The Family Caregivers' Stress Pathways by Types of Long Term Care Services for the Elderly (장기요양보호서비스 유형별 가족 부양스트레스 경로)

  • The purpose of this study is to analyse the family caregivers' stress pathways by types of long term care services for the elderly, and then to discuss the findings of analysis. For this research, primary caregivers that provide care the elderly requiring long term care services sanctioned by National Health Insurance Corporation were drawn and surveyed. Among collected data, data for 258 primary caregivers by type of long term care services were used for this study. The results of this study can be summarized as follows. First, on average, the elders that utilize care service in institutions for the elderly were higher proportion of women, older, higher rate of bereavement, more children than the elders that utilize in-home care service, but some cases were vice-versa. Second, the elders that utilize care service in institutions more ADL dependency, higher proportion of severe dementia or severe stroke, higher care rank by National Health Insurance Corporation than the elders that utilize in-home care service on average. Third, primary caregivers with elders that utilize in-home care service were higher proportion of women, older, lower education level, higher rate of spouse and daughter-in-law in relationship with care recipient, less health, lower income than primary caregivers with elders that utilize care service in institutions. Fourth, subjective indicators representing caregivers' reactions to caregiving for the elderly significantly impact on caregivers' stress(ie, depression), and pathway of caregivers' stress are differentiated by type of long term care services. Fifth, stressors that have direct impacts on depression as caregiving family are differentiated by type of long term care services. Therefore, policies or programs to reduce negative mental health or stress of caregivig families should be designed differently by reflecting pathway of various stressors and stress by use types of long term care services for the elderly.

The Study on the Development of the 'Caregiving Affirmation Scale' for Korean Caregivers for the Aged (한국 노인가족부양자의 '부양긍정감척도' 개발을 위한 연구)

  • This study attempts to develop the scale of 'Caregiving Affirmation Scale' for Korean Caregivers for the Aged and to testify the possibility of generalizing the scare. Mixed methods of qualitative and quantitative researches were used to achieve the purpose of the study. The steps of developing the scale were proceeded as follows: Firstly, 40 items were identified through the qualitative study and the literature reviews. Secondly, the content validity was verified through the professional group survey in addition to confirm the questionnaire items by the 1st survey. And thirdly, the construct validity and concurrent validity of the factors of 'Caregiving Satisfaction' were explored by performing the 2nd survey. As the results of the study, the total 16 items of 'Caregiving Affirmation scale' identified with the four sub-factors as follows; 1) Improvement of the care skill and a sense of fulfillment; 2) Possibility of contributions to other persons; 3) Acceptance of the aged; and 4) Improvement of Self-value. The reliability value of Cronbach's α was 0.89 with the high internal consistency and the confirmatory factor analysis showed the relationship between the items and each factor were also statistically significant. Thus, the content validity was verified. Also the correlation with the affirmative care recognition and the life satisfaction was 0.65, which meant the concurrent validity exists. The Caregiving Affirmation Scale for Korean Family Caregivers for the aged is expected to be used at the social welfare practice settings for the aged.

A Study on the Caregiving in Elderly of the Female Family Caregivers (여성가족부양자의 노인부양에 관한 연구)

  • Purpose: The purpose of this Study was to identify the factors that affect the caregiving burdens felt by the female family caregivers and quality of caregiving. Method: The subjects were 247 female family caregivers who were living with the elderly and were residing in Seoul. Data was collected from June 30, 2000 to Agust 11 by using questionnaire. The collected data was analyzed using descriptive statistics and hierarchial multiple regression with SAS/PC. Result: Situational factors had the greatest influence on the caregiving burdens. While the interactive factors of discrepancy between past and present image of elder and the caregiving belief were proven to be significant, the former had greater influence. The factors affecting the quality of caregiving, this is greatly influenced by situational factors and interactive factors. The discrepancy between past and present image of elder had relatively little influence while caregiving belief had the greatest influence. Among the situational factors, family stress had the greatest influence, while the caregiving burdens had little influence on the quality of caregiving. Conclusion: In order to improve the quality of caregiving in elderly by the female caregiver in the family, and to reduce the caregiving burdens, it is important to consider variables related to interaction as well as those directly concerned with caregiver and the elderly for nursing intervention.

Cost Analysis of Long Term Care Services (장기요양보호서비스의 비용분석)

  • The purpose of this study is to evaluate cost side by type of long term care services economically, and then to discuss the findings and implications for the results of analysis. For this research, primary caregivers that provide care the elderly requiring long term care services sanctioned by National Health Insurance Corporation were drawn and surveyed. Among collected data, data for 422 primary caregivers were used for this study. The subjects used in this study consisted of family caregivers from various settings that give care to the elderly. The results of this study can be summarized as follows. First, caregiver's household income level is low. Therefore, caregiving families with the elderly are likely to have financial difficulties. Second, under coverage of long term care insurance system, the direct cost caregiving households pay for the elderly is still very high. Third, indirect cost of caregiving households accounted for the larger proportion caregiving costs. Fourth, social cost burden for caregiving the elderly is very high. This cost amount is appropriately equal to household income of caregivers surveyed in the research. Fifth, service use cost of caregiving households is differentiated by type of long term care service. Sixth, direct cost of caregiving households is statistically significantly differentiated by type of long term care service, but is differentiated less than service use cost. Seventh, social indirect cost for caregiving the elderly is statistically significantly differentiated by type of long term care service. Eighth, social cost amounts for the elderly utilizing long term care service is very high, total social cost per capita by types of long term care service tend to converge on average social cost per capita of total service.

A Study on the Major Factors Affecting Health-Related Quality of Life of Elderly Stroke Survivors (뇌졸중 생존 노인들의 건강관련 삶의 질 영향 요인)

  • The main purposes of the study were to identify factors affecting health-related quality of life of elderly stroke survivors, and to suggest social work interventions for improving their quality of life. The participants were 328 elderly stroke survivors over 55 years old recruited from 18 long-term care hospitals and 13 nursing care facilities in Chung-Nam Province, Korea. The data was collected through a direct survey method from Dec. 16, 2008 to Jan. 17, 2009. Statistical analysis revealed that the overall health-related quality of life among participants was relatively low, especially in the social relationships domain. As a result of hierarchical multiple regression analysis, depression emerged as the most significant predictor of elderly stroke survivors' health-related quality of life, followed by financial burden in future, activities of daily living, acceptance of disability, comorbidity, and major financial supporter(spouse). Health-related quality of life of elderly stroke survivors in long-term care facilities was affected by a variety of variables from disease-related factors to environmental ones. Particularly psychosocial variables were the most powerful factors to predict HRQOL of elderly stroke survivors. Based on the results, social work implications were discussed in the context of developing clinical strategies to alleviate elderly stroke survivors' health-related quality of life in long-term care facilities. Living with chronic illness and disability is a great challenge to the elderly. Most of all long-term residence in care facilities can cause deterioration of quality of life in old age. From now on, studies on health-related quality of life of elderly with chronic illness and disability should be activated in social work field.
