Analysis of the Relationship between Abdominal Ultrasound Based Kidney Stones, Obesity and Metabolic Syndrome (복부 초음파 검사 기반 신장결석과 비만 및 대사증후군 관련성 분석)

  • Kidney stones are a common disease with an annual prevalence of about 30,000 people in Korea, and are deeply related to an increase in chronic kidney disease, high blood pressure, coronary artery disease, metabolic syndrome, type 2 diabetes, hyperlipidemia, and abdominal obesity. Therefore, in this study, 135 examinees who visited a general hospital in Dalseong-gun from May 2019 to June 2020 for a medical examination were examined. The relationship between kidney stones and factors related to obesity and metabolic syndrome were found in abdominal ultrasound. I tried to find out the relevance. As a result of the study, the risk of kidney stones in the abnormal group was increased by 4.255 times compared to the normal group in total cholesterol factor, and the risk of kidney stones in the abnormal group was increased by 2.072 times compared to the normal group in the low-density lipoprotein cholesterol factor. Total cholesterol factor and low-density lipoprotein cholesterol It was found that the factor affects the prevalence of kidney stones and metabolic syndrome. Since the risk of kidney stones is related to total cholesterol and low-density lipoprotein cholesterol factors, active attention should be paid to preventive purposes through health check-ups.

Optimal Cutoff Points of Rate Pressure Product in Each Stage of Treadmill Exercise Test According to the Degree of Metabolic Syndrome in Korean Adults (한국성인의 대사증후군 예방을 위한 운동부하 검사시 각 단계별 심근부담률의 적정 임계점)

  • The rate pressure product (RPP) is expressed as a product of the heart rate and systolic blood pressure as an index indirectly measuring the myocardial oxygen consumption, and it indicates the burden on the myocardium. The aim of this study was to determine the optimal level of RPP for preventing metabolic syndrome in a treadmill exercise test in Korean adults. Metabolic syndrome was the diagnosis of the third executive summary report on the National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATP III) criteria. According to the criteria, the metabolic syndrome diagnosis group (MetS, N=25), pre-metabolic syndrome group (Pre-MetS, N=106), and non-risk factor group (Non-MetS, N=65) were classified. The exercise stress test was performed based on the Bruce protocol. The RPP was calculated as (heart rate${\times}$systolic blood pressure)${\div}1,000$. The results showed that the maximum systolic blood pressure was high despite the low daily dose reached in the diagnostic group of metabolic syndrome. The optimal threshold of the RPP at the time of the exercise treadmill test for a metabolic syndrome prediction was $12.56mmHg{\times}beats/min{\times}10^{-3}$ in the first stage of the exercise stress test. The second stage of the exercise test was $16.94mmHg{\times}beats/min{\times}10^{-3}$, and at the third stage of the exercise test was $21.11mmHg{\times}beats/min{\times}10^{-3}$.

The Relationship between Metabolic Syndrome and Quality of Life in Korean Adult Women (한국 성인 여성의 대사증후군과 삶의 질)

  • Metabolic syndrome is the most important risk factors of cardiovascular diseases So, it is judged that Quality of life of persons with metabolic syndrome inevitably deteriorate. For the purpose, the study aims to analyse the number of risk factors, and the relationship between abnormality of each risk factor and Quality of life, targeting adult women aged more than 19. For this study, data from the National Health and Nutrition Behavior Survey in 2007 and 2008 were incorporated and 4,365 subjects were collected for the study. Version 17.0 SPSS was used for data analysis. Statistical significance was defined as p<0.05. As a result of the study, the prevalence rate of metabolic syndrome was 24.2%. According to the number of risk factors of metabolic syndrome the regression coefficient of Quality of life was -0.024 when the number of risk factor was one, -0.048 when there were two risk factors, -0.090 when there were three risk factors, -0.117 when there were four risk factors, and -0.168 when there were five risk factors. The regression coefficient(abnormal/normal)of risk factors of metabolic syndrome were as follows; Waist circumference -0.035, Blood pressure was -0.064, Fasting glucose was -0,026 and HDL cholesterol was -0.012. In conclusion, as it was discovered that risk factors of metabolic syndrome had significant relations with Quality of life, it is suggested that the relations between some significant variables and Quality of life should be actively considered so that adult women can perceive metabolic syndrome properly and endeavor to improve their Quality of life.

Association between the Components of Metabolic Syndrome and Total Bilirubin at the Health Care Check Up in Korean Men and Women: Based on Adults Except for Diabetes and Hypertension (당뇨와 고혈압 환자를 제외한 건강검진을 실시한 한국 성인남녀의 대사증후군 구성요소와 총 빌리루빈간의 관련성)

  • Bilirubin has an anti-inflammatory effect as an endogenous antioxidant and has been reported to be inversely correlated with hypertension, diabetes, metabolic syndrome, and insulin resistance. On the other hand, there is a lack of research evaluating the association between bilirubin and metabolic syndrome compared to liver-specific indicators. This study examined the association between the total bilirubin and metabolic syndrome prevalence in Korean adults who underwent health screening compared to ALT and GGT. This cross sectional study included 22,568 adult males and females aged 20 years or older who underwent a health examination at the General Hospital of Gyeonggi Province from March 2015 to February 2018. Metabolic syndrome was diagnosed based on the American Heart Association/National Heart, Lung, and Blood Institute (AHA/NHLBI) guidelines. In both genders, the metabolic syndrome diagnosed group had higher liver enzymes, and the total bilirubin and direct bilirubin were lower than those without the metabolic syndrome risk factors. The serum total bilirubin showed a weak inverse correlation with abdominal obesity (r=-0.066 vs r=-0.055) and triglyceride (r=-0.127 vs r=-0.136) in both males and females, which is weaker than ALT and GGT The data were statistically significant. In addition, total bilirubin as well as ALT and GGT in both men and women were not predictors of the likelihood of developing metabolic syndrome.

Prevalence of the Nonalcoholic Fatty Liver Disease in Obese Children (소아 비만증에서 비알코올성 지방간염의 유병률)

  • Purpose : Obesity is, along with metabolic syndrome, closely related with nonalcoholic fatty liver disease. This study tried to evaluate the prevalence of nonalcoholic liver disease in obese children and verify the factors associated with the disease. Methods : Two hundred and seventy nine children who showed a body mass index of 95 percentile over the baseline in health examinations of surrounding schools were evaluated. Questionnaires, body measurements, blood examinations, and ultrasonographic measurements of abdominal fat were examined. Results : Out of 279 children enrolled for the study, 27 children were found to possess nonalcoholic liver disease(9.7%). Among those found to be positive for nonalcoholic liver disease, it's prevalence increased to 15.2%(22 out of 144 children) among children with severe obesity. Factors known to be involved with metabolic syndrome, namely waist/hip circumference ratio and thickness of abdominal fat, were found to be closely related to nonalcoholic fatty liver as well. Conclusion : The prevalence of nonalcoholic fatty liver in obese children was 9.7%, with higher incidence observable in severer obesity. Factors responsible for metabolic syndrome were closely associated with nonalcoholic fatty liver disease, and the level of insulin resistance, which is an useful index in both diseases, can be utilized in evaluation of the effect of treatment and control of risk factors.

Comparison of the Usefulness of Lipid Ratio Indicators for Prediction of Metabolic Syndrome in the Elderly Aged 65 Years or Older (65세 이상 고령자에서 대사증후군 예측을 위한 지질비율 지표의 유용성 비교)

  • The purpose of this study was to compare the usefulness of the lipid ratio indicators for the diagnosis of metabolic syndrome in the elderly aged 65 years or older. From January 2018 to December 2020, 1,464 people aged 65 years or older who underwent a health checkup at a general hospital in Seoul were included. Lipid ratio indicators were measured through blood tests. The prevalence of metabolic syndrome according to the quartiles of the lipid ratio index was confirmed by logistic regression analysis. In addition, the metabolic syndrome predictive ability and cutoff value of the lipid ratio indices were estimated with the receiver operating characteristic(ROC) curve. The correlation between atherogenic index of plasma(AIP) and waist circumference was the highest in both men and women(r=0.278, p<0.001 vs r=0.252, p<0.001). As for the lipid ratio indices, the incidence of metabolic syndrome was higher in the fourth quartile than in the first quartile. The area under the ROC curve(AUC) value of AIP was higher at 0.826(95% CI=0.799-0.850) and 0.852(95% CI=0.820-0.881) for men and women, respectively, compared to other lipid ratio indicators, and the optimal cutoff values for both men and women was 0.44(p<0.001). Therefore, the AIP among the lipid ratio indicators was found to be the most useful index for diagnosing metabolic syndrome in the elderly aged 65 years or older.

Association between intake of antioxidant vitamins and metabolic syndrome risk among Korean adults (한국 성인의 항산화 비타민 섭취와 대사증후군 위험 간의 관련성)

  • Purpose: The purpose of this study was to examine the association between intake of antioxidant vitamins and prevalence of metabolic syndrome (MetS) among Korean adults. Methods: A total of 614 subjects aged 30~60 years were recruited from those who received a medical checkup at a general hospital in South Korea between 2009 and 2012. Presence of MetS was determined based on criteria issued by the NCEP ATP III. Intakes of antioxidant vitamins (vitamin A, retinol, carotenoids, vitamin C, and vitamin E) were estimated by combining 3-day diet records with an antioxidant vitamin database for common Korean foods. We used multiple logistic regression analysis to assess the association between dietary intakes of antioxidant vitamins and MetS. Results: Men in the highest tertile for retinol (OR = 0.40, 95% CI = 0.23~0.71, P for trend = 0.0009), carotenoids (OR = 0.57, 95% CI = 0.32~1.00, P for trend = 0.0470), and vitamin E (OR = 0.52, 95% CI = 0.30~0.92, P for trend = 0.0190) intakes had a lower likelihood of having Mets than those in the lowest tertile. The OR of high fasting blood glucose among men in the highest tertile for vitamin A (${\mu}g$ RE: OR = 0.55, 95% CI = 0.32~0.97, P for trend = 0.0417, ${\mu}g$ RAE: OR = 0.52, 95% CI = 0.29~0.92, P for trend = 0.0211), carotenoids (OR = 0.41, 95% CI = 0.23~0.73, P for trend = 0.0036), and vitamin E (OR = 0.47, 95% CI = 0.26~0.82, P for trend = 0.0080) intakes was lower than those in the lowest tertile. In women, subjects in the highest tertile of retinol intakes had a lower prevalence of MetS than those in the lowest tertile group (OR = 0.55, 95% CI = 0.30~0.98). The OR for abdominal obesity was lower among women with the highest vitamin A (${\mu}g$ RE) intakes compared to those in the lowest tertile (OR = 0.51, 95% CI = 0.28~0.93, P for trend = 0.0293). Conclusion: These results suggest that dietary intakes of antioxidant vitamins might be associated with reduced risk of having MetS among Korean adults.

The Prevalence of Metabolic Syndrome in Inpatients with Schizophrenia (정신분열병 입원 환자에서의 대사 증후군 유병률 연구)

  • Objectives : The objective of this study is to assess the prevalence and risk factors of metabolic syndrome (MS) among inpatients with schizophrenia, and to compare with general population. Methods : Nine hundreds seventy inpatients were included to assess the prevalence of MS from two mental hospitals in Yong-in city. Assessment of MS was performed based on modified American Heart Association criteria with abdominal obesity threshold of 90cm for men, 85cm for women. Comparative analysis was performed using age- and gender matched sample from Korea Health and Nutritional Examination Survey III. For evaluating risk factors, analysis included 766 inpatients taking consistent medications at least for 30 days. Results : The prevalence of MS of inpatients with schizophrenia was 26.2%, in females it was 33.2%, compared to 20.2% in males(p < 0.001). Compared to general population, the prevalence of MS was significantly lower in male and higher in female patients. In terms of criteria prevalence, who met abdominal circumference criterion were more prevalent in patient group(p < 0.001). After multivariate analysis, female gender and old age remained as risk factors of MS. Conclusions : The prevalence of MS of inpatients with schizophrenia was 26.2% and significantly low compared to general population. Female gender and old age were risk factors of MS.

  • Recently, Korea has undergone rapid social and economic development, and with that came various changes in living habits and disease patterns; the nation, accordingly, has been alerted to the associated risks of chronic ailments. As risk factors of metabolic syndrome commonly include causal factors of chronic diseases, prevention is important. The prevalence of metabolic syndrome in Korean adults is 22.4%, which comprises a large portion of the whole disease (Korean National Health and Nutrition Examination Survey, 2013). This study aims to examine the elements of healthy living practices by analyzing the risk factors of metabolic syndrome and health behaviors, and provide basic materials to contribute to the activation of health promotion. Among 12,481 subjects of the source data from the 2014 community health survey, which was performed on adults aged 19 and over in Jeonlabuk-do, 12,185 people were analyzed in this study using SPSS 23.0. There were significant differences with respect to sex, smoking status, age, education level, and number of days of physical activity among the results of the difference of the risk factors of the metabolic syndrome according to the characteristics of the health behavior of the subjects. Of the risk factors for metabolic syndrome, this study emphasized the necessity of strategies to promote health programs regarding the management of drinking, smoking, physical activities, depression, subjective health condition and quality of life to the group of those with dyslipidemia, hypertension, and diabetes.
