Influential Factors on Fatigue in Patients with Type 2 Diabetes Mellitus (제 2형 당뇨병 환자의 피로와 영향요인)

  • This descriptive study was performed to identify the factors that influence fatigue in patients with type 2 diabetes mellitus. A total of 117 subjects were recruited from a university hospital in J city. For data collection, a questionnaire survey was performed between May 1 and June 28 2013. Data were analyzed by descriptive statistics, t-test, one way ANOVA, Pearson Correlation, and multiple regression analysis using the SPSS WIN 15.0 package. The mean fatigue scores of the subjects was 2.80. Depression, age, and time since the diagnosis were the variables influencing fatigue. The total explanatory power of these factors on fatigue was 40%. In conclusion, it is essential to develop a fatigue control nursing program considering depression, age and time since the diagnosis for patients with type 2 Diabetes mellitus.

Factors Influencing Use of Medical Alert Identification in Diabetic Patients Using Insulin (인슐린을 사용하는 당뇨병 환자의 당뇨인식표에 대한 인식, 소지실태 및 소지의 영향 요인)

  • Purpose: The aim of this study was to identify perception, use status and factors influencing use of medical alert identification (ID) in diabetic patients who are on insulin. Methods: A survey was conducted from December 2013 to April 2014 with 198 people who were asked to participate in the survey. Results: The medical alert ID perception score was 3.54 on a 5-point scale. Most of respondents (82.8%) knew about the medical alert ID, but only 23.2% of respondents wore a medical alert ID and only 43.5% of respondents always wore a medical alert ID. Perception and frequency of possession were positively correlated (r=.41, p<.001). The most influential factor for the possession of a medical alert ID was the perception of the medical alert ID (${\beta}=.41$, p<.001). The perception of the medical alert ID explained 17% of variance in possession of a medical alert ID. Conclusion: Most of diabetic patients who take insulin received education about hypoglycemia and knew about the medical alert ID, but only few of them wore a medical alert ID. Findings from this study indicate that perception is an important variable related to possession of a medical alert ID in this population.

Experiences of Diabetes Education among Educators of Diabetes : a content analysis approach (당뇨병 교육자의 당뇨교육 경험: 내용분석적 접근)

  • Purpose: This study explored experiences of diabetes education among educators of diabetes. Methods: Data were collected from individual in-depth interviews with 10 nurses and 2 dieticians with had at least 3 years of experience in diabetes education. Data collection was conducted between May 2014 and February 2015. All interviews were recorded and stored as digital audio files, which were then transcribed verbatim. Data were analyzed through qualitative content analysis. Results: Analysis showed that four categories could be derived from the data: 1) barriers of diabetes education from an educator's perspective, 2) barriers of diabetes education form a patient's perspective, 3) facilitating factors of diabetes education from an educator's perspective, and 4) facilitating factors of diabetes education from a patient's perspective. Conclusion: This study suggests the necessity to strengthen the policy systems and financial support at a national level to provide diabetes education with higher quality to patients. In addition, it is required to develop various diabetes education programs with consideration to patient characteristics.

Relationship between Sleep Time and Diabetes Diagnosis Experience in Adults (성인의 수면시간과 당뇨병 진단경험군과의 관련성)

  • Sleep is known to be a major factor that reduces the quality of life in the human body.This study aimed to investigate the relationship between sleep and sleep in a group that has been diagnosed with diabetes. This study used raw data from the Community Health Survey and targeted a total of 227,754 people. Data analysis used cross-tabulation and logistic regression analysis. As a result of the analysis, gender, age, diabetes experience, subjective health status, sleep time, smoking, drinking, and blood sugar awareness were different from the diabetes experience group. In the group with diabetes diagnosis experience, the odds ratio for sleep time was 1.4 times higher in the group with 5 hours of sleep than in the group with 6 to 7 hours of sleep. In the future, empirical research may be needed to determine the relationship with sleep time, and this study showed the importance of sleep time. Based on the importance of sleep time, there is a need to develop a health management program that considers the importance of sleep for those who have been diagnosed with diabetes.

  • 당뇨병은 고혈당을 조절 안할 경우에는 돌이킬 수 없는 신체적 합병증은 물론 정신적 손상까지 받지만, 고혈당은 노력만 하면 조절이 가능하며, 정상에 가까울 정도로 조절되면 정상인과 같은 삶을 영위할 수 있다. 그러나 보건의료 전문가의 인슐린 주사 요법, 식사 및 운동요법 등의 일방적인 처방만으로 성공적인 당조절을 기대하기에는 어려움이 있다. 그래서 당뇨병 환자에게 당뇨병이라는 것이 무엇이며, 어떻게 이 병을 스스로 조절할 수 있게 만드느냐가 더욱 중요한 것이다. 따라서 우리나라에서도 당뇨병에 대한 관심이 높아짐에 따라 각 지역과 의료기관마다 다양한 당뇨병 교육 프로그램이 실시되고 있다. 특히 소아 당뇨병 환자는 이러한 전통적인 당뇨병에 관한 교육을 통해서 당뇨병에 관한 지식은 많이 가지고 있지만, 당뇨병 자가 조절을 위한 환자 역할 행위(sick-role behavior)를 변화시키는데는 그다지 성공절이라 할 수 없다. Bandura(1977)는 자기 효능은 ‘할 수 있다는 자신감’으로 수행 성취, 대리 경험, 언어적 설득, 정서적 각성에 관한 정보에 의해 결정된다고 하였다. 따라서 이러한 정보들은 자기 효능을 증진시키기 위한 중재의 방향을 제시해 준다. 오늘날 이러한 정보를 활용한 중재 프로그램 중에서 가정과 학교를 떠나 자연 환경 속에서의 집단 활동을 통하여 사회 학습 경험을 하는 캠프 프로그램에 대한 관심이 높아지고 있다. 캠프에 참여한 당뇨병 아이들은 캠프 활동속에서 다른 동료 아이들이 자신이 갖고 있는 문제들을 성공적으로 해결해 나가는 것을 관찰하여 대리 경험하게 될 때 희망을 가지게 되며, 당뇨병을 가진 다른 동료들과 공통의 경험을 공유할 수 있는 기회를 갖게 되어 그들은 자신이 더 이상 혼자가 아니며 남과 ‘다르지’도 않다는 것을 깨닫게 되어 점차 자신감을 가지고 살아갈 수 있게 된다. 본 연구는 캠프 프로그램이 소아 당뇨병의 자기 효능을 증진시키고, 당뇨병 환자 역할 행위 이행 정도를 높여주는지를 규명해 봄으로써, 소아 당뇨병 환자를 위한 효과적인 간호 중재 방안을 제시하고자 비동등성 대조군 전후 실험 설계의 유사 실험 연구를 시도하였다. 1996년 8월 10일 부터 12월 12일까지 종합병원에서 추후 관리하고 있는 소아 당뇨병 환자중 선정 기준에 맞는 환자 41명을 연구대상으로 하였으며, 그 중 19명은 실험군으로 Bandura의 자기 효능 증진을 위한 정보원을 활용한 캠프 프로그램을 5박 6일간에 걸쳐 실시하였고, 다음 22명은 대조군으로 캠프 프로그램을 실시하지 않았다. 자료수집은 자기 효능 척도와 환자 행위 역할 이행 척도를 캠프 프로그램을 실시하기 전에 사전 조사를 하고 중재 후 4주째 사후 조사를 하였다. 수집된 자료는 SPSS/PC+로 Chi-square test, t-test, ANCOVA, Pearson correlation을 이용하여 분석하였으며, 그 결과는 다음과 같다. 캠프 프로그램은 소아 당뇨병 환자의 자기 효능을 증진시키고 환자 역할 행위 이행을 높여주는데 효과적 이었다. 소아 당뇨병 환자의 자기 효능은 환자 역할 행위 이행과 순 상관 관계가 있어, 자기 효능이 증진될수록 환자 역할 행위 이행 정도가 높아졌다.

Factors Affecting the Control of HbA1c in Type 2 Diabetic Patients (제2형 당뇨환자의 당화혈색소(HbA1c) 수치 조절에 미치는 영향요인)

  • Objects: This study is a descriptive study to identify the factors influencing the glycated hemoglobin control in type 2 diabetes patients. Methods: This study used raw data from the third year of the National Health and Nutrition Examination Survey performed from 2015. The subjects were 285 people who were classified as type 2 diabetes patients among the 7,380 participants of the National Health and Nutrition Examination Survey. Findings: The factors influencing the glycated hemoglobin control in type 2 diabetes patients were found to be age, education level, prevalence period and sleeping time. Improvements: In type 2 diabetes patients, it is very important to have appropriate education on diabetes when they discovered the disease morbidity in the middle age rather than in the aged period and to have enough sleeping time. A practical program to help this education and training should be developed.

The Relationship between HbA1c Control and Diabetes Self-care Knowledge, Competence, Behavior and Quality of Life on Diabetes elderly (노인 당뇨환자에서 당뇨 자가관리 지식, 자신감, 행위 및 삶의 질과 당화혈색소 조절의 관련성)

  • This study was conducted to identify the diabetes self-care (DSM) knowledge, DSM competence, DSM behavior, and quality of life according to HbA1c control in diabetic elderly. The participants were 205 diabetes elderly who visited the citizen health promotion center located in D city, Korea. Data were collected from April, 4 to August 31 2015, and were analyzed by the t-test and chi-squared test using IBM SPSS 23.0. The mean scores of DSM knowledge, DSM competence, DSM behavior and quality of life were $50.61{\pm}16.39$, $71.27{\pm}10.21$, $62.78{\pm}1.29$ and $0.86{\pm}0.11$, respectively. Additionally, DSM behavior (t=2.17, p=0.031), education level (t=11.80, p=0.0019) l, BMI (t=0.001, p=0.012), and number of visits to citizen health center (t=16.497, p=0.001) differed significantly between the controlled HbA1c group and uncontrolled HbA1C group. However, ANCOVA revealed that the level of HbA1c did not affect the DSM behaviors. Therefore, it is necessary to develop and apply a DSM education program that reflects the characteristics knowledge level of the elderly. In addition, healthcare institutions with high accessibility in terms of distance and cost to guide and manage desirable diabetic self-care behaviors should be provided everywhere.

The Mediating Effects of Depression on Loneliness and Suicidal Ideation in Elderly Living Alone with Diabetes Mellitus: A Secondary Data Analysis (독거 당뇨병 노인의 외로움이 자살사고에 미치는 영향에 대한 우울의 매개효과 검증: 2차 자료 분석)

  • The purpose of this study is to analyze suicidal ideation of elderly people with diabetes living alone and to investigate the relationship between suicidal ideation, loneliness, and depression. This study was conducted through secondary data analysis using total survey data for the elderly living alone in O province. The subjects of this study were 466 elderly people who were diagnosed with diabetes among the elderly living alone. The data of this study were analyzed using descriptive statistics, t-test, Pearson's correlation coefficients, multi-linear regression analysis, and the three-step mediating effect verification procedure of Baron & Kenny using the SPSS 26.0 program. The average loneliness score and average depression score of the subjects were 4.52±3.30 and 4.88±4.03, respectively, and 27.3% of the subjects said they had a suicidal ideation. The subject's loneliness (β=.20, p=.005) and depression (β=.30, p<.001) was found to be a significant factor in predicting suicide ideation. In addition, depression was found to be partially mediated in the relationship between loneliness and suicidal ideation. Through the results of this study, loneliness and depression were found to be important factors related to suicidal ideation in the elderly with diabetes living alone. In addition, in order to improve suicidal ideation in the elderly with diabetes living alone, intervention to lower the level of loneliness and depression is necessary.

The Effect of Exercise Program in Korea for Controlling Blood Glucose Levels in elderly women patients undergoing type II diabetes mellitus (제 2형 당뇨병 여성 노인 환자의 혈당조절을 위한 국내 운동 프로그램의 효과분석)

  • The purpose of this study was to investigate the effects of exercise program intervention in Korea for the control of blood sugar levels in elderly women patients undergoing diabetes mellitus typeII and to investigate the effects of exercise program intervention. Research methods were searched through RISS, KISS, KoreaMed, and KMbase as domestic databases and limited to domestic papers published from 2010 to May 2017. Diabetes, female diabetes, and diabetes mellitus typeII were used as the search terms related to the patient group. Programs related to intervention, intervention, exercise, blood glucose, glucose insufficiency, glucose control, fasting glucose and HbA1c were used. A total of 12 papers were provided according to the intervention method. The mediation period was 12 weeks(50%), 3 times per week(58.3%), and 50 to 69 minutes(83.4%) was the most. The combined aerobic and anaerobic exercise was found to be an effective exercise method to control not only blood glucose level but also HbA1c. Individualized exercise program intervention should be provided considering the physical fitness level and health status of elderly patients undergoing diabetes mellitus typeII and should be encouraged to maintain effective blood glucose control through continuous exercise rather than one-off exercise.

The Association between Personal Health Behaviors and Emergency Room Visit among Elderly Patients with Diabetes (당뇨병 노인의 건강행위 실천과 응급실 내원 간의 관련성)

  • This study was to understand the relationship between emergency room visit due to illness and personal health behaviors as well as population and illness characteristics of elderly patients with diabetes. This study was a descriptive correlation study that analyzed 956 respondents aged 65 or older and diagnosed with diabetes in the Korean Health Panel's data from 2014 to 2017. Data were analyzed using the SPSS 26.0 program with Mann-Whitney U test, chi-square test, and logistic regression analysis. The respondents who visited emergency room had fewer days of moderate-intensity physical activity and walking activity, were older, had activity restrictions due to disability, had depression, and had a large number of comorbid chronic diseases. In order to prevent elderly people with diabetes from visiting emergency room, it is important to encourage moderate-intensity activity and walking.
