Carcinoid tumor of gallbladder in a dog (개의 담낭에서 발생한 carcinoid tumor 증례 보고)

  • A 7-year-old female mixed-breed dog was presented to a veterinary clinic for pyometra. During abdominal ultrasonography, an abnormal finding was noted in the gallbladder. A cholecystectomy was performed. Grossly, the gallbladder wall was thickened with a polypoid nodular projection into the lumen. Microscopically, the polyp consisted of a solid sheet of round to polygonal-shaped cells that formed small packets with fibrovascular septa. The neoplastic cells contained abundant cytoplasmic eosinophilic granules. Immunohistochemically, the neoplastic cells were positive to chromogranin A and neuron-specific enolase, whereas they were negative to vimentin and cytokeratin. This case was diagnosed as a primary gallbladder carcinoid tumor.

Management of Asymptomatic Gallstones in Renal Transplantation

  • Background: In solid organ transplantation patients, complications of cholelithiasis may run a fulminant course, resulting in high morbidity and mortality under immunosuppression and may even result in rejection. Here, we reviewed medical records of 66 patients in order to determine the outcome of management approach for asymptomatic gallstones in renal transplantation patients. Methods: We retrospectively reviewed clinical courses of 66 cases of renal transplantation performed between 2000 and 2012 at Kosin University Gospel Hospital. Results: Among 66 cases, eight had gallstones before transplantation. Three of these cases had undergone previous cholecystectomy for symptomatic gallstones, one had a simultaneous laparoscopic cholecystectomy and renal transplantation, and four were observed by regular abdominal ultrasonography. One patient was found to have cholangitis, and endoscopic retrograde biliary drainage was performed, resulting in alleviation of symptoms. Among 58 cases without preoperative gallstones, three developed gallstones after transplantation. One patient had cholecystitis, and the symptoms subsided after conservative treatment. Conclusions: For patients with asymptomatic gallstones who are awaiting renal transplantation, expectant management should be considered.

A Case Report on a Patient Treated with Combined Korean Medicine for Functional Dyspepsia after Cholecystectomy (담낭절제술 후 발생한 기능성 소화불량 환자에 대한 한방복합치료 치험 1례)

  • Objectives: The purpose of this study is to report the case of a patient with functional dyspepsia after cholecystectomy whose discomfort after eating and upper abdominal pain improved with a combined Korean medicine treatment. Methods: A combination of treatments, including acupuncture, cupping, Chuna, and herbal medicine, was provided for 48 days to a patient with functional dyspepsia. Her progress was assessed with NRS, EQ5D, and GIS every 2 weeks, including before and after treatment. Results: The patient made significant improvements in NRS, EQ-5D, and GIS after treatment. No adverse events were observed in the patient. Conclusions: The findings indicate that combined Korean medicine treatment can be a safe and effective alternative to treating functional dyspepsia after cholecystectomy.

  • However, technical advances in ultrasono imaging have had a remarkable impact on the study of biliary system oral cholecystography is a contrast of the gallbladder which is very frequently performed even with the application of Extra Shock Wave Lithotripsy(ESWL) in clinical use. Oral GB requires a stringent preparation if it is to be fully successful and a considerable amount of time to complete all its procedures and its objects of the radiographs. 1) to obtain a firm diagnosis of the presence of gallstones. 2) to essess function of the gallbladder that is, its ability to concentrate and store bile. After a times sequence of X-ray exposures taken in various positions to show the gallbladder to be satisfactorily filled, the patient is given a fatty meal, for instances two eggs or a cup of milk. The gallbladder which is drained by the cystic duct stores and concentrates the bile and is stimulated to contrast and excrete the bile by hormone "cholecystokinin" secreted in the intestinal mucosa. To evaluate the effect of the fatty meal which caused the gallbladder to constrict and empty, and by so doing the contrast medium passes through the cystic and bile ducts which are shown in radiographs exposed from 15-30 minutes after the variety practice of fattymeal, such as soft-boiled 2 eggs, raw 2 eggs, 100g of peanuts, and 200ml of milk. If the concentration of the opaque medium in the gallbladder is adequate, then not only the size, shape and position of the gallbladder will be shown from firms taken at intervals, the rate of concentration of the opaque medium and of the emptying of gallbladder has been measured and analyzed.

Clinical Application of $^{99m}Tc-HIDA$ Cholescintigraphy in the Cholecystitis (담낭염 환자에서의 $^{99m}Tc-HIDA$ Scan의 진단적 의의)

  • Cholescintigraphic studies with $^{99m}Tc-HIDA$(dimethyl iminodiacetic acid) were performed in 22 cases of normal subjects, 21 of acute cholecystitis, 12 of chronic cholecystitis and 12 others, with the results of, 1) In normal control group, liver and intrahepatic biliary tree, CBD and gall bladder, and G-I tract appeared at 10, 20 and 30 minutes after intravenous injection of $^{99m}Tc-HIDA$ respectively, 2) In acute cholecystitis, 20 among 21 cases showed non-visualization of gall bladder with the diagnostic accuracy, sensitivity and specificity of 93.2%, 95.2% and 100% respectively. 3) In chronic cholecystitis, 5 among 12 cases showed non-visualization of gall bladder and remained 7 among 12 cases showed poor contraction of gall bladder (4), delayed visualization of gall bladder (1) and normal findings (2). 4) In the other disease group, the 12 cases which initially suspected as acute cholecystitis, revealed normal scan findings to exclude the cystic duct obstruction easily, With the above results, this scintigraphic procedure was found very rapid, accurate and easily available method for the determining of the cystic duct patency.

  • This study was performed to evaluate diagnostic sensitivity and specificity of sonography on clonorchiasis. During the 9 months from March to November 1994, sonography skin test, stool examination, and medical examination were performed to 609 volunteers of Pohang which is located along the Hyongsan-gang (Rivers and is one of well-known endemic areas of clonorchiasis. The sensitivity of sonography was either 21% if the intrahepatic ducts dilatation was assumed to be the only positive anding or 52% if the periductal echogenicity was also included as one of positive findings. The sensitivity of skin test was 62%. The sensitivity was 46% if the diagnostic criteria were arbitrarily assumed as being positive in skin test and having either sonographic findings. The sonographic finding of fusiform, non-shadowing, weakly echogenic foci in gallbladder or in billiaiy tree indicating the flukes or aggregates of flukes was not obvious in this study. The specificity of sonography was either 95% if the intrahepatic ducts dilatation was assumed to be the only positive finding or 82% if the periductal echogenicity was also included as one of positive findings. The specificity of skin test was 52%. The specificity was 90% if the diagnostic criteria was arbitrarily assumed as being positive in skin test and having either sonographic findings.

  • Background: The dosage of the anesthetic drugs is generally determined by the total body weight of the patients. However, the drugs can be overdosed when the patient is morbidly obese. We have determined anesthetic induction dose based on lean body mass estimated from bioelectrical impedance analysis (BIA). Case: We report a case of morbidly obese patient (161 cm, 138 kg and body mass index 53.1) who had an elective laparoscopic cholecystectomy. The dose of induction agent was determined by lean body mass estimated by BIA, and the sedation was assessed by the observer's assessment alertness/sedation scale. Conclusions: Dose determination through lean body mass measured by BIA is useful in highly obese patients.

Study on Effect of Warmth Therapy Nursing Intervention Program after Laparoscopic Cholecystectomy (복강경담낭절제술 후 가온요법 간호중재 프로그램 적용 효과)

  • Purpose: This study is a non-equilibrium control-like experimental study that attempts to develop a nursing intervention plan and to present basic data by grasping the effect of the heating therapy nursing intervention program on patients entering the recovery room after surgery. Method: The subjects of the study were 31 in an experimental group and 31 in a control group who applied one heating therapy program located in D city. The warming therapy was conducted in a recovery room in the range of 38 to 43℃ through a heater. The general characteristics of the subjects were determined by real number and percentage, and a homogeneity test between the experimental group and the control group was determined by 𝑥2-test and independent t-test. The difference in body temperature, pain, and tremor over time between the two groups was determined by repeated measure ANOVA. Results: First, the results of repeated measurements immediately after surgery showed significant differences in body temperature, pain, and tremor between the two groups. Second, temperature comfort showed a significant difference. Conclusion: The results confirmed that there was a positive effect on the patient in the intervention of warming therapy after surgery.

  • Cholelithiasis, one of the digestive, disease is one of the subject to have to be solved in the medical field because it is very important health problem in clinic and gives people much pain. The formulative process of gall stone is not certainly known yet, but an approach to the size, location, bile sludge of gall ston and the existence of pus is very contributive to the cure and prediction of the disease. As above, the early diagnosis of gall stone is very important. A few study on the possessing rate of gall stone in normal Korean Group is reported. But no report has submitted to the Korean Life Insurance Medical Association until now. We, the Medical Dept. of Dae Han Kyouk Life Insurance Co. Ltd., practiced ultrasonic examination for the recent 17 months and obtained 84 data of gall stone potency out of 4201 examples. We analyzed them on the basis of sex, age, gall stone incidence rate and the results are as follows; 1) The possessing rate of gall stone in female showed 1.22 times higher than in in male. 2) The older the medical examined group grow, the more the frequency of gall stone incidence increase regardless of sex. 3) Our report coincides with foreign one in sex and age, but in total gall stone incident frequency our country has 2%, which is much less than foreign one.

  • The measurement of gastric emptying time is useful in differentiation of the hypochondriac neurosis from the functional gastrointestinal disturbance, and also useful in searching the hidden gastrointestinal disease in patients who are believed as the functional gastrointestinal disturbance. We confirmed in this study that the more delayed gastric emptying time was measured in functional gastrointestinal disturbance compared to normal group, and more shortened gastric emptying time was found after treatment with dopamine antagonist (cleboril) in this group.
