Complications and Perinatal Factors According to the Birth Weight Groups in the Infants of Diabetic Mothers (당뇨병 산모아에서 출생 체중군에 따른 합병증 및 주산기 인자)

  • Purpose : This study was performed to compare complications and perinatal factors according to the birth weight groups in the infants of diabetic mothers(IDM). Methods : Three hundred and one singleton diabetic mothers and their babies of more than 30 weeks' gestational age admitted in the department of Pediatrics, Chonnam University Hospital from January 1996 to March 2002 were enrolled. Complications and perinatal factors were compared between large for gestational age(LGA) and appropriated for gestational age(AGA) infants. Results : Hypomagnesemia was observed in 37.5%, jaundice in 21.3%, hypoglycemia in 11.1%, hypocalcemia in 7.0%, and birth injury in 19.6%. Congenital anomaly was noted in 24.9% with cardiovascular anomaly most commonly. In the LGA group, the frequencies of jaundice, hypoglycemia, tachypnea, and birth injuries were higher, and the interventricular septum was thicker than the AGA group. In the LGA group, Cesarean section rate, maternal height, weight before pregnancy, weight gain during pregnancy, and the incidence of unawareness of gestational DM were significant compared with the AGA group. Conclusion : In the LGA group, the frequencies of jaundice, hypoglycemia, tachypnea, and birth injuries were higher, and the interventricular septum was thicker than the AGA group. In the LGA group, maternal height, weight before pregnancy and weight gain during pregnancy were larger, and the incidence of unawareness of gestational DM was higher compared with the AGA group. These results suggest that careful examination and management are needed to detect the high risk, pregnant DM mothers with possible LGA babies.

Effects and adverse-effects of growth hormone therapy in children with Prader-Willi syndrome: A two year study (Prader-Willi 증후군 소아에서 성장호르몬 치료의 효과와 부작용)

  • Purpose : The objective of this study was to evaluate the effects and adverse side-effects of growth hormone (GH) therapy in children with Prader-Willi syndrome (PWS). Methods : Forty-one patients who had been treated with GH for more than two years (24 boys and 17 girls, mean age $7.3{\pm}3.3$ years during treatment initiation) were enrolled for this study. Results : After 2 years of GH therapy, the height and weight standard deviation scores (SDS) increased significantly ($-1.19{\pm}1.37$ vs. $-0.02{\pm}1.45$, and $1.02{\pm}2.42$ vs. $1.63{\pm}2.22$, P<0.002); however the percentage body fat decreased ($44.6{\pm}9.9%$ vs. $38.1{\pm}10.5%$, P<0.001). Further, no change was observed in the thyroid and serum glucose levels, but the total cholesterol level decreased. GH therapy did not impact glucose control in the patients with diabetes. The most common adverse effects of GH therapy were the progression of scoliosis and adenoid hypertrophy. Conclusion : GH therapy improved the height SDS and body composition in patients with PWS. However, GH should be used with caution in patients with scoliosis and adenoid hypertrophy.

  • Magnetization Transfer (MT) imaging generates contrast dependent on the phenomenon of magnetization exchange between free water proton and restricted proton in macromolecules. In biological materials in knee, MT or cross-relaxation is commonly modeled using two spin pools identified by their different T2 relaxation times. Two models for cross-relaxation emphasize the role of proton chemical exchange between protons of water and exchangeable protons on macromolecules, as well as through dipole-dipole interaction between the water and macromolecule protons. The most essential tool in medical image manipulation is the ability to adjust the contrast and intensity. Thus, it is desirable to adjust the contrast and intensity of an image interactively in the real time. The proton density (PD) and T2-weighted SE MR images allow the depiction of knee structures and can demonstrate defects and gross morphologic changes. The PD- and T2-weighted images also show the cartilage internal pathology due to the more intermediate signal of the knee joint in these sequences. Suppression of fat extends the dynamic range of tissue contrast, removes chemical shift artifacts, and decreases motion-related ghost artifacts. Like fat saturation, phase sensitive methods are also based on the difference in precession frequencies of water and fat. In this study, phase sensitive methods look at the phase difference that is accumulated in time as a result of Larmor frequency differences rather than using this difference directly. Although how MT work was given with clinical evidence that leads to quantitative model for MT in tissues, the mathematical formalism used to describe the MT effect applies to explaining to evaluate knee disorder, such as anterior cruciate ligament (ACL) tear and meniscal tear. Calculation of the effect of the effect of the MT saturation is given in the magnetization transfer ratio (MTR) which is a quantitative measure of the relative decrease in signal intensity due to the MT pulse.

Quality Characteristics of Brown Rice Makgeolli Produced under Differing Conditions (발효조건을 달리하여 제조한 현미 막걸리의 품질특성)

  • In this study we investigated the possibility of preparing brewed brown rice makgeolli, a traditional Korean rice wine, under diverse conditions. For this purpose the physicochemical characteristics of makgeolli brewed at different temperatures, utilizing a variety of nuruks, the traditional Korean fermentation agent, were studied. The alcohol content was seen to be highest when brewing occurred at $30^{\circ}C$, with the nuruk TN producing 16.2%. At $20^{\circ}C$TN produced 14.1% alcohol content. The alcohol content was therefore higher, by about 2%, for $30^{\circ}C$ fermentations than $20^{\circ}C$ fermentations. Similarly, saccharifying activity was influenced by temperature and sugar content, with a higher activity seen at $30^{\circ}C$ than at $20^{\circ}C$. As the fermentations progressed acidification petered out, with titratable acidity being 0.50-0.67% in all end samples. On the Hunter L, a, b scale; the a value decreased slightly, while the b value increased steadily during the fermentation process. Measurements of total organic acids were highest at $30^{\circ}C$, with the nuruk AK, at about 550 mg%. The content of citric acid was the highest at $30^{\circ}C$, being 230-310 mg% in all samples. However, more lactic acid was detected at $20^{\circ}C$ than at $30^{\circ}C$. Total free amino acid was highest at $30^{\circ}C$, with TN at $8,605{\mu}g/ml$, AK at $6,083{\mu}g/ml$, and RJ at $2,381{\mu}g/ml$. Total free amino acid and essential amino acid was shown to be higher at $30^{\circ}C$ than at $20^{\circ}C$. The bioactive substance ${\gamma}$-aminobutyric acid was also higher at $30^{\circ}C$, with TN at $223{\mu}g/ml$. From all of these results, we surmise that brown rice makgeolli manufacturing conditions are optimal at $30^{\circ}C$ fermentation temperatures and using the nuruk TN for brewing vinegar. In addition, the nuruk used clearly affects the quality of brown rice makgeolli and an appropriate method to determine the best nuruk for various purposes should be pursued.

A study of the development of macrovascular complications and factors related to these complications in young adults with childhood/adolescence-onset type 1 diabetes mellitus (소아청소년기에 1형 당뇨병으로 진단된 젊은 성인에서 대혈관 합병증의 발생과 이에 연관된 인자에 대한 연구)

  • Purpose : Macrovascular complications are the main cause of mortality in type 1 diabetes mellitus (T1DM). The purpose of this study was to clarify the presence of early vascular changes and to assess the risk factors of macrovascular complications in young adults with T1DM diagnosed in childhood and adolescence. Methods : Seventy-two patients ($23.9{\pm}2.4$ years) with T1DM diagnosed before 18 years of age and twenty normal controls were included. The incidence of hypertension, dyslipidemia, and other risk factors of macrovascular complication were reviewed. Flow-mediated vasodilation (FMD) and mean intima-media thickness (IMT) measured by ultrasound were compared between patients and control subjects, and their correlations with macrovascular risk factors were analyzed. Results : Of the 72 patients, 32 (44.4%) had hypertension. The proportions of maleness (P=0.03) and mean body mass index (P=0.04) were higher in the hypertensive patients than in normotensive patients. Thirty-one (N=69, 44.9%) patients had dyslipidemia and LDL-cholesterol was positively correlated with mean HbA1c (r=0.32, P=0.008) and total daily insulin dose (r=0.27, P=0.02). The mean IMT was significantly higher in patients than in control subjects ($0.43{\pm}0.06$ mm vs $0.39{\pm}0.06$ mm, P=0.03). There was no difference in the value of FMD between patients and controls, but the duration of the disease after pubertal onset was negatively correlated with FMD (r=-0.34, P=0.01). Conclusion : Hypertension, dyslipidemia and atherosclerotic vascular change were observed in young adults with T1DM diagnosed during childhood and adolescence; this strongly suggests that meticulous screening of macrovascular complications and control of their risk factors should be conducted.

Effects of a low glycemic load diet on body weight loss in overweight or obese young adults (식단의 당부하량에 따른 20대 성인의 체중 감량 효과 연구)

  • Purpose: This study compared the effects of a high glycemic load (high GL) diet and low glycemic load (low GL) diet on the body weight, body fat, blood pressure, and blood lipid indicators. Methods: Twenty-one young adults aged between 21 and 28 years who were overweighted or obese (body mass index [BMI] between 23 and 33.5 kg/㎡) before the study and after calorie reduction diets with either low GL or high GL for 2 weeks each were examined. The study was a randomized crossover design with a 2-week washout period between the 2 types of diet. The order of the low GL and high GL diet periods was randomized. The body weight, body fat, blood pressure, levels of blood lipids, fasting glucose, insulin, homeostatic model assessment (HOMA) insulin, C-peptide, and HOMA C-peptide were measured at the baseline, as well as 2, 4, and 6 weeks after starting the experiment. Results: When subjects were on the low GL diet, they lost more weight than those eating the high GL diet (mean ± SD, -2.77 ± 1.09 vs. -1.56 ± 0.78 kg; p < 0.001); there were greater decreases in body fat mass (-1.62 ± 1.19 vs. -0.88 ± 0.91 kg; p = 0.024) and BMI (-0.95 ± 0.32 vs. -0.56 ± 1.08 kg/㎡; p < 0.001). On the other hand, there were no significant differences in changes in biochemical parameters, such as blood lipids and fasting glucose levels, and blood pressure. The body weight, body fat mass, BMI, percent body fat, blood pressure, cholesterol (total, low-density lipoprotein, and high-density lipoprotein), fasting glucose, C-peptide, HOMA-insulin resistance-C-peptide levels were decreased significantly at 6 weeks. Conclusion: The low GL diet may be more effective in losing body weight, body fat mass, and BMI than the high GL diet for 2 weeks in healthy young overweight or obese adults.

Differences in serum ferritin and vitamin D levels of Korean women with obesity and severe obesity (비만과 고도비만 한국 여성의 혈청 페리틴과 비타민 D 수준의 비교)

  • Purpose: This study examined the relationships among serum ferritin, vitamin D, folate, iron, and vitamin B12 as indicators of obesity. The results provide the basic data for the prevention and treatment of obese and severely obese people. Methods: This study selected 44 people from 18 years of age or older to 59 years. This study used the indicators of the body mass index (BMI) to analyze obesity as the obesity group (BMI of 25.0-29.9 kg/㎡) and as the severe obesity group (BMI ≥ 30.0 kg/㎡). Of the 44 subjects, 23 and 21 subjects were in the obesity and severe obesity groups, respectively. Their height, weight, body fat, skeletal muscle mass measured using bioimpedance analysis, and measured serum nutrients and biochemical parameters. Results: The obesity group showed a significantly lower age, body weight, BMI and body composition, body fat mass, and body fat percentage, and the height was significantly lower in the severe obesity group. The results of the biochemical parameters of the subjects showed that the levels of aspartate transaminase, alanine transaminase, hemoglobin A1c, total cholesterol, and triglyceride were within the normal range, and there was no significant difference between the 2 groups. The levels of folate, vitamin B12, 25-hydroxyvitamin D3, iron, and ferritin were almost normal, and there was no significant difference between the 2 groups. Conclusion: This study revealed an association with the serum nutrients and obesity, but there was no difference between the obesity group and severe obesity group. Observations of the nutrient levels in not only the blood in obesity and severe obesity but also in red blood cells and tissues will be necessary.

Effects of Combined Exercise on Irisin, Body Composition and Glucose Metabolism in Obese Elderly Women with Type 2 Diabetes Mellitus (복합운동이 제2형 당뇨병 비만 여성노인의 Irisin, 신체조성 및 당 대사에 미치는 영향)

  • The purpose of this study was to investigate the effects of combined exercise on irisin, body composition and glucose metabolism in obese elderly women with type 2 diabetes mellitus. The subjects were thirty-six obese elderly women with type 2 diabetes volunteers, aged 65 to 85 years, composed of the combined exercise type 2 diabetes mellitus group (n=20) and non-exercise type 2 diabetes mellitus group (n=16). The 60 minute combined exercise program (outdoor walking exercise & elastic-band exercise) was performed 3 times per week for 12 weeks. Exercise intensity of outdoor walking exercise was performed as medium intensity (RPE 5~6) and elastic-band exercise was progressively increased every four weeks (1-4 weeks: OMNI-RES 3~4, 5-8 weeks: OMNI-RES 5~6, 9-12 weeks: OMNI-RES 7~8). The results of the study in the combined exercise type 2 diabetes mellitus group were as follows; Irisin and skeletal muscle mass had significantly increased (p<.001), percentage of body fat had significantly decreased (p<.001). Further, HbA1c (p=.020) and fasting glucose (p<.001) was significantly decreased, and HOMA-β was significantly increased (p<.001). Correlation results showed that change of irisin had a significant negative correlation between percentage of body fat mass (r=-.423, p=.010), HbA1c (r=-.351, p=.036) and fasting glucose (r=-.424, p=.010). Also, irisin changes showed a positive correlation with aerobic endurance (r=.355, p=.034) and HOMA-β (r=.411, p=.013). In conclusion, the practice of regular combined exercise was found to increase the level of irisin in elderly women with type 2 diabetes and have a positive effect on body composition changes. In addition, HbA1c, fasting glucose and insulin secretion was improved, which helped to regulate glucose metabolism. Walking exercise and elastic band exercise are recommended as effective exercise for the prevention and management of diabetes in obese elderly women with type 2 diabetes mellitus.

Fungal Microflora on Korean Home-made Meju (재래식(在來式) 메주에 분포(分布)하고 있는 진균(眞菌)에 관한 조사(調査) 연구(硏究))

  • For this study 3 samples of the home-made meju and 3 samples of the improved meju were selected according to their characteristics. A total of 25 strains of true fungi were isolated from the samples of the home-made meju and identified by the Key of Alexopoulos and Raper, using a strain Aspergillus oryzae A-113 as a control. Amylolytic and proteolytic enzyme activities of the isolated strains were investigated ana the results obtained were as follows. 1. The 25 strains from the home-made meju were identified into 2 Aspergillus oryzae, 14 Asp. flavus, 6 Penicillum spp. 1 Candida sp 1 Spicaria sp and 1 Rhizopus sp. 2. The 3 strains from the improved meju were all identified as Aspergillus oryzae. 3. Aspergillus flavus, A-B, from the home-made meju was found to he the strongest strain in ${\alpha}-amylase$ activity and also to be similar to the strains of Aspergillus orzae from improved meju. 4. Aspergillus flavus, A-7, from the home-made meju was found to be the strogest strain in ${\beta}-amylase$ activity and stronger than that from the improved meju. 5. Aspergillus flavus B-3, was found to be the strongest strain in protease activity and stronger than that from the improved meju. 6. Some of the strains from the home-made meju turned out to be harmful strains, such as Penicillium spp. which secrete antibiotics, Asp. flavus which secretes mycotoxin, Candida sp which causes skin diseases, Spicaria sp. which is a insect pathogen. 7. Rhizopus sp was also found but it has not been proved to be harmful.

Preparation of Nanoporous Activated Carbon with Sulfuric Acid Lignin and Its Application as a Biosorbent (황산 가수분해 잔사 리그닌을 이용한 나노 세공 활성탄 제조 및 친환경 흡착제로의 활용 가능성 평가)

  • In this study, catalytic activation using sulfuric acid lignin (SAL), the condensed solid by-product from saccharification process, with potassium hydroxide at $750^{\circ}C$ for 1 h in order to investigate its potential to nanoporous carbon In this study, catalytic activation using sulfuric acid lignin (SAL), the condensed solid by-product from saccharification process, with potassium hydroxide at $750^{\circ}C$ for 1 h in order to investigate its potential to nanoporous carbon material. Comparison study was also conducted by production of activated carbon from coconut shell (CCNS), Pinus, and Avicel, and each activated carbon was characterized by chemical composition, Raman spectroscopy, SEM analysis, and BET analysis. The amount of solid residue after thermogravimetric analysis of biomass samples at the final temperature of $750^{\circ}C$ was SAL > CCNS > Pinus > Avicel, which was the same as the order of activated carbon yields after catalytic activation. Specifically, SAL-derived activated carbon showed the highest value of carbon content (91.0%) and $I_d/I_g$ peak ratio (4.2), indicating that amorphous large aromatic structure layer was formed with high carbon fixation. In addition, the largest changes was observed in SAL with the maximum BET specific surface area and pore volume of $2341m^2/g$ and $1.270cm^3/g$, respectively. Furthermore, the adsorption test for three kinds of organic pollutants (phenol, 2,4-Dichlorophenoxyacetic acid, and carbofuran) were conducted, and an excellent adsorption capacity more than 90 mg/g for all activated carbon was determined using 100 ppm of the standard solution. Therefore, SAL, a condensed structure, can be used not only as a nanoporous carbon material with high specific surface area but also as a biosorbent applied to a carbon filter for remediation of organic pollutants in future.
