호장근의 일시적 국소뇌허혈 흰쥐 모델에 대한 신경보호효과 (Neuroprotective Effect of the Roots of Polygonum Cuspidatum on Transient Focal Cerebral Ischemia in Rats)

  • Objectives : The purpose of the present study was to development of neuroprotective antioxidant agents. For the purpose, we investigated the neuroprotective effect of anti oxidant herb, the root of Polygonum cuspidatum on transient focal cerebral ischemia in rats. Methods : The roots of Polygonum cuspidatum were extracted by 85% MeOH (PCE). Radical scavenging effects were investigated using DPPH assay and TBARs (Thiobarbituric acid reaction substance) assay in brian homogenates. Neuroprotective effect was investigated using transient focal cerebral ischemia rat model (2 h of ischemia, 22 h of reperfusion) by behavioral test and measurement of brain damage using 2, 3, 5-triphenyltetrazolium chloride staining. Results : PCE showed potent and dose dependent radical scavenging effects in DPPH and TBARs assay. Oral administration of PCE reduced brain infarct volume by 29.7% and improved the sensory motor functional deficit by 29% compared with vehicle treated group. Conclusions : PCE showed radical scavenging effects and neuroprotective effect on stroke rat model. Therefore, Polygonum cuspidatum could be a candidate for the development of neuroprotective-antioxidant agents.

  • 두꺼워진 경동맥 내중막 두께는 일과성 뇌 허혈, 뇌졸중, 그리고 심근경색과 같은 관상동맥질환을 예고하는 독립적 인자로서 매우 중요하다. 경동맥 내중막 두께를 측정하기 위한 기존의 영상처리 방법은 전체 초음파 영상 가운데 수작업에 의해 구획을 임의로 설정한 다음, 그 부분만의 색도분포 미분치 산출에 의한 구간 분할로 두께를 추정하는 것이었기 때문에 임의성에 의한 한계는 물론이고 구간별 색도분포가 혼재하는 경우 정확한 두께 측정에는 역부족이었다. 우리는 본 논문을 통하여 경동맥과 같이 영상입력 대상물이 일관된 벡터 형성 특성을 갖고 관측영상의 화소 색도 집합의 원소들 또한 그 고유한 방향성을 내재하고 있는 경우, 이를 층위별로 군집 처리하여 원하는 특징을 효과적으로 추출하는 새로운 영상처리기술을 소개한다 우리는 보다 정확한 측정을 위해, 인체를 구성하고 있는 장기나 혈관과 같은 여러 조직이 동일조직 내에서 타조직과 다른 층위적 특성이 있음에 착안하여, 이의 초음파영상을 수직 및 수평축으로 성분 분석하여 차별된 군집들로 분류해 내고 군집들 사이의 색도 특성과 상호 연관을 수학적으로 규명한 다음, 특징별로 축진행에 따른 순차적인 영역처리를 시행함으로서 혈관 조직에 대한 층위적 형성 판별과 혈관 막(膜)들의 정확한 두께측정이 가능함을 보이고 영상 분석과 실험을 통해 입증하였다. 이러한 접근 방법은 경동맥영상과 같이, 영역간 색도분포의 혼재와 문턱치 산출의 어려움에도 불구하고 일관된 벡터 흐름과 방향성을 형성하고 있는 영상인 경우, 그 영역처리와 특징추출에 효과가 높으므로 응용이 가능하다.

당귀약침(當歸藥鍼)의 혈해(血海) 자입(刺入)이 Intraluminal Filament 삽입술(揷入術)에 의(依)해 유발(誘發)된 백서(白鼠)의 허혈성(虛血性) 뇌손상(腦損傷)에 미치는 영향(影響) (Effects of Angelica gigas Nakai herbal acupuncture into Hyolhae(SP10) of brain ischemic injury induced by Intraluminal Filament insertion in the rats)

  • Objective : The aim of this study was to investigate effects of Angelica gigas Nakai(AGN) on the ischemic injury by intraluminal filament insertion in the rats. Methods : The ischemia was induced by intraluminal filament insertion into middle cerebral artery. AGN herbal acupuncture into SP10 was carried out during 3 weeks after ischemic injury. Eight-arm radial maze was designed for the behavioral task. AGN herbal acupuncture showed neuroprotective agents in cresyl violet, acetylcholinesterase(AchE), choline acetyltransferase(ChAT) and nerve growth factor(NGF)-stain. Then check the effect of regional cerebral blood flow(rCBF) according to AGN herbal acupuncture in rats. Results : The errors in the eight-arm radial maze task were significantly decreased in normal group compared with control group on 1~6days, AGN2(0.02g/kg) herbal acupuncture group on 1~5days, AGN3(0.1g/kg) on 1~3days, AGN4(0.5g/kg) on 1, 3~6days. The rate of correct choice was significantly increased in AGN1(0.01g/kg) and AGN4 herbal acupuncture groups. The density of neurons in the hippocampal CA1 was the most increased in normal group and AGN1, AGN3, AGN4 herbal acupuncture groups compared with control group. The density of AchE in the hippocampal CA1 had a tendency to increase in all the groups when they were compared with control group, but not significant. The density of ChAT in the hippocampal CA1 was significantly increased in normal group and AGN1, AGN4 herbal acupuncture groups compared with control group. The density of NGF in the hippocampal CA1 was significantly increased AGN4 herbal acupuncture group compared with control group. The rCBF was significantly increased in AGN1, AGN3 and AGN4 herbal acupuncture groups without the change of blood pressure. Conclusions : These results suggest that AGN herbal acupuncture can be used for controlling stroke in early stage as herbal medication.

소라에서 분리한 Vibrio균으로 제조한 다시마 Single Cell Detritus(SCD)의 품질특성 (Quality Characteristics of Sea Tangle Single Cell Detritus (SCD) Manufactured by Vibrio sp. Isolated from Batillus cornutus)

  • 해조류를 식품으로 이용하기에는 분말화가 필수적이나 분쇄 등의 분말화 과정에서의 온도상승은 이들 생리활성성분의 열화를 가져올 수 있으며, 수십$\sim$수백 ${\mu}m$의 분말입자는 입자 상호간의 응집으로 인하여 유동성이 좋지 않다. 해조류의 기계적 분쇄의 단점을 해결하기 위하여, 다시마(Laminaria japonica)의 분해능이 강한 Vibrio sp.를 소라(Batillus cornutus)에서 찾아 다시마 SCD(Single Cell Detritus)를 제조하였으며 SCD가 가지는 여러 가지 성분 및 생리활성을 분석하였다. 다시마 SCD의 조단백 함량과 총아미노산의 함량은 다시마분말보다 높았으나 ash의 함량은 낮았다. 다시마 SCD와 분말시료의 항암활성은 각각 31.20과 29.27%로 차이가 없었다(p<0.05). ACE저해효과는 분말시료가 39.31%로 SCD의 26.07%보다 약간 높았다. 항혈액응고 측정 결과 다시마분말은 55.3초로 다시마 SCD의 34.5초보다 높았다. 또한 다시마 SCD와 분말은 항산화효과와 뇌허혈에 의한 신경세포사 억제효과는 없는 것으로 나타났다. 비록 다시마 SCD의 생리활성은 분말보다 약간 떨어지나 조단백질, 아미노산 및 지질의 함량이 높기 때문에 빵, 국수 및 수산 연제품 등의 첨가제로서 응용할 수 있는 큰 장점이 있다고 판단된다.

희렴 이 뇌허혈에 미치는 효과 (Effects of Siegesbckiae Herba on the Brain Ischemia)

  • This experimental study was designed to investigate the effects of SIEGESBECKIAE HERBA extract (SHE) on the change of cerebral hemodynamics 〔regional cerebral blood flow (rCBF) and mean arterial blood pressure(MABP)〕 in normal condition and cerebral ischemic rats, and to determine the mechanism of action of SHE. This study was designed to investigate whether or not SHE inhibit lactate dehydrogenase (LDH) activity in neuronal cells and cytokines production in serum of cerebral ischemic rats. The results were as follows SHE increased rCBF significantly in a dose-dependent manner, but MABP was not changed by SHE in normal rats. The SHE-induced increase in rCBF was significantly inhibited by pretreatment with indomethacin (IDN), an inhibitor of cyclooxygenase but was increased by methylene blue (MTB), an inhibitor of guanylate cyclase. SHE inhibited lactate dehydrogenase (LDH) activity significantly in neuronal cells. rCBF was increased significantly and stably by SHE(10㎎/㎏, i.p.) during the period of cerebral reperfusion, which contrasted with the findings of rapid and marked increase in control group in ischemic rats. In serum by drawing from femoral arterial blood after middle cerebral arterial occlusion(MCAO) for 1hr and reperfusion for 1hr, the sample group was decreased IL-1β production significantly compared to that of the control group. In serum by drawing from femoral arterial blood after MCAO 1hr and reperfusion 1hr, sample group decreased TNF-α production significantly compared to that of the control grolilp. In serum by drawing from femoral arterial blood after reperfusion 1hr, sample group increased TGF-β production significantly compared to that of the control group. In serum by drawing from femoral arterial blood after MCAO for 1hr and reperfusion for 1hr, IL-10 production of the sample group was similar to that of control group. These results suggested that SHE had inhibitive effect on the brain damage by inhibited LDH activity, IL-1β and TNF-α production, but accelerated TGF-β production.

계혈등(鷄血藤) 추출물이 뇌허혈에 미치는 실험적 연구 (Experimental Study of Patholobi Caulis on the Transient Cerebral Ischemia in Rats)

  • The study was designed to investigate the mechanism of Patholoobi Caulis freeze dried powder (PCF) on the regional cerebral blood flow (rCBF) and mean arterial blood pressure (MABP) in normal rats and cytokines production ($IL-1{\beta}$, $TNF-{\alpha}$, IL-10, $TGF-{\beta}$) in cerebral ischemic rats. The results in normal rats were as follows ; Increase of PCF-induced rCBF was significantly inhibited by pretreatment with methylene blue (10 ${\mu}g/kg$, I.p.), an inhibitor of guanylate cyclase, and was inhibited by indomethacin (1 mg/kg, i.p.), an inhibitor of cyclooxygenase. Increase of PCF-induced MABP was decreased by pretreatment with methylene blue, but was increased by indomethacin. These results suggested that the mechanism of action PCF was mediated by cyclic 3',5'-guanosine monophosphate. The results in cerebral ischemic rats were as follows ; In cytokine production in serum from femoral arterial blood 1 hr after middle cerebral arterial occlusion, PCF (10 mg/kg. i.p.) significantly decreased $IL-1{\beta}$ and $TNF-{\alpha}$ production, and increased IL-10 production compared with control group. In cytokine production in serum from femoral arterial blood 1 hr 1 hr after reperfusion, PCF (10 mg/kg, i.p.) significantly decreased $IL-1{\beta}$ production, and incresed IL-10 production compared with control group. These results suggested that PCF was significantly and stably increased regional cerebral blood flow by inhibiting $IL-1{\beta}$ production, and by accelerating IL-10 production.

말기 심부전증 환자에 대한 심장이식술의 조기 성적 (Early Results of the Heart Transplantation for End Stage Heart Failure)

  • 서울대학교병원 흉부외과에서는 1994년 3월부터 1996년 5월까지 14명의 환자에 있어서 심장이식술을 시행 하였다. 남자가 9명 여자가 5명이었으며 평균나이는 40.8 $\pm$ 12.4세 이었다. 수술전 모든 환자들은 UiID Fc III 또는 IV이었으며 술전 진단은 확장성 심근병증이 11명, 제한성 심근병증이 3명이었다. 장기 공여자의 평균연령은 24.9 $\pm$ 10.2세 이었으며 뇌사의 원인으로는 교통사고에 의한 뇌손상이 8명으로 가장 많았으며 거미막하 출혈이 2명, 이 물질에 의한 기도폐쇄, 추락사고, 뇌종양, 익사사고 등이 각각 1명이 었다. 수혜자와 공여자의 혈액형은 11명에 있어서는 일치되었고 2명에 있어서는 적합하였으나 1명의 환자에 있 어서는 부적합하였다. 11명의 환자에 있어서는 양대정맥을 직접 문합하는 방법을 사용하였으며 3명의 환자에 있어서는 우심방을 연결하는 방법을 사용하였으며 평균 이식심장 허혈시간은 157.8 $\pm$ 43.8분(94-220분) 이었다. 2명이 사망하 여 병원 사망률은 14.3%이었다. 사망원인은 우심실부전, 사이클로스포린 유발성 용혈성요독증후군, 거부반응 으로 인한 다발성 장기부전과 대동맥 문합부위 가성동맥류의 파열로 의심되는 酉\ulcorner銖汰潔駭\ulcorner 평균추적기간은 16.2 $\pm$ 9.0개월(3-28개월) 이었으며 만기사망은 1례 있었다. (8.3%) 마지막 추적당시 급성 거부반응으로 치료를 받고 있는 1명을 제외한 모든 환자들은 UnD Fc I이었다. 병원사망을 포함한 1개월 및 6개월, 2년 생존율은 각각 n.9 $\pm$ 6.9%, 85.7 $\pm$ 9.4% , 77.1 $\pm$ 11.7% 이었다. 결론적으로 심장이식술은 말기 심부전환자의 이상적인 치료법이며 앞으로 장기적인 추적검사가 필요하리라 생각된다.

  • Objective : The aim of this study was to investigate effects of Gagambosim-Tang(GBT) of focal brain ischemic injury induced by intraluminal filament insertion in the rats. Method : The ischemia was induced by intraluminal filament insertion into middle cerebral artery Eight-arm radial maze was designed for the behavioral task. Gagambosim-Tang was orally administrated in SD rats for 21 days. The task was started on the 4th week after focal brain injury, and performed two trials per day for 6days. The effects of Gagambosim-Tang on neuroprotective agents in cresyl violet, choline acetyltranferase(ChAT), nerve growth factor (NGF)-stain and c-Fos with ischemic injury were investigated. Results : The errors in the eight-arm radial maze task were significantly decreased in normal group compared with control group on 2-6days, GBT lX(42.2 mg/ml)orally administrated group on 1days, GBT 6X(253.2 mg/ml) on 3, 5, 6days. The rate of correct choice was increased in normal and GBT 6X groups. The neuroprotective effect in the hippocampal CA1 was increased in normal and GBT 1X, GBT 6X groups compared with control group. The density of ChAT in the hippocampal CA1 was increased in normal and GBT6X groups compared with control group. The density of NGF in the hippocampal CA1 was increased normal and GBT6X groups compared with control group. The number of c-Fos-positive neurons in the hippocampal CA1 was increased in normal and GBT 6X groups compared with control group. Conclusion : These results suggest that Gagambosim-Tang may have protective effect on dementia.

대군자탕이 뇌허혈에 미치는 억제 효과 (The Inhibitive Effects of Yukgunja-tang on the Cerebral Ischemia)

  • This experimental study was designed to investigate the effects of Yukgunja-tang(YGJT) on the inhibition of cerebral ischemia in rats. And We measured regional cerebral blood f1ow(rCBF) and pial arterial diameter(PAD) in cerebral ischemic rats, and cytokines production in serum Of cerebral ischemic rats. The results were as follows; Both rCBF and PAD were significantly and stably increased by YGJT(10 mg/kg, i.p.) during the period of cerebral reperfusion, which contrasted with the findings of rapid and marked increase in control group. In cytokine production of serum by drawing from femoral arterial blood after middle cerebral arterial occlusion(MCAO) 1 hr, IL-1β and TGF-β production of sample group were similar to that of control group, but sample group was decreased TNF-α production compared with control group, and was significantly increased IL-10 production in compared with control group. This results were suggested that YGJT has inhibitive effect on the brain damage by inhibited IL-1β production and TNF-α production, but accelerated IL-10 production. We thought that YGJT should have an anti-ischemic effect through the improvement of cerebral hemodynamics and inhibitive effect on the brain damage.

순환계에서 Nitric Oxide의 생리-병리학적 역할과 그 임상적 의의 (Nitric Oxide: The Pathophysiological Roles and Clinical Implications in Circulatory System)

  • 대기오염물질이면서 동시에 생체내 화학반응의 산물이기도 한 nitric oxide(NO)는 그 생체내 분포가 광범위하고 생리적 역할이 다양하여, 최근의 생명과학 분야에서 가장 크게 주목받는 몇가지 연구대상 중 하나이다. 세포에서의 NO 산생은 nitric oxide synthase (NOS)에 의해 촉매되는데, 이들은 brain form (bNOS, neuronal; nNOS, NOS I), inducible form (iNOS), 및 endothelial form(eNOS)로 구분되는데, 이중 bNOS(nNOS)와 eNOS는 inducible form에 대비되는 constitutive form(cNOS)에 해당하므로 각각 ncNOS 와 ecNOS로도 불리운다. NOS는 아미노산인 L-arginine을 산소와 결합시켜 L-citrulline으로 변환시키면서 NO를 유리하고, 이 NO는 세포내의 guanylate cyclase를 활성화하여 cyclic GMP를 생산하거나, superoxide(O2-) 및 수소이온과 차례로 결합하여 반응성이 매우 높은 수산화기(-OH)를 발생시켜 세포독작용을 유발하기도 한다. 정상상태에서 뇌혈관내피세포의 ecNOS로 부터 유리된 NO는 혈관을 확장시켜 신경세포에 대한 산소공급을 원활히 유지해 주지만, 순환장애를 일으켰을 때는 뇌조직내의 iNOS로부터 대량의 NO가 유출되어 신경세포의 손상을 가져온다. 호흡기에서는 NO가 기도평활근을 이완시키고 폐혈류를 개선하므로, 미숙아나 성인의 호흡장애시에 소량의 NO를 흡입시키면 oxygenation을 호전시킬 수 있다. 그러나 대기오염이나 흡연 등으로 대량의 NO를 흡입할 경우 치명적인 폐부종이나 methemoglobin혈종을 일으킬 수 있다. 순환계에서는 cNOS가 혈관을 확장시켜 조직의 혈류를 유지하는데 일익을 담당한다. 세균내 독소(lipopolysaccharide; LPS)나 각종 명역조절물질들이 혈관내피세포와 혈관평활근세포로 부터 과다한 NO를 유리시키면 혈압이 급격히 떨어져 순환허탈상태에 빠지게 된다. 심장에서는 관상혈관 내피세포의 eNOS가 심근의 혈류를 유지해 주지만 허혈이나 세균내독소 또는 면역조절물질 등에 의하여 심근세포나 침윤된 대식세포의 iNOS로 부터 과량의 NO가 유리되면 심근세포의 손상이 초래된다. 신장에서는 내피세포의 cNOS에 의하여 사구체여과가 조절되고 있는데, 세균내독소나 면역 조절물질 등에 의하여 사구체관막세포(mesangial cell)등의 iNOS로 부터 과량의 NO가 유리되면 신조직과 사구체의 손상을 초래한다. 위와 같이 대부분의 장기에서 ecNOS는 조직의 혈류를 유지하는 역할을 하며, iNOS는 애초 세균 등 침입자에 대한 세포독작용이 그 존재 목적이라고 풀이할 수 있겠으나 일종의 부작용으로 자체조직의 손상을 초래하게 되는 것으로 본다. 따라서 NO와 관련된 각종 병변의 치료를 위해서는 NOS의 비선택성 억제제인 arginine 유도체 보다는 iNOS에 대한 선택적 억제제인 S-methylisothiourea(SMT), aminoethylisothiourea(AETU), aminoguanidine (AMG), agmatine, L-canavanine, transforming growth factor b1(TGF-b1) 등의 사용을 검토해 보는 것이 타당할 것으로 사료된다.
