Various torque generating devices have been developed and employed to apply a proper torque. These devices are usually calibrated by the manufacturer to apply appropriate torque levels for their specific implants and attachments. The purpose of this investigation was to determine and compare the accuracy of the torque controllers. In this study, 4 types of torque controllers were used; torque limiting device(TLD), torque indicating device(TID) and contra angle torque driver(CA), electronic torque controller(ETC). Digital torque gauge was employed to measure the de-torque value. Thirty cycles of tightening and loosening were repeated with each torque controller. All implant torque controllers have shown slight errors and deviations. The contra angle torque driver exhibited the most accurate data. In the limitation of this study, it would be recommended that the implant torque controllers should be checked whether uniformed and precise torque can be generated and a measuring error should be corrected.

Purpose: The purpose of this study was to compare the axial displacement of the hexagonal and conical abutment in internal conical connection implant after screw tightening and cyclic loading. Materials and Methods: Internal conical connection implants were divided into two groups (n = 10): group HEX, hexagonal abutment; and group CON, conical 2-piece abutments. The axial displacement and removal torque values were measured after 30 Ncm torque tightening and 250N loading test of 100,000 cycles. The Student t test with 5% significance level was used to evaluate the data. Results: HEX group demonstrated significantly higher axial displacement values after 30 Ncm tightening in comparison to the CON group (P < 0.05). No significant difference was found in axial displacement after cyclic loading (P = 0.052). Removal torque loss before and after the cyclic loading both revealed no significant difference between groups (P = 0.057 and P = 0.138). Removal torque value decreased after cyclic loading in both groups (P < 0.05). Conclusion: Overall, both abutment with or without hexagon index presented similar biomechanical performance except HEX group demonstrated significantly more axial displacement after applying tightening torque.

Tightening of the screws in implant restorations should be accurate and precise. If applied torque is too low, screw loosening would be occurred. With too high torque, the screw fracture might take place. Various torque generating devices are developed and employed to apply a proper torque. The purpose of this investigation was to determine and compare the accuracy of the torque controllers. In this study, 4 types of torque controllers were used; electronic torque controller, torque limiting device, torque indicating device and contra angle torque driver. Digital torque gauge was employed to measure the de-torque value. Thirty cycles of tightening and loosening were done with each torque controller. All implant torque controllers have shown slight errors and deviations. The torque liming device exhibited the most accurate data. No significant difference was found among the mean de-torque values of the electronic torque controller, torque indicating device and contra angle torque driver. In the limitation of this study, it would be recommended that the implant torque controllers should be checked whether uniformed and precise torque can be generated and a measuring error should be corrected.
Purpose: The purpose of this study was to investigate screw joint stability and sagittal fit between internal connection implant fixtures of two different manufacturers and customized abutments. Materials and methods: Internal connection implant systems from two different manufacturers (Biomet 3i system, Astra Tech system) were selected for this study (n=24 for each implant system, total n=48). For 3i implant system, half of the implants were connected with Ti ready-made abutments and the other half implants were connected with Ti CAD-CAM custom ones of domestic-make (Myplant, Raphabio Co., Seoul, Korea) and were classified into Group 1 and Group 2 respectively. Astra implants were divided into Group 3 and Group 4 in the same way. Micro-CT sagittal imaging was performed for fit analysis of interfaces and preloading reverse torque values (RTV) were measured. Results: In the contact length of fixture-abutment interface, there were no significant differences not only between Group 1 and Group 2 but also between Group 3 and Group 4 (Mann-Whitney test, P>.05). However, Group 2 and Group 4 showed higher contact length significantly than Group 1 and Group 3 in abutment-screw interface as well as fixture-screw one (Mann-Whitney test, P<.05). In addition, RTV was lower in CAD-CAM custom abutments compared to ready-made ones (Student t-test, P<.05). Conclusion: It is considered that domestically manufactured CAD-CAM custom abutments have similar fit at the fixture abutment interface and it could be used clinically. However, RTV of CAD-CAM custom abutments should be improved for the increase of clinical application.
Statement of problem: It is difficult about assessing the loosening tendency of conical connection type implant after cyclic loading with diamond like carbon coating one-piece abutments, and also about the retightening effect. Purpose: This study was performed to investigate the influence of one-piece abutment screw retightening after $5.0{\times}10^{4}$ cyclic loading and Diamond like coating Material and methods: Thirty two ITI implant were divided to 4 groups. Group 1,3-titanium abutment, group 2,4 - diamond like carbon coated abutment. Group 1,2 - $20.0{\times}10^{4}$ cyclic loading after $5.0{\times}10^{4}$ cyclic loading, Group 3,4- after $20.0{\times}10^{4}$ Cyclic loading. After cyclic loading, periotest values were taken and removal torque values of abutments were measured with a digital torque gauge. Results: 1. The removal torque of group 2 after $5.0{\times}10^{4}$ cyclic loading is slightly greater than the other groups but not significantly higher than others (P>0.05). 2. The final removal torque values after $20.0{\times}10^{4}$ cyclic loading of group 1 is bigger than group 3, and group 2 is bigger than group 4, but not significantly higher (P>0.05). 3. The final removal torque values after $20.0{\times}10^{4}$ cyclic loading of all groups are not significantly different (P>0.05).
Purpose: The purpose of this study was to investigate the effect of heat applied to disintegrate cement on the removal torque value and fracture strength of titanium abutment and abutment screw. Materials and methods: Implants, titanium abutments and abutment screws were prepared for each 20 piece. Implant abutments and screws were classified as the control group in which no heat was applied and the experimental group was heated in a vacuum furnace to $450^{\circ}C$ for 8 minutes and cooled in air. The abutments and screws were connected to the implants with 30 Ncm tightening torque at interval 10 minutes and the removal torque value was measured 15 minutes later. And the fracture strength of abutment screw was measured using universal testing machine. Results: The mean removal torque value was $27.84{\pm}1.07Ncm$ in the control group and $26.55{\pm}1.56Ncm$ in the experimental group and showed statistically significant difference (P < .05). The mean fracture strength was $731.47{\pm}39.46N$ in the control group and $768.58{\pm}46.73N$ in the experimental group and showed statistically no significant difference (P > .05). Conclusion: The heat applied for cement disintegration significantly reduced the removal torque value of the abutment screw and did not significantly affect fracture strength of the abutment screw. Therefore, in the case of applying heat to disintegrate cement it is necessary to separate the abutment screw or pay attention to the reuse of the heated screw. However further studies are needed to evaluate the clinical reuse of the heated screw.
Purpose: This study was to evaluate the accuracy of the implant torque controller used in dental clinics and to investigate whether it was applied appropriately. Materials and methods: Fifty dentists who work in dental clinics were enrolled in this study. Dental (implant) practice career, experience frequency of implant screw loosening and fracture, education of implant torque controller application and infection control methods were included in the survey. 25 Ncm and 30 Ncm of the tightening torque applied to the implant screw were measured by 50 clinicians. After measuring the torque value by using the torque controller, the torque mean according to where education about the implant torque controller was received was analyzed with independent t-test at the significance level of 0.05. Results: The torque controller used in private dental clinics showed 4.78% error ratio. When 50 dentists applied 25 Ncm to the implant screw was $29.0{\pm}8.4$ Ncm, and that in 30 Ncm was $34.3{\pm}9.1$ Ncm. Statistical significance was found between the group that was educated about implant torque application and the group that was not educated. Conclusion: During the prosthodontic treatment with implant, there was difference between actual applied torsion force and the amount torque controller indicated. Clinicians have to not only be well-informed about the accurate usage method of the torque controller, but also keep and manage the torque controller so as to maintain continuous and accurate torque values. Through this, it is considered to achieve clinical results to minimize problems of screw loosening or fracture.

Purpose: The aim of this study was to investigate the effects of a titanium component for the zirconia abutment in the internal connection implant system on screw loosening under thermocycling conditions. Materials and Methods: Internal connection titanium abutments and external connection zirconia abutments with titanium sockets were connected respectively to screw-shaped internal connection type titanium implants with 30 Ncm tightening. These implant-screw-abutment assemblies were divided into two groups of five specimens each; titanium abutments as control and zirconia abutments with titanium sockets as experimental group. The specimens were subjected to 2,000 thermocycles in water baths at $5^{\circ}C$ and $55^{\circ}C$, with 60 seconds of immersion at each temperature. The removal torque values (RTVs) of the abutment screws of the specimen were measured before and after thermocycling. RTVs pre- and post-thermocycling were investigated in statistics. Results: There was not screw loosening identified by tactile and visual inspection in any of the specimens during or after thermocycling. The mean RTV difference for the control group and the experimental group were $-1.34{\pm}2.53Ncm$ and $-1.26{\pm}2.06Ncm$, respectively. Statistical analysis using an independent t-test revealed that no significant differences were found in the mean RTV difference of the groups (P > 0.05). Conclusion: Within the limitations of this in vitro study, the titanium socket for the zirconia abutment did not show a significant effect on screw loosening under thermal stress compared to the titanium abutment in the internal connection implant.
Purpose: The purpose of this study was to evaluate the effect of abutment material on screw-loosening before and after cyclic loading. Among the different materials of abutments, zirconia and metal abutment were used. Material and methods: Two types of implant systems: external butt joint(US II, Osstem Implant, Korea) and internal conical joint(GS II, Osstem Implant, Korea) were used. In each type, specimens were divided into two different kinds of abutments: zirconia and metal(n=5). The implant was rigidly held in a special holding to device ensure fixation. Abutment was connected to 30 Ncm with digital torque gauge, and was retightened in 30 Ncm after 10 minutes. The initial removal torque values were measured. The same specimens were tightened in 30 Ncm again and held in the cycling loading simulator(Instron, USA) according to ISO/FPIS 1480. Cycling loading tests were performed at loads 10 to 250 N, for 1 million cycles, at 14 Hz,(by subjecting sinusoidal wave from 10 to 250 N at a frequency of 14 Hz for 1 million cycles,) and then postload removal torque values were evaluated. Results: 1. In all samples, the removal values of abutment screw were lower than tightening torque values(30 Ncm), but the phenomenon of the screw loosening was not observed. 2. In both of the implant systems, initial and postload removal torque of zirconia abutment were significantly higher than those of metal abutment(P<.05). 3. In both of the implant systems, the difference in removal torque ratio between zirconia abutment and metal abutment was not significant(P>.05). 4. In metal abutments, the removal torque ratio of GS II system(internal conical joint system) was lower than that of US II system(external butt joint system)(P<.05). 5. In zirconia abutments, the difference in removal torque ratio between the two implant systems was not significant(P>.05). Conclusion: Zirconia abutment had a good screw joint stability in the condition of one million cycling loading.

To analyze the thermal characteristics of wire connection part that is connected to a screw-clamping terminal block depending on screw torque, the normal screw torque defined in KS C 2625 and the abnormal screw torque that can occur due to loosened screws resulting from defective work or aged deterioration were used as experimental variables. After the same load was applied to normal and abnormal screw torques, the thermal characteristics of the wire connector were measured and compared with a thermal imaging camera. The findings from this study will be used to detect heating due to defective screw torque at terminal block connections during electric safety inspection in the future and minimize the hazard of electric fire.
