Hybrid Procedure for Aortic Arch Aneurysm: Arch Vessels Debranching andThoracic Endovascular Aortic Repair? -2 case Reports- (대동맥궁 대동맥류에서 Hybrid 술식의 적용 - 2예 보고 -)

  • Conventional surgery for aortic arch aneurysms have many postoperative complications and a high mortality rate due to prolonged cardiopulmonary bypass time, especially in high risk patients. In this report, we present two cases of a hybrid procedure that involves open brachiocephalic bypass with concomitant endovascular arch stent grafting in high risk patients with distal aortic arch aneurysm.

  • Subvalvular aortic stenosis developed after patch closure of perimembranous'VSD is rarely reported. A 18-month-old, 8 kg child with this complication after VSD closure 8 months ago in other hospital has been treated medically and was admitted to this hospital because of severe cardiomegaly and sign of heart failure. Cardiac catheterization revealed 55 mmHg of pressure gradient between aorta and LV cavity. We report one successful redo case of surgically relieved subvalvular aortic stenosis in a child after patch closure of perimembranous VSD.

  • Penetrating atherosclerotic ulcer involving the aortic arch has rarely been reported on in the literature. Acute rupture of a penetrating atherosclerotic ulcer involving the distal arch and the proximal descending thoracic aorta was found in a 78-year-old male, and he originally presented with acute-onset shoulder pain and hoarseness. Patch repair of the perforated arch and the proximal descending thoracic aorta was successfully done under total circulatory arrest.

Ages of the Thrust Tectonics of Mungyeong Area ; Insight from Field Relationships (문경지역 야외조사자료에 의한 드러스트의 발달시기)

  • Field relationships indicate that the Jumchon granite intruded the Pyungan Supergroup but the Daedong Supergroup overlies Jumchon granite nonconformably. This relationship suggests that the Jumchon granite intruded after the sedimentation of the Pyungan Supergroup (at Late Permian or younger), but before the sedimentation of the Daedong Supergroup (at Early Triassic). The Jumchon granite intruded thrusts within Pyungan Supergroup indicating that the thrust event occurred after the sedimentation of the Pyungan Supergroup but before the intrusion of Jumchon granite. This justifies a narrow age bracket of the first thrusting event of the Mungyeong area, from Late Permian to Early Triassic. In other localities, rocks of the Daedong Supergroup override the rocks of Pyungan Supergroup by thrusts, indicating that another thrust event occurred after sedimentation of the Daedong Supergroup (after Early Devonian).

  • Pedicled omentoplasty is effective in thoracic surgery, but it is associated with several postoperative complications. A case of diaphragmatic hernia as a complication of pedicled omentoplasty in a 65-year-old male is reported. Because aortoesophageal fistula occurred three months after the patch aortoplasty for mycotic aneurysm of descending thoracic aorta, he underwent ascending thoracic aorta to abdominal aorta bypass surgery with resection of thoracic aortic aneurysm and esophagorrhaphy with wrapping of the esophageal suture line and the stumps of aorta with pedicled omental flap. Three years after the operation, herniation of the stomach developed. The pedicled omental flap was ligated and divided, and the diaphragm defect was repaired.

  • Traumatic aneurysm of both the thoracic aorta and the left ventricle are extremely rare in children because it is characterized by high mortality. We report a case which we experienced recently with sucessful outcome. A Five-year-old boy had a blunt trauma by bongo bus. He had pulmonary hemorrage and pericardial effusion complicated by multiorgan failure threatening his life. Aneurysm of LV and Descending aorta were showed by 2-D echocardiogram and MRI. The atient underwent successful corrective surgery 2 and half momths after trauma, the Postoperative status of this patient was uneventful, now he is being followed up the OPD.

A Comparative Study of Place Names on "Daedongyeo-jido" and "Cheonggu-do" (목판본 "대동여지도"와 "청구도"의 지명 비교 연구)

  • This study is to comparatively analyse the place names on "Cheonggu-do"("靑邱圖", manuscript edition, 1834) and "Daedongyeo-jido"("大東輿地圖", woodblock-edition, 1861). Though the same person-Kim, Jungho(金正浩)- published those maps, and the latter was based on the former map, place names on thos maps are very different. The hypothesis is that those different information on maps were not from the cartographic, but from the social aspects at that times. The whole township names on "Cheonggu-do" were erased in "Daedongyeo-jido". The other names such as mountain, river, port, castle, beacons, official waer houses were recorded instead of township. Especially military names which were recorded newly in "Daedongyeo-jido", such as castle, beacon, ranches for the defense of nation, was geographical distributed in a strategic and key point in the northern and coastal area. This shows that there were rules for the mapping of "Daedongyeo-jido" and those rules could be come from the social demand.

  • As the average age of the general population increases, a growing number of elderly patients are presenting for cardiac operations. Although aortic valve replacement in patients aged 80 years and older has been shown to have excellent outcomes with good long-term survival rates, some physicians are still hesitant to refer elderly patients for surgical intervention. A 95-years old female was admitted to our hospital with cardiogenic shock and an emergency operation was required. She was successfully treated with emergency aortic valve replacement. We report here on a case of successful emergency surgical treatment for aortic stenosis in a 95 years old woman.

  • Difficulty in exposing anastomotic sites is a frequently encountered problem during surgical repair of a distal aortic arch aneurysm via median sternotomy or lateral thoracotomy. Endovascular repair has th limitation that it usually requires surgical rerouting of some of the brachiocephalic branches in order to get sufficient length for proximal fixation of the stent-graft. To take advantage of each approach, we fixed the distal end of the prosthetic graft by means of a pre-mounted metallic stent instead of performing conventional surgical anastomosis during the repair of distal arch aneurysms with using median sternotomy and hypothermic circulatory arrest. We report here on our experience with such 3 patients.

Successful Treatment of Lower Extremity Malperfusion Caused by Acute Aortic Dissection (급성 대동맥 박리증에 의한 하지 관류부전의 치료)

  • Organ malperfusion may mask the presence of aortic dissection and is one of major cause of death in patient with aortic dissection. Several key mechanism appear to contribute to the development of malperfusion, therefore optimal choosing of treatment is necessary to obtain better result. In cases of extremity malperfusion, open bypass procedures have been used for primary treatment, but noninvasive interventional procedures are also recommended as good alternatives in some cases. Here in, we report a case of successful aortic replacement followed by stent insertion in patient with extremity malperfusion caused by acute aortic dissection.
