Purpose: Although several neuroanatomical models of panic disorder have been proposed, little is known regarding the neurological mechanisms underlying cognitive-behavioral therapy (CBT) in patients with panic disorder. This study was performed to identify the brain structures that show changes of regnioal cerebral blood flow (rCBF) after CBT in patients with panic disorder. Materials and Methods: Seven patients who were diagnosed as panic disorder by DSM-IV were treated with CBT for 8 weeks and twelve healthy volunteers joined in this study. Serial $^{99m}Tc-ECD$ brain perfusion SPECT images were acquisited and PDSS-SR (Self-Report version of Panic Disorder Severity Scale) and ACQ (Agoraphobic Cognitive Question) scores were measured just before and after CBT in all patients. Data were analyzed using SPM2. Results: Subjective symptoms were improved, and PDSS-SR and ACQ scores were significantly reduced ($14.9{\pm}3.9\;vs.\;7.0{\pm}1.8$, p<0.05; $30.3{\pm}8.5\;vs.\;21.6{\pm}3.4$, p<0.05, respectively) after CBT in panic patients. Before CBT, a significant increase of rCBF was found in the cingulate gylus, thalamus, midbrain, both medial frontal and temporal lobes of the panic patients compared to the normal volunteers. After CBT, we observed a significant rCBF decrease in the left parahippocamus, right insula and cingulate gyrus, both frontal and temporal lobes, and a significant rCBF increase in both the occipital lobes, left insula, both frontal and left parietal lobes. Conclusion: These data suggested that CBT is effective for panic disorder and diminish the activity of the brain areas associated with fear in panic disorder.
Objectives : The multimodal telepresence systems have been adopted in a variety of applications, such as telemedicine, space or underwater teleoperation and videoconference. Multimedia, one of the telepresence systems, has been used in various fields including entertainment, education and communication. The degree of subjective telepresence is defined as the probability that a person perceives to be physically in the remote place when he/she experiences a multisensory feedback from the multimedia. The current study aimed to explore the neural mechanism of telepresence related to multisensory feedback in patients with schizophrenia. Methods : Brain activity was measured using functional magnetic resonance imaging while fifteen healthy controls and fifteen patients with schizophrenia were experiencing filmed referential conversation at various distances (1 m, 5 m and 10 m). Correlations between the image contrast values and the telepresence scores were analyzed. Results : Subjective telepresence was not significantly different between the two groups. Some significant correlations of brain activities with the telepresence scores were found in the left postcentral gyrus, bilateral inferior frontal gyri, right fusiform gyrus, and left superior temporal sulcus. There were no main effects of group and distance. Conclusion : These results suggest that patients with schizophrenia experience telepresence as appropriately as healthy people do when exposed to multimedia. Therefore, patients with schizophrenia would have no difficulty in immersing themselves in multimedia which may be used in clinical training therapies.

목적 : 파킨슨 병은 기저핵 흑질의 치밀부에서 도파민성 신경세포의 퇴행으로 인하여 발생하는 질병으로 신경 염증이 주요 병인으로 밝혀져 있다. 이 연구는 MPTP 유발 파킨슨 병 동물 모델에서 신수혈($BL_{23}$)에 대한 봉독 약침의 항염증 효과 및 그 기전을 확인하기 위해 시행되었다. 방법 : $C57_{BL}$/6쥐를 무처치군, MPTP+saline군, MPTP+BVA(0.06mg/kg)군, MPTP+BVA(0.6mg/kg)군의 4군으로 나눈 뒤 무처치군을 제외한 모든 그룹에 총 8시간 동안 2시간 간격으로 MPTP-HCl(20mg/kg per dose$\times$4)을 복강내로 주입하였다. MPTP+BVA 군에서 봉독약침은 마지막 MPTP 주입 2시간 후부터 48시간 간격으로 신수혈($BL_{23}$)에 양측으로 각 20${\mu}\ell$씩 주입하였고 MPTP+saline군에서는 봉독약침 대신 Saline을 주입하였다. 마지막 MPTP 주입 후 7일째에 쥐의 뇌를 적출한 후 면역조직화학법을 시행하였다. 결과 : MPTP 유발 파킨슨 병 동물 모델에서 신수혈에 대한 봉독약침은 농도 의존적으로 TH-Immunoreactivity neuron의 감소와 microglial activation을 억제하였다. HSP70-IR neuron은 모든 군에서 나타나지 않았다. 결론 : 봉독약침이 용량의존적으로 microglial activation을 억제하는 효과를 통해 도파민성 신경세포의 파괴를 억제함으로써 항염 효과를 나타냄을 알 수 있었다. 이 결과는 봉독약침이 microglial activation 억제를 통해 임상적으로 파킨슨 병과 같은 신경 퇴행성 질병에 있어 유용한 치료수단이 될 수 있음을 시사한다.

The relationship between neoplastic disease and thromboembolic disorders has been recognized since 1865, when Armand Trousseau first reported a high incidence of venous thrombosis in a series of patients with gastric carcinoma. The overall incidence of thromboembolic disease in patients with cancer has been reported to vary 1% to 15%. In a prospective study, Ambrus and associates reported that thrombosis and/or bleeding was the second most common cause of death in haspitalized cancer patients. We report a case who presented as a thromboembolic disease and subsequently confirmed to have an underlying lung malignancy. This 45 years old male patient visited our hospital with abdominal pain and distention of 3 days duration. Abdominal CT scan revealed multiple splenic and renal infarctions. On 20th haspital day, drowsy mental status was developed and hemorrhagic cerebral infarction was noted in brain CT scan. Chest CT scan revealed a 4cm sized spiculated mass on left lung apex and multiple paratracheal lymph adenopathy. With surgical biopsy of left supraclavicular lymph nodes, this patient was confirmed to have adenocarcinoma.

Cheonmabanhwa-Tang(CBT) has been used in the Oriental Medicine for many centuries as a therapeutic agent for dizziness due to Poong-Dam. This Study was designed to investigate the mechanism of Prescription on cerebral hemodynamics [regional cerebral blood flow(rCBF) and pial arterial diameter(PAD)J in cerebral ischemia rats, The results in cerebral ischemic rats were as follows: Both rCBF and PAD were significantly and stably increased by CBT (10 ㎎/㎏, i.p.) during the period of cerebral reperfusion, which contrasted with the findings of rapid and marked increase in control group. Pretreatment with indomethacin(1 ㎎/㎏, i.p.), an inhibitor of cyclooxygenase significantly but unstably increased the CBT-induced increases in PAD as well as rCBF during the period of cerebral reperfusion. Pretreatment with methylene blue(10 (.1.㎍/㎏, i.p.), an inhibitor of guanylate cyclase significantly but unstably increased the CBT-induced increases in PAD as well as rCBF during 150 minutes of cerebral reperfusion, but decreased stably the CBT-induced increases in rCBF and PAD after 180 minutes of cerebral reperfusion. In conclusion, the present authors thought that CBT caused effect on cerebral hemodynamics via mediation of cyclooxygenase.

This study was designed to investigate the effects of Mokhyangjoki-san extract (MJS) on the improvement in cerebral blood flow (rCBF) in cerebral ischemic rats, and further to determine cytokines production (IL-1${\beta}$, TNF-${\alpha}$, IL-10, TGF-${\beta}$) of MJS. The results in cerebral ischemic rats were as follows ; The rCBF was significantly and stably increased by MJS (10 mg/kg, i.p.) during the period of cerebral reperfusion, which contrasted with the findings of rapid and marked increase in control group. In cytokine production in serum from femoral arterial blood 1 hr after middle cerebral arterial occlusion, MJS significantly decreased IL-1${\beta}$ and TNF-${\alpha}$ production, and increased IL-10 production compared with control group. In cytokine production in serum from femoral arterial blood 1 hr after reperfusion, MJS significantly decreased IL-1${\beta}$ and TNF-${\alpha}$ production compared with control group. IL-10 and TGF-${\beta}$1 production in MJS group were significantly increased compared with control group. These results suggested that MJS significantly and stably increased rCBF by inhibiting the production IL-1${\beta}$ and TNF-${\alpha}$, and accelerating that of IL-10 and TGF-${\beta}$. The result in nerve cells was as follows ; MJS significantly inhibited lactate dehydrogenase activity in vitro in a dose-dependent manner. This result suggested that MJS prevented the neuronal death.

Infantile osteopetrosis is a rare congenital disorder caused by abnormal bone resorption. Patients with osteopetrosis can have severe anemia, thrombocytopenia, hepatosplenomegaly, rickets, visual impairment, and deafness. Cytomegalovirus also can cause a congenital infection with anemia, thrombocytopenia, hepatosplenomegaly, and calcifications in the brain. We report a 38-day-old infant with severe hepatosplenomegaly, thrombocytopenia, hypocalcemia, and growth failure. Real time polymerase chain reaction detected cytomegalovirus in the plasma. Skeletal radiography revealed generalized bone sclerosis. He was diagnosed with osteopetrosis along with cytomegalovirus infection. Only the test for mutation of the CLCN7 gene, representing the most common and heterogeneous form of osteopetrosis, was available, and the result was negative. With supportive care and antiviral treatment, severe thrombocytopenia due to the cytomegalovirus infection almost normalized despite the possible immunosuppression caused by osteopetrosis. We present the first report of an infant who suffered from osteopetrosis and CMV infection which was successfully treated by long term antiviral agent therapy.
Objectives : Noemyeong-san (NMS) is a novel herbal prescription composed of five oriental medicinal herbs including Prunellae Spica, Betulae Cortex, Foeniculi Fructus, Asiasari Radix, and Clematidis Radix for treating Alzheimer's disease. In the present study, we investigated the effects and molecular mechanisms of NMS on BV2 microglia to evaluate the potential action of this formula for preventing or treating neurodegenerative disease such as Alzheimer's disease. Methods : To determine the cytotoxicity of NMS on BV2 microglia, the MTT assay was performed. The effects of NMS on lipopolysaccharide (LPS)-stimulated BV2 microglia were determined with a nitric oxide (NO) assay and western blots for inflammatory mediator-related proteins, mitogen activated protein kinases (MAPKs), nuclear factor kappa B (NF-${\kappa}B$) pathway-related proteins, and heme oxygenase-1 (HO-1). Result : NMS inhibited induction of iNOS and COX-2 as well as NO production without affecting the cell viability in LPS-stimulated BV2 microglia. NMS also suppressed activation of ERK and p38 MAPK among main kinases of MAPKs as well as NF-${\kappa}B$ by LPS stimulation. Furthermore, NMS dose-dependently induced the expression of HO-1 and the inhibitory effect of NMS on the production of NO were blocked by pretreatment with an HO-1 inhibitor, Snpp. Conclusions : These results demonstrate that NMS has potent anti-neuroinflammatory effect on the LPS-stimulated microglia. These findings provide evidences for NMS to be considered as a new prescription for preventing or treating neurodegenerative disease such as Alzheimer's disease.

CheonghunHwadam-tang(CHT) have been used in oriental medicine for many centuries as a therapeutic agent of vertigo by wind, fire and phlegm. CHTS was CHT adding Schizonepetae Herba. The effects of CHTS on the cerebral blood flow and blood pressure is not known. The purpose of this Study was to investigate effects of CHTS on the regional cerebral blood flow(rCBF) and mean arterial blood pressure(BP), action-mechanism of CHTS-induced changed-rCBF and BP. The changes of rCBF and BP was determinated by Laser-Doppler Flowmetry(LDF). The results were as follows ; CHTS extract was increased significantly rCBF in a dose-dependent, but was not changed BP compared with CHTS non-treated group. Pretreatment with propranolol, indomethacin and methylene blue were inhibited CHTS induced increase of rCBF, propranolol(all CHTS-treated group) and indomethacin(CHTS 0.01 mg/kg) of them were significantly decreased. Pretreatment with propranonol and indomethacin were inhibited CHTS induced increase of BP, but pretreated with methylene blue was significantly accelerated BP in high dosage. This results suggest that CHTS increased rCBF by dilating pial arterial diameter and the action of CHTS is also mediated by adrenergic β -receptor and cyclooxygenase.
The neuronal ceroid-lipofuscinoses (NCLs) are a kind of neurodegenerative storage disorders. The NCLs are charecterizated by accumulation of autofluorescent lipofuscin or lipopigment in the brain. All NCL group belongs to in lysosomal storage disorders (LSDs), except Northern epilepsy. NCLs are the most common group of progressive neurodegenerative disorders in childhood, with an incidence as high as I in 12,500 live births. Four main clinical types have been described based on the onset age : infantile, late infantile, juvenile and adult types. Clinical symptoms of NCLs include loss of vision, seizures, epilepsy, progressive mental retardation and a premature death. Although mutation causes neurodegeneration in NCLs, the molecular mechanism by which mutation leads to neurodegeneration remains unclear. In this paper, we review the characteristics of these NCLs.
