  • 올해 대단했던 겨울이 이제야 그 기세가 한풀 꺾이고 기온도 평년 기온을 회복하면서 봄을 알리는 3월이 왔다. 하지만 겨울이 물러나고 봄이 되는 환절기에 접어들면서 지병을 앓는 노인들이 숨지는 사례가 늘고 있다. 환절기에는 노인들의 자연 돌연사가 잦아져 주변에서 관심을 더욱 기울여야 한다고 전문가들은 조언한다.

Analysis of considerations in oral health education for elderly diabetic patients: Focus group interview (당뇨병 노인환자의 구강보건교육 시 고려사항 분석: 포커스 그룹 인터뷰)

  • The purpose of this study is to identify considerations for oral health and oral health education through group interviews with the general public and those diagnosed with diabetes among the elderly over 65 years of age. The subjects of this study were divided into two groups: 5 healthy people and 5 diabetic patients over 65 who visited the Senior Welfare Center in G city, and each group had a 60-70 minute interview. As a result of the study, four key themes were derived: 'Knowledge on the relationship between diabetes and oral disease', 'Oral health status', 'Oral health management method', and 'Considerations for oral health education'. Healthy people who participated in this study answered that the contents they thought necessary for oral health education were periodontal management method, implant, holistic oral care, and systemic health, and diabetic patients answered that diabetes, periodontal management method, and importance of practicing oral care. Based on the results of this study, it is considered that it is necessary to develop a customized oral health education program for diabetic elderly patients in the future.

The Relationship between HbA1c Control and Diabetes Self-care Knowledge, Competence, Behavior and Quality of Life on Diabetes elderly (노인 당뇨환자에서 당뇨 자가관리 지식, 자신감, 행위 및 삶의 질과 당화혈색소 조절의 관련성)

  • This study was conducted to identify the diabetes self-care (DSM) knowledge, DSM competence, DSM behavior, and quality of life according to HbA1c control in diabetic elderly. The participants were 205 diabetes elderly who visited the citizen health promotion center located in D city, Korea. Data were collected from April, 4 to August 31 2015, and were analyzed by the t-test and chi-squared test using IBM SPSS 23.0. The mean scores of DSM knowledge, DSM competence, DSM behavior and quality of life were $50.61{\pm}16.39$, $71.27{\pm}10.21$, $62.78{\pm}1.29$ and $0.86{\pm}0.11$, respectively. Additionally, DSM behavior (t=2.17, p=0.031), education level (t=11.80, p=0.0019) l, BMI (t=0.001, p=0.012), and number of visits to citizen health center (t=16.497, p=0.001) differed significantly between the controlled HbA1c group and uncontrolled HbA1C group. However, ANCOVA revealed that the level of HbA1c did not affect the DSM behaviors. Therefore, it is necessary to develop and apply a DSM education program that reflects the characteristics knowledge level of the elderly. In addition, healthcare institutions with high accessibility in terms of distance and cost to guide and manage desirable diabetic self-care behaviors should be provided everywhere.

The Analysis of Self-care Knowledge, Competence, Behavior, and Quality of Life in Community Diabetes Elderly (지역사회 당뇨노인의 당뇨 자가관리 지식, 자신감, 행위 및 삶의 질 분석)

  • This study was conducted to identify the level of diabetes self-care knowledge, confidence, behavior and quality of life in elderly people with diabetes. The participants were 205 elderly with diabetes who visited a citizen health center located in D city. Korea. The data were collected from April, 20th to August 31th, 2015, and were analyzed by the t-test and ANOVA. The results were as follows; 1) The mean scores for diabetes self-care knowledge, confidence, behavior, and quality of life were 6.99 (2.17), 71.27 (10.21), 62.78 (1.29) and 0.86, respectively. 2) There were significant differences in the self-care behavior according to the education level, duration of diabetes mellitus and subjective health, and the quality of life differeddepending on the level of subjective health(F=35.253, p=0.000). 3) Knowledge of diabetes diet was the lowest among the various aspects of diabetes self-care knowledge. Confidence for exercise was lowest, while confidence for medication and blood sugar check was highest. However, the participants exhibited good medication behavior, whereasthey did not carry out blood glucose checks in their self-care management behavior. The most uncomfortable aspect of the quality of life was found to be pain and discomfort. Based on the results of this study, it is necessary to develop guidelines and methods for dietary education reflecting the dietary habits of Koreans, and to investigatethe reasonsfor thelow performance of theblood glucose check. In addition, customized nursing interventionstaking into consideration the characteristics of elderly with diabetes need to be developed and verified.

Convergence Study on Diabetes Self-Management Knowledge, Social Support, Educational Needs of the diabetes in rural elderly (농촌 지역 노인 당뇨병 환자의 자기관리 지식, 사회적 지지, 교육요구도에 관한 융합연구)

  • The purpose of this study was to identify convergence factors related to DSMK, social support, and educational needs on the diabetes in rural elderly. The subjects were 115 elderly patients who were registered in diabetes control center, taking medication more than 6 months in U city. The survey period was from August 24 to October 31, 2016. The results showed that DSMK showed significant differences in gender, education level, duration of diabetes, experience of diabetes education, experience of family participation in diabetes education, and perceived health status(p<.05). The degree of social support was found to be significant differences in marital status, diabetes education experience, diabetes education needs, gender, duration of diabetes, and perception of diabetes self-management difficulty(p<.05). There were significant positive correlations between DSMK and social support, social support and educational needs(p<.05). Based on these results, we hope to develop a convergent self - management program based on the needs of the elderly patients.

The Association between Personal Health Behaviors and Emergency Room Visit among Elderly Patients with Diabetes (당뇨병 노인의 건강행위 실천과 응급실 내원 간의 관련성)

  • This study was to understand the relationship between emergency room visit due to illness and personal health behaviors as well as population and illness characteristics of elderly patients with diabetes. This study was a descriptive correlation study that analyzed 956 respondents aged 65 or older and diagnosed with diabetes in the Korean Health Panel's data from 2014 to 2017. Data were analyzed using the SPSS 26.0 program with Mann-Whitney U test, chi-square test, and logistic regression analysis. The respondents who visited emergency room had fewer days of moderate-intensity physical activity and walking activity, were older, had activity restrictions due to disability, had depression, and had a large number of comorbid chronic diseases. In order to prevent elderly people with diabetes from visiting emergency room, it is important to encourage moderate-intensity activity and walking.

Convergence Research on Oral Care and Subjective Oral Health of Elderly with Diabetes: Focus Group Interview (노인 당뇨병 환자의 구강관리 및 주관적 구강건강에 관한 융합연구: 포커스그룹 면담)

  • This study was intended to prepare the basic data required for the development of oral products for the patient with diabetes by analyzing the relation between diabetes and oral health and the actual condition of oral care from the senior citizens diagnosed with diabetes out of the senior citizens over 65. In this study, 8 senior citizens who visited the senior welfare center in D city, were organized into the group of 4 senior citizens and the interview approximately for 40 minutes per group was conducted. In the results of this study, 5 core topics of 'Diabetes and Oral Health", 'Oral Discomfort', 'Oral Care Method', 'Visit to Dentist' and 'Demand for Oral Care' were drawn. For the relation between diabetes and the oral health, the most of research subjects answered that they heard about it for the first time, They answered that for the oral health care, they brushed the teeth using toothpaste 3 ~ 4 times a day and for the visit to dentist, they visited the dentist only when they had oral problem. For what is necessary for oral care, they answered that they wish that the oral care education customized for patient with diabetes and diverse oral care products such as toothpaste, etc. would be developed. Considering the results of this study, the development of oral care management program and the development of oral products in future are deemed to be necessary.
