Diagnosis of Obstructive Sleep Apnea Syndrome Using Overnight Oximetry Measurement (혈중산소포화도검사를 이용한 폐쇄성 수면무호흡증의 흡증의 진단)

  • Objectives: The gold standard for diagnosing obstructive sleep apnea syndrome (OSAS) is nocturnal polysomnography (NPSG). This is rather expensive and somewhat inconvenient, however, and consequently simpler and cheaper alternatives to NPSG have been proposed. Oximetry is appealing because of its widespread availability and ease of application. In this study, we have evaluated whether oximetry alone can be used to diagnose or screen OSAS. The diagnostic performance of an analysis algorithm using arterial oxygen saturation ($SaO_2$) base on 'dip index', mean of $SaO_2$, and CT90 (the percentage of time spent at $SaO_2$<90%) was compared with that of NPSG. Methods: Fifty-six patients referred for NPSG to the Division of Sleep Studies at Seoul National University Hospital, were randomly selected. For each patient, NPSG with oximetry was carried out. We obtained three variables from the oximetry data such as the dip index most linearly correlated with respiratory disturbance index (RDI) from NPSG, mean $SaO_2$, and CT90 with diagnosis from NPSG. In each case, sensitivity, specificity and positive and negative predictive values of oximetry data were calculated. Results: Thirty-nine patients out of fifty-six patients were diagnosed as OSAS with NPSG. Mean RDI was 17.5, mean $SaO_2$ was 94.9%, and mean CT90 was 5.1%. The dip index [4%-4sec] was most linearly correlated with RDI (r=0.861). With dip index [4%-4sec]${\geq}2$ as diagnostic criteria, we obtained sensitivity of 0.95, specificity of 0.71, positive predictive value of 0.88, and negative predictive value of 0.86. Using mean $SaO_2{\leq}97%$, we obtained sensitivity of 0.95, specificity of 0.41, positive predictive value of 0.79, and negative predictive value of 0.78. Using $CT90{\geq}5%$, we obtained sensitivity of 0.28, specificity of 1.00, positive predictive value of 1.00, and negative predictive value of 0.38. Conclusions: The dip index [4%-4sec] and mean $SaO_2{\leq}97%$ obtained from nocturnal oximetry data are helpful in diagnosis of OSAS. CT90${\leq}$5% can be also used in excluding OSAS.

Comparison of Positional and Non-Positional Obstructive Sleep Apnea Patients by Nocturnal Polysomnography (야간수면다원검사를 이용한 자세성 및 비자세성 수면무호흡증 환자의 비교 연구)

  • Objectives: The aim of this study was to evaluate the differences in the polysomnography data between positional and non-positional obstructive sleep apnea (OSA) patients. Methods: Forty-seven patients diagnosed with OSA were evaluated using full night polysomnography. According to the criteria of Cartwright et al., the patients were classified into two groups with 37 positional (supine apnea-hypopnea index [AHI] $\geq$ 2x's the lateral AHI) and 10 non-positional (supine AHI < 2x's the lateral AHI) OSA patients, and the differences of polysomnography data between the two groups were evaluated. Results: There were no significant differences in demographic variables (age, gender, and BMI), daytime sleepiness, overall AHI, total arousal index, and percent time of snoring between two groups. However, AHI, arousal index, and mean oxygen saturation ($SpO_2$) of the REM sleep stage were significantly more severe in the positional OSA group than the non-positional OSA group. Mean $SpO_2$ and the lowest $SpO_2$ during overall sleep stage were also significantly lower in the positional OSA group than the non-positional OSA group. Conclusions: Our results of differences in the polysomnography data of REM sleep stage suggest that non-positional OSA patients may have higher collapsibility of the oropharyngeal airway during sleep than positional OSA patients.

  • Changes in political and economic environments require a new concept of 'culture' and a paradigm shift in cultural policies. Especially, broadcasting is needed to construct a productive infra-structure in order to play its role as culture industry in multi-channel environments caused by the progress of digital technology. In addition, Korea-USA FTA Agreement and a subsequently expected open policy of broadcasting market raise issues of a flow of foreign capital and a compatibility of cultural diversities and cultural identities. From this perspective, this study attempts to suggest alternatives for cultural diversity of program contents in new global media environments. More specifically, these alternatives examine the meaning and achievements of co-production of broadcasting programs as an active and direct method to preserve cultural identities and universalities of cultural contents at the same time. Details of this study are as follows: thorough review of internation co-production and program format industries and their possibilities to overcome cultural harriers and to provide local alternatives.

  • The mutual interchange and cooperation giving first consideration to the communication technology and industry makes it possible to advance the day of unification of North and South Korea. For the sake of this, it is necessary to build communication network connecting all parts of Korea synthetically. This paper presents the plan to build the realizable communication system in North Korea. It bases on an objective view and prospect of domestic and foreign conditions and will help to construct the communication inflation. Therefore, consideration of pluralistic, economic and relay aspects must be proceeded and also the least expense of unification must be deliberated.

  • 폐쇄성수면무호흡증(Obstructive sleep apnea syndrome : OSAS)은 신경근육계, 호흡계, 심혈관계의 복합적인 질환으로서 사회적, 생리학적으로 심각한 문제를 발생시킬 수 있는 질병이다. 수술적치료를 결정하기에 앞서 환자의 증상 및 징후, 신체 상태와 습관 등을 정밀검사하고, 기도폐쇄가 일상생활 및 정신건강에 미치는 영향을 고려하여 수술여부를 결정하여야 한다. 폐쇄성수면무호흡증후군의 진단에 유용한 검사인 수면다원검사와 Videofluoroscopy의 검사결과를 바탕으로 수술소견을 비교분석하고자 하였다.

  • The purpose of this article is to depict the relationship between the frames of leadership and the dependent variables such as job satisfaction, respect from subordinates, promotion, and effective attainment of unit objectives. To attain this purpose, four types of multiple leadership frame, such as structural, human resources, political, and symbolic frames were surveyed through questionnaire and analyzed by SPSS. The major findings are : 1) In general, the symbolic frame is an important factor having a stronger influence on job satisfaction, respect from subordinates, promotion, and effective attainment of unit objectives. 2) In details, the human resources frame shows a positive effects on job satisfaction and respect; the structural frame on effective attainment of unit objectives; the political frame, to a certain degree, on promotion; and the symbolic frame, especially on respect from subordinates, promotion, and effective attainment of unit objectives.

Cultural Collage in Men's Fashion (남성 패션에 표현된 문화 꼴라쥬 현상)

  • The purpose of this study is to analyze the meaning and the value of the cultural collage expressed in men's fashion. As for the research methodology, literary research was under taken to study psychoanalytical and socioeconomic contexts in which cultural collage has been formed. In addition, demonstrative studies in men's fashion were undertaken through the analysis of pictures. The cultural collage is defined as a phenomena of making creativity by combining cultures which have no relevance. In psychological and sociological aspects, the rise of the cultural collage is caused by multinational corporation, pluralization of identity, information society, consumer oriented production, consumer society and inundation of kitsch. On the basis of the characteristics and social backgrounds of cultural collage, the aesthetic values of men's fashion can be explained by pluralism de-generalization and relativity.

Polysomnographic Findings in Kleine-Levin Syndrome (Kleine-Levin Syndrome의 수면 다원 검사 소견)

  • Kleine-Levin syndrome is a disorder characterized by recurrent episodes of hypersomnia, hyperphagia and hypersexuality that typically occur weeks or months apart. A 17-years-old male showed these episodes and took nocturnal polysomnography(NPSG) and multiple sleep latency test(MSLT). As results of NPSG, sleep latency was 82.5min, sleep efficiency was 82.5min, sleep efficiency was 82.5%, latency and percentage of REM sleep were 106.5min and 14.6% and percentage of slow wave sleep was 12.7%. In 4 times MLST, average of sleep latency and REM latency were 8min 7sec and 5min 20sec with 3 times sleep onset REM period(SOREMP). These findings are consistent with these of Keine-Levin syndrome. And the possible causes and classification of this syndrome were discussed.

  • Polysomonography, a way of diagnosing patients for Sleep Apnea, measures several kinds of bio-signal simultaneously. So it makes patients' behavior restricted and the examination more expensive. In this paper, presents another way of examining patients' ECGs, which are transferred to a computer system through a new wireless communication, Zigbee. This way of using Zigbee has solved restrictions of places and time for Polysomonography; and thus it is possible to reduce the cost, as well as improving patients' liberty.

  • Objectives: We attempted to compare the performance of 2 commercially available actigraphies with focus on sleep parameters, using polysomnography as standard comparison tool. Methods: Fourteen normal volunteers (5 males and 9 females, mean age of $28{\pm}4.6\;years$) participated in this study. All the participants went through one night of polysomnography, simultaneously wearing 2 different kinds of actigraphies on each wrist. Polysomnographic and actigraphic data were stored, downloaded, and processed according to standard protocols and then statistically compared. Results: Both $ActiWatch^{(R)}$ and $SleepWatch^{(R)}$ tended to overestimate the total sleep time, compared to the polysomnography. $SleepWatch^{(R)}$ tended to underestimate the sleep latency. The two actigraphs and the polysomnograph did not show significant difference of sleep efficiency, when compared with one another. In addition, all of the sleep parameters from the instruments showed linear correlations except in $SleepWatch^{(R)}'s$ sleep latency. The sleep parameters from the two actigraphs did not show much noteworthy difference, and linear relationships were found between the sleep parameters from the two actigraphs. There was no significant distinction in the results of the two different actigraphs. Conclusion: The results of two actigraphies can be used interchangeably since the sleep parameters of the two different actigraphies do not show significant differences statistically. Overall, it is not legitimate to use actigraphy as a substitute for polysomnography. However, since sleep parameters except sleep latency show linear correlations, actigraphy might possibly be used to follow up patients after polysomnography.
