ASSESSMENT OF THE NEEDS OF SELF-CARING RETIREMENT VILLAGE RESIDENTS FOR COMMUNITY SERVICES (RETIREMENT VILLAGE에 거주하는 자가 돌봄이 가능한 노인들의 지역사회 서비스에 대한 욕구 사정)

  • 지역 사회 서비스는 노인들이 질적으로 보다나은 삶을 영위할 수 있도록 돕는 기능을 해야 한다. 적절한 서비스의 공급을 위해서는 지역사회 서비스 욕구 사정의 과정이 우선적으로 요구되어지며, 이에 본 연구는 호주 NSW주 Illawarra지역내의 retirement village에 거주하는 자가 돌봄이 가능한 노인들의 지역사회 서비스에 대한 욕구를 평가하는데 그 목적을 두었다. Illawarra지역내의 여덟개 retirement village에 거주하는 98명의 자가 돌봄이 가능한 노인들을 대상으로 그들의 1) functional status; 2) 지역 사회 서비스에 대한 지식정도; 3) 지역 사회 서비스 사용정도; 4) 지역 사회 서비스 만족도; 5) 잠재적인 지역 사회 서비스 욕구 등에 대한 자가 보고형 설문조사가 이루어졌다. 수집된 자료들은 JMP(for Mackitosh) program으로 통계 처리되었고, computed frequencies가 본 연구의 목표 달성을 위해 주로 사용되었다. 연구 결과를 요약하면 아래와 같다. 1. 평균 연령 76세인 대상자들의 80%이상이 그들의 일상 생활 운영에 다른 사람의 도움을 필요로 하지 않으며 건강상태 또한 양호하다고 자각했다. 2. 90%이상의 대상자들이 지역 사회 서비스를 이용하지 않았다. 3. 70%이상의 대상자가 지역 사회 서비스의 존재에 대해 지각하고 있었다. 4. 60%이상의 대상자가 필요시에는 지역 사회 서비스에 관한 정보를 어렵지 않게 얻을 수 있다고 생각했다. 5. 40%의 대상자가 기존의 지역 사회 서비스에 대한 양과 질에 만족을 나타냈고, 그 외의 대상자들 (60%)은 그 지역 사회 서비스에 대한구체적인 지식이 없다고 보고했다. 6. 25%의 대상자들이 여러 지역 사회 서비스에 관한 관심을 보였다. 결론적으로 대다수의 대상자들은 대부분의 지역 사회 서비스들을 필요로 하지 않았고, 이러한 결과는 현재 자가간호가 가능한 retirement village 거주 노인들에게 당연한 것으로 보여진다. 그러나, 자존감이 강한 노인들에게 서비스 욕구는 자가 돌봄이 건강에 대한 포기정도로 왜곡되어 자각될 가능성이 있는 만큼, 본 연구를 통해 보고된 대상자들의 지역 사회 서비스에 대한 관심도는 고려해 볼만한 가치가 있다고 여겨지며, 이에 노인들의 자존감이 서비스 욕구에 미치는 영향에 대한 연구를 제언하는 바이다.

Health-Promoting Lifestyle Patterns and Health Perception in Elders Using Welfare Service Centers (복지관 이용 노인의 건강증진생활양식과 건강지각)

  • Purpose: The purpose of this study was to identify health promoting lifestyle patterns (HPLP) and health perception (HP), and related factors, in elderly people using welfare service centers. Methods: Three hundred elders were interviewed using a structured questionnaire through convenience sampling at two welfare centers. Data were analyzed using the SPSS WIN 12.0 program. Results: Most of the elders had some kind of diseases, such as hypertension, diabetes, or osteoarthritis. The mean HPLP score was 2.53, and significant differences were found according to the elders' general characteristics. The sub-domain nutrition had the highest mean score, and stress management and exercise, the lowest. The mean HP score was 3.38, and significant differences were found according to education level and number of underlying diseases. HPLP showed a correlation with HP. Conclusion: The results indicate that elders have poor practices in stress management and exercise reflecting need for community based stress management programs for welfare service centers elderly clients.

A Prediction Model for Health Promoting Behavior of The Korean Elderly (한국노인의 건강행위 예측모형구축)

  • This study was designed to construct a model that predicts the health promoting behavior of the Korean elderly. Data were collected by self-reported questionaires from 254 Korean elderly in seoul, from June 1 to July 15, 1998. Data were analyzed by descriptive statistics and correlational analysis using pc-SAS program. The Linear Structural Modeling(LISREL) 8.0 program was used to find the best fit model which predicts causal relationships of variables. The overall fit of the hypothetical model to the data was moderate[X$^2$=249.83(df=83, p=.00), RMR=.07, GFI=.90, NNFI=.92, NFI =.91]. The predictable variables of health promoting behavior of the Korean elderly were social activity. social support. self-integrity and helplessness except the perceived health status. These variables explained 17.1% of health promoting behavior of the Korean elderly.

The Use of Professional Health Care Systems among the Elderly in Rural Communities (농촌 지역 노인의 전문 건강관리 체계 이용 행위에 관한 연구)

  • Purpose: This ethnography is aimed at describing the health care seeking behavior of elderly details in their socio-cultural context. The research question is 'under what conditions did elderly informants decide to use certain professional health care services and how do they make use of all the available resources?' Method: 10 sessions of fieldwork were conducted in the two agricultural villages between Sep. 1999 and Oct. 2002. The data for this paper came from participant observation with 14 informants. In the process of analysis I used proxemic and taxonomic techniques. Result: Informants decided to use a certain health care system according to their folk definition of illness. They prefer to use the health services where they felt more comfortable and free. They wished to be care from intuitive and holistic healers. Social network and having health resources was also important factor. Conclusion: We need more comprehensive research model to reach a plausible explanation. Combined qualitative-quantitative research is needed to get practical data to develop effective health care systems for the elderly.

The Effects of Digital Literacy and Health Empowerment on Elders' Communication with Doctors: Focusing on Moderating Effect of Health Beliefs (디지털 리터러시와 건강 임파워먼트가 노인의 의사와의 소통에 미치는 영향 연구: 건강신념의 조절효과를 중심으로)

  • Purpose: This study explored the effects of the elderly group's digital literacy and health empowerment on communication with doctors, considering moderating effect of health beliefs about chronic diseases. Methods: A one-on-one interview survey was conducted with 500 older adults in South Korea. The main variables were digital literacy, health empowerment, communication with doctors, and health belief of chronic diseases. Results: The interaction effect between health empowerment and susceptibility, and health empowerment and perceived barrier were significant. Conclusion: Communication with doctors greatly increased when the elders had high levels of health empowerment and low levels of susceptibility. Also, communication with doctors greatly increased when the elders had high levels of health empowerment and low levels of perceived barriers.

Factors Influencing Depression in Community-dwelling Elderly with Low Income (재가 저소득계층 노인의 우울에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the level of and factors influencing depression in community-dwelling, low-income elderly. Methods: A randomized sample of 306 subjects aged 65 and over was selected from one communities. in Korea. Data were collected through self-reported questionnaire, during the period from January to September, 2007. The data were analyzed by t-test, ANOVA, Pearson's correlation coefficient and multiple regression with SPSS/Win program. Results: The prevalence of depression was 43.8%, the mean score of depression was 6.37. Depression of the elderly significantly correlated with education level, types of the health insurance, economic levels, smoking, hypertension, arthritis, incontinence, life satisfaction, perceived health status and cognitive function. Stepwise multiple regression analysis reveled that a combination of life satisfaction, cognitive function and health insurance accounted for 67.6% of depression in elderly. Conclusion: The findings of this study suggest that the approach to effective depression prevention and management program for community-dwelling elderly should consider life satisfaction, physical and mental health and economic levels.

A Convergence Study on the Relationships between Self-Efficacy and Health Behavior in eldery women with chronic low back pain (만성 요통을 가진 여성 노인의 자기 효능감과 건강행위간의 관계에 대한 융합연구)

  • Purpose: The purpose of this study is to examine the relationships between self efficacy and health behavior of elderly woman with chronic low back pain. Methods: A descriptive correlational design was used and the participants were 130 eldery women patients with chronic low back pain from one general hospital in 2012. The structured questionnaire included Perceived Self Efficacy Scale, Health Behavior Assessment tool of the Korean Elders. Data analysis included t-test, ANOVA and Pearson's correlation. Results: The mean scores were $67.34{\pm}18.32$ for self efficacy, $99.70{\pm}14.46$ for health behavior. There were significant correlations between self efficacy and health behavior(r=.80). Conclusion: The findings suggest that elderly woman with chronic low back pain need nursing interventions to improve health behaviors by promoting self efficacy.

Effects of a Health Diary Program on Fall-Related Outcomes in Low-Income Elderly Women with Osteoarthritis (건강다이어리 프로그램이 저소득 관절염 여성노인의 낙상관련 심리적 변수에 미치는 효과)

  • Purpose: This study was done to identify the effects of a health diary program on fall related outcomes the low-income elderly women. Methods: The study was carried out with a nonequivalent control group with pretest-posttest design. The study was composed of two groups, each made up of 24 subjects: experimental group and control group. The subjects were low-income women aged over 65 years with osteoarthritis and both the experimental and control groups were made up of subjects with the same age profiles. The independent variable was the health diary program, and the dependent variables were fall related outcomes (fear of falling, fall-efficacy, knowledge of fall) difficulty of performing activity, and mood state. The health diary program was performed for 50 minutes each session and twice a week for 8 weeks. Data were collected before the health diary program 10 weeks after the beginning of the program. Results: The experimental group showed significant differences in knowledge of fall, fear of falling, and mood state compared to the control group. However there was no significant differences in difficulty of performing activity and fall efficacy. Conclusion: The results of this study may be used as part of an education to prevent falls for low-income elderly women with osteoarthritis.

Influence of Professional Self-concept, Moral Sensitivity on Elderly Care Performance of Geriatric Hospital Nurses (요양병원 간호사의 전문직 자아개념과 도덕적 민감성이 노인간호실천에 미치는 영향)

  • This study was conducted to investigate the relationship among professional self-concept, moral sensitivity and elderly care performance nurses in geriatric hospitals, as well as to clarify factors that affect nurses' elderly care performance. Participants consisted of 153 nurses working in geriatric hospital in G Province were evaluated. Date were collected from September 15 to October 15, 2018 and analyzed using the SPSS/WIN 23.0 program. The mean of professional self-concept was $2.87{\pm}0.35$ out of 4, that of moral sensitivity was $4.70{\pm}0.47$ out of 7 and that of elderly care performance $3.51{\pm}0.41$ out of 4. Elderly care performance was siginificantly positively correlated with professional self-concept (r=.48, p<.001) and moral sensitivity (r=.31, p<.001). Factors influencing elderly care performance were professional self-concept (${\beta}=.32$, p=.001), moral sensitivity (${\beta}=.18$, p=.021), educational experience of geriatric nursing (${\beta}=.17$, p=.029), which explained 31.6% of elderly care performance. The results of this study suggest that supporting systems should be arranged to provide various learning opportunities as a way to increase the professional self-concept of nurses with short career as well as intervention strategies may be necessary to make education of geriatric nursing obligatory.

The Influence of Care Stress for Older Patients, Self-perceptions of Aging, Aging Anxiety on Retirement Preparation in Nurses (간호사의 노인간호 스트레스, 노화인식, 노화불안이 노후준비에 미치는 영향)

  • Purpose: The purpose of this study was to explore the influence of care stress for older patients, self-perceptions of aging, and anxiety about aging on preparation for retirement in clinical nurses. Methods: This was a cross-sectional descriptive study in which participants were 222 nurses who had at least six months of work experience and were involved in caring for older patients in the last six months at the work site. Data were collected from August to September, 2020 using an online survey questionnaire. Data were analyzed using the IBM SPSS/WIN 25.0 program. Results: The mean level of preparation for retirement in the participants was moderate (3.21 out of 5 points). Among the sub-domains of preparation for retirement, financial preparation had the lowest score. Self-perceptions of aging and aging anxiety were significant predictors for retirement preparation in nurses, accounting for 16% of the variable's total variance. Conclusion: A greater level of preparation for retirement was associated with a positive perception of aging and a decreased level of aging anxiety in the clinical nurses. Further research should focus on exploration of specific determinants of financial preparation for retirement and development of intervention strategies for improving preparation for retirement in the nursing workforce.
