The Study of tear film stability for normal eyes in adults (성인 정상안의 눈물 안정성에 대한 연구)

  • This study is to research the tear film stability for 48 eyes, agreed the purpose of study, didn't have no contact lens wearing and were tested all two times in a day. Their visual acuity was more than 0.8, corrective visual acuity more than 0.8, the fattest K-reading, apical corneal radius, was in 7.40~8.40mm, corneal astigment was less than 1.00D and orthophoria. The tests, tear scope test, tear film break up time test(TBUT) and schirmer test order, were performed their functions before and after their working at interval of 6 hours over. The tearscope test results was no difference, TBUT was no difference, although 1.2 seconds more in the afternoon, shcirmer test was decreased 2.0mm in the afternoon.

Change in Axial Rotation of Toric Soft Contact Lens according to Tear Volume (눈물양에 따른 토릭 소프트콘택트렌즈의 축 회전양 변화)

  • Purpose: The present study was aimed to investigate the effect of tear volume on a change of axial rotation according to wearing time of toric soft contact lens and gaze directions. Method: Toric soft contact lenses with double thin zone design applied on 62 eyes. Then, changes in non invasive tear film break-up time and the rotational direction/amount of lens when changing gaze direction were respectively measured after 15 minutes and 6 hours of lens wear. Results: Lens rotation to temporal direction was more found when changing gaze direction after lens wear. However, its rotation was varied according to wearing time and the subjects' tear volume. Furthermore, the frequency of lens rotation to temporal direction was higher in dry eyes compared with normal eyes at nearly all gaze directions after 15 minutes and 6 hour of lens wear. The rotational amount of lens was generally greater in dry eyes after 15 minutes of lens wear. However, its difference between normal eyes and dry eyes was not great after 6 hours of lens wear. Conclusion: The present study revealed that axial rotation of toric soft contact lens was varied according to the wearer's tear volume and lens rotational patterns at the initial, and extending periods of lens wear were different. The change in rotational pattern of toric soft contact lens from these results means the possibility of visual change after extending lens wear, and the identification of its correlation with tear volume suggests the necessity of considering factors for choosing appropriate toric soft contact lens.

Comparison of Success Rates after Silicone Tube Intubation with or without Lacrimal Endoscopy for Epiphora (눈물흘림 환자에서 눈물길미세내시경의 사용 여부에 따른 실리콘관삽입술의 성공률 비교)

  • Purpose: To compare the success rates between silicone tube intubation using a lacrimal endoscope and using a conventional nasal endoscope alone in adult patients suffering from epiphora. Methods: We conducted a retrospective chart review of 80 eyes of 55 patients who underwent silicone tube intubation from January 2014 to June 2017. Patients were preoperatively diagnosed with syringing and dacryocystography. The silicone tube was removed 3 months after surgery and success rates were evaluated at 4 and 12 months. Success rates were analyzed by dividing the patients into two groups, according to lacrimal endoscope use. Results: A lacrimal endoscope was used in 40 eyes. In the group using a lacrimal endoscope, preoperative diagnoses were partial obstruction in 26 eyes and complete obstruction in 14 eyes. In the group without lacrimal endoscope use, preoperative diagnoses were partial obstruction in 35 eyes and complete obstruction in 5 eyes (p = 0.018). The success rates at 4 and 12 months after surgery in the two groups (with and without lacrimal endoscope use) were 87.5% and 80.0% and 72.0% and 62.1% (p = 0.546 and p = 0.565), respectively. The success rates of patients with partial obstruction in the two groups were 92.3% and 82.9% at 4 months and 71.4% and 69.2% at 12 months (p = 0.448 and p = 1.000), respectively. The success rates of patients with complete obstruction in the two groups were 78.6% and 60.0% at 4 months and 72.7% and 33.3% at 12 months (p = 0.570 and p = 0.505), respectively. Site differences, the degree of obstruction, and lacrimal endoscope use had a significant impact on the success rate at 4 and 12 months (p = 0.001 and p = 0.022, respectively). Conclusions: Although silicone tube intubation using a lacrimal endoscope cannot guarantee a significant success rate, it is possible to observe the anatomical structure of the nasolacrimal pathway in real time, such that the appropriate diagnosis and treatment can be performed simultaneously. Because patients diagnosed as having a complete obstruction had a good success rate, we can extend indication of silicone tube intubation as a less invasive approach.

Comparisons of Adherence Level of Micro-organisms According to Contact Lens Materials and Protein Deposition and Disinfection Efficacy of Multipurpose Solution (콘택트렌즈 재질 및 침착 단백질에 따른 균 흡착 정도와 다목적용액의 살균력 비교)

  • Purpose: The present study was aimed to compare the difference in adherence level of microorganisms according to contact lens materials and protein deposition and to evaluate disinfection efficacy of multipurpose solution. Methods: The evaluations of micro-organisms' adherence and disinfection efficacy of multi-purpose solution were conducted by employing the Part 2. Regimen Procedure for Disinfecting Regiments in the Disinfection Efficacy Testing under the "FDA Evaluation Criteria & Method". Results: Pseudomonas aeruginosa, Serratia marcescens, Candida albicans except Staphylococcus aureus adhered more on etafilcon A lens and disinfection efficacy of total 4 products investigated was almost perfect except Candida albicans. The 3 micro-organisms except Serratia marcescens adhered more to albumin-predeposited lens. Disinfection efficacy of multi-purpose solution was higher against the micro-organisms adhered to albumin-deposited lens than against the micro-organisms adhered to the lysozyme-deposited lens. Furthermore, disinfection efficacy of multi-purpose solution was different according to types of micro-organisms. Conclusions: It was revealed that the type of micro-organisms, the lens materials and type of absorbed tear protein affected the amount of adhered micro-organisms to contact lens and that adhesion of tear protein could induce the change of disinfection efficacy of multi-purpose solution. It suggest that the hygienic condition of contact lens can vary by these factors influencing on disinfection efficacy and the occurrence of adverse effect can be affected.

Comparison of Blinking Patterns When Watching Ultra-high Definition Television: Normal versus Dry Eyes (초고선명 텔레비전 시청 시 정상안과 건성안에서의 눈깜박임 양상 비교)

  • Purpose: To analyze blinking patterns when watching an ultra-high definition (UHD) television and to compare the results between normal eyes and dry eyes. Methods: A total of 59 participants aged from 13 to 69 years were instructed to watch a colorful and dynamic video on a UHD television for 10 minutes. Before and after watching the UHD television, we measured the best corrected visual acuities, autorefraction, tear-break-up-time, degree of corneal erosion and conjunctival hyperemia via slit lamp biomicroscopy. In addition, questionnaires for the evaluation of eye fatigue and symptoms of a dry eye were completed. The definition of dry eye syndrome was that the tear-break-up-time of one of the eyes was less than 5 seconds, conjunctival injection, or marked corneal erosion. The number of blinks and the duration of blinking were both measured and analyzed at the early and late phases of video-watching. Results: After watching the UHD television in the normal eye group, the tear-break-up-time was significantly decreased (p < 0.001) and the degree of corneal erosion was significantly increased (p = 0.023). However, the subjective symptoms of participants were not aggravated (p = 0.080). There were no significant differences in blinking patterns in the dry eye group. On the other hand, in the normal eye group, the mean blinking time was significantly increased (p = 0.030). Conclusions: Watching an UHD television changes the tear-break-up-time, degree of corneal erosion, and blinking pattern in normal eyes, which may increase the risk of dry eye syndrome.

Comparison of In Vitro Lipid Deposition and Change of Optical Characteristics on Daily Disposable Lenses (1-day) and 3-days Lenses Over 3 days (3-days lenses와 daily disposable lenses(1-day)의 착용 시간 별 지방 침착량 및 광학적 특성 변화의 비교)

  • The study aimed to investigate in vitro lipid deposition of oleic acid, oleic acid methyl ester and cholesterol on a daily disposable (1-day lenses) and 3-days lenses over 3 days and changes of optical characteristics is also investigated. Artificial tear solutions were prepared to simulate actual tear compositions. Two types of contact lenses (1-day lenses (Senofilcon A) and 3-days lenses (silicone tripolymer)) were soaked in the artificial tear solutions within an incubator at 37 ℃ with 150 rpm for 8, 16, 24 hours. Lipid deposition (oleic acid, oleic acid methyl ester and cholesterol) were measured using high performance liquid chromatography (HPLC) instrument. In addition, measurements of oxygen transmissibility, light transmittance and observation of lens surface were conducted. The amount of lipid deposition on the 1-day lenses were 127.55 ㎍/lens for Day 1, 302.96 ㎍/lens, for Day 2, and 353.30 ㎍/lens for Day 3. The 3-days lenses were 46.22 ㎍/lens for Day 1, 66.07 ㎍/lens for Day 2, and 67.45 ㎍/lens for Day 3. Oxygen transmissibility were 81×10-9(cm/sec)(ml O2/ml×mmHg)(Baseline) and 48×10-9(cm/sec)(ml O2/ml×mmHg) (Day 3) for the 1-day lenses, it were 13.23×10-9(cm/sec)(ml O2/ml×mmHg)(Baseline) and 9.6×10-9(cm/sec)(ml O2/ml×mmHg) (Day 3) for the 3-days lenses. Transmittance of each lenses were 97.21% (Baseline) and 94.25% (Day 3) for the 1-day lenses, 97.65% (Baseline) and 95.15% (Day 3) for the 3-days lenses. Observation of surface deposition indicated greatest deposition for the 3-days lenses type on Day 3. Lipid deposition for both lens types increased by day and was greater for the 1-day lenses type. Surface deposition appeared to differ as it was greatest for the 3 days lens type, which may suggest other deposits such as protein may be present.
