Bronchiectasis and Pneumonia in an Elderly Patient Treated through Paeong(肺癰) (기관지확장증과 폐렴이 병발한 고령의 환자를 폐옹(肺癰)으로 변증(辨證)한 치험 1례(例))

  • As age distributions of developed countries grow increasingly top-heavy with elderly, instances of bronchiectasis and pneumonia increase. An elderly patient was diagnosed with pneumonia by radiational examination, which showed pound glass opacity in the right middle lobe. Also, he was diagnosed with bronchiectasis in the honeycomb-like recticular area of the right lower lobe. He said that he was suffering from a cough, phlegm and general weakness. He was treated by oriental medicine treatment exclusively, through herbal medicine and acupuncture therapy. Yukmijihwabg-Tang(六味地黃湯), Gamijinhae-Tang(加味鎭咳湯), and Samchulbobi-Tang(參朮補脾湯) As a result, symptoms of cough phlegm and general weakness improved considerably. These results suggest that oriental medicine treatment is effective in improving quality of life fer chronic respiratory patients.

  • A 69 year-old female patient with Bronchiectasis was treated. She was suffering from cough, sputum, fever, sweating, chills and general weakness. Her symptoms were diagnosed as Pe-Ong(肺癰), and she was administered Wikyung-Tang(葦莖湯). After taking Wikyung-Tang(葦莖湯) her symptoms improved. Results suggest that Wikyung-Tang(葦莖湯) is an effective treatment fer Bronchiectasis. Though, results support a role for oriental medical treatment for lung disease, further research into the effectiveness of oriental medical treatments for various lung disease is needed.

A Case Report of a Patient with Bronchiectasis Managed with Sikyungbanha-tang (시경반하탕(보험제제)을 통한 기관지확장증 환자의 관리 1례)

  • Objective: This case reports the management of a patient with bronchiectasis treated with Sikyungbanha-tang, an herbal extract covered by national health insurance. Methods: Twice a year, the patient was treated for a 30-day period with the herbal extract Sikyungbanha-tang. History-taking and a chest X-ray (CXR) were used to evaluate the effects. Results: Following treatment with herbal medicine, the patient's complaints of symptoms decreased, and improvement was shown on CXR. Conclusions: Sikyungbanha-tang in herbal extract form in insured herbal extracts has effects on the management of bronchiectasis patients.

  • Since the advent of potent chemotherapy, the incidence and severity of bronchiectasis have been on the decrease. Up to date, however, the medical treatment of bronchiectasis has not given us much satisfaction. Our purpose here is to show our experiences with pulmonary resections of bronchiectasis to clarify its surgical results and define its surgical indications. Sixty-five patients with bronchiectasis, treated surgically from January 1973 to December, 1982 at the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital, were given clinical assessment. Of the 65 patients involved in the series, 49 were male and 16, female. The patients ranged from 8 to 51 in ages, with 59 cases [91%] between 10 and 39 years old. The prominent clinical features were as follows: cough [88%], sputum production [78%], recurrent upper respiratory infection [51%], and frequent hemoptysis [32%]. The past history of the patients included measles 29%, whooping cough 14%, pulmonary tuberculosis 12%, and pneumonia or bronchitis 12%. The cylindrical type of the bronchiectasis was found to be most common, accounting for 43% of the cases. The operations were performed on the left lung in 52 cases and the right lung in 13 cases. The most common operative procedure was the left lower lobectomy and lingular segmentectomy, which showed 34%. The second most frequent procedure was the simple left lower lobectomy which was 22%. Incomplete resections of the multisegmental bronchiectasis were carried out in 12 cases, of which 7 cases were satisfactory Four patients underwent bilateral pulmonary resections for the severe multisegmental bilateral bronchiectases, during the first and second operations. Improvement in pulmonary symptoms was generally obtained in all four cases. The follow-up ranged from 1 week to 7 tears, with an average of 18.8 months. The overall results revealed that 87% of these and excellent or good conditions, but 13% had persistent symptoms. There was one operative death, which is a 1.5% mortality.

Recurrent Desquamative Interstitial Pneumonia with Fibrotic Lung Disease (폐섬유화를 동반한 재발성 박리성 간질성 폐렴)

  • Desquamative interstitial pneumonia is an uncommon form of interstitial lung diseases and it has a good prognosis compared with other types of idiopathic interstitial pneumonia. A 69-year old man was admitted to our hospital because of a 3-month history of dyspnea. The patient presented with hypoxemia. High-resolution computerized tomography of the patient showed ground glass opacity and traction bronchiectasis with subpleural early honeycombing on the both lung fields. The pathologic findings of the video-assisted thoracoscopy lung biopsy were compatible with desquamative interstitial pneumonia, and irregularly distributed interstitial fibrosis and inflammation were observed at the peripheral parenchyme. Oral predinsolone was started; his symptoms and chest x-ray were improved, and so he stopped taking the prednisolone. Ten months later, the desquamative interstitial pneumonia recurred. We report here on a case of recurrent desquamative interstitial pneumonia with fibrotic lung disease.

Herbal medicine prescription analyses of bronchiectasis patients with claim data during 5 years (2013~2017) (최근 5개년 (2013~2017)간 기관지확장증(J47) 환자에게 처방한 급여한약제제 현황 분석 - 건강보험청구자료 중심으로)

  • Objectives : Korean national health insurance data is a useful real-world data representing whole medical bills submitted to Health Insurance Review Agency. This study aims to understand recent benefit trend of insurance herbal preparations for treating bronchiectasis(disease code J47) utilizing insurance data. Methods : We reviewed national health insurance claims data from 2013 to 2017 which have main diagnosis or sub diagnosis code of J47 and with the record of prescribing insurance herbal medication. Frequency analysis was performed to analyze the most frequently prescribed prescription. Results & Conclusions : Both the number of claims statement(770 to 1,746cases) and patients(266 to 484) of insurance herbal preparations increased considerably from 2013 to 2017. Top 10 preparations based on the number of claims statement were 'Samso-eum', 'Yeonkyopaedok-san', 'Socheongryong-tang', 'Bojungikgi-tang', 'Hyangsapyungwi-san', 'Yijin-tang', 'Saengmaek-san', 'Jaeumganghwa-tang', 'Ojeok-san' and 'Gungha-tang'. Top 10 preparations based on the number of patients were 'Samso-eum', 'Socheongryong-Tang', 'Saengmaek-san', 'Yeonkyopaedok-san', 'Haengso-tang', 'Hyangsapyungwi-san', Yijin-tang', 'Jaeumganghwa-tang', 'Bojungikgi-tang' and 'Hyeonggaeyeongyo-tang' in respectiv order. Claims of top 10 frequent preparations occupied more than 60% of total claims. We hope this finding to be utilized as basic data for future research of evidence-based bronchiectasis treatment utilizing Korean traditional medicine.

  • Immunoglobulin G4 (IgG4)-related lung disease has been actively studied over the past few years. Radiologic findings of IgG4-related lung disease vary among patients, but there are no long-term follow up studies regarding variations in imaging features over the course of disease progression. In two cases with relatively long-term follow up, diverse early and late computed tomography (CT) findings of IgG4-related lung disease are reviewed in this report. In contrast to nodular or diffuse ground-glass opacity, which was predominantly noted in CT scans at earlier stages of disease, honeycombing and traction bronchiectasis were regarded as late radiologic manifestations. Solid nodules might be visible in both early and late stages; however, development of new solid nodules and enlargement of preexisting nodules could occur during disease progression. Interlobular septal thickening and mediastinal/hilar lymphadenopathy were persistent, even in later stages of the disease. These findings might be useful in making an accurate and timely diagnosis of IgG4-related lung disease.

The Differences of the Smoking Habit Between Emphysema and Chronic Bronchitis (폐기종과 만성기관지염의 흡연습관 차이)

  • Background : Smoking is the most important and consistent determinant of the development and progression of COPD(Ed Note : Define COPD). The fact that cigarette smokers develop a different type of COPD, chronic bronchitis and emphysema, with different clinical and pathological aspects, suggests that the development of COPD has a relationship with other smoking-associated factors beyond just a simple smoking history. The aim of this was to analyze the smoking habits and history of patients with COPD and to evaluate the development of different types of COPD according to patient's smoking habits. Method : To evaluate the differences in the smoking patterns of patients with chronic bronchitis and emphysema, a pulmonary function test was conducted, and the smoking history and patterns was obtained through a smoking history questionnaire by a direct personal interview from 333 male cigarette smokers diagnosed with COPD, in the Yeungnam university medical center(190 patients diagnosed with chronic bronchitis, 143 patients diagnosed with emphysema). Result : The patients with emphysema smoked earlier and had a higher smoking history(ie, more packyears, more total amounts of smoked cigarette, and more deep inhalation and longer duration of plain cigarette exposure) than those with chronic bronchitis. The depth of inhalation was also significantly higher in the emphysema patients after taking into account age, cumulative cigarette consumption and the type of cigarette smoked. Conclusion : Emphysema was more associated with the increasing degree of inhalation as assessed by the depth of inhalation. A high alveolar smoke exposure may be a significant risk factor for the development of emphysema.

Effect of bronchial artery embolization in the management of massive hemoptysis : factors influencing rebleeding (대량객혈 환자에서 기관지 동맥색전술의 효과 : 색전술후 재발의 원인과 예측인자)

  • Background : Bronchial artery embolization has been established as an effective means to control hemoptysis, especially in patients with decreased pulmonary function and those with advanced chronic obstructive pulmonary disease. We evaluated the effect of arterial embolization in immediate control of massive hemoptysis and investigated the clinical and angiographic characteristics and the course of patients with reccurrent hemoptysis after initial succeseful embolization. Another purpose of this study was to find predictive that cause rebleeding after bronchial artery embolization. Method : We reviewed 47 cases that underwent bronchial artery embolization for the management of massive hemoptysis, retrospectively. We analyzed angiographic findings in all cases before bronchial artery embolization and also reviewed the angiographic findings of patients that underwent additional bronchial artery embolization for the control of reccurrent hemoptysis to find the clauses of rebleeding. Results : 1) Underlying causes of hemoptysis were pulmonary tuberculosis(n=35), bronchiectasis(n=5), aspergilloma(n=2), lung cancer(n=2), pulmonary A-V malformation(n=1), and unknown cases(n=2). 2) Overal immediate success rate was 94%(n=44), an6 recurrence rate was 40%(n=19). 3) The prognostic factors such as bilaterality, systemic-pulmonary artery shunt, multiple feeding arteries and degree of neovascularity were not statistically correlated with rebleeding tendency (p value>0.05). 4) At additional bronchial artery embolization, Revealed recannalization of previous embolized arteries were 14/18cases(78%) and the presence of new deeding arteries was 8/18cases(44%). 5) The complications(31cases, 66%) such as fever, chest pain, cough, voiding difficulty, paralytic ileus, motor and sensory change of lower extremity, atelectasis and splenic infarction were occured. Conclusion : Recannalization of previous embolized arteries is the major cause of recurrence after bronchial artery embolization. Despite high recurrence rate of hemoptysis, bronchial artery embolization for management of massive hemoptysis is a effective and saute procedure in immediate bleeding control.

Clinical Investigation of Tuberculous Pneumonia (결핵성 폐렴환자의 임상적 고찰)

  • Background : It is difficult to differentiate between tuberculous pneumonia and Community Acquired Pneumonia, so the diagnosis and treatment of tuberculous pneumonia can be delayed frequently. In this study, we attempted to retrospectively evaluate the clinical and radiologic characteristics of tuberculous pneumonia. Methods : We conducted a retrospective analysis of clinical characteristics of 58 patients diagnosed with tuberculous pneumonia from Nov. 1997 to May 2001 at Korea university kuro hospital. Result : The male to female ratio was 1:1 and the mean age at diagnosis was $54.5{\pm}18.6$ years. Fifty five patients were confirmed microbiologically and three patients pathologically. There were 20 patients(34.5%) who had diabetes mellitus(8cases), chronic obstructive pulmonary disease(3cases), malignancy(3cases), bronchiectasis(2cases), chronic renal failure(1cases) or long term history of corticosteroid treatment(3cases). Many patients had multilobar infiltration in chest X-ray, dominantly in the lower lobe. thirty two patients(55.2%) had infiltration in more than 2 lobes and 5 patients in more than 4 lobes. The significant correlation between the diabetes mellitus and the infiltrated Rt lower lobe(RLL) was found on the borders of confidence limit.(P=0.07<0.1). There was significant correlation between woman and infiltrated lobe(RML, RLL, LLL) excluding the both upper lobe(P=0.029). Conclusion : We must consider tuberculous pneumonia when lobar pneumonia with consolidation resistant to antibiotics, especially in the patients who have diabetes mellitus, chronic obstructive pulmonary disease, malignancy, bronchiectsis, chronic renal failure or long term history of corticosteroid treatment.
