Evaluation of Oral Malodor for Patients Visiting A Dental Clinic (일 치과의원 내원환자의 구취에 대한 평가)

  • The purpose of this study was to find out the degree of self-consciousness of oral malodor, the status of oral hygiene care, some oral examination factors, and measurement values of malodor compounds through conduction oral examination and questionnaire survey, and measuring volatile malodor compounds by instrumental analysis. The data were collected from 155 patients visiting a dental clinic by using a self-administered questionnaire, conducting oral examination for halitosis, and measuring malodor compounds with Oral Chroma. The rate of recognizing their breath as 'somewhat bad' were 80.0% for the female patients and 74.3% for the male, and, however, 14.3% of the male recognized their breath as 'very bad' while 18.8% of the female did not recognized oral malodor, showing statistically significant difference between gender(p=0.004). The average concentrations of volatile sulfur compounds measured by Oral Chroma were 1.65 ng/10 ml for hydrogen sulfide, 1.71 ng/10 ml for methyl mercaptan and 1.66 ng/10 ml for dimethyl sulfide, on the average, respectively, exceeding malodor threshold levels of all 3 compounds, and were significantly higher in the male group than those in the female, also exceeding all threshold levels except hydrogen sulfide values of the female group. The type of oral malodor was the most prevalent for Type I as 23.2%, followed by Type V, Type IV, Type II whileas Type I and Type IV in the female as 30.6% and 25.9% respectively, showing statistically significant difference by gender(p=0.006). The correlations among oral examination indices was the highest between tongue fur score and simplified oral hygiene index, followed by a significant reverse correlation between the number of fixed prosthodontic teeth and the number of dental caries(p=0.000).

The Degree of Fatigue Depending on Constitution in Middle-Aged Women (중년 여성의 체질별 피로 - 한방병원 내원환자를 중심으로 -)

  • Purpose: This study was conducted to investigate the subjective degree of fatigue, the degree of fatigue depending on constitution, and the factors related to fatigue in middle-aged women. Method: Data were collected from 51 patients, registered at Oriental Medicine Hospital in J-city and K-city respectively. The instrument used in this study were the Multidimensional Assessment of Fatigue and the Questionnaire of Sasang Constitution Classification Ⅱ. Data analysis was done SPSS Result: The characteristics of constitution in middle-aged women could be divided into three: twenty six people of Soeumin, thirteen people of Taeumin, and twelve people of Soyangin. The degree of fatigue in middle-aged women: the average score 30.61 out of the total score 40. Subdivision of fatigue scores: common fatigue degree 7.71, distress due to fatigue 7.67, degree of daily activity fatigue 7.43, and fatigue frequency in the previous week 7.84. There were significant differences in the degree of fatigue depending on constitution in middle-aged women(p=.033). This study revealed weight, exercise, and drinking are important factors that can exert influence on the degree of fatigue. Also, there was a big difference in the degree of fatigue depending on constitution. Conclusion: In conclusion, fatigue is one of the difficult problems with middle-aged women. And this study revealed that change in the weight(especially loss of weight), physical exercise, and drinking are important factors that can exert influence on the degree of fatigue. Also, there was a big difference in the degree of fatigue depending on constitution. Therefore it is necessary to develop Korean nursing intervention to providing an appropriate environment where there is the harmony of the dual principles of the negative and positive, and patients can be taken care of properly.

An Analysis of Factors Affecting Satisfaction of Physical Therapy Patients (물리치료 내원환자의 만족도에 영향을 미치는 요인 분석)

  • Patient satisfaction is an important factor in evaluating the quality of care. Patient satisfaction may be used to evaluate provider services and facilities, and used to predict the patient returns to a facility. The patients d whether the patient returns to a facility or whether the patient recommends the facility to other people may be affected by a variety of factors of patient satisfaction. Low satisfaction may result in poor compliance with the potential of waste of resources and suboptimal clinical outcome. This study is to identify factors of patient satisfaction that will affect patients decision whether the patient returns or not. A self-administered questionnaire survey was conducted in Seoul, Chung-Joo and Bu-Cheon cities, Survey data was obtained from 743 patients who visited the physical therapy practice at university hospitals, general hospitals and clinics. Response rate was 94.4%. The instrument developed by Goldstein et al. (2000) was used and translated into Korean. Several items were added to the instrument. Patient's opinions of service in each domain measured using 5-point Likert-type scales that ranged from strongly disagree to strongly agree. A multiple-regression analytic approach was used to predict overall satisfaction of physical therapy. Age, kindness, scheduling, convenience of parking, privacy, and waiting time predicted the overall satisfaction of physical therapy. The older patients had higher level of satisfaction with physical therapy compared with the younger patients. Patient satisfaction were more affected by access (scheduling and waiting time), administrative technical management (convenience of parking), and interpersonal management (kindness of physical therapists and other staffs) than clinical technical management (physical therapists' skills).

A study on the practice application of oral hygiene auxiliary supplies and oral health status ofpatients in 'S' university dental clinic (치위생과 실습실 내원환자의 구강위생보조용품 사용유무와 구강건강상태와의 관계)

  • Objectives : The purpose of this study was to investigate the relationship among the practice application of oral hygiene auxiliary supplies, oral health state of patients in S university dental clinic. Methods : The subject in this were 261 patients who got a scaling at the oral hygiene practice lab in the department of dental hygiene in S university dental clinic from April 1 to May 31, 2010. For the data analysis, an SPSS WIN 11.5 program was used and its signification level was 0.05. Results : 1. For the oral health state according to sex distinction, it showed the men's 0.78 MT index was higher than women's 0.48 MT index and statistically significant difference. 2. For FT index, women(4.72) was higher than men(3.50) and it showed statistically significant difference(p<0.05). 3. For the oral health state according to sex distinction, Why not use oral hygiene auxiliary supplies showed statistically significant difference(p<0.05). 4. For the practice application of oral hygiene auxiliary supplies according to age distinction, 18.5% more than 30 years replied as I use interdental brush and it showed statistically significant difference. 18.5% more than 30 years replied as I use powered brush and it showed statistically significant difference(p<0.05). 5. For the oral health state according to the practice application of oral hygiene auxiliary supplies distinction, there were significant difference that dental floss, interdental brush, mouth rinse product, Why not use oral hygiene auxiliary supplies(p<0.05). Conclusions : The findings of this study were lower than the utilization of oral hygiene auxiliary supplies. Therefore, to increase the use of oral hygiene auxiliary supplies to patients of the appropriate selection and correct usage of oral hygiene auxiliary supplies and the resulting effects have sufficient training to practice more efficiently should be.

A Study on the Relationship Between the Locational Characteristics of Oriental Medicine Hospitals and the Number of Patients (한방병원의 입지특성과 내원환자 규모 간의 관계에 관한 연구)

  • The purpose of this study was to analyze the relationship between the locational characteristics of areas surrounding oriental medicine hospitals and the number of patients who visited study hospitals. Administrative data collected from the annual report of 5 ward offices in Daejeon used to assess the geographical attributes. Two oriental medicine hospitals operated in Daejeon provided data for the number of inpatient and outpatient. Number of patients who visited study hospitals was calculated in each Dong which is the smallest administrative district. The geographical attributes of Daejeon were evaluated by the demographic and economic factors which were assumed to influence the health care demand. Each criterion was measured from each Dong. Weights of factors was calculated by Analytic Hierarchy Process (AHP) method. Evaluation scores which representing the geographical attributes of Dong was computed by multiplying the eight factors and weights. Results showed positive correlation coefficients between the evaluation scores of Dong and the number of patients. One hospital which was more closely located to areas with high evaluation scores had higher number of patients than that of the other hospital. Buffering analysis with varying size support the analysis results. This finding proposed the importance of location for the management of oriental medicine hospitals in a metropolitan city. Applying study model to other cities will enhance the validity of study results.

Comparative Analysis of Patients Visiting Department of Korean Internal Medicine in a Korean Medicine Hospital Before and During COVID-19 - From July 2018 to June 2021 at Wonkwang University Jeonju Korean Medicine Hospital - (COVID-19 전후 단일 한방병원 한방내과 내원환자들에 대한 비교 분석 - 2018년 7월부터 2021년 6월까지 원광대학교 전주한방병원을 중심으로 -)

  • Objectives: This study aimed to analyze the healthcare utilization behavior of patients visiting the department of Korean internal medicine in the Korean medicine hospital of Wonkwang University in Jeon-ju from July 2018 to June 2021. Methods: We retrospectively analyzed the medical records of 26,108 patients and sorted the data by period, month, visiting types, new or returning types, sex, and age group. IBM SPSS 26.0 and the R 4.05 'changepoint' package were used with various statistical methods, such as Independent t-test, Mann-Whitney test, Chi-square test, Simple regression analysis. The P-value was set at 0.05. Results and Conclusions: Females outnumbered males regardless of period, and the ratio of females fell after COVID-19. Regardless of visiting types, patients in their 50s, 60s, and 70s outrated any other age group. The average number of females among the returning patients decreased significantly after COVID-19, but did not in males. Outpatients under 10 and in their 10s decreased significantly after COVID-19, as did inpatients in their 40s and 60s. The average duration of hospitalization was extended significantly after COVID-19. The number of outpatients and inpatients decreased as time passed after COVID-19. We expect that the results of this study will be used as reference materials in analyzing the effects of COVID-19 on healthcare utilization.

A Case of Septic Arthritis due to Streptococcus intermedius in a Healthy Child (특이 과거력이 없는 소아에서 발생한 Streptococcus intermedius 화농 관절염 1예)

  • Streptococcus intermedius is a small, non-motile, Gram-positive, non-sporeforming, and aerotolerant anaerobic coccus. It is a part of the normal microflora in the oral cavity and upper respiratory, gastrointestinal and female urogenital tracts. It is an opportunistic pathogen that causes serious infections in patients with immunocompromised states or cardiac diseases as a result of trauma or invasive procedures. We describe a case of septic arthritis of the hip caused by S. intermedius in an immunocompetent healthy 7-year-old boy without a history of periodontal disease or invasive procedures. He had hip joint pain three weeks ago, and the fever began on the day of the visit. He had been healthy and had not undergone any invasive procedures recently. Septic arthritis of the hip was indicated in the magnetic resonance imaging of the hip. S. intermedius was identified in the hip joint fluid aspiration and blood culture. He was successfully treated with surgical intervention and antibiotic therapy with ceftriaxone followed by amoxicillin for five weeks.

The Research about Expected Hospital Management in Gynecology area based on the Medical Information Record of a University Hospital (일개 대학병원 의무기록정보 통계를 활용한 산부인과 병원경영 예측에 관한연구)

  • This research is based on the medical information record of a university hospital on analysis of outpatients, inpatients and delivery related information in gynecology area. The result of application having analysis of patients as basic material for hospital management is as follows. The number of new patients in average was 140.9, and as the year passes by, it seemed to decrease noticeably(p=0.000). In the first year (2001) the number was only 212.6, but increased to 140 by the year of 2007, showing increase of 80.9. The actual number of patients in the hospital was 124.6 in average and it showed noticeable decrease after each year (p=0.000), from 144.6 patients on the first year 2001, to 104.8 patients in 2007, showing approximately 40 patients decreased. Multiple regression analysis was performed having independent variables as characteristics of patients and cause of delivery related factors, and dependent variables as the number of patients in the hospital. According to analysis, the cause of affecting the number of patients in the hospital was selected as the number of new patients, the number of delivery per year. The reliability rate was recorded as 62.8%. Therefore, apart from the services on marketing and patient management which must come first, the effect of inviting them cannot be avoided, which directly links to trust resulting from the consideration to patients.

  • Hemophilia is a group of gender-linkage inherited bleeding disorders that impair the body's ability to control blood clotting or coagulation. This sex-linked disorder is transmitted on the X chromosome. These genetic disorders have lower blood plasma clotting factor level of coagulation factor. Most common form is Hemophilia A and B. Restorative dental care and simple surgery for the hemophiliac patient are quite often neglected for fear of bleeding during procedures. Even dental specialist avoid these patients and make them severe problem patients. On the dental treatment especially, invasive procedure, special considerations of bleeding control are required. Inter-consultation with the hematologist will provide orientation on the best approach to dental treatment, such as the need of replacement therapy, the modification of antifibrinolytic therapy, the application of local hemostatic methods. In this case reports, we successfully treat early childhood caries of patient with hemophilia B under the general anesthesia.

The Analysis of Acute Drug Intoxication in Emergency Medical Center (응급의료센터에 내원한 음독환자의 약물중독 분석)

  • In order to investigate acute drug intoxication trends in the elderly who visited emergency medical center, Data were collected from the records of poisoning patients visited five different hospitals from January 1, 2007 to December 31, 2007. The analysis was conducted by using 624 cases from these hospitals. The results are as follows. The study was designed to be divided into two categories of elderly and those under age 65. The criteria were as follows ; male to female ratio, those living together with families, past suicide attempts, acute drug intoxication reasons, management, mental status and the level of sobriety on arrival at the medical centers, and monthly distribution. There was no difference between two groups. In conclusion, it is necessary to develop suicide prevention program for the suicide vulnerable group. Accidental poisonings are going to continue. This means that poisoning prevention education programs must also be developed for periodic use.
