The quantitative analysis of Diffusion Weighted Imaging in Breast MRI (유방 MRI 검사에서 확산강조영상의 정량적 분석)

  • The purpose of this study was to examine the usefulness of diffusion weighted images in breast MRI by performing a quantitative comparative analysis in patients diagnosed with DCIS. On a 3.0T MR scanner, diffusion weighted images and ADC map images were obtained from 20 patients histologically diagnosed with ductal carcinoma in situ (DCIS). The findings from the quantitative image analysis are the following: The diffusion weighted images showed higher SNR and CNR at the lesion area. In addition, the ADC values were lower at the lesion area.

  • The objective of this study is structural analysis of rope brake for elevator. The finite element model was developed to compute the stress, strain and friction force for rope brake. The ANSYS code was used for this analysis. In order to structural analysis of rope brake, many variables such as internal pressure, boundary condition, load condition and constraints were considered.

  • The diagnosis of carcinoma in situ of urinary bladder is difficult in that the symptoms and cystoscopic findings are nonspecific. The cytology of urine could be helpful for diagnosis of carcinoma in situ of urinary bladder. We present a case of bladder washing cytology of carcinoma in situ. A 54-year-old man presented with dysuria for 1 year. Cystoscopic findings revealed multifocal reddish trabeculated lesions. The bladder washing cytology revealed rather uniform tumor cells which were singly scattered of forming syncytium in the clean back-ground. The nuclei were round to oval with inconspicious nucleoli. The cystoscopic biopsy revealed typical histologic features of carcinoma in situ of urinary bladder.

  • The purpose of this study was to examine the usefulness of 3D reconstruction images in breast MRI by performing a quantitative comparative analysis in patients diagnosed with DCIS. On a 3.0T MR scanner, subtraction images and 3D reconstruction images were obtained from 20 patients histologically diagnosed with ductal carcinoma in situ (DCIS). The findings from the quantitative image analysis are the following: The 3D reconstruction images showed higher SNR at the lesion area, ductal area, and fat area that of the subtraction image. In addition, the CNR were not significantly different in the lesion area itself between the subtraction images and 3D reconstruction images.

  • Incidence of basal and squamous cell carcinoma is increasing among Korean. With increasing this incidence we need to attend the management and prevention of these cutaneous malignancies. Basal cell carcinoma (BCC) is the most common skin malignancy. Sun exposure is the most important environmental cause of BCC. Daily sun protection especially avoiding sun exposure between 10 a.m. and 4 p.m. provides most effective prevention against chronic ultraviolet-induced skin damage. Hedgehog pathway inhibitors are used locally advanced and metastatic BCC lesions in Korea. Until recently, there are no reports whether it developed resistance of Hedgehog pathway inhibitors with advanced BCC in Korean patients. Among malignant skin tumors, 19.1% were squamous cell carcinoma (SCC). The incidence of SCC was rapidly increased over the past years. However, the incidence rate of SCC in Japan has not been changed from the period 1976-1980 to 1986-1990. Sentinel lymph node biopsy for SCC did not provide diagnostic value. Bowen's disease and Erythroplasia of Queyrat are considered as carcinoma in situ of the skin. Erythroplasia of Queyrat have worse prognosis with a higher rate of malignant degeneration. Dermoscopy may helpful tool in assisting the noninvasive diagnosis of carcinoma in situ of the skin.

  • Basalold squamous carcinoma (BSC) is a rare, aggressive neoplasm of the upper aerodigestive tract or esophagus. It is characterized by a biphasic pattern in which basaloid tumor is intimately associated with a neoplastic squamous component which can be either Invasive r in situ. Despite its characteristic histologic appearance, the BSC of the esophagus has been confused with esophageal neoplasm variously reported as adenoid cystic carcinoma or carcinoma with adenoid cystic differentiation Their distinction is important because genuine adenoid cystic carcinoma is much less as- gressive than BSC. The biologic course of BSC is similar to that of the more frequent squamous cell carcinoma of the esophagus. We have experienced a case of BSC of the esophagus in a 60-year-old male patient. The lesion was located in the middle third of the esophagus. The patient was treated with surgery followed by radio- therapy.

  • The purpose of this study was to examine the usefulness of IDEAL technique in breast MRI by performing a quantitative comparative analysis in patients diagnosed with DCIS. On a 3.0T MR scanner, fat-suppressed T2-weighted images and T1-weighted images before and after contrast enhancement were obtained from 20 patients histologically diagnosed with ductal carcinoma in situ (DCIS). The findings from the quantitative image analysis are the following: 1) On T2-weighted images, SNR were not significantly different in the lesion area itself between the CHESS and IDEAL groups, while the IDEAL group showed higher SNR at the ductal area and fat area than the CHESS group. In addition, the CNR were higher for the IDEAL group in those regions. 2) On T1-weighted images before enhancement, SNR were not significantly different in the lesion area itself between the CHESS and IDEAL groups, while the IDEAL group showed higher SNR at the ductal area and fat area than the CHESS group. In addition, the CNR were higher for the IDEAL group in those regions. 3) On T1-weighted images after enhancement, SNR were not significantly different in the lesion area itself between the CHESS and IDEAL groups, while the IDEAL group showed higher SNR at the ductal area and fat area than the CHESS group.

  • The microsurgery of the laryngeal lesions was introduced by Kleinsasser in 1965. This has been utilized for diagnosis, surgical management and education of the laryngeal lesions. The laryngomic-roscopic technique appear to be useful in the evaluation of the lesions which may be precancerous, since minor degrees of epithelial thickening and alterations in the pattern of fine vessels beneath the epithelium may be observed with greater clarity. With suspension laryngoscopy, the authors experienced 11 cases of the laryngeal lesions: polyp (4 cases), web formation between false cords and true cords (1), papilloma (4), laryngeal trauma (1), and squamous cell carcinoma (1).

Clinical and Histopathological Studies on Carcinoma of the Uterine Cervix in Taegu (대구지방으 자궁경부암에 재한 임상 및 병리조직학적 연구)

  • Clinical and histopathological studies were made on 202 cases of malignancy of cervix, that were visited to the Department of Obstertrics and Gynecology, Yeungnam University Hospital, during 5 years from 1983 to 1987. The results were summarised as follows. 1. Malignancies of the uterine cervix were 10% of total female malignancies. 2. Among 202 cases, 195 cases(96.5%) were squamous cell carcinoma, in which 60 cases(30.0%) of carcinoma in situ, 9 cases(4%) of microinvasive, and 126 cases(62.5%) of invasive carcinoma were included. 3. The average age of the patients with squamous cell carcinoma was 49.4 years old. ; In cases of carcinoma in situ, it was 43.8, microinvasive, 40.0, invasive 52.1 years old. 4. Clinical symptoms of the patient with squamous cell carcinoma in order of frequency were as follows. ; vaginal bleeding(47.5%), abnormal cytology(15.4%), and abnormal vaginal discharge(9.4%). 5. Duration of the chief complaints was most commanly less than 6 months(73.2%), and the average duration was 3.8 months. 6. The most common age of marriage was between 19 to 22 years 0Id(46.5%). The average was 21.5 years old. 7. The gravity was 51.5% in 5~8 times, and average 6.2 times. The parity was 61.9% in 1~4 times, and average 3.9 times. 8. The subdivision of 126 cases of invasive carcinoma was made according to FIGO stage classification, stage I, 40 cases(31.8%), stage II, 54 cases(42.9%), stage III, 11 cases(8.7%), and stage IV, 8 cases(6.3%). 9. The histologic subtypes of invasive squamous cell carcinoma were distributed as follows. ; large cell keratinizing type, 25 cases(19.8%), large cell nonkeratinizing type, 101 cases(80.2%). 10. In the cytologic diagnosis, class I was 2 cases(l.9%), class II was 16 cases(15.1%), class III was 33 cases(31.1%), class IV was 31 cases(29.3%), class V was 24 cases(24.6%). 11. The frequency of lymph node metastasis was 7.5% in stage I, and 11.1% in stage II.
