Experience as a Vulnerable Elderly Individual with Diabetes (당뇨병을 가지고 살아가는 취약계층 노인의 경험)

  • Purpose: This phenomenological study was conducted to describe and understand the experience of vulnerable elderly individuals with diabetes by identifying the meanings and structures of the experience. Methods: The data were collected through in-depth interviews of six vulnerable elderly individuals with diabetes aged over 65 years. The interview data were audio-recorded, transcribed verbatim and checked for accuracy. The Colaizzi's method of phenomenology was used to analyze the data. Results: Four theme clusters were extracted as follows: 'Life adversity', 'Adaptability to the life', 'Replacement of absence of family with social support', 'Difficulty of self-care'. Conclusion: The results provide an in-depth understanding of life experiences of vulnerable elderly individuals with diabetes. The findings will be useful to nurses caring for this population.

Factors Affecting Highly Educated Elders' Diabetic Health Leader Attitude (고학력 노인의 당뇨병건강리더 태도에 영향을 미치는 요인)

  • Purpose: This study was done to identify the relationships among diabetic health leader attitude, diabetic knowledge, and health conservation in highly educated elders. Methods: The participants were 119 elders aged 65 or more who were attending the senior welfare centers in D and K City and had at least a high school degree. Data were collected from June 10 to July 7 in 2013. Data analysis included one-way ANOVA, independent t-test, Pearson's correlation, and stepwise multiple regression, done with the SPSS/WIN 19.0 program. Results: A positive correlation was found among diabetic health leader attitude, diabetic knowledge, and health conservation. Health conservation explained 26% (${\beta}$=.31) and diabetic knowledge explained 2% (${\beta}$=.18) of the variance in diabetic health leader attitude. Conclusion: The results indicate that to increase diabetic health leader attitude among highly educated elders, health leader programs are needed for increasing their diabetic knowledge and health conservation.

Relationships of Motivational Factors and Diabetes Self-management Behavior in Community Dwelling Older Adults (당뇨병이 있는 노인의 행위 변화 동기 요소와 자기관리 행위)

  • Purpose: Diabetes Self Management Behavior (DSMB) is crucial for the elderly with diabetes to prevent diabetes complications and to improve their quality of life. The Purposes of this study were to investigate the current status of DSMB and to identify motivational factors related to DSMB in community dwelling older adults with diabetes. Methods: The subjects were 150 diabetic elderly who visited 2 community senior centers in S city. DSMB scale consisted of 5 sub-domains; Being active, healthy eating, regular medication, glucose monitoring, and foot care rated by a scale with a range of 0 to 7. Personal motivation(i.e., intention to behavior) and social motivation including family support and health professional support were measured. Results: The mean score of DSMB was 4.27. The mean score of intention to behavior was 2.52. DSMB was related to Intention to behavior (r=.461, p<.001), family support (r=.342, p<.001), and health professional support (r=.284, p<.001). In regression analysis, a total of 33.4% of variance in DSMB was accounted for by intention to behavior, family support, and health professional support. Conclusion: To improve DSMB of the elderly, diabetes educator should consider on the strategies across both personal and social motivation related to DSMB.

Nutritional Status and Health Risks of Low Income Elderly Women in Gwangju Area (광주지역 저소득층 여자노인의 영양상태와 건강위험요인에 관한 연구)

  • This study was performed to identify association between nutritional status and health risks of the elderly. This was a cross-sectional study involving low income elderly women in Gwangju, Korea (${\geq}$65y, n = 92). Socio-demographics, life style characteristics, health conditions, dietary intakes based on 24h-recall method, anthropometric measures, and clinical biochemistry parameters were examined. Anthropometric and clinical parameters included wt, ht, waist, hip, body protein, body fat, abdominal fat, total cholesterol, HDL-cholesterol, triglyceride, total protein, albumin, hemoglobin, hematocrit, fasting blood glucose, ferritin, IL-2, IL-6, TNF-${\alpha}$, CRP, TAS, TBARS, systolic blood pressure, and diastolic blood pressure. The subjects were divided into three groups based on age (65-74y, 75-84y, 85y${\leq}$) and were divided into two groups according to the sum of the Nutrition Screening Initiative (NSI) checklist score (adequate nutritional status, NSI score ${\leq}$3; at risk of malnutrition, NSI score >3). Mean and frequency of variables were estimated. Analysis of Variance, Tukey test, Chi-square test, and Multiple linear regression analyses were performed. Mean BMI and body fat were 25.1 $kg/m^2$ and 40.0%, respectively. However, for over 80% of subjects, the intakes of energy, fiber, thiamin, riboflavin, niacin, folate, Ca, K, and Zn were less than the Korean DRI (EAR or AI). The subjects who had lower NSI score tended to have better health status, eat meals frequently, have less depression, and exercise regularly. The subjects who had higher NSI score tended to have tooth problems, to eat alone most of time, and to be physically unable to cook or feed. Serum IL-6 and TNF-${\alpha}$ were significantly related with nutritional status which suggested higher tendency of inflammatory response. Serum IL-2, TAS, and glucose were significantly correlated with body fat (%) or abdominal fat (%). These results suggest that improving the nutritional status, increasing regular exercise, maintaining normal weight are beneficial to health care of low income elderly women.

System Thinking on Diabetes Prevention and Management (당뇨병 예방 및 관리의 시스템 사고)

  • This research aims to identify important variables and their effects on diabetes prevention or progression from prediabetes to diabetic complication using system thinking. Based on the existing studies, we have constructed a causal loop diagram explaining dynamics of diabetes and have found 7 important reinforcing loops and 3 balancing loops in the causal loop diagram. The CLD displays an effect of increasing prediabetes early detection and awareness on diabetes prevention. The findings indicate that the projects in Korea for reducing incidence of diabetes and potential risk of complication have focused on variables which are related only on diabetes even though prediabetes has been a critical point on diabetes prevention and management. Thus, it also concludes that the effect of projects focusing only on diabetes have faced limitation to manage diabetes accordingly.

당뇨병 돋보기 II-당뇨병성 급성 합병증-케톤산증

  • 당뇨병환자에서 발생하는 가장 중요한 급성 합병증인 당뇨병성 케톤산증이나 고삼투성 비케톤성 혼수는 인슐린의 투여를 중단하였거나 감염, 외상 등의 신체적 손상이나 정신적 스트레스에 의하여 인슐린에 대한 길항 호르몬들이 과분비돼 발생한다. 일반적으로 당뇨병성 케톤산증은 인슐린 의존형 당뇨병환자 중 주로 젊은 층에서 자주 나타나며 병의 경과가 빠르게 진행되기 때문에 고삼투압 상태보다는 케톤산혈증이 주요 문제가 된다. 반면에 고삼투압성 비케톤성 혼수는 인슐린 비의존형 당뇨병환자 중 주로 노인층에서 호발하며 병의 경과가 서서히 진행하기 때문에 심한 고혈당 상태로 인한 고삼투압 상태가 주요 문제가 된다. 이와같은 당뇨병의 급성 대사성 합병증들은 치료가 지연되거나 적절한 치료가 행해지지 못했을 때는 예후가 극히 불량하며 사망률 또한 높은 것으로 보고되고 있다. 이에 급성 대사성 합병증 중 당뇨병성 케톤산증의 병인, 진단 및 치료 등을 알아보기로 한다.

A comparative study for the development of a health promotion policy through an analysis of the mortality rates in Korea and Japan, $1983{\sim}2003$ (한국과 일본의 사망률 비교 분석을 통한 건강증진정책 함의)

  • This study attempts to provide fundamental information for a health policy and health services by looking at the trends and types of the mortality rates in Korea and Japan. In this study, data of the death statistics of Korea and Japan over the 21-year period from 1983 to 2003 are analyzed. Mortality data are standardized ${\times}100,000$ to the 2003 Japan population, according to the direct method. In 2001/2003, the ranking of major causes of death in Korea and Japan has been greatly changed. In Korea, mortality rates from malignant neoplasms, diabetes mellitus, renal failure, falls and suicide have increased. In Japan, the mortality rates from pneumonia increase more than those from diabetes mellitus. In 2001/2003, the proportions of the mortality rates from chronic diseases are higher than those from acute diseases. In the Korean health promotion policy "Health Promotion 2020", a more intensive goal management is needed.

The Comparison of Health Status and Health Behavior among Hypertension Group, DM Group, and Hypertension DM Group for the Aged Provided with Customized Home Care Service by Visiting Nurses (맞춤형 방문건강관리사업 노인대상자의 고혈압군, 당뇨군, 고혈압.당뇨군별 건강상태 및 건강행태 비교)

  • Purpose: This study aims to compare health status and health behavior among the hypertension group, the DM group, and the hypertension-DM group for aged clients of customized home visiting health care services. Methods: This study was conducted as cross-sectional research. The subjects of this study were 2,235 aged people over 65 living in J City. Data were collected using structured questionnaires and measurements. The collected data were analyzed using the SPSS/WIN 12.0 program, and descriptive statistics, $x^2$ test, t-test and ANOVA were used for the analyses. Results: BP and glucose control showed a significant difference among the groups. BMI and waist circumference were significantly higher in the hypertension-DM group than in the other groups. Stroke, arthritis, and perceived health status were significantly different among the three groups. Depression was high, but not significantly different among the groups. Smoking and drinking were not significantly different among the groups. Physical activity was very low, especially in the hypertension group. Medication was high, especially in the hypertension-DM group. Conclusion: It is necessary to consider care plans for the hypertension-DM group, and educate the group for care management. Also, depression and physical activity programs are needed for the age.

Effects of Diabetic Foot Care Education for the Aged with Low Health Literacy (건강정보 이해능력이 낮은 당뇨병 노인을 위한 발 관리 교육의 효과)

  • Purpose: The purpose of this study was to examine the effects of diabetic foot care education for the older adults with low health literacy. Methods: A quasi-experimental design with a non-equivalent control group pretest-posttest was used. The participants who were diagnosed with diabetes, were adults over 65 years old at the welfare center of Y and B city. They were divided into the experimental group (n=32) and the control group (n=31). Inclusion criteria were a score of 5 or under on the Short form of Korean Functional Health Literacy Test and 24 or more on the Korean version of Mini-Mental State Examination. Foot care education was conducted in a small group for 40 minutes, once a week, for three weeks. The education materials are composed of an easy term, picture and photographs to understand easily. Results: The scores of diabetic foot care knowledge (t=4.57, p<.001), foot care self-efficacy (t=6.07, p<.001), and foot self-care behavior (t=4.18, p<.001) were significantly increased in the experimental group compared to the control group. Foot health status was not significantly improved. Conclusion: The findings indicate that this education program can be used as a nursing intervention improving foot care knowledge, foot care self-efficacy, and foot self-care behavior in order to prevent the diabetic foot problems of elderly diabetic persons with low health literacy.
