우리나라는 65세 이상 노인인구가 전체인구의 9.5%로 이미 '고령화사회'로 진입하였다. 현재 그 속도가 더욱 빨라져 이제는 '초고령사회'로 나아가고 있는 실정에서 노인들을 위한 전문 의료시설이 절실하다. 이에 발 맞춰 노인성 질환을 중심으로 진료하고 어르신들을 위한 특성화된 치료를 제공하고 있는 노인전문병원이 있어 찾아가 봤다.

According to the rapid increase of the elderly population, especially frail older population, many kinds of elderly care facilities have been supplied within a relatively short period. Among them, elderly hospitals and elderly welfare facilities have occupied a major portion. The elderly hospital, which had emerged from 1994, has recorded sharp increase in facility numbers and bed numbers by the support of Korean government together with the increase of care demand. However, the concept and fundamental planning criteria of elderly hospitals have not yet been set up. This paper has derived the concept of the elderly hospital from the Medical Law and Elderly Welfare Law, and prospected the supply of it from domestic and international statistics. Also this paper has explored the fundamental design issues of elderly hospitals by analysing precedent studies and designs, and by surveying some facilities.

이 논문은 노인의료복지시설의 현황과 시설체계에 관한 논문으로 부산시를 대상으로 하였다. 현황 분석과 설문조사을 통해 그에 따른 문제점을 분석하고, 해결방안으로 새로운 유형의 시설체계를 제시하였다. 현재 노인의료복지시설들은 입소대상자들을 다양하게 수용하고 있지 못하며, 각각 독립적으로 운영되고 있어서 한 시설 안에서 전문적인 치료와 요양을 동시에 할 수 없다. 이러한 문제점들을 토대로 본 논문에서는 그 해결방안으로 좀 더 넓은 범위에 걸쳐서 한 장소에서 요양과 전문적인 치료가 가능한 'one-stop service system' 체계를 제언하였다. 설문조사 결과들을 종합하여, 구상한 5가지 유형의 시설체계는 다음과 같다. 1) 무료전문요양시설 + 노인전문병원, 2) 유료전문요양시설 + 노인전문병, 3) 노인전문병원 + 실버타운 주거공간 형식의 입원실, 4) 실비전문요양시설 + 노인전문병원 + 실버타운 주거공간 형식의 입원실, 5) 노인전문병원 + 재가복지서비스. 노인관련시설은 더 이상 특수한 시설이 아니라, 누구든 언젠가는 이용 하게 될 시설이라는 점에 착안할 때, 그에 대한 대책 마련은 시급하다고 생각된다. 앞으로 노인의 질환의 종류 및 정도가 보다 중복, 중대화 될 것으로 보인다. 따라서 보다 다양하고 전문화된 시설이 요구될 것으로 예상된다.

According to the increase of medical demand for aged, many of geriatric hospitals have been needed. But Korea has been short of them and has not prepared the planning guidelines. This paper aimes to identify the design concept of geriatric hospitals and provides the basic data for architectural planning of these hospitals.

2008년에 장애인차별금지법이 제정되었고, 2013년 4월 11일부터 웹 접근성 준수가 시행되었다. 따라서 모든 법인과 단체는 2015년까지 웹 접근성 준수를 충족해야 한다. 본 논문은 한국형 웹 접근성 평가도구인 K-WAH 4.4를 적용하여 노인전문요양병원의 웹 접근성을 평가하였고 그에 대한 개선 방안을 제안하였다. 웹 콘텐츠 접근성지침 2.0에 따라 노인전문요양병원을 평가한 결과 매우 낮은 웹 접근성 준수율을 보였다.

Considering the rapid growth of elderly population and the increase of medical demand for aged, many of geriatric hospitals are needed in Korea. On the other hand, supplies of hospitals for acute care are considered to be excessive. So many of small and medium hospitals facing management problems have been converged to geriatric hospitals. These attempts are considered resonable for efficient utilization of health care resources in Korea. This paper aims to identify the concept of geriatric hospital and to analyze the problems and then to seek the alternatives for architectural planning on the basis of surveying the geriatric hospitals converged from hospitals for acute care.

본 연구는 노인전문병원 간호사의 노인에 대한 지식, 태도 및 간호실천과의 관계를 확인하고 간호실천에 미치는 영향요인을 파악하기 위하여 시도되었다. 자료수집은 2013년 3월 17일부터 4월 20일까지 D시와 K도 노인전문병원 간호사 188명을 대상으로 하였다. 연구도구는 노인에 대한 지식, 태도 및 간호실천을 이용하였다. 자료분석은 SPSS와 Amos 프로그램을 이용하여 빈도분석, 평균과 표준편차, t-test, One-way ANOVA, Pearson's correlation coefficients, multiple regression analysis, 최대우도법을 이용해 분석하였다. 대상자의 노인에 대한 지식은 1점 만점에 0.49점, 태도는 7점 만점에 3.72점, 간호실천은 4점 만점에 3.54점으로 나타났다. 대상자의 연령, 총 임상경력, 최근 노인관련 교육 경험, 노인과 동거 경험에서 간호실천 차이가 있었다. 대상자의 노인에 대한 지식, 태도 및 간호실천은 정적 상관관계가 있었다. 노인간호 실천에 영향을 미치는 요인은 노인에 대한 지식, 태도, 최근 노인관련 교육 경험으로 나타났다.
