영국 음악치료 사례연구 - 자폐아 치료를 중심으로 - (THE CURRENT TRENDS OF BRITISH MUSIC THERAPY & TWO CASE STUDIES OF AUTISTIC CHILDREN)

  • 영국에서는 현재 음악치료가 자폐아 치료의 주요 수단으로 자리잡고 있고, 그 치료의 효과 또한 의학계와 교육계에서 널리 인정받고 있다. 이 글은 영국 음악치료의 주 동향이라 할 수 있는 즉흥연주를 바탕으로한 정신역동적 음악치료법(Psychodynamically Informed Improvisational Music Therapy)이 자폐증에 효과적 치료법임을 두 개의 사례연구를 통해 소개하고자 한다. 두 개의 사례연구는 매순간 개개인에 알맞게 응용되는 즉흥연주를 바탕으로한 음악치료의 과정이 어떻게 자폐아와 치료자간의 치료 관계를 형성시키고 발전시키는가를 보여준다. 첫번째 사례 Charlie를 통해서는 음악이 비언어적, 언어적 의사 소통과 초기 놀이발달 과정에 미치는 영향을 살펴보겠다. 두번째 사례에서는, 음악치료라는 환경이 어떻게 Mark에게 안전한 자기표현과 의사 소통의 장을 열어 주고, 자신과 다른 사람들에 대한 인식의 발달에까지 영향을 미치는가를 보겠다. 음악치료의 임상 과정에 대한 이해를 돕기 위하여 위의 사례는 발달적, 음악역동적, 정신역동적인 시점에서 고찰되어진다.

감각통합치료를 하고 있는 작업치료사의 놀이에 대한 인식 (Play Awareness of Occupational Therapists With Using the Sensory Integration Approach)

  • Objective : This study is to know about the play awareness of occupational therapists with using the sensory integration approach, and is to suggest educational strategies of play as occupation and research. Methods : 124 occupational therapists completed the questionnaires including the general use of play in occupational practice and the use of play assessments with preschoolers, and factors influence on the use of play in occupational therapy for children. Results : Respondents used the play as therapy modality and performed much with based on sensory integration and development approach in individual occupational therapy room with variable play tools. It was restricted to use as objective assessment tools and methods, and had important factors that influence on the use of play in occupational therapy for children; role of occupational therapy in work setting, personal frame of reference and availability of play assessments. Conclusion : The results suggest a need for increasing the emphasis on play in entry-level curricula and continuing education, improving clinician access to valid and reliable play assessments, and completing studies designed to examine the use and efficacy of play in sensory integration therapy intervention.

  • 장애자들에게 필요한 재활치료의 횟수와 시간은 실제 이용회수에 못 미치는 상황으로 재활치료의 관심과 콘텐츠에 지속적인 투자와 인프라가 필요한 상황이며, 또한 재활치료의 종류에도 미술, 음악, 놀이치료가 가장 일반적인 장애자 재활방법이다. 제안한 논문에서는 장애자의 동작을 인식하는 과정을 통하여 사운드와 시각적 요소를 포함하고 있는 게임형 재활 콘텐츠를 제공함으로써 다양한 유형의 장애자들이 콘텐츠의 레벨관리가 가능한 디지털 게임 재활프로그램을 설계한다.

  • 수부재활훈련도구는 저하된 수부 기능 회복 및 전체적인 수부 기능 향상을 필요로 하는 환자와 사용자들을 위해 병원 및 가정에서 치료와 함께 병행한다. 이에 따라 다양한 목적의 수부재활훈련도구의 개발 또한 함께 이루어 지고 있으며, 많은 도구가 보급되어 각 병원에서 규정한 프로세스로 재활훈련을 진행하고 있다. 그러나 현재 병원에서 활용하는 훈련도구들은 점진적인 시스템으로 구성되어 있지 않고 일정 시간동안 단순반복성 동작을 유도한다. 따라서 재활훈련을 하는 환자들이 무료함을 느끼며 재활에 대한 동기부여를 갖지 못하므로 지속적이고 효과적인 훈련이 어렵다. 또한, 훈련에 활용되는 도구들은 고가인 경우가 대부분이며, 공간적 제한성으로 인해 환자의 자발적 재활훈련을 어렵게 한다. 이는 환자의 경제적 부담을 가중시킴과 동시에 올바른 신체적 향상을 가져오지 못하는 문제로 이어질 수 있다. 이러한 논점을 바탕으로 본 연구는 놀이요소 중 하나인 실뜨기를 접목해 과제를 통한 효과적 재활훈련이 이루어질 수 있도록 하고, 경제적이며 자유로운 재활이 이루어질 수 있도록 하고자 한다. 이를 위해 도구의 특성과 사용자 환경을 조사한 후 실뜨기 놀이와, 훈련도구와의 관련성 및 지속적인 재활이 가능한지에 대한 타당성을 확인하고 전문가 인터뷰를 통해 디자인 요소를 도출하였다. 이후 디자인 가이드라인 및 프로토타입을 제작해 사용성 테스트 및 디자인요소 평가를 진행하여 문제점을 보완하였다.

재해를 당한 소아청소년에 대한 치료적 접근 (THERAPEUTIC APPROACH FOR CHILD AND ADOLESCENT AFTER DISASTER)

  • 본고는 최근 미국의 911 사태이후 정신의학 분야에서 관심사로 떠오르고 있는 재해정신의학 (Disaster Psychiatry)에대한 고찰을 통해 국내에서 대형재해 발생시 소아청소년 정신과 의사로서의 일정역할을 모색코자하는 목적의 일환이다. 우선 성인과 달리 소아청소년 재해환자에대한 치료를 다룰 때 어떤점을 고려하여야하는가에대한 연령 발달적 특이성를 살펴보고, 예방프로그램과 소아청소년 특유의 치료법은 무엇인가에대해 살펴보았다. 이를 요약하면 1) 소아는 타인에게 의존해야하는 존재로 부모 혹은 가족의 아이에대한 태도와 지지가 예후 결정에 가장 중요한 요소임으로 가족치료 부모교육의 중요성이 강조되며, 2) 개인치료에서는 인지행동적 요소가 여러 연구 문헌을 통해 가장 효과가 입증된 치료기법이고, 3) 놀이면담치료, fellow victim으로 구성된 집단치료 역시 효과적이며, 4) 1회성 재해를 당한 공포환자의 경우 특수기법인 EMDR(Eye Movement Desensitization and Reprocessing)의 시도가 추천되고, 5) 약물치료의 경우 2차적 혹은 보조치료수단이 되는데 성인에서 시도되는 다양한 약물의 적용이 가능하지만 부작용에대한 주의가 필요하다.

아동의 자아개념형성과 자아발달 촉진을 위한 집단모래놀이치료 효과 (The Effect of Group Sandplay Therapy for Children's Self-concept Construction and Ego-development Enhancement)

  • This study was conducted in order to inquire into the impact of children's self-concept construction and ego-development enhancement during Group Sandplay Therapy. The Group Sandplay Therapy sessions were held once week, for a total of 20 weeks. The Group Sandplay Therapy Process consisted of playing with sand and creating a sand tray in groups. There were two group in total. Each group had 4 children of the same sex aged from six to seven years old. In order to study the self-concept and ego-development, we used a self-concept test and ego-development as a research tool. In addition, the present research analyzed any changes which tool place by dividing each aspect in each sand tray of the therapy process into a positive subject and a negative subject, and analyzed the changing patterns seen in the sand tray worldas it unfolded. It was proven that an efficient treatment in changing the sand tray world and children's action, had an effect on rearranging the children's mental schemas. In terms of the progress of the sandplay journey, it was seen that negative elements decreased dramatically and positive elements were observed to have increased. As a result it was confirmed that Group Sandplay Therapy had a number of positive effects in the construction of children's self-concept and in terms of the enhancement of children ego-development.

국내.외 놀이치료 시설의 사례분석을 통한 사용 색채 특성에 관한 연구 (A Study on Used Colors Characteristic of Case Study for Children Play Therapy Center)

  • This study aims to be the useful resources for selecting play therapy facility in the future by analysis of used colors in the play therapy facility as the subsequent studies of the thesis of KIID in 2010, titled 'Study on the planning factors of case study for children's play therapy center'. As research method of this study, the overall understanding about play therapy facility was improved through theoretical considerations, and it was about the Moon-Spencer's theory of color harmony which will be used for the framework of case study. In Section 3, the field trips at 6 facilities were conducted as the case research and the data research through website was conducted for case study. Moon-Spencer's theory of color harmony used the Munsell's color Order System, so after measuring CMYK value of the picture for the case research through photo shop, the color, brightness and saturation were measured through Munsell Conversion program. The following three kinds of features were drawn by research of the harmony and disharmony of used colors for the cases after substituting the theory of color harmony through the measured color. brightness, and saturation. First. series Y and YR were used mainly for warm image as the concept of play therapy room. Second, the various colors were used for comfortable image like home. But the various patterns were used by mixing for the therapy room F, so it gives confusing impression. Third, series YR and achromatic color as the color of wood were used unavoidably for using materials of manufactured goods.

커플모래놀이치료가 대학생의 자아분화와 이성관계만족에 미치는 효과 (The Effect of Sandplay Therapy for Couples on College Students' Self-Differentiation and Heterosexual Relationship Satisfaction)

  • This study was conducted to investigate the effect of sandplay therapy for couples on the self-differentiation, and heterosexual relationship satisfaction, of couples who are college students'. The subjects were four couples who are college students living in Seoul with low self-differentiation and heterosexual relationship satisfaction. I carried out sandplay therapy on four couples for once a week for 10 weeks, and each session was 70 minutes long. In this study, the self-differentiation scale was modified for Korea on the basis of Bowen's family systems theory (1976), and it was used as a research tool. Yu (2000) and Lee (2000) made a scale to measure heterosexual relationship satisfaction after they adjusted the Korea Marital Satisfaction Inventory (K-MSI). The scale was also used in order to compare pre-therapy and post-therapy traits and identify any changes. In addition, the group's behavior and interactions were observed and recorded in each session throughout the process of sandplay therapy with the couples, and the behanior and interactions were analyzed along with the change flow in the sand trays. The study's outcomes are summed up as follows. First, sandplay therapy with couples advances the level of self-differentiation in college students. Second, sandplay therapy with couples improves the heterosexual relationship satisfaction on college students. Third, the trays used in sandplay therapy with couples showed positive changes as the therapy progressed. As the sessions continued, negative factors in the trays decreased and the trays converged into one. As the sessions proceeded, it was observed that the couples became closer, felt more comfortable with each other, compromised and reflected on their relationship. The study's results show that sandplay therapy with couples increased the level of self-differentiation in college students and heterosexual relationship satisfaction, thus proving the therapy's effectiveness.

자녀가 상담을 받고 있는 부모의 양육스트레스와 타액 코티졸에 미치는 모래놀이치료 효과 (The Effects of Sand Play Therapy on Parenting Stress and Saliva Cortisol Levels of Parents Undergoing Child Counseling Programs)

  • The purpose of this study is to investigate the effects of sand play therapy on the parenting stress and saliva cortisol levels of parents undergoing child counseling programs. The study was conducted with 13 parents who were undergoing child counseling programs. The sessions were held every week for 45 minutes from July, 2011 to September, 2011. To evaluate the effects of sand play therapy, pre test and post test were conducted and the results were then analyzed. The therapy sessions consist of parents making sand boxes, and this was done without consideration of any particular theme. The instrument used was Abidin (1990)'s Parenting Stress Instrument (PSI). Saliva cortisol levels were obtained at the pre-post stage of the sand play therapy. The data were analyzed by means of frequency and the Wilcoxon Rank Sum Test was conducted by the SPSS. The major findings were as follows; There were significant differences in terms of the decreases in the areas of perception of all the parenting stresses examined, including daily life stress, child temperament stress, child relationship stress, and learning-expectation stress, as well as in saliva cortisol levels. Finally, this study suggests that parenting stress and saliva cortisol levels have a positive relationship, and the effects of sand play therapy were significant with decreases in parenting stress and saliva cortisol levels. There is a clear need for parents undergoing child counseling programs to engage in sand play therapy to decrease parenting stress and saliva cortisol levels.

애착문제 유아 가족을 위한 인형을 이용한 가족놀이치료 프로그램의 개발 및 효과 (The Development and Effectiveness of a Family Play Therapy Program Using Puppets for Families with Children Having Attachment Problems)

  • The purpose of this study was to develop a "Family Play Therapy Program" using puppets to decrease attachment problems for families with children having attachment problems and to examine its effectiveness. The participants were an experimental group of 16 individuals(8 children with attachment problems whose ages ranged from 4 to 5 and 8 mothers) and a control group of 16 individuals (8 children with attachment problems whose ages ranged from 4 to 5 and 8 mothers). The experimental group was treated through the Family Play Therapy Program, which involved the use of puppets and was held for twelve sessions twice a week. The changes in the participants were measured in order to examine the effects of the program. The researcher measured children's emotional and behavioral expression(TBP), the mothers' sense of self(Self-Differentiation Scale), personal relationships(ECR-R), and the perception of family functions(ICPS-FFS) both before and after the FPT program and compared their differences. The MIM Rating Scale and Marschak Behavior Rating Scale were administered to examine the interactions between mothers and children, and 1:1 interviews were also conducted. The data thus gathered were used for non-parametric analysis(Mann-Whitney U test and Wilcoxon rank sum test)using SPSS WIN 17.0. The results of this study were as follows: First, the program had a positive effect on children's emotional expression. After the problem were over, negative emotional and behavioral expression in the experimental group decreased. Second, the program had a positive effect on mothers' self-differentiation and personal relationships. Third, the program had a positive effect on changes in the interaction behaviors between the mother and child. Fourth, the program had a partially positive effect on the responses from their group developmental stages, especially on the subscale of both a program for the reinforcement of mothers' emotions and family play program utilizing puppets.
