Factors affecting HRQOL in Elders with Hearing Impairment : Based on the 2018 KNHNES (청력저하 노인의 건강과 관련된 삶의 질 영향요인: 2018 국민건강영양조사 자료를 이용하여)

  • The Purpose of this paper was to explore the effect of hearing impairment on HRQOL in Korean elders. We carry out a cross-sectional analysis using nationally representative data from the KNHANES, 2018. A total of 528 elders who had hearing impairment were included. Statistical analysis was performed using SPSS 25.0, χ2-test, t-test, correlation, and regression analysis to confirm the quality of life, and the influencing factors. There were significant differences in HRQOL according to gender, age, education, marital status, income, occupation, number of household members, tinnitus, number of comorbidities, smoking status, alcohol drinking status, subjective health status, regular exercise, restriction of activity, depressive mood and perceived stress. The results indicated that age, BMI and number of comorbidities had a significant negative association with HRQOL. In addition, marital status, number of comorbidities, body mass index, subjective health status, regular exercise, restriction of activity and perceived stress were predictors of life quality in elders with Hearing Impairment and the variable in the health-related quality of life was explained by 37.3%. In order to improve the HRQOL of elders with hearing impairment, multidisciplinary efforts and development of educational programs are required.

Influence of Fall-preventive Occupational Therapy Applied to Elderly in the Community upon Balance Ability (여성노인에게 적용한 낙상예방 작업치료가 균형 능력에 미치는 영향)

  • This study is to look into the effectiveness of fall-preventive occupational therapy of elderly by clarifying the influence of occupational therapy applied to female elderly in the community upon balance ability. A 6-week occupational therapy program including both training and exercise for prevention of fall down was applied by dividing into 13 persons of experimental group and 13 persons of control group for female elderly aged more than 65 years who were capable of independence gait and have not taken any drug to adversely affect balance. For balance ability assessment, the Berg balance scale and the Tetrax fall index were used. The Berg balance scale in experimental group was shown to be 48.38 to 50.69 through the 6-week program, indicating changes in significance level(p=.017). The Tetrax fall index in experimental group was shown to be 46.77 to 35.54 through the 6-week program(p=.000). Changes in significance level were not found for both the Berg balance scale and the Tetrax fall index in control group. As seen from the results of this study, it is found that 6-week application of the fall-preventive occupational therapy program including fall prevention training and exercise was effective for promotion of elderly's balance ability, and further studies just on the effect of fall prevention training are required to be made.

A Preliminary Study on a Performance-Based Cognitive Function Test : With the Normal Elderly (수행기반 인지기능평가를 위한 예비연구 : 정상 노인을 대상으로)

  • Objective: The purpose of this study is to conduct a preliminary research for the development of a Performance-Based Cognitive Function Test (PCFT) to screen the elderly for cognitive function impairment, and examine the reliability and validity of the test. Methods: A draft version of the Performance-Based Cognitive Function Test (PCFT) was developed and utilized in nine healthy elderly individuals. In order to verify its reliability, we analyzed the internal consistency of the PCFT. In order to verify the concurrent validity of the PCFT, this study analyzed the correlation between motor-cognitive dual task assessments and the Korean version of the Mini-Mental State Examination (MMSE-K). Results: The internal consistency of the PCFT for motor and cognitive tasks was 0.871 and 0.959 (Cronbach's ${\alpha}$), respectively. Concurrent validity of the PCFT, which was performed through motor-cognitive dual task assessments, ranged from 0.755 to 0.964 (Spearman's rho statistic, p < 0.05). In addition, correlation between the cognitive assessment tool and the MMSE-K ranged from 0.849 to 0.943 (p < 0.01). Conclusion: This study verified, and established the reliability and validity of the PCFT. Further studies are required to examine other psychometric properties in a modified PCFT, for screening cognitive function impairments in the elderly.

Development of community-based intensive health care program for the community dwelling elderly (재가노인을 위한 지역사회 중심의 집중건강관리프로그램의 개발과 적용)

  • The purpose of the present study was to develop a community-based intensive health care program for the community dwelling elderly to strength their functional status and to verify the effect on their geriatric syndrome. A one-group pretest-posttest design was used for the study. A total of 69 frail elderly, who lived in the area within 20 minutes by car, were committed themselves to the day care center(Sangikjae), and had the ability of verbal communication were selected from G city in Kyunggi province. The participants completed a set of questionnaires to measure the sub-score of frailty, fall, urinary incontinence, malnutrition, and mild cognitive disorder domain, using the Otasha-Kensin through the physical examinations and interviews. After 4 weeks of intervention, the outcome was measured to evaluate the effects of the program, and the data obtained were analyzed using descriptive statistics, paired t-test and McNemar test. The results showed that the sub-score of frailty, fall, urinary incontinence, and malnutrition domain were statistically significantly decreased after intervention except those of urinary incontinence and mild cognitive disorder domain, implying that the risk of frailty, fall, and malnutrition was decreased. These findings indicated that community-based the intensive health care program is effective for relieving geriatric syndrome of the community dwelling elderly.

Effect of Heat Stress of Extreme Heat Lever on Muscle functionand Muscle Injury Markers in Elderly Women (열 스트레스가 여성노인들의 근기능 및 근손상에 미치는 영향)

  • The purpose of this study was to examine the influence of extreme heat on muscle function and muscle injury marker in elderly women. The subjects of this study were eight post-menopausal elderly women without any metabolic disease. All eight subjects were asked to perform the knee joint isokinetic exercise using isokinetic equipment (cybex) in the laboratory and experimental temperature within laboratory was adjusted to two conditions: extreme heat temperature(33±0.5℃) and normal temperature(20±0.5℃) maintained in 50±3% humidity conditions. Each experimental exercise was monitored and analyzed the change of HSP70, LDH and CK. Muscular functions (peak torque, total work, percentage of peak torque body weight, fatigue index, average power and total work) were significant differences at exercise between temperatural conditions (p<.05). In extreme heat temperature, muscular injury markers (HSP70, LDH and CK) were increased, threfore resulted in significantly higher than normal temperature(p<.05). These results show that extreme heat temperature can decrease muscle function in elderly women.

Biomechanical Alterations in the Lower limb Joints during the Punching Motion of Elderly Women after 12-Weeks of Taekwonaerobics Training (여성노인의 태권에어로빅스 12주 훈련 후 몸통지르기 동작시 하지관절의 생체역학적 변화)

  • The purpose of this study was to investigate the biomechanical alterations in the punching motion of 10 elderly women after 12 weeks of taekwonaerobics training. Seven infrared cameras(Qualisys MCU-240) and 2 force platforms(Kistler-9286AA) were used to acquire raw data. The results were as follows. First, the minimum joint angles of the lower limbs had a statistically significant difference between both the dorsiflexion/plantar flexion(1eft, $p=0.001^*$) and the inversion/eversion(both, $p=0.009^*$, $p=0.04^*$) from the ankle angle. There were differences in abduction/adduction(left, $p=0.04^*$) from the knee angle, as well as internal/external rotation(both, $p=0.07^*$, $p=0.02^*$) from the hip angle. Second, the maximum resultant joint moments of the lower limbs had statistically significant differences in the inversion/eversion moment from the ankle joint(both, $p=0.05^*$, $p=0.05^*$), the abduction/adduction moment(left, $p=0.08^*$) from the knee joint, and the internal/external rotation moment(right, $p=0.09^*$) from the hip joint. Third, the maximum resultant joint powers of the lower limbs had a statistically significant difference both in flexion/extension joint powers(both, $p=0.05^*$, $p=0.01^*$) and in abduction/adduction(both, $p=0.02^*$, $p=0.00^*$) from the hip joint, as well as abduction/adduction(left, $p=0.00^*$) from the knee joint, In conclusion, the elderly women were somewhat changed after 12 weeks of taekwonaerobics training.

Changes in Life Habits of Health Associated with Participation in a Wellness Improvement Program in the Elderly (웰니스 건강 증진 프로그램 참여에 따른 고령자 건강생활습관 변화)

  • This study was conducted with the elderly who used a senior welfare center to help their health improvement. After the status of body composition, senior fitness, life habits, and nutrition levels in the elderly were investigated, a wellness health improvement program was conducted for 12 weeks. Effects before and after the program were examined. The following conclusions were drawn. 1. For the changes of body composition by age associated with sex, while elderly men had a significant difference in BMI, elderly women had a significant difference in B.M.I and body fat percentage. 2. For the changes of senior fitness by age associated with sex, while elderly men had a significant difference in lower body muscle, upper body muscle, upper body flexibility, and transformed general endurance, elderly women had a significant difference in lower body muscle and upper body muscle. 3. For the changes of life habits and nutrition levels by age associated with sex, while elderly men had a significant difference in diet habits and exercise habits, elderly women had a significant difference in exercise habits, stress relief habits, and total nutrition level. Based on the findings, the elderly's participation in the wellness improvement program can enhance lower body muscle which is a risk factor of daily activities and fall and increase upper body muscle which is good to carry and move everyday objects. In addition, it may play a positive role in improving diet habits and changing stress relief and exercise habits, resulting in better health and quality of life of the elderly.

The Process of Change, Decision Making, Self-efficacy and Perception of Subjective Health by the Stage of Exercise Behavior among Older Adults (노인의 운동행위변화단계에 따른 변화과정, 의사결정평가, 자아효능감 및 주관적 건강지각)

  • Purpose: The purpose of this study was to examine the differences of the process of change, decision making, self-efficacy and perception of subjective health according to the stages of exercise behaviors suggested by transtheoretical models among older adults. Method: The subjects consisted of 291 older adults. Trained research assistants interviewed the older adults using structured questionnaires. ANOVA analyses with post hoc test were used to examine the mean differences for each stage of exercise behavior using SPSS for windows. Result: The subjects were divided into five stages of exercise behavior: 24.4% precontemplation, 12.4% contemplation, 17.9% preparation, 12.4% action, 33.0% maintenance stage. There were significant differences in process of change, decision making, self-efficacy and subjective health perception according to the exercise stage of change. 'Self-liberation' and 'stimulus control' were important strategies for intention of exercise, while 'reinforcing management' for the actual practice of exercise. 'Self-efficacy' was an effective strategy to change older adults from pre-contemplation to preparation stage. Conclusion: For exercise intervention, older adults should be classified into groups according to their stages of exercise behaviors and provided effective individualized intervention depending on their stage.

The Relationship between Health Belief and Exercise Compliance among Elderly Adults at Senior Centers (경로당 노인의 건강신념과 운동이행의 관계)

  • Purpose: The purposes of this study were to examine the relationship between health belief and exercise compliance among elderly adults at senior centers and to identify factors influencing their exercise compliance. Methods: The subjects of this study were 100 elderly adults who were using senior centers in J City. Data were collected from the 5th of August to the 14th of September in 2014 using a questionnaire about general characteristics, health belief, and exercise compliance. Data analysis included one-way ANOVA, independent t-test, Pearson's correlation, and stepwise multiple regression using the SPSS/WIN 18.0 program. Results: The mean score for exercise compliance was 3.85 (range 1~5), and for perceived health state 3.17 (range 1~5). The mean score for each of the sub-factors of health belief was 3.89 for benefit, 1.94 for barrier, 3.34 for severity, 2.43 for sensitivity, and 3.65 for exercise self-efficacy (range 1~5). There was a significant correlation between exercise compliance and exercise benefit, and 28% of variance in exercise compliance was explained by exercise benefit in health belief, family history of illnesses, and perceived sensitivity in health belief. Conclusion: To promote exercise compliance among elderly adults at senior centers, exercise programs emphasizing exercise benefit should be developed.

The Effects of Low Intensity Exercise Program with Telephone coaching on Physical Fitness and Physiological Index in the Elderly Women in Home Visiting Health Program (전화코칭을 동반한 저강도 운동프로그램이 방문건강관리 여성노인의 체력과 생리적 지표에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of a low intensity exercise program with telephone coaching on the physical fitness and physiological index of elderly women who are receiving home visiting health program. Methods: This study was conducted using a non-equivalent control group pretest-posttest design. The participants were 53 senior citizens. The subjects (15) in experimental group 1 performed only a low-intensity exercise program and the subjects (16) in experimental group 2 performed a low intensity exercise program with telephone coaching for 16 weeks. The low intensity exercise program consisted of muscle strength band exercise, stretching, and hand exercise using balls. Results: There were statistical differences in the right grip strength (p=.030), left grip strength (p=.035), raising behind of right arm (p=.046), raising behind of left arm (p=.045), postprandial blood sugar (p=.021), body mass index (p=.029). Conclusion: The low intensity exercise program with telephone coaching is recommended as a physical fitness and physiological index for elderly women in a home visiting health program.
