, we propose a method to provide the direction information of the endoscope and the force applied to the probe in real time. We also propose and evaluate an interpolated complementary filter considering the very slow movement of the endoscope probe. The proposed interpolated complementary filter showed 34.6% and 27.6% performance improvement. The expected user interface for applying the proposed method to the endoscopic system is also presented. This shows that the proposed method is applicable to endoscopy system.

  • A fistula between the respiratory and gastrointestinal systems is generally caused by infection and trauma. We experienced a 51-year old man with a broncho-pleuro-gastro-colonic fistula. He complained of chronic foul odor during respiration. He had suffered a traumatic diaphragmatic rupture 30 years ago. The infection of the diaphragm caused necrosis of the right lower lobe of the lung. It also caused a broncho-pleural fistula. The infection also created adhesion and a perforation of the gastric cardiac portion and the colonic splenic flexus portion of the gastro-intestinal track. We performed left lower lobectomy of the lung, reconstruction of the diaphragm and gastro-intestinal reanastomosis.

  • A clinical study was performed on 152 cases of surgical esophageal disease treated by the Department of Thoracic & Cardiovascular Surgery of Korea University Hospital from Jan. 1989 through July 1994. The most common esophageal disease was cancer which was seen in 73 cases (48%) among 152 cases. All were treated surgically' 52 patients (71%) were managed by curative or palliative resection with reconstruction and feeding gastrostomy or jejunostomy, otherwise Celestine tube insertion was performed on the remaining 21 patients for palliatio'n. Esophageal leiomyoma occurred in 6 cases(3.9%), among them 1 case was performed with trio recoscopic enucleation . Achalasia were in 7 cases (4.6%) and was treated with modified Heller's m otomy and with Belsey Mark IV operation. Diverticulum were in 11 cases (7.2%). Esophageal stricture occurred in 20 cases (14.1 %) and 17 of 20 cases were managed with bypass surgery. Esophageal perforation was seen in 20 cases, its cause was instrumental trauma in 7 cases, stab wound in 4 cases, foreign body in 4 cases, spontaneous perforation in 3 cases, and others 1 case Other disease including congenital lesion was seen In 1 Scases.

Characterization of Porcine Tissue Perforation Using High-Power Near-Infrared Laser at 808-nm Wavelength (808 nm 파장의 고출력 근적외선 레이저 조사 시 돼지 조직의 천공 특성 연구)

  • A fundamental study on laser-tissue interaction was conducted with the aim of developing a therapeutic medical device that can remove lesions on the intestinal wall by irradiating a high-power 808-nm infrared laser light incorporated in an endoscopic system. The perforation depth was linearly increased in the range of 1~4 mm in proportional to laser output (3~12 W) and irradiation time (5~20 s). We demonstrated that the perforation depth during laser irradiation was varied according to the tissue property of each extracted porcine organ. The measurement of the temperature distribution suggests that the energy is localized in the irradiation spot and transferred to deep tissue, which protects the surrounding tissue from thermal injury. These results can be used to set the driving parameters for a laser incision technique as an alternative to conventional surgical interventions.

The Significance of Small Polyp of Colon in Koreans (한국인에 있어서 대장의 작은 용종의 의의)

  • Background: There arc two theories in the development of colon cancer. One is the adenoma-carcinoma sequence theory and the other is the de novo cancer theory. Western countries believe in the adenoma-carcinoma sequence theory, however there are many recent reports from, Japan about cancers developing from small adenomas. Methods: The present study analyzed 408 polyps from 508 cases that were taken by colonoscopic polypectomy at the Department of internal Medicine, Yeungnam University Hospital. Results: The percentage of patients who had a polyp was 41.3% (210 cases out of 526 cases), and the peak incidence was noted in patients in their 50s and 60s, There was no difference between the sexes, but we noted significant increase in the incidence of polyps in patients over the age of thirty. We found 395 polyps below 1cm and 13 polyps above 1cm. Among 408 polyps, 5 cases were cancerous polyps and 3 cases showed polyp size of less than 1cm each. The first case was a polyp of 0.4cm in size with elevated mucosa at the ascending colon. The second was 0.5cm in size with round elevation and hyperemic mucosa in the rectum. The third was 0.6cm in size with tubular elevation at the hepatic flexure. Conclusions: Colon polyp is a common disease in Koreans, Even small polyps can have cancer tissue, which should be removed if discovered during colonoscopy. We believe that not all colon cancer originates in the manner described by the adenoma-carcinoma sequence theory. However further studies with a larger sample population arc needed to determine the exact role of colon polyps in the development of colon cancer.
