The Necessary Conditions and Deterring Factors of Good Care (좋은 돌봄의 필요조건과 저해요인에 관한 연구 - 노인돌봄을 중심으로 -)

  • Social care service has grown much with the introduction of Long-Term Care Insurance, but it remains difficult to answer in the affirmative to the question: are we providing sufficiently good care? This study has its purpose in figuring out what conditions are necessary to realize good care in our society. The study has used focus group interview (FGI) as a way to acquire realistic knowledge on the conditions that create good care and its deterring factors. The focus group interview, which targeted long-term care workers and facility heads, was conducted three times from January through March 2014, with each session taking about 3 hours. The analysis showed that the components of good care were mutual understanding and recognition through active interaction, making a good relationship based on mutual trust and respect, professionalism of care worker with flexible judgment that provides customized services, professionalism of center manager with appropriate intermediation, and the tripartite partnership among family, elderly, and care worker. Meanwhile, the deterring factors of good care were identified as devaluation of care labor, ambiguous job description and abuse, unprofessionalism of care worker and manager, inappropriate interference and indifference of family, and the structural constraints on long-term care environment.

A Case Study on the Community-based Elderly Care Services Provided by the Social Economy Network in Gwangjin-Gu, Seoul (사회적경제 조직의 지역사회 돌봄 네트워킹 가능성에 대한 비판적 고찰: 서울시 광진구 노인돌봄 클러스터 사례연구)

  • This study analyzed the case of elderly care cluster in Gwangjin-gu to explore the possibilities of social economy as a provider of community-based social services. Community-based means the approach by which community organizations build a voluntary and collaborative network to enhance collective problem-solving abilities. Therefore, it is very likely that the social economy that emphasizes people, labor, community, and democratic principles can contribute to community-based social services. This study analyzed social economic network by using four characteristics of social economy suggested by OECD community economy and employment program as an analysis framework. The results of this study are as follows: First, it is found that social economy would hardly supply community-based social services through network cooperation because of a large variation in community identity, investment to new product, and labor protection. Second, community users are not the consumers of the social economy and the products of the social economy stay in market products only for the organizations in social economy. In order to create good services that meet the needs of residents, community development approaches are required at the same time. The importance of community space where local residents and social economy meet is derived. Third, public support such as purchasing support has weakened the ecosystem of social economy by making the distinction between public economy and social economy more obscure. On the other hand, public investment in community infrastructure is an indirect aid to social economy to communicate with residents and to promote good supply and consumption. In the end, community-based social services need a platform where the social economy and the people meet. This type of public investment can create the ecosystem of the social economy.

Dementia Strategies using Welfare-technology (복지기술을 활용한 치매전략 방안)

  • South Korea faces elderly care crisis with both low fertility rate and rapid ageing rate. Long-term care insurance in 2008 has taken care burden from family to nation. However, few options, lack of tailored services, and unstable community care plan caused blankness of care in some areas. Especially, increasing dementia rate has become an emergent social issue. Welfare technology could be a useful alternative to fill out the gap between demand and supply. Using FGI method on two groups(high-risk dementia group and MCI group), intimacy, accessibility and preventive way of dementia with welfare technology are analysed in this paper. Despites of a few differences of dementia progress, interests towards technology are expressed. More suggestions using TV and normal telephone to improve daily independence are presented as well. Amongst MCI group, help-service for technology-use at ADS and linkage ways of in-home services are also suggested.

Effects of the Fall Prevention Education Program (FPEP) for Caregivers in Elderly Care Facilities on Fall-related Knowledge, Fall-related Burden, and Caring Behaviors for Fall Prevention (노인요양시설 요양보호사를 위한 낙상예방교육프로그램이 낙상관련 지식, 낙상관련 부담감 및 낙상예방 돌봄행위에 미치는 효과)

  • The purpose of the study was to examine the effects of the Fall Prevention Education Program (FPEP) on those providing direct care in elderly care facilities regarding fall-related knowledge, fall-related burden, and caring behaviors for fall prevention. The FPEP developed in the study was implemented for 80 minutes per session and four sessions in total. Participants included a total of 47 subjects, 24 in the experimental group and 23 in the control group. The program was implemented from July 16 through August 31, 2012. Data were analyzed using ${\chi}^2$-tests, Fisher's exact probability tests, independent t-tests, and analyses of covariance using the SPSS/Win 19.0 program. According to the pre-and post-test, the experimental group participated in the program showed an increase in fall -related knowledge (t=6.71, p<.001), a decrease in fall-related burden (t=-2.31, p=.026), and an increase in caring behaviors for fall prevention (F=49.50, p<.001) as compared to the control group. The results of the study demonstrated that the FPEP developed for those providing direct care in elderly care facilities was an effective intervention for decreasing fall-related burden and increasing fall-related knowledge and caring behaviors for fall prevention.

The Convergent Influence of Knowledge, Attitudes toward Caring for the Elderly and Geriatric Nursing Practice in Nursing Students (간호학생의 노인에 대한 지식, 노인돌봄태도가 노인간호실천에 미치는 융복합적 영향)

  • The purpose of this study was to identify the relationships among knowledge, attitudes toward caring for the elderly and geriatric nursing practice and the influencing factors on geriatric nursing practice in nursing students. Data were collected from 255 nursing students with clinical practice experiences in three nursing schools in B city, with the measurement of Fact on Aging Quiz Part I, Attitudes toward Caring for the Elderly and Geriatric nursing practice. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and Stepwise multiple regression. Significant correlations were found among knowledge of, caring attitude towards the elderly and geriatric nursing practice. The significant factors influencing geriatric nursing practice were caring attitude towards the elderly, grade, and knowledge of the elderly, and these factors explained 27.4% of the variance. The findings suggest that effective educational strategies need to be developed and implemented to foster positive caring attitude towards and knowledge of the elderly in order to improve geriatric nursing practice in nursing students.

Policy Supports for Informal Caregivers: Focusing on Policy Changes in the United States and United Kingdom (비공식 돌봄자를 위한 정책 지원 비교 연구: 미국과 영국 최근 정책 동향 고찰 중심으로)

  • Diverse official policies in community are available for caregivers to take care of the elderly in the US and the UK. This study aims to examine the recent changes in government supports based on The Recognize, Assist, Include Support and Engage (RAISE) in the US, and those by Carer Act 2014 in the UK, to take any good lessons for Korean policy. Caregivers will play a new role to develop innovative treatment for patient-centered care. The UK strengthens to provide various efforts for working carers while assuring economic efficiency in labor market with empirical evidence. The major four ways to support carers were developed agreed with the acknowledge of caregiving value and their human right; financial support, direct services for carers, flexible work time, and advocacy. Korean supports policies for carers in long-term care and social welfare will be more effective in community care system if more evidence based policies are prepared.

Nurses' Perceptions of Person-Centered Care in Long-term Care Hospitals: Focus Group Study (인간중심돌봄에 대한 요양병원 간호사의 인식: 포커스 그룹 연구)

  • This study was conducted to understand and describe Korean nurses' perception of person-centered care for elderly patients in long-term care hospitals. Qualitative data were collected through four focus group interviews consisting of 21 nurses working in four long-term care hospitals in Jeolla and Gyeongsang provinces. Participants completed interviews from July 19 to 30, 2018. All interviews were recorded, transcribed, and analyzed by employing the thematic analysis method. Six main themes for the attributes of person-centered care for elderly patients in the long-term care hospitals were conceptualized: respecting individual needs, walking to the end, supporting hidden dreams, becoming a family partner, helping patients live like they are at home, and changing culture. Person-centered care perceived by nurses was conducted to provide individualized nursing according to elderly's preferences and help them discover the value and meaning of life through various activity programs. Nurses also recognized person-centered care to maintain cooperative relationships with their family members and share their decision-making process, as well as to form a physical environment and organizational culture that respects the rights and autonomy of the elderly. Based on the results of this study, it is necessary to identify the diverse needs of the elderly and develop nursing intervention programs based on person-centered care.

The Changing Shape of Care-time Diamond: Social Care Expansions in the 21st Century in Korea (변화하는 케어-타임 다이아몬드: 한국의 21세기 사회적 돌봄의 확대)

  • Traditionally, caring for young children and the elderly has been largely assumed and practiced intensively within the family in Korea. The Korean government established residual protection systems for the elderly as well as children whose needs could not be met by their family members alone. However, in the 21st century, a number of social forces have made it necessary to expand the state's intervention in the care provisions. The primary forces include the ageing process, low fertility, change in the women's labour market participation, changes in the family formation and dissolution, and changes in the people's perceptions of familial responsibilities regarding caring for other family members. This paper employs and further develops the idea of the care diamond conceived by the United Nations Research Institute for Social Development Project in relation to the political and social economy of care and applies it to Korea's social care expansions. The analysis demonstrates that the roles of the public and the market sector, in case of child care, increased while those of the third sector decreased. Apropos of the elderly care, the role of the market expanded dramatically, followed by that of and the state and the third sector. Nonetheless, it is important to note that the fundamental characteristics of Korea's care provision for children and the elderly have remained unchanged and even strengthened where the elderly care is concerned. The bulk of personal care demand is still met within the family, particularly by female members of the household.

The Experiences of Long Term Care Workers Who care for People with dementia at Home (요양보호사들의 치매노인 돌봄 경험)

  • This study examined the experience of long-term care workers who care for people with dementia at home. A descriptive exploratory methodology was applied within the qualitative research paradigm. The data were collected using individual in-depth interviews with 10 long-term care workers who had at least 3 years working experience caring for people with dementia. The data were analyzed within the qualitative content analysis. Job selection, struggling and strategies for the role accomplishment were found to be the main categories. This study emphasized the role of caring as social filial piety from the concept of nonpayment of work in the family.

Awareness of Patient-Centeredness and Transitional Care Services for Older Patients Among Primary Care Physicians in Korea (일차진료 의사들의 환자중심성 및 노인환자 대상 전환기 돌봄서비스 인식)

  • 목적: 병원에서 재가 및 시설로 퇴원한 환자가 지역사회에서 건강을 유지하기 위해서는 전환기 돌봄서비스(Transitional care services)가 필요하다. 이를 위해 지역사회 내 의료서비스와 자원을 연계하는 주치의의 역할이 중요시된다. 본 연구에서는 선행연구를 바탕으로 일차진료 의사들의 환자중심성에 대한 인식을 파악하여 환자중심 기반의 서비스 제공을 위해 필요한 정책을 제시하였다. 또한 Transitional Care Service에 대한 일차진료 의사들의 인식을 확인하고 인구사회학적 요인과의 관계를 확인함으로써 서비스 우선순위를 도출하고자 하였다. 방법: 본 연구는 전국의 가정의학과, 내과, 신경과 등 노인 질환과 관련 있는 과의 전문의 자격증이 있으며 자발적으로 온라인 설문조사에 참여할 의사를 표현한 일차진료 의사 259명을 대상으로 수행되었다. 환자중심성 및 전환기 돌봄서비스에 대한 인식을 살펴보기 위해 구조화된 설문지를 개발하였으며, 조사전문업체를 통해 2019년 10월 28일부터 2019년 11월 22일까지 온라인으로 설문조사를 수행하였다. 결과: 본 연구에 대한 주요 결과는 다음과 같다. 첫째, 일차진료 의사들을 대상으로 9가지 전환기 돌봄서비스 인식에 대해 살펴본 결과 "입원 시 진단, 건강상태, 치료계획 및 결과 에 대한 설명(4.4)"과 "퇴원 후 자가 건강관리를 위한 정보 및 훈련 (4.2)"에 대한 필요성이 높게 나타났다. 둘째, 35세 이상 일차진료 의사가 34세 이하 일차진료 의사보다 전환기 돌봄서비스에 대한 인식이 높게 나타났다(F=7.3, p<0.01). 또한, 환자중심성에 대한 인식이 높을수록, 연령이 높을수록, 서울 외 지역에서 근무할수록 전환기 돌봄서비스에 대한 인식이 높게 나타났다. 결론: 본 연구에서는 일차의료를 제공하는 의료진들을 위한 교육프로그램과 지역사회에서 일차의료 의사들을 중심으로 하는 지역 연계 방안을 제시하였다는 점에서 의의가 있다.
