뇌실내 Morphine의 척수세포 억제작용

  • Morphine은 강력한 진통작용으로 약효를 나타내고 있고 이와 같은 진통작용은 대체로 크게 대별하여 두 가지 기전으로 설명이 된다. 그 첫째는 수의 opioid수용체에 작용하여 척수에서 상행으로 전달되는 통각정보를 차단하는 것이며 또 다른 하나는 뇌에 존재하는 opioid수용체에 작용하여 진통작용을 발현한다고 알려져 있다. 실지로 임상에서 morphine을 실내에 주입하여 강한 진통작용을 가진바가 많이 보고되었다. 그러나, 아직 마약성 진통제가 뇌를 통하여 진통작용을 가지는 그 기전에 대해선 잘 밝혀지지 않았다. 많은 연구가 마약성 진통제가 뇌에 주입되면 뇌에 존재하는 opioid수용체에 작용하고 이 수용체를 가진 신경세포가 하향성 억제를 함으로써 척수에서 통각정보를 차단하여 진통작용을 나타냄을 시사한다. 그러나 아직도 이의 정확한 기전이 명쾌히 밝혀지지 않고 있다. 즉, 뇌실내에 morphine을 주입하면 이것이 과연 척수세포를 억제할 것인가에 대한 답이 없기 때문이다. 그와는 반대로 뇌실내 morphine주입은 척수세포에 아무런 영향을 미치지 못한다는 보고가 있다. 이에 본 실험은 뇌실에 주입된 morphine이 척수세포를 어떤 영향을 미치는가를 확인하고자 하였다.

  • The purpose of this study was to investigate the relationship between psychomotorik and brain function in domestic and foreign literature, and to clarify the mechanism of psychomotorik from the viewpoint of brain science, and to provide the teachers, therapists, The purpose of this study is to provide basic data which can enhance the understanding of the mechanism of psychomotorik. From the viewpoint of brain science, the study of domestic and foreign literature related to the mechanism of psychomotorik and the positive learning environment and various physical experiences in early life through the movement of psychomotorik are very important from the brain physiological point of view. The results of this study showed that the more the children move, the more the learning is improved and the healthy emotional control ability is improved and the brain is more likely to be optimized. Therefore, it is considered that this study, which has been studied through the brain science approach, provided very important basic information to understand the relation between movement of psychomotorik and brain function. In particular, understanding the possibility of optimizing the brain by cultivating a healthy emotional control ability and improving learning as children move more and more is considered to be a necessary process for educating our children well in the future. Until now, research on psychomotorik and brain function was lacking. Through this study, I hope that more people will have an opportunity to expand their understanding of the psychomotorik.

fMRI로 관측한 침의 진통 기전

  • 목적: 침에 의한 진통 기전은 동양에서 경험이나 임상적으로 그 효능이 입증되어 왔으나 서양에서 요구하는 과학적인 접근 방법에 의한 증거가 매우 희박하며, 과학적인 접근이 매우 어려웠다. fMRI를 이용하여 뇌의 기능 영상을 관측함으로 그 증거를 제시하고자 한다. 대상 및 방법: 집게손가락에 특정시간동안 온도에 의한 자극을 주면서 기능적 뇌 영상을 받고 침 자극 전후의 활성 영역을 비교함으로써 침이 통증의 진통 기전에 효과가 있음을 보여 준다. 신뢰성 있는 데이터를 위해 여러 명의 영상을 평균하여 잡음을 제거하였다.

  • The purpose of this study was to investigate the existing theories related to overtraining syndrome and to examine the mechanism of overtraining syndrome from the viewpoint of brain science by examining domestic and foreign literature related to the relationship between overtraining syndrome and brain neurotransmitter. The aim of this paper is to provide basic data that can improve the understanding of the mechanism of overtraining syndrome and the role of neurotransmitters and neuromodulators. The results of this study and a number of hypotheses about the overtraining syndrome were proposed, each with strengths and weaknesses. Similar symptoms that occur when the concentration of serotonin in the neurotransmitter increases are related to signs and symptoms of overtraining syndrome. However, it has not been validated to date because it can not distinguish the mechanism of the mediator between the central nervous system and the peripheral nerves. This study suggests that the mechanism of overtraining syndrome will provide important basic information to understand the complex causes of overtraining syndrome through the interaction of existing theory and brain neurotransmitter. Although there has been a lack of studies on the mechanism of overtraining syndrome and brain neurotransmitters so far, we hope that this study will provide an opportunity for more and more people to broaden their understanding of overtraining syndromes.

  • 두부 외상은 많은 발생율과 사망율을 차지하고 있으며, 건강한 생활을 위해 큰 관심을 갖게 되었다. 신경방사선영상은 외상성 뇌손상 환자들의 진단과 치료에 필수적인 방법이다. 뇌손상의 기본 기전, 병리, 그리고 영상 소견을 이해 하는 것은 매우 중요하다. 1970년대에 Glasgow coma scale의 형상과 전산화단층촬영(CT)의 발달은 임상의사들이 두부외상에 대한 평가와 환자들의 경과를 예상하는데 극적인 변화를 주었다. 최근 자기공명영상(MRI)의 발달로 외상성 뇌손상의 형태를 더욱 이해 하게 되었고, 두부 외상의 발견율이 높아지게 되었다.

  • This review describes the neurophathological mechanisms that are implicated in perinatal brain injury. Perinatal brain injury is the most important cause of morbidity and mortality to infants, often leading to spastic motor deficits, mental retardation, seizures, and learning impairments. The immature brain injury is usually caused by cerebral hypoxia-ischemia, hemorrhage, or infection. The important form of perinatal brain injury is the hypoxic-ischemic injury and the cerebral hemorrhage. The pathology of hypoxic-ischemic injury include delayed energy failure by mitochondrial dysfunction, neuronal excitotoxicity and vulnerability of white matter in developing brain. The immature brain has the fragile vascular bed of germinal matrix and can not effectively centralize their circulation. Therefore, the cerebral hemorrhage process is considered to be involved in the periventricular leukomalacia.

Anxiety Disorders after Traumatic Brain Injury (외상성 뇌손상 후의 불안장애)

  • Traumatic brain injury(TBI) is generally considered to be a risk factor for psychiatric disorders, especially depression and anxiety disorder. Despite the anxiety disorders are frequent sequelae after traumatic brain injury, the patients have not been payed medical attention and treated by doctors properly. The factors of precipitating and sustaining the anxiety disorders after TBI are brain injury itself, and the patient's or caregiver's response to the disability after TBI. To diagnose and treat them effectively, the knowledge about the mechanisms of and symptoms after TBI have to be needed. Psychiatrist should be a supportive and good listener to the patients who are complaining anxiety symptoms and differentiate whether the psychiatric symptoms are due to TBI or not. Because the TBI patients are very sensitive to drug side effects, doctors have to be familiar with the side effects as well as the mechanisms of action of the common psychotropics.

Occupational Therapy in Hypoxic-Ischemic Brain Injury Patient by Suicidal Attempt: Case Report (자살시도로 인한 저산소성 허혈성 뇌손상 환자의 재활치료 - 인지 재활과 연하 재활을 중심으로: 사례연구)

  • Objective : This case study was to verify effects of cognitive rehabilitation and swallowing rehabilitation on Hypoxic-Ischemic Brain Injury patient by Suicidal Attempt. Methods : The subject was a 32-year old Hypoxic-ischemic brain injury patient by suicidal attempt. He received treatment once a day five times a week, for a half an hour for each session from September 8th to December 16th, 2016. Treatment were cognitive and swallowing rehabilitation. He was assessed based on Mini-Mental State Examination-Korean (MMSE-K), Korean-Modified Barthel Index (K-MBI), Computerized Neurocognitive Function Test (CNT), Videofluoroscopic Dysphagia Scale (VDS), American Speech-Language-Hearing Association National Outcomes Measurements System (ASHA-NOMS). Results : The patient's total MMSE-K score increased from 25 to 27. His K-MBI score increased from 74 to 88. His memory, attention span, and executive function (DST, VST, SWCT, WCST) by CNT scores were improved. VDS score has no changes to 34, 44.5 and 34. ASHA-NOMS score also has no change to 6, 2 and 6. Conclusion : The study showed that the application of the treatment of cognitive and swallowing in hypoxic-ischemic brain injury patient by suicidal attempt results has positive effects on cognitive functions, and swallowing function.

  • The neurophysiological study has been widely used in search of the relationship between brain and behavior. The basic techniques for the animal experiments of this kind such as stereotaxic techniques, lesioning methods, the methods of electrical stimulation and recording, and confirmation of histological location were briefly reviewed. Nevertheless, the importance of complementary neurochemical, neuroanatomical and behavioral studies can not be neglected.
