Opinions of Medical Practitioners in a Local Area about Rural Medical Care Practices (일부지방(一部地方) 개업의(開業醫)들의 농촌의료(農村醫療)에 대(對)한 의견(意見))

  • During 2 months from December 1979 to January 1980, Medical practitioners in Chonnam province were grouped into Si (city), Eup (town) and Myun (rural area) groups according to the locations of their clinic. 40 practitioners were randomly selected in each group and were asked their opinions about rural medical care in general. Total of 88 practitioners replied to the question as 73.9 percent of response rate in average. The most frequently mentioned advantages of rural practice were ease of clinic opening in Si-and Myun-group respondents and good social relationship in Eup-group respondents. The most frequently mentioned disadvantages were medical isolation in Si-group respondents and residents' ignorance in medicine in Eup-and Myun-group respondents. In all groups, most respondents thought that rural medical care should be delivered by and controlled by physician. Suggestions made by Eup-and Myun-group respondents for improving rural medical care by structural change focused on the regional medical insurance system, while Si-group respondents noted district hospital or hospital linkage as the most preferrable system.

Historical Review of Modern Public Health Nursing (근대 보건간호의 역사적 고찰)

  • Objectives: The purpose of this study is to examine the modern history of public health(PH) and suggest a way forward for PH nursing(PHN). Methods: This paper is a review article that derives results from literature review. Results: In the period of beginning (up to 1944), PHN began as the PH Department was created in the Hygiene Bureau in 1908 and tasks about nurses were legislated. PHN was limited to infectious disease tasks and performed mostly by missionaries. In the period of foundation formation (1945 to 1961), the Republic of Korea was founded, and PH policies and tasks were defined with the establishment of the central government organization and the applicable laws. In the period of foundation establishment (1962 to 1979), the Regional PH Act was amended, and as a result, PH Centers(PHCs) spread across the country. In the period of foundation expansion (1980 to 1994), the PH referral system of PHCs, PH Units, and Primary Health Care Post was established. In the period of organization in each area (1995 to 2005), PH programs reflecting changes in disease structure and public needs for the quality of life. A regional health care plan was launched. In the period of funtion expansion (2006 to present day), Centers for support health living were established. Conclusions: In the future, PH nurses need to have a macroscopic perspective that views PH through the overall PH system, and to expand from the existing healthcare concept to the national and global healthcare one.

Need Assessment for Enlargement of Oriental Medical Care Service Room in Rural Community Health Center (농촌지역 보건소 한방진료실 확대설치 요구도 및 관련요인 - 일개 군 보건소 한방진료실 내소자를 대상으로 -)

  • Objectives: The oriental medical care has been getting popular in community health centers because of uniqueness, changing of disease patterns, and increasing of elderly population. From 1998, oriental medical doctors has been working in several rural community health centers for their military obligations. At this point of time, it is necessary to evaluate the oriental public health doctors system. This study was performed to investigate the utilization patterns, the degree of satisfactions, needs of oriental medical care service provided by community health center in a designated Gun area. This study focused on the need for extending over Myun area of community health center's oriental medical care services. Methods: Person-to-person interview survey method through a structured questionnaire was done by a personnel at a oriental medical care service room in a Gun community health center. The major statistical method used for the analysis were the t-test, ANOVA, and logistic analysis. Results: The total number of responded subjects in this study was 163 residents. Among these 65.0% were aged 61 or over, and only 13.5% recognized themselves were healthy. 73.7% of the respondents demanded establishment of more oriental medical care services provided by community health center to other Myun area. Factors affecting the need for enlargement of oriental medical care service room were education level, subjective awareness of access to community health center, and cost satisfaction of oriental medical service provided by community health center. Thus, a resident who had graduation of middle school achievement or above(OR=3.35), had a long way to center(OR=2.47), satisfied with oriental medical service cost(OR=2.78) had demonstrated increased chance of need by logistic regression analysis.

  • Purpose: The purpose of the study quantitatively investigates the experience of unmet healthcare service utilization by rural populations in vulnerable areas during the COVID-19 pandemic based on Andersen's behavior model. At the same time, this study attempts to describe the experiences of unmet healthcare service utilization among participants in vulnerable rural areas by analyzing qualitative contents through open-ended question. Methods: Data were collected from October to November 2022 using Qualtrix, a web-based survey platform. A total of 863 participants completed an online survey. Quantitative data were analyzed using 𝑥2 test and logistic regression analysis. Qualitative data were analyzed using content analysis. Results: The factors affecting participants' unmet healthcare service utilization were type of residential area and underlying disease. The qualitative analysis identified; four categories and nine sub-categories. Conclusion: Based on these findings, it is necessary to develop a disaster nursing response model according to the type of residential areas and the number of people.

The Influence of Private Health Insurance on Admission among Some Patients with Cervical or Lumbar Sprain (민영의료보험 가입이 일부 경·요추부 염좌 환자의 입원에 미치는 영향)

  • Background: In Korea, private health insurance has neglected to induce externality on national health insurance by moral hazard. Therefore, we conducted this study in order to explore the influence of private health insurance on unnecessary medical utilization among patients with cervical or lumbar sprain. Method: The study examined a population of 449 patients (admission, 384; out-patient; 85) diagnosed with simple cervical or lumbar sprain without neurological symptoms at 20 small hospitals or clinics in Gwangju and Jeollanam provinces from Jul. 1 to Aug. 31 2008. The data were collected using structured, self-administrated questionnaire which collected information such as whether or not the patient was admitted (as a dependent variable), whether or not they had private health insurance (as a independent variable), and covariates such as socio-demographic characteristics, the factors related to the sprain, and characteristics of the insurance provider. Results: From hierarchical multiple logistic regression analysis, it was found that the admission rate of patient with private health insurance was higher than that those without it (Odds ratio=3.31, 95% Confidence interval; 1.14-9.58), meaning that private health insurance was an independent factor influencing the admission of patients with these conditions. Other determinants of admission were patient age and physician referral. Conclusions: This study is the first empirical study to explore the influence of private health insurance on inducing moral hazard in admission services, specifically among patients with cervical or lumbar sprain. Regulation of benefits provided by private health insurance may be necessary, as the effect of this moral hazard may mean existence of externality.

  • 거제(巨濟) 지역사회(地域社會) 개발(開發) 보건원(保健院)이 목적하는 바대로 사업이 진전되고 있는지, 바람직한 지역사회의학 사업을 수행하고 있는지를 평가하는 일은 필요하다고 생각한다. 이러한 평가의 하나로서 프로젝트에 영향력을 줄 수 있는 세 그룹 즉 지역사회의 유지, 프로젝트를 이용하는 주민 그리고 의료문제에 관심이 있는 사람들 중 프로젝트에 다녀간 일이 있는 사람에 대해서 그들이 이 사업에 대해 이해하고 있는 바 또는 태도 등을 알아 보았다. 거제 지역사회 개발 보건원에 대한 반응은 긍정적이었다. 즉 저렴한 수가로 농촌 보건사업을 하고 있고, 간단한 치료사업과 예방의학 사업을 하고 있으며, 한국에서 지역사회의학의 교육을 발전시키는 데 자극적인 역할을 하고 아울러 지역사회의 개발을 위한 자극을 주었다고 하였다. 문제점으로 지적된 것은 위치가 거제군 전체의 의료를 위해서는 적합하지 못하며, 국내 의료인이나 지역주민의 참여가 불충분하였고, 보건소 사업과의 협조가 덜 이루어지고 있다는 점 등이었다. 한편 사업의 목적이나 계획 및 운영면이 약하지 않은가 하는 것도 우려하였다.

The Relationship Between Dietary Pattern and the Community Periodontal Index in Elderly People Living Alone in Rural Areas (농촌지역 독거노인의 식이패턴과 지역사회치주지수와의 관련성)

  • Objectives : The purpose of this study was to investigate the relationship between dietary pattern and the Community Periodontal Index in elderly people who live alone in rural areas and to provide necessary data to strengthen nutrition education about the oral health of elderly people who live alone. Methods : Dental examinations and questionnaires were conducted with 380 elderly people who live alone in rural areas of Gyeonggi-do. Results : The Community Periodontal Index was higher when the elderly people who live alone had a low intake of vegetables and fruits, a high intake of sugars, a low number of breakfasts, a high frequency of overeating and a high frequency of instant ingestion. Conclusions : It is necessary to provide nutritional management services for the elderly people who live alone in rural areas and to provide preventive centered comprehensive oral care.

Self-Rating Perceived Health: The Influence on Health Care Utilization and Death Risk (자가건강인지도에 따른 3년간의 의료이용도와 사망위험 비교)

  • Objectives: This 3-year longitudinal study was conducted to evaluate the influence of self-rating health perception on health care utilization and all cause-death risk. Methods: The hypothesis was tested using a community-based samples, among which subjects 3,414 were interviewed in 1995, Self-rating health perception was assessed by single-item question. Three components of health care utilization amount(number of visits, number of medications, yearly health care expenses) per year were measured using medical insurance data during 3-year follow-up period among subjects in district health care insurance. There were 123 deaths from all causes among 3,085 subjects interviewed. Results: The results showed that those who had poor health perception revealed more increases in the amount of health care utilization than good health perception group (p<0.05). After adjusting for age and sex, the poor health perception group had higher death risk over 3 years than good health perception group(hazard ratio=1.88). but, after adjusting health care utility, supplementary, was not significant. Conclusion: These results suggest that self-rating health perception was associated with difference in health care utilization and all cause-death risk.

  • Using a method of Community-Based Research, Reconstructed action plaining for community change within the rural community is to mobilize and develop community resources, to develop and change community service delivery system, to communicate on community issues and advocacy. Research findings are as follows: Firstly, community outreach, its utility is finding some needy client within the lack of resources absolutely. Secondly, one is linking communities between a poor community and a wealthy community. Finally, one is building initiative social capital. The results suggest that social policy for rural community should built up service delivery system. It needs for local government to maintain community change by financial support. It has to link social welfare system and medical system.

The Effects of a Living-Lab Program on the Musculoskeletal Health Problems of Rural Women (농촌 여성의 근골격계 건강 문제 해결을 위한 리빙랩 프로그램의 효과)

  • Purpose: The purpose of this study is to develop a living lab program to solve the musculoskeletal health problems of rural women and analyze its effects. Methods: The subjects included eight rural women and this study involved pretest and posttest designs for a single group. The program ran from July to August 2020 and consisted of one in-person training session and three weeks of management. The effectiveness of the program was evaluated by the change in the degree of pain experienced in the wrists, shoulders, and back, along with the general health status of the subjects. The Wilcoxon Sign-Rank test was used in the analysis. In addition, the program satisfaction was analyzed with five items based on the factors of the health belief model. Results: While the program seemed to have no significant impact on the health status of the subjects, all the participants did report reduced pain in their wrists, shoulders, and lower back. The 'sensitivity' and 'cue to action' metrics also increased with participation in the program. Conclusion: This program was effective in relieving some pain associated with the musculoskeletal problems in rural women. Therefore, such programs should be sustained and spread around community organizations
