Factors Influencing Post Stroke Depression in Acute Stroke Patients (급성기 뇌졸중 환자의 뇌졸중 후 우울에 영향을 미치는 요인)

  • This study was carried out to investigate the post stroke depression (PSD) occurrence in acute stroke patients and to identify the factors that influence PSD. The study subjects were 104 adults in their 20s or older who were scheduled to be discharged due to inpatient treatment for ischemic stroke in two hospitals. Data were collected using Post Stroke Depression Scale for PSD, Multidimensional Scale of Perceived Social Support for social support, National Institutes of Health Stroke Scale for stroke severity, and Modified Rankin Scale for disability. The average length of stay after stroke onset of the study subjects was 5.9±2.1 days, and 79.8% were within 7 days. Stroke severity score was an average of 2.4±2.5 out of 42 points, and disability score was an average of 1.6±1.1 out of 6 points. Among the subjects, 32.7% had mild or more severe depression after stroke. The subjects with no religion, severe disability, high stroke severity, and less family support had a higher likelihood of experiencing PSD. These results show that depression can appear in the early stages of stroke onset. Therefore, it is necessary to develop nursing guidelines for depression intervention after acute stroke, including continuous early assessment of depression from the acute phase of stroke and religion or family support.

  • 어떤 질병도 예방보다 나은 치료방법은 없겠다. 뇌졸중이 특히 그러한데, 뇌졸중은 그 원인이 어느 정도 밝혀져 있어 조금만 주의한다면 충분히 예방이 가능한 병이다. 또한 일단 죽은 뇌세포는 재생되지 않기 때문에 뇌졸중이 발생한 후라고 하더라도 예방이 매우 중요하다. 그러기 위해 뇌졸중의 원인으로 알려져 있는 것들로서 `위험인자` 라고 불리는 것들을 먼저 알아야겠다. 뇌졸중을 예방하는 데는 의사가 처방하는 약만으로는 부족하다. 앞으로는 위험인자를 잘 조절하여 더 이상 뇌졸중이 생기지 않도록 노력해야 한다. 환자가 꼭 협조하여야만 예방이 제대로 된다는 사실을 잊지 말자.

Association between sleep quality and depression in the elderly after a stroke (뇌졸중 후 노인의 수면의 질과 우울의 관련성)

  • This study was conducted to investigate the level of depression in the elderly after stroke and to determine the relationship between sleep quality and post stroke depression. The study participants were 110 elderly people aged 60 or older who visited the physical therapy room after stroke, and the survey was conducted from August to November 2020. Descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and multiple regression analysis were performed for data analysis. As a result, 77.3% (85 patients) of the participants were depressed. The factor influencing on depression was the quality of sleep, and the explanatory power was 27.8%. It was confirmed that the sleep quality of the elderly after a stroke was a major influencing factor on depression. Therefore, it is thought that measures to improve the quality of sleep should be prepared in order to reduce the post stroke depression of the elderly.

  • Authors report a case of poststroke bipolar disorder that occurs much less frequently than poststroke depression(PSD). A MRI study performed to identify the etiology of a secondary manic episode in a patient with preceded PSD after left basal ganglia infarction revealed newly developed right basal ganglia infarction associated with poststroke bipolar disorder. It is interesting to note that (1) the temporal relationship was found between the occurrence of PSD after left hemisphere stroke and the occurrence of poststroke bipolar disorder following right hemisphere stroke, and that (2) the occurrence of PSD and the occurrence of poststroke bipolar disorder are associated with lesion location respectively. It has been reported that bipolar disorders were associated with subcortical lesions of the right hemisphere, whereas right-cortical lesions led to unipolar mania and that risk factors for mania included a family history of psychiatric disorders and mild subcortical atrophy. In this case, MR image shows subcortical lesions of the right hemisphere and mild subcortical atrophy. The investigation of the relationships among stroke lesion locations and potstroke mood disorders and risk factors for poststroke bipolar disorder may contribute to understanding the neurobiology of primary mood disorder. A clinical implication is that the risk of secondary bipolar disorder after cerebral infarction should be highlighted.

Recovery and Associated Factors of Cognitive Function in Patients with Hemorrhagic Stroke (출혈성 뇌졸중 환자의 인지기능 변화 및 연관 요인에 대한 추적조사)

  • We investigated the improvement of cognitive functions, activity of daily living (ADL), and quality of life (QoL) after hemorrhagic stroke and identified associated factors. For this research, twenty-five patients with a hemorrhagic stroke were enrolled. We measured cognitive function, activity of daily living (ADL), and quality of life (QoL) from 7 days to 12 months after onset of stroke. Then we analyzed the correlation between cognitive function and other risk factors. According to results, cognitive functions improved during 12 months with statistically significant differences. Other functions were similar to cognitive functions. Improvement of cognitive functions were correlated with age, the type of hemorrhagic stroke and Glasgow coma scale at 7 days after stroke. Overall, cognitive function in patients with hemorrhagic stroke recovered from acute to 12 months after onset of stroke. And, improvement of cognitive function at this phase were associated with age, the type of hemorrhagic stroke and GCS score at 7 days. These results would provide us an information to plan cognitive rehabilitation in patients with hemorrhagic stroke.

  • 본 연구는 가상현실시스템을 이용한 훈련이 뇌졸중 환자의 균형과 낙상효능감에 미치는 영향을 알아보기 위해 실시하였다. 연구 대상자는 뇌졸중 환자 20명으로 실험군 10명 대조군 10명씩 각각 배정하였다. 실험군과 대조군 모두 일반적인 주 5회의 물리치료와 작업치료를 받았다. 실험군은 추가적으로 가상현실 프로그램을 이용하여 8주간 1일 20분 주 3회 시행하였다. 본 연구의 측정은 균형수행 능력검사, 낙상효능감 척도검사를 사용하였다. 두 군 간 훈련 후 측정 사이에 균형수행 능력검사와, 낙상효능감 검사에서 통계적으로 유의한 차이를 보였다(p<0.05). 또한 각 군 간 훈련 후 측정 사이에 균형수행 능력검사와, 낙상효능감 검사에서 통계적으로 유의한 차이를 보였다(p<0.05). 따라서 가상현실 프로그램 병행한 훈련이 뇌졸중 환자의 균형과 낙상효능감을 향상시키는데 긍적적인 도움을 주어 유용한 뇌졸중 환자의 치료 프로그램으로 사용될 수 있을 것으로 보여 진다.

The Effects of Task Oriented Activity and Modified Constraint Induced Movement Therapy on Quality of Life for Patients With Stroke (과제 지향적 훈련과 수정된 강제유도 운동치료가 뇌졸중 환자의 삶의 질에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effect of task oriented activity and modified constraint induced movement therapy (mCIMT) on Quality of Life (QOL) for patients with stroke. Methods : Thirty stroke patients were participated voluntarily and were assigned randomly into task oriented activity and mCIMT groups. The QOL of both groups were assessed using Stroke Specific Quality of Life (SS-QOL). Results : The QOL showed a statistically meaningful difference for both groups (p<.05), but after the intervention, the both groups showed no statistically meaningful difference in terms of the QOL (p>.05). Conclusion : We found that task oriented activity and mCIMT improve the QOL of patients with stroke through increasing their affected upper extremity function and movement. It is expected that task oriented activity and mCIMT will have a positive effect on the QOL of stroke patients by applying them to clinic with occupational therapy.

Effects of Mechanical Horseback Riding Training on Trunk Control and Balance function in Stroke patients (뇌졸중 환자의 체간조절, 균형능력에 기계적 승마치료가 미치는 영향)

  • The purpose of this study was to determine the effect of mechanical horseback riding training on trunk control, balance function in stroke patients. 15 stroke patients were recruited for this study. The subjects were randomly assigned to two groups: experimental(n=7) and control(n=8). All subjects received a routine physical therapy. The mechanical horseback riding training was for 30 min in the case of the EG subjects. This study was carried out approximately 8 weeks from March 4 to May 3. The assessment tools of this study involved the trunk control test, postural assessment scale for stroke, Berg balance scale and timed up and go test. The measurements were recorded before the intervention and after the intervention. In the cases of the EG subjects and CG subjects, the variables measured after the intervention significantly differed from trunk control test, postural assessment scale for stroke, Berg balance scale and timed up and go test before the intervention (p<.05). Also, there were significant differences in trunk control test, postural assessment scale for stroke, Berg balance scale and timed up and go test at post test between the 2 groups (p<.05). The findings indicate that mechanical horseback riding training exerts a positive effect on trunk control, balance function in stroke patients. Further studies are required to generalize the result of this study.

Change in Health Behaviors of Patients Before and After Stroke (뇌졸중 환자의 발병전후 건강행위의 변화)

  • This study was conducted to compare the health behaviors of patient s before and after a stroke and propose health education program to reduce risk factors related to stroke recurrence. Data were collected from eighty- eight stroke patients registered at the Gyeongju-si Health Center between July 1, to August 30, 1999, by interviewing patients from a prepared structured questionnaire, which included questions on that patients ' general characteristics, health- related behaviors, family-related characteristics, and pre- and post - stroke health status. Smoking rate of 51.1% before stroke reduced to 25.0% after stroke; drinking rate of 52.3% before stroke reduced to 17.0% after stroke; daily smoking amount of 20.1 packs per day before stroke significantly reduced to 14.9 packs per day after stroke; and daily drinking amount of 92.4ml before stroke significantly reduced to 23.7ml after stroke. Smoking rate according to sex showed a marked decrease in the male subjects, but 31.6% still smoked even after their stroke. Among the female subjects, smoking rate of 16.1% before stroke reduced to 12.9% after stroke. Observation of the change in health- related behaviors of stroke patient s showed significant change in smoking rate, drinking rate and intake of regular meals etc. of patient s with a spouse and patients who received preventive health education. Health education on quitting smoking, temperance, low fat diet, exercise and regular meals for stroke patient s are needed, and public and private organizations can do their part in development and providing continuing health education programs and health education.
