  • The author reviewed history and development of variable approaches of play psychotherapy. To understand basic concepts and practical factors of play, the functions of play behavior and it's clinical application were summarized. As major therapeutic approaches, both theoretical concepts and practical techniques of psychoanalytic approach and those of nondirective approach were presented. For clinician it is important to know developmental theories which can be applied to various therapeutic technics and to apply these theories to play therapy situations. So, to know the developmental approach in play psychotherapy, play as a developmental process, its' therapeutic application, play in the perspective of cognitive development and the therapeutic process of play psychotherapy were reviewed. With the knowledge of developmental concept therapists can increase their abilities to prescribe an appropriate type of play therapy.

  • A 8 year old boy with problems of hyperactivity, impulsivity received playpsychotherapy. Through the detailed examination of their contents and group supervision of play therapy session, common mistakes novice therapist make frequently and tasks in the initial phase of play psychotherapy are illustrated. Therapist's personality and anxiety frequently interferes therapeutic processes. Common mistakes therapist makes during initial phase of play therapy are ; therapist's over-enthusiasm, overinvolvement or non-responsiveness, premature or inappropriate interpretation and direct connection of play theme to reality. It is utmost important for the therapist to make positive and trusting relationship with the patient in the initial phase of the therapy. In order to do so therapist must accept the patient as he is, permit the patient takes his leads. And as a result, the Patient can go through corrected developmental experience. In conclusion, it is a therapist's main task to establish relationship.

  • The study presented the experience of playtherapy of a 6-yr old girl with elective mutism and attempted to understand her psychopathology and formulated her psychodynamic which was expressed though the therapeutic process of playtherapy in the viewpoint of developmental model. Therapist's attitude was nondirective and supportive. Main theme of her play, fantasy and art was separation anxiety and abandonment fear as the result of continued psychic traumas such as separation from her grandmother and physical abuse by her father and mother etc. So she couldn't achieve developmental tasks such as autonomy and separation individuation and became generally inhibited and selectively mute. The process of play therapy was summarized and coceptualized as her successful progress through sequential developmental phases within the therapeutic relation ship. By reexperiencing this developmental process, she could accomplish her autonomy and separation individuation and developed to oedipal stage and successfully resolve oedipal conflict.

TREATMENT OF REACTIVE ATTACHMENT DISORDER (반응성 애착 장애의 치료)

  • Most articles of Reactive Attachment Disorder reported Disinhibited Type children adopted from other countries. Reports from only Turkey and Korea focused on Inhibited Type of Reactive Attachment Disorder children whom raised by their own parents and whose symptoms are very similar to Autistic Disorder. Since articles of treatment of Reactive Attachment Disorder, especially for Inhibited Type are very rare, this article informed the author's experiences of treatment for Korean Reactive Attachment Disorder children since 1987. To treat Reactive Attachment Disorder patients and their parents, three important areas must be included : 1) to make a therapeutic environment for a Reactive Attachment Disorder child, 2) to make an attachment between Reactive Attachment Disorder child and his/her mother through individual play therapy, filial therapy, and group therapy with sibling or peer, 3) to catch up developmental delay by speech therapy, cognitive therapy and therapeutic education. This treatment methods can be more easily and more effectively applied to Korean patients than other methods from western countries including USA or England.

The Clinical Effect of Sandplay Therapy on Adolescent Behavior and Emotion - Through Minesota Multiphasic Personality Inventory (모래놀이치료가 청소년의 행동, 정서에 미치는 임상효과 - 다면적 인성검사를 중심으로)

  • The sand play therapy showed a clinical treatment effect to such children with lots of problem behaviors at the previous studies. This study aimed to evaluate the clinical effect to emotional and behavioral problems with 20 adolescents after 10 weeks group sand play therapy. After sand play therapy, the subjects showed a significant effect such that the MMPI depression scale score, somatic symptoms complaints, social discomfort, psychomotor retardation, somatic dysfunction, affection desire, bizarre sensation experience, shyness/self-consciousness were decreased from baseline to 10 weeks of the study. We suspected that the sand play caused to clinical effect to physical problems as well as emotional problems of the adolescents with school maladaptation.

COMPARATIVE STUDY OF CHARACTERISTICS IN RORSCHACH RESPONSE AND SANDPLAY BETWEEN ADHD AND NORMAL CHILDREN (ADHD 아동과 정상아동의 Rorschach반응 및 모래놀이치료 내용 비교)

  • Objectives : Present study compared the characteristics in Rorschach responses and Sandplay between ADHD and normal children group. Method : ADHD (excluded inattention type & emotional problem) group and normal group was composed of 10 children respectively. All of them were controlled by Intelligence Quotation. Rorschach responses were scored by Exner system and compared by t-test. For sandplay work, the time was measured and photo was taken. Results : 1) ADHD group showed that the reaction time in Rorschach was significantly fast and they easily failed to recall the responses. The statical significances are 3r+ (2)/R, X-%, Xu%, (2), AG, Afr, SCZI, Wsum 6, FD, Zd, Blends, a. 2) ADHD group showed that the reaction time in Sandplay was significantly fast, they used low materials (total numbers of toy, vehicles) and the work was less coordinated. Conclusion : This results suggest that ADHD children are apt to react, think and coordinate impulsively. Also, Sandplay could be used fur diagnostic method as Rorschach test.

Mediation Effect of Play on the Relationship Between Sleep Habits and Cognitive Problem-Solving in Toddlers (유아기 아동의 수면 습관과 인지적 문제해결 능력의 관계에서 놀이의 매개효과 )

  • Objective : This study aimed to investigate the mediating effect of play on the relationship between toddlers' sleep habits and problem-solving. Methods : In total, 1,734 participants were selected from the 3rd wave of the Panel Study on Korean Children. A structural equation modeling approach was utilized to examine the relationship among toddlers' play, sleep habits, and problem-solving, as well as to investigate the mediating effect of play. Results : The monthly age of the study participants ranged from 23 to 32 months, with 885 (51.0%) boys and 849 (49.0%) girls. The indirect effects of play on problem-solving skills (β = 0.137, p = .006) were statistically significant, but the direct effects of sleep habits on problem-solving skills (β = -.015, p = .871) and the total effect (β = 0.122, p = .057) were not significant. Conclusion : This study indicated that sleep habits did not have a direct effect on problem-solving ability, but that the indirect effects were significant and fully mediated by play. Incorrect sleep habits can negatively affect lifelong development. Therefore, parents would need to be aware of whether their child is developing good sleep habits during the toddler age.

  • The social attention about disaster psychiatry was increased after 911 terror in New York. The role of child psychiatrist and specific consideration for the treatment of child victim in disaster were reviewed. The following were main points. 1) The most single determining factor of prognosis is supporting system and parental attitude to their child victim. So family therapy and parental eucation are needed. 2) Cognitive Behavior Therapy is known to the most effective treatment in many literature. 3) Brief group therapy with fellow victim is cost effective preventive methods and screening tool for more serious victim, 4) Eye Movement Desensitization and Reprocessing(EMDR) could be a very amazing method in reducing repetative horrible traumatic image. 5) Many kinds of drug using in adult are considered with caution.

  • This study was purposed to find out clinical characteristic of intra and extrafamilial sexual abuse, nature of offenders, difference of symptoms describe that sexually abused children and adolescents were experienced. Subjects were 10 sexually abused children and adolescents who visited Seoul National Mental Hospital, from Jan. 1992 to Dec. 1993. In intrafamilial sexual abuse, they had more chronic course than extrafamilial abuse and had more family psychopathology. In symptomatology, The preschool children mainly presented to somatic symptoms and anxiety, and The school children prominently exhibited depressive reaction and withdrawal. Most common reaction of parents was anger and guilt. They were treated with crisis intervention, play therapy, and hospitalization, if needed. The maintenance of treatment was difficult due to parent's avoidance and repression.

  • The purpose of this paper is to introduce the current trends of British Music Therapy and its effectiveness as a therapeutic intervention for the autistic children. This paper consists of two parts;Part one will provide a general picture of music therapy by looking at its fundamental concepts and therories. Part II(two case studies) will illustrate the actual processes of music therapy which can engender a therapeutic relationship between the therapist and the autistic child. Through the first case study of Charlie, a 3 year and 9 month non-verbal autistic boy, we shall see how music therapy facilitates both non-verbal and verbal communication and a form of pretend play. The second case of Mark, a 10 year old autistic boy with complex emotional, behavioural and learning difficulties who had been sexually abused in the past, presents how the therapist’ acceptance of the child’ being and the shared experience of music therapy enable the child to express his difficulties and to develop the awareness of self and others. By exploring both musical and interpersonal aspects of music therapy, this gives an in depth examination of therapeutic processes. In order to clarify clinical procedure, this paper is viewed in musical, developmental and psychodynamic perspectives.
