ICF Core Sets for Measuring Functional Status of Acute Stroke Patients (급성기 뇌졸중 환자의 기능상태 측정을 위한 ICF 핵심지표)

  • Purpose: This study was conducted to extract international classification of functioning, disability, and health (ICF) core sets for measuring functional status in acute stroke patients, and to evaluate clinical applicability of the core sets. Methods: A set of 22 ICF items on functional status in acute stroke patients were extracted from the Korean general ICF core sets and ICF core sets for stroke patients. The extracted ICF functional items were assessed at the time of admission and discharge among 100 stroke patients who were admitted in a university hospital. Results: Comparing to functional status at admission of acute stroke patients, the overall functional status at discharge was improved. However, functions on defecation, skin protection, and relationship with immediate family at the time of discharge were not significantly changed. Conclusion: The set of ICF functional items identified in this study may be reliable and valid to assess acute stoke patients' body functions, activities and participation and environmental factors in the holistic and comprehensive nursing context. Nursing interventions on bowel elimination and skin protection for acute stroke patients need to be developed.

Assessing the Functional Status of Acute Stroke Patients using ICF Core Set-short Form (단축형 ICF 핵심지표로 측정한 뇌졸중 환자의 기능상태)

  • Purpose: The International Classification of Functioning, Disability and Health (ICF) model was proposed by the World Health Organization for enhancing interdisciplinary communication. The purpose of this study was to examine the functional status of acute stroke patients using ICF core sets-short form. Methods: A set of 15 ICF items on functional status was used by nurses for their daily nursing assessment on functional status in 44 acute stroke patients in a university hospital. Results: The highest level of function was "consciousness", while the lowest was "defecation". Compared with functional status on the first day, consciousness, ingestion, language (mental), and defecation functions were significantly improved on the 14th day. However, there was no significant changes in items on activity and participation or environmental functions. Conclusion: These findings suggest that the function-specific nursing interventions for acute stroke patients need to be developed. Future research needs to test the usefulness of ICF items as an measurement tool.

A Study on the Burden, Health Status and Well-being in Spouse Caregivers of Patients with Stroke (뇌졸중환자 배우자의 부담감과 건강상태, 안녕 간의 관계 연구)

  • Purpose: The purpose of the study is to investigate the correlation between burden, health status and well-being in spouse caregivers of patients with stroke. Method: The subjects of this study were 160 spouse caregivers registered at general hospital in Seoul and Seongnam. The data were collected from July 2th to October 30th, 2003. The Suh & Oh's burden scale, Yang's health status scale and Park's well-being scale were employed to measure burden, health status and wellbeing respectively. Collected data were analyzed using SPSS 12 version to obtain summary statistics for the descriptive analysis, Pearson Correlation, and Stepwise Multiple Regression. Results: 1. The mean score of the degree of burden, health status and well-being were 3.52, 5.92 and 3.64 points. 2. The burden of the subjects were significantly negative correlated with psychological health status(r=-.482. p<.01) and wellbeing(r=-.455, p<.01). 3. With the result of stepwise multiple regression, psychological health status and wellbeing were the main variables which could explain burden by 27.1%. Conclusion: The findings of this study will provide practical guidelines for developing emotional nursing interventions for the spouses taking care of CVA patients who would experience a heavy burden and distress.

The study on functional condition evaluation in discharging of stroke patients using FIM (FIM을 사용한 뇌졸중 환자의 퇴원시 기능상태 평가에 관한 연구)

  • This study, whose subjects are forty stroke patients in discharging who have been using FIM Western medical (Neurology, Neurosurgery, Rehabilitation Medicine) and Chinese herb medical base of D Hospital in Pusan from the first of May to the eleventh of July, has been made to evaluate the patients' functional conditions and analyze the factors affecting them. The result of study is as followings: FIM total score in discharging is avarage $85.83{\pm}28.96$; the motor FIM score is $57.55{\pm}24.40$ and the cognitive FIM score $29.95{\pm}6.99$. The items recorded the highest score reveal the eating bowel management; the former is $5.68{\pm}1.73$, the latter $5.33{\pm}2.23$. The item recorded the lowest score reveal bathing and stairs, each $2.35{\pm}1.69$ and $3.23{\pm}2.13$. Analysis reveal it is in the case of age(p=.005) and durations of admission(p=.01) that there is significant difference of FIM score when stroke patients in discharging.

Effects of Neurofeedback Training and Computer-Assisted Cognitive Rehabilitation on Cognition and Upper Extremity Function in PostStroke. (신경되먹임 뇌파 훈련과 컴퓨터보조 인지 재활훈련이 뇌졸중 환자의 인지와 상지기능에 미치는 영향)

  • Objective : This study was to evaluate the effects of a Neurofeedback(NFB) and Computer Assisted Cognitive Rehabilitation(CACR) training to improve on cognition and affected arm function in stroke subjects. Methods : Participants were randomly allocated to three groups: NFB(n=14), CACR(n=14) and control(n=16). All groups received occupational therapy and physical therapy for 5 session 30 minutes per week during 6 weeks. Also NFB and CACR group practiced additional NeuroComp training and RehaCom training for 30 sessions 30 minutes during 6 weeks. Results were evaluated by cognition, affected arm function. Results : There were significantly increased by CACR training that outcomes of MMSE-K(p<.05). And there weren't significantly difference by NFB and CACR training that outcomes of the affected arm function. And a difference between three groups wasn't found. Conclusion : The NFB and CACR training improves cognitive function. These results suggest that NFB and CACR training is feasible and suitable for individuals with stroke.

  • Various adult diseases (cerebral apoplexy, athymiait, etc.) result from hypertension, blood circulation disturbance and increment of HBP. In early diagnosis of these diseases, MRI, X-ray and PET have been used rather aim for treatment than for a prevention of disease. Since. cerebral apoplexy and athymiait could appear to the regular/irregular persons, it is very important to measure HBP which has connection with cerebral blood flow state. HBP has more diagnosis elements than that of BP. So, we can diagnose accurate hypertension by measuring of HBP. But, existing sphygmomanometers and automatic BP monitors can not measure HBP, and can not execute complex function(measuring of BP/HBP, blood flow improvement). Purpose of this paper is to develop a system and algorithm which can measure BP/HBP for accurate diagnosis. Also, we extracted diagnosis factors by correlativity analysis of BP/HBP. Maximum pressure of HBP corresponds to 62% that of BP, Minimum pressure of HBP corresponds to 46% that of BP. Therefore, we developed the multi-function automatic blood pressure monitor which can measure BP/HBP and improve cerebral blood flow state.

The Long-Term Effects of High-Frequency Transcutaneous Electrical Nerve Stimulation(TENS) on the Lower Limb Spasticity and the Balance in the Chronic Stroke Patients (장기간 고빈도 경피신경전기자극이 뇌졸중 환자의 하지 경직 및 균형에 미치는 영향)

  • The purpose of this study was to investigate effects of the long-term high-frequency transcutaneous electrical nerve stimulation(TENS) on the spasticity and the balance in chronic stroke patients. Twenty-six subjects with spasticity over lower limbs were allocated randomly into two groups under standard rehabilitation: (1) TENS group, (2) placebo-TENS group. TENS stimulation was applied on the both the gastrocnemius for 30 minutes, 5 days a week for 4 weeks(100 Hz, 0.25 ms, 2 times sensory threshold). The Modified Ashworth Scale(MAS) and Hand-held manual muscle tester were used to assess the ankle plantarflexor spasticity. Balance function under three conditions was measured by using force-plate and the amount of postural sway was assessed; in (1) the condition of standing with eyes opened, (2) with eyes closed and (3) the condition of standing on unstable surface with eyes opened. Both groups showed significant improvement in spasticity and balance function after treatment for 4 weeks(p<.05). Especially, TENS group showed a significant reduction of spasticity compared to placebo-TENS group(p<.05). These results suggested that additional stimulation of a long-term high-frequency TENS to standard rehabilitation induced an improved balance function and a spasticity reduction. The long-term application of high-frequency TENS will be an effective intervention for reducing spasticity and increasing balance ability in the chronic stroke patients.

The Effects of Visuo-perceptual Task Exercise on Postural Balance and Independent Gait Pattern in Hemiplegic Patients (시지각 운동 과제 프로그램이 편마비 환자의 자세 균형 및 독립적 보행에 미치는 효과)

  • 본 연구는 뇌졸중으로 인한 편마비 환자 20명을 대상으로 총 8주간 시지각 운동 과제를 적용하여 자세 균형, 보행 양상에 미치는 효과를 검증하고자 시행하였다. 실험군(n=10)과 대조군(n=10) 모두 보존적인 물리치료를 실시하였고 실험군에는 시지각 운동 과제 프로그램을 더하여 실시하였다. 본 연구의 결과는 실험군의 되먹임 측정을 통한 자세 균형 검사에서 편평한 지면 위에서 눈을 뜨고 감은 상태의 안정성 지수는 유의한 감소를 보였으며, pillow 위에서 눈을 뜨고 감은 상태의 안정성 지수도 유의하게 감소하였다. 체중분포지수는 편평한 지면에서 눈을 뜨고, 감은 상태와 pillow 위에서 눈을 뜨고 감은 상태 모두에서 유의하게 감소하였다. 실험군에서 10m 보행 속도는 대조군과 달리 유의하게 증가하였다. 본 연구에서의 시지각적 운동과제 프로그램은 편마비 환자의 신체기능과 관련된 자세균형 및 보행을 증진시키는 효과가 있었고 이에 보다 효과적인 운동방법으로 제시할 수 있다.

Predicting Factors of Post-Stroke Depression (뇌졸중 후 우울에 영향을 미치는 요인)

  • Predicting factors of post-stroke depression were investigated. The subjects were 254 stroke patients who had been discharged and visited regularly the outpatient clinic for follow up care. The influencing factors were classified into five categories : demographic, disease-related, current state, social support, and illness intrusiveness. Sex, age, job, and educational level were defined as the demographic factor. The disease-related factors included stroke type, illness duration, attack frequency, and NIH score. ADL, cognitive function, and social activity were considered as the current state factors. The social support was measured as the perceived amount of social support. Illness intrusiveness means the perceived illness induced life style disruptions. Demographic, disease-related, current state, and social support were hypothesized to directly and indirectly affect post-stroke depression through illness intrusiveness. The illness intrusiveness was hypothesized to directly affect post-stroke depression. The hierarchial multiple regression was used to identify significant factors. The result showed that this model explained 43.3% of variance of post-stroke depression. And the prevalence of post-stroke depression was 38.8%. Among the demographic factors, job was identified as a main contributor to indirectly increase the post-stroke depression. Among the disease-related factors, stroke type, attack frequency, and NIH score were found to indirectly affect the post-stroke depression. Among the current state factors, ADL and social activity indirectly affected the post-stroke depression through illness intrusiveness. Social support and illness intrusiveness were identified to directly affect the post-stroke depression. This study has proved the factors likely to be implicated in the development of post-stroke depression. Based upon these results, it is recommended that the nurses who take care of post-stroke patients consider the risk factors such as social support, illness intrusiveness et al. Also programs which decrease the illness intrusiveness and increase the social support to reduce post-stoke depression recommended to be developed.

A Research of Prehospital 119 Emergency Medical Service for Stroke Patients (병원 전 뇌졸중 환자의 구급의료 실태 분석)

  • This study analyzes the influencing factors of prehospital 119 Emergency Medical Service for Stroke Patients in prehospital. From July, 2010 to June, 2011, 123 patients were diagnosed with Stroke in Emergency Medical Center. Among 123 patients, the prehospital assessment rates were as follows : blood pressure checked 73.2 %, pulse checked 73.2 %, respiratory checked 64.2 %, $SpO_2$ checked 79.7 %, pupil reflex test 88.6 %, Electrocardiogram checked 14.6 %, blood sugar checked 19.5 %. Prehospital emergency medical care, Oropharyngeal airway insertion 2.4 %, Manual airway maneuvers 17.1 %, endotracheal intubation 2.7 %, oxygen supply 35.4 %, and suction and intravenous fluid therapy 0 %. The property of mental status evaluation by 119 Emergency Medical Service was nearly perfect, but the patients assessment and emergency medical care were not.
