Stereotactic radiofrequency dorsal root ganglionotomy can be very useful procedures for the treatment of pain emanating from the lumbar segmental nerves. This procedure is reserved for patients who have failed conservative interventional treatments and in whom open surgical intervention is not an option. The advantages of the radiofrequency lesion method are presented, excellent control of the lesion process using temperature monitoring to quantify the lesion size, prevent boiling, and to produce differential destruction of neural tissue. The afferent fibers in the ventral root which are spared by dorsal rhizotomy but nerve fibers with their cells in the ganglion from either dorsal or ventral root can be destructed with stereotactic radiofrequency ganglionotomy. This technique is performed using a 100 mm cannula with a 5 mm active tip. Repeated lateral fluoroscopic view should be taken to make sure that cannlua still resides within the superior, dorsal quadrant or the foramen. With the cannula in this position, electrostimulation is performed and good paresthesia on the leg should be noted with 0.3 and 0.5 volt at 50 Hz stimulation. At 2Hz stimulation distinct dissociation between motor and sensory should be shown. Percutaneous lumbar ganglionotomy have carried out under local anesthesia on inpatient basis in 6 patients. A series of 5 patients with metastatic cancer pain and a patient with compression fracture have been relieved of pain without serious complications.
목적: 본 연구는 임상에서 종종 마주치는 만성적이고 낫지 않는 항문거근증후군 환자의 한의학적 치료 유용성을 공유하고자 한다. 방법: 14년 전에 치질수술 후에 발생한 항문통증이 개선되지 않고 지속되어 고통스러웠던 47세 남자 환자의 병력과 한의학적 치료 후의 임상적 개선과정을 자세히 제시하였다. 결과: 환자는 평소 건강하였는데, 치질수술 후에 대변을 보면 시작되는 항문통이 발생하였으며, 경우에 따라서는 오전 내내 불편함이 지속되었다. 14년 동안 종종 줄어들기도 하였지만 점진적으로 심해지는 경과를 보였고, 특히 1년 전부터는 더욱 심해졌으나 다양한 치료에도 호전이 없었다. 외국에 거주하는 이유로 한국에 방문하는 기회에 본원에서 한의학적 변증 하에 한약치료 및 약침을 포함하는 침치료와 뜸 치료 후 빠르게 호전되었다. 8주 정도의 치료 후에 NRS 2로 개선되어 만족한 상태로 출국하였다. 결론: 본 증례는 특별한 치료법이 부재하는 만성적이거나 혹은 난치성 항문거근증후군에 대하여 한의학적 치료법이 하나의 훌륭한 치료법일 수 있음을 보여주는 임상 예로서 의미가 있다고 하겠다.
Objective : Although cingulotomy has been applied to patients with affective disorders more frequently, there are numerous reports of its use for the control of severe pain. The goal of this study was to investigate the role of stereotactic bilateral anterior cingulotomy for intractable cancer pain. Method : Between January and June, 2000, we underwent stereotactic bilateral anterior cingulotomy in 6 patients for intractable cancer pain with poor response to opioids. The patients were suffering from widespread musculoskeletal or visceral pain. We made four lesions along the two tracks on either side of the cingulate cortex. Result : In all patients, pain reliefs after cingulotomy were dramatic and immediate. Five out of six patients did not require any opioids and one patient could reduce dose of opioids. There were no deaths or serious complications related to the procedure. Conclusion : These results suggested that a bilateral anterior cingulotomy might be useful method to control intractable cancer pain associated with the widespread metastatic disease. To provide rationale of bilateral anterior cingulotomy in intractable cancer pain, the theoretical mechanisms and role of bilateral anterior cingulotomy are discussed, along with our surgical techniques and the course of our patients.
Many patients with intractable chest pain visit pain clinics, two of which, with rare cases of an intraspinal tumor and malignant mesothelioma were experiences at our clinic. A 37-year old female patient presented with exacerbating chest pain, but without neurological manifestations, of 15-months duration. Her laboratory findings, such as blood tests, chest X-ray, EKG, abdominal ultrasonography and chest CT, were normal. MRI revealed an intradural extramedullary schwannoma at the T 5 and 6 levels of the thoracic spine. She completely recovered following a laminectomy, with removal of the tumor. The other case was a 65-year old male patient, who presented with chest and back pain in the thoracic area of 6 months duration. He had no cough and dyspnea, and was initially misdiagnosed with intercostal neuralgia; therefore, pain control medication was administered, but all trials were ineffective. Finally, chest CT revealed a malignant mesothelioma, with multiple spine metastases. In conclusion, patients with intractable chest pain should be re-examined both clinically and radiographically.
We describe the accidental injection of local anesthetics containing steroid into the subdural space during an attempted lumbar epidural injection for intractable radiculopathy in a patient with failed back surgery syndrome. A 24-year-old man complained of severe radiating pain to left lower extremity and showed a walking disturbance and severe lumbar scoliosis. The MRI finding was a left paramedian recurred disc herniation on L4-5 in a laminectomy state. Several therapeutic modalities such as epidural steroid injection, transforaminal injection, L2 root block, medication, and exercise therapy, etc failed. Initially, during epidural block at L4-5 under fluoroscopic guidance, a railroad track appearance appeared on epidurogram suggesting the presence of a subdural space. A second epidural block was tried at L5-S1. Following confirmation of epidural space upon epidurogram, 6 ml of 0.5% lidocaine including triamcinolone 40 mg was injected. The patient showed signs of the subdural injection including an unexpectedly high sensory block (T2) and a motor weakness of both lower extremities. Following this event, the severe radiculopathy and lumbar scoliosis were improved. Therefore, we conclude that subdural injection of steroid could be helpful in intractable radiculopathy, especially in the failed back surgery syndrome. However, it must be used cautiously with careful patient selection.
Hiccup is defined as an abrupt involuntary contraction of the diaphragm and intercostal muscles with sudden closure of the glottis 35 msec after onset. The term "intractable" is given to those hiccups with a duration ranging from 24 hours to more than 25 years. Short hiccup bouts are mostly associated with gastric distention or alcohol intake, resolved spontaneously or with simple remedies. In contrast, intractable hiccup is a rare but disabling condition which can induce depression, weight loss and sleep deprivation. Although the pathophysiologic mechanisms of hiccup are still poorly understood, wide variety of pathological conditions such as: brain tumor, abdominal tumor, myocardial infarction, renal failure, abdominal surgery etc., can cause intractable hiccup. A 58-year-old male who had suffered from hepatocellular cancer was consulted from medical department due to intractable hiccup. Initial treatment modalities with administrations of metoclopramide and chlorpromazine and nerve block including phrenic nerve block, cervical epidural block and glossopharyngeal nerve block were not effective. Administration of midazolam and baclofen however achieved desirable effect.
A 57-year-old female diagnosed with L5-S1 lumbar intervertebral disc herniation, suffering from severe pain despite taking tapentadol received combined Korean medicine treatment, including acupotomy, acupuncture, pharmacopuncture, and herbal therapies for 53 days. To assess pain, Numeric Rating Scale (NRS) and lumbar range of motion (ROM) were checked daily from the day of admission. Moreover, the Oswestry Disability Index (ODI) and European Quality of Life-5 Dimensions (EQ-5D) were used to evaluate function and quality of life. After combined Korean medicine treatment, reabsorptioin of intervertebral disc was confirmed by radiological examination; pain reduced from NRS 5~7 to NRS 1~2; lumbar ROM in extention increased from 20° to 30°; and function and quality of life improved. The results suggest the possibility that a combined Korean medical treatment, including acupotomy, can be used as an alternative to opioids for pain management of lumbar vertebral disc herniation.
Arthritis of hip joints deteriorates the quality of life in ankylosing spondylitis (AS) patients. Secondary to the articular inflammatory process, the shortened hip-girdle muscles contribute to the decreased joint mobility which may lead to the functional impairment. As the limitation of range of motion (ROM) usually progress slowly, clinicians regard it as a chronic condition and prescribe long-term therapy. However, by short-term intensive multimodal treatment, a 20-year-old man diagnosed as AS with severely limited hip joint ROM who relied on crutches doubled the joint angle and could walk independently only within 2 weeks. The combination included intra-articular steroid injection, electrical twitch obtaining intramuscular stimulation, extracorporeal shock wave therapy, heat, manual therapy, and stretching exercises. The management focused on the relaxation of hip-girdle muscles as well as the direct control of intra-articular inflammation. Hereby, we emphasize the effectiveness of intensive multimodal treatment in improving the function even within a short period.
체외충격파치료술(Extracorporeal Shock Wave Therapy, ESWT)은 보존적인 치료가 어려운 근골격계 난치성 통증 치료 뿐 아니라 심혈관계 질환까지 적용 분야가 확대되고 있는 혁신적인 치료술이다. 본 연구에서는 국내에서 사용되는 집속형 ESWT 치료기의 성능을 결정하는 충격파 음향 출력의 분포를 조사했다. 분석에 사용된 데이터는 식약처에 등록된 30개 기술 문서를 통해 수집했다. 조사 결과, ESWT 치료기 충격파의 집속 특성은, 초점 거리가 5 mm ~ 65 mm, 초점 폭이 3 mm ~ 30 mm, 초점 깊이가 4 mm ~ 108 mm 범위에서 변화하고 있다. 충격파의 최대 양압(P+)는 7 MPa ~ 280 MPa, 초점에서 에너지 밀도 Energy Flux Density(EFD)는 0.0035 mJ/㎟ ~ 35 mJ/㎟, 펄스당 에너지(E)는 0.737 mJ ~ 80.86 mJ로 매우 넓은 범위에서 분포하고 있다. P+ 및 EFD 상관성 분석에 포함된 모든 국내산 PE 방식 제품(5개) 및 1개의 EM 방식 국산 치료기는 P+ 및 EFD가 통상적인 범위를 크게 벗어나고 있으며, 예상되는 상관성을 따르지 않고 있어, 데이터의 신뢰성을 인정하기 어려운 상태이다. 음향 출력의 값에 신뢰성을 부여하기 어려운 경우, 식약처에서 인정하는 시험 검사 기관을 통해 사후 시험 검사 및 관리가 요구된다. 충격파 음향 출력에 대한 통과 기준이 치료기의 적응증에 대한 임상 시험 결과를 근거로 설정될 수 있도록 식약처의 규정 및 가이드라인 개선이 필요하다.
골수염은 골수의 염증을 의미하며 임상적으로는 골을 구성하고 있는 조직등의 염증을 포함한다. 일반적으로 골수에서 시작되어 수질내로 확장되며 피질골, 골막등에 이환되어 골 전반에 걸친 골괴사를 야기시킨다. 원인은 일반적으로 세균에 의한 감염으로 여겨지며 항생제의 발달로 그 유병율이 감소하였으나 여전히 외상, 매독, 만성 신질환, 알코올 중독, 영양결핍, 방사선 조사나 화학적 항암요법은 골의 감염을 위험하는 인자로 알려져 있다. 골수염의 치료는 비교적 까다로우며 난치성으로 진행될 가능성이 있다. 특히 소아에서는 하악구조가 성숙이 덜되어 있어 염증이 쉽고 빠르게 퍼진다. 따라서 소아에서는 골수염을 조기에 발견하고 치료하는 것이 중요하다. 또한 소아는 성장중이므로 악골의 성장 또한 고려해야 하므로 조기발견이 더욱 중요하다고 볼 수 있다. 특히 하악골중에서 하악과두는 하악골 성장의 중요한 곳으로 하악과두의 질환발생시 하악성장율감소, 안면비대칭과 같은 큰 문제를 야기할 수 있어 주의가 요구된다. 하악과두의 골수염은 대부분 치성감염이나 하악골절후의 감염으로 발생한다. 단, 소아의 경우에는 특별한 감염원인 없이 하악의 골수염이 발생했다는 보고도 있다. 이에 저자는 9세의 소아에서 뚜렷한 원인을 찾기 어려운 하악과두의 골수염을 경험하였기에 증례보고와 함께 문헌고찰을 하고자 한다.
