THE CYTOTOXIC EFFECTS OF ROOT CANAL SEALERS ON FIBROBLASTS IN HUMAN PERIODONTAL LIGAMENT (근관 충전용 시멘트의 치주인대 섬유아세포 독성에 관한 연구)

  • The purpose of this study was to evaluate for the cytotoxicity of root canal sealers - Tubliseal, N2, AH26, and Sealapex - on fibroblasts cultivated from human periodontal ligament. Succinate dehydrogenase activity test and $^{51}Cr$ release test were performed to evaluate the shortterm cytotoxicity. According to l,3 and 6 days vital cell count and $^{14}C$-leucine incorporation rate to fibroblasts were evaluated. The results of this study were as follows: 1. In the test of SDH activity by millipore filter method, Sealapex showed mild cytotoxicity but Tubliseal, N2 and AH26 showed severe cytotoxicity. 2. In $^{51}Cr$ release test, Tubliseal was the most cytotoxic sealer tested, and rank ordered the relative cytotoxicity of the other sealers tested as follows: AH26, N2, Sealapex. 3. In the test of viable cell count, cytotoxicity in Tubliseal was continued because vital cell number reduced with time. Because vital cell was not showed in N2 and AH26 at 1 day, it was recognized that N2 and AH26 exhibited severe cytotoxicity. In Sealapex, vital cell number increased remarkably with time, so it showed that cytotoxicity decreased with time. 4. In $^{14}C$-leucine incorporation rate test, protein sythesis was not produced in Tubliseal, N2 and AH26 after 3 days, it showed that cytotoxicity in Tubliseal, N2 and AH26 was severe. Although protein synthesis in Sealapex decreased with time, it continued after 6 days. Therefore Sealapex has been exhibited mild cytotoxicity.

A STUDY ON THE RADIOPACITY OF ROOT CANAL SEALERS (근관충전용 시멘트의 방사선 불투과성에 관한 연구)

  • The aim of this study was to establish reproducible method for measurements of radiopacity and to investigate the level of radiopacity of root canal sealers. The experiments were performed in two parts. In the first part, densitometric readings were performed using an aluminum step wedge as a reference at variable voltages and exposure times. Then standard curves for the aluminum step wedge were compared to comprehend the effect of voltage and exposure time. In the second part, on the basis of these results, appropriate conditions for exposure were adopted for standardized measurements of radiopacity. Under standardized set of conditions, densitometric measurements of ten root canal sealers and one gutta-percha point were performed and the levels of radiopacity referable to an equivalent thickness of aluminum were compared. The following results were obtained : 1. At 50 and 60 kVp, increasing the exposure time caused a decrease in the slope of the standard curve for the aluminum step wedge. However, at 70 kVp increasing the exposure time causing a parallel shift of the standard curve to the right. 2. At constant exposure time, increasing the voltage caused a decrease in the slope of the standard curve. 3. The radiopacity of root canal sealers and a gutta-percha point varied between 2.43 mm Al and 9.20 mm Al equivalent. 4. All the root canal sealers had radiopacities more than dentin, and the radiopacity of the gutta-percha point was approximately 5 times as much as that of dentin in terms of equivalent thickness of aluminum. 5. The AH26 had radiopacity more than the gutta-percha point, and the radiopacities of ZOE, Vitapex, Canals, Kerr PCS, Nogenol were similar to that of the gutta-percha point, and Tubliseal, Apatite II, Apatite III, Silapex were less radiopaque than the gutta-percha point.

  • The purpose of this study was to examine the influence of several root canal sealers on the discoloration of internal surface after root canal obstruction. Twenty four sound human premolars, extracted for orthodontic or prosthodontic purposes, were randomly selected and divided into eight groups. Extracted premolars were prepared, and the following seven materials were introduced into the pulp cavities: AH 26, Fuji ionomer (Type I) cement, N2, Oxypara "Murakami", Kerr sealer, PCA sealer, and G-C's Propac ZOE cement. After 7 weeks of incubation; the discolored tooth crowns were hemisectioned, and the internal staining patterns were examined. Then, with an association of observed values the mean intensity scores and percentage of coverage scores of the internal staining patterns in teeth attained by two observers using for Chisquare test were analyzed. The results were as follows: 1. All the experimental premolar's crown showed various ranged discoloration of internal surface. 2. There was no significance between the association scores of two observers participated into this experiments:% coverage scores (P > 0.05) and intensity (P > 0.05) 3. The crowns filled with PCA sealer, AH 26, and Fuji ionomer cement was visible within a depth of one third of dentin. (P> 0.05) 4. For N2, Kerr sealer, and G-C's Propac cement, A slight dentinal staining was recorded, which penetrated up to half way into the dentin. (P> 0.05) 5. It was noticed that the teeth filled with Oxypara "Murakami" were discolored more than two thirds of the dentinal layer. 6. On the control group, there was no discoloration.

COMPARISON OF THE SEALING ABILITY OF VARIOUS ROOT CANAL FILLING TECHNIQUES (근관충전방법에 따른 폐쇄효과의 비교)

  • The purpose of this study was to evaluate the sealing ability of various canal filling methods. Palatal roots of ninety extracted human maxillary molar teeth were resected at cementoenamel junction. Eighty of them were randomly assigned to four experimental groups, ten were served as positive and negative controls. All canals were prepared to # 40 using Profile. Experimental groups were obturated by lateral condensation technique, Thermafil technique, Continuous Wave of Condensation Technique, and down-pack & back-fill technique of Obtura-II, each with root canal sealer. Control groups were not obturated. Teeth were immersed in resorcinol-formaldehyde resin for 5 days at $4^{\circ}C$, and the resin was allowed to polymerize completely for 4 days at room temperature. Teeth were then ground horizontally at 1.5mm(level 1), 2.5mm(level 2), 3.5 mm(level 3) from the anatomical apex and examined with a stereomicroscope at ${\times}40$ magnification. The gap between the canal wall and the filling material, which was filled with the resin, was measured at each of the three levels. Each ratio of leakage was obtained by calculating the ratio of the area of the resin to the total area of the canal and was analyzed statistically (Rank-sum test). The results were as follows : 1. At the level 1, there was the greatest leakage in the Thermafil group and Obtura-II group, and the difference between the Obtura-II group and Continuous Wave of Condensation Technique group was statistically significant(p<0.05). 2. At the level 2, there was the least leakage in the Continuous Wave of Condensation Technique group, but there was no statistically significant difference between each group (p>0.05). 3. At the level 3, there was no statistically significant difference between each group(p>0.05).

A STUDY ON THE EFFECT OF MOISTURE ON APICAL SEALING PROPERTY OF ROOT CANAL (근관충전 시 수분오염이 치근단 폐쇄성에 미치는 영향에 관한 연구)

  • The purpose of this study was to investigate the effect of moisture on apical sealing properties of root canal. Fifty five single rooted human teeth were selected from maxillary and mandibular teeth. After removing crown portion at the cemento-enamel junction, all teeth were routinely prepared with step-back method. And then, the canals were dried with paper point and the teeth were randomly divided into 3 groups of 15 teeth each, and remaining 10 teeth were used as positive and negative control teeth : Group 1 were irrigated with 1ml of 95% alcohol and dried with air and paper point. Group 2 and 3 were intentionally contaminated with 0.05ml of 3.5% NaOCl or saliva, respectively. All the teeth were obturated with sealapex and gutta percha cone by lateral condensation technique, and covered with two coat of nail varnish after 48 hours of obturation. The teeth were immersed in india ink for 1 week and cleaned with methyl salicylate and then the degree of dye penetration were measured with stereomicroscope. The data were analyzed statistically by one-way ANOVA. The results were as follows : 1. All experimental groups showed varying degrees of dye penetration, and the mean degree of dye penetration was 0.1mm to 0.7mm. 2. Saliva contamination group(group 3) showed the highest amount of dye penetration, followed by NaOCl contamination group, then alcohol dried group, but there was no significant difference among three experimental groups. * This results suggest that there was no significant differences of apical leakage after canal obturation between alcohol dried canal and moisture present canals and the use of alcohol instead of paper point is unnecessary to dry the canals prior to canal filling. But other factors such as bacterial contamination and sealer discoloration by moisture must be considered in application of this results to clinical practice.

A STUDY ON THE SEALING ABILITY OF THE THERMAFIL ENDODONTIC OBTURATION TECHNIQUE (Thermafil 충전법의 근관폐쇄성에 관한 연구)

  • The purpose of this study was to evaluate the apical sealing ability of the Thermafil endodontic obturation technique and to compare it with lateral condensation technique. 42 straight canals from extracted human anterior teeth and 42 curved canals(> $25^{\circ}$) from maxillary and mandibular molar teeth were selected. And 80 of them were divided into four groups, 20 canals respectively. The teeth in prior two groups had straight canals and the other two groups had curved canals. The rest of four canals served as positive and negative controls. After resecting anatomical crowns, all canals were prepared using a standard step-back technique. Lateral condensation was used to obturate two groups, one group of straight ones the other curved. And Thermafil obturators were also used in the same two groups. Obturated teeth were infiltrated by India ink for a week, decalcified and cleared with 5% nitric acid and methyl salicylate. The apical leakage and the frequency of filled lateral and accessory canals were measured with stereomicroscope and also apical extrusion of sealer and gutta-percha and obturation time were checked and the data were analyzed statistically(one-way ANOVA, t-test, Chi-square test). The results were as follows : 1. There was no significant difference in the degree of dye penetration between Thermafil and lateral condensation groups(p>0.05). 2. Apical extrusion of sealer and gutta-percha occurred significantly more often with Thermafil obturators in straight canals(p<0.05), but not significantly different in curved canals(p>0.05). 3. Canal obturation time with Thermafil obturators was significantly faster than lateral condensation (p<0.05). 4. The Thermafil groups showed a higher frequency of filled lateral and accessory canals than in the lateral condensation groups. But the difference was not statistically significant (p>0.05).

HISTOLOGIC STUDY OF THE TISSUE RESPONSE TO THREE ROOT CANAL CEMENTS (수종 근관 충전용 시멘트의 조직반응에 관한 조직학적 연구)

  • This study was conducted to evaluate the tissue responses histologically to three root canal cements : Sealapex, AH-26, and zinc oxide-eugenol cement. Twelve white female Sprague-Dawley rats, weighing between 350 and 400 gm, were anesthetized with an intraperitoneal injection of Ketamine hydrochloride(0.4 ml). After shaving the sites selected(left and right scapular areas, left and right pelvic areas), the animal's backs were scrubed with soap and water, and sterilized with absolute alcohol. Each material was mixed to a thin consistency to flow out easily through a 24-guage needle, and loaded into a sterile, disposable plastic 1-ml syringe. All of the rats were injected subcutaneously with 0.1 ml of the three test sealers. Normal saline was used as a control. Animals were sacrificed after 48hr, 1, 4, and 12 weeks by overanesthetization using jars containing anesthetic ether. The tested sites were surgically removed with the surrounding tissue and fixed with 10% formalin. After 48 hours specimens were embedded in paraffin, sectioned to an average thickness of $6{\mu}m$ thick, stained with hematoxylin-eosin. The slides were examined under the light microscope. The results were obtained as follows 1. All material except the control showed various degree of inflammation on 48 hr. 2. Sealapex : In early stage, severe inflammatory cell infiltration was observed. At the 4th weeks observation, graunlomatous tissue with macrophage and foreing body giant cells containing many dark particles in their cytoplasm was observed. 3. AH-26 : Mild inflammatoy reaction was observed with AH-26 throughout the experimental period. 4. Zinc oxide-eugenol cement : Severe inflammatory cell infiltration, necrosis along the material, edema could be seen in early stage. Zinc oxide-eugenol cement maintained a moderate/severe reaction throughout the experimental period.

A effect of calcium hydroxide endodontic materials on the differentiation and the activation of osteoclast (수산화칼슘계 근관 충전제가 파골 세포의 분화 및 활성에 미치는 영향)

  • The purpose of this study was to investigate the direct inhibitory effect of calcium hydroxide materials on differentiation and activation of osteoclast. we used the osteoclast progenitor cells isolated from bone marrow cell of chick embryo tibia and four experimental materials [$Ca(OH)_2$ powder, Metapaste$^{(R)}$, Vitapex$^{(R)}$, Pulpdent$^{(R)}$] diluted at 0.1, 0.01, $0.05{\mu}g/ml$. There were measured both the number of differentiated osteoclast and the area of resorption lacunae. Also, we conducted MTT assay on U2OS osteoblast to examine of cytotoxic effect and obtained following result. 1. Considering the result of the inhibitory effects upon osteoclast differentiation, There were shown a significant difference increased in the following order: Metapaste$^{(R)}$, $Ca(OH)_2$ powder, Vitapex$^{(R)}$. But no significant difference was found in pulpdent group that the number of differentiated osteoclast was increased at all concentrations(p<0.05). 2. Among the three experimental groups, that is, $Ca(OH)_2$ powder, Metapaste$^{(R)}$, Vitapex$^{(R)}$ at $0.1{\mu}g/ml$ dilution that were statistically significant in reduction of the number of differentiated osteoclast. Vitapex group showed significant cytotoxic effect compared to control and another two groups exhibited no significant difference. Also, 0.2% DMSO group was shown statistically siginificant cytotoxicity (p<0.05). 3. Examining pattern and measured area of resorption lacunae in the control and the three experimental groups ,that is, $Ca(OH)_2$ powder, Metapaste$^{(R)}$, Vitapex$^{(R)}$ at $0.1{\mu}g/ml$ dilution, except $Ca(OH)_2$ powder group, statistically significant differences were found between experimental groups and control group. Also, DMSO group showed statistically significant decrease (p<0.05). From these results, we think that calcium hydroxide is responsible for suppression of hard tissue resorption by a direct inhibition of dfferentiation and activation of osteoclast.

ANTIMICROBIAL EFFECT OF ANTIBIOTICS AND ROOT CANAL CEMENTS ON THE PREDOMINANT PATHOGENIC ANAEROBIC MICROFLORA IN ROOT CANALS (근관내 주요 혐기성 병인균에 대한 수종 항생제와 근관충전용 세멘트의 항균효과에 관한 연구)

  • The purpose of this study was to evaluate the susceptibility of anaerobic microorganisms to certain antibiotics and root canal cements. Prevotella intermedia(Bacteroides intermedius) ATCC 25611(serotype A), Fusobacterium nucleatum ATCC 25586, Actinomyces viscosus ATCC 15987 which are the predominant pathogenic anaerobes in dental root canals were cultured in BHI for 48 hours(Fig.1). After each $200{\mu}l$ of those broths with microorganisms was streaked on each surface of blood agar plate, 2 to 5 antibiotic discs which are impregnated with Tetrncycline, Erythromycin, Ampicillin, Clindamycin, or Vancomycin were applied on each surface of blood agar plate and cultured for 5 days anaerobically in the anaerobic chamber (Fig.2). 15 antibiotic discs for each kind of antibiotics and each species of microorganisms were tested. Also each kind of root canal cement tubes which include Zinc oxide eugenol cement, Zinc phosphate cement, Calcium hydroxide powder+DD.W., Calcium hydroxide paste(Pulpdent Tempcanal), or Vitapex(Table 1) were applied on the inoculated BAPs after $200{\mu}l$ of each experimental species of microorganisms was streaked on the surface of blood agar plates, and they were cultured for 5 days anaerobically in the anaerobic chamber(Fig.3). The sensitivity(antimicrobial effect) was determined by the diameter of the inhibition zone. The results are as follows: 1. The results of antibiotic susceptibility test(Table 2) 1) All of the tested antibiotics had antimicrobial activity with various degrees. 2) In Prevotella intermedia (old Bacteroides intermedius), the diameter of inhibition zone to Erythromycin($37.87mm{\pm}2.20$) was largest, those to Tetracycline($26.20mm{\pm}2.96$), Vancomycin($21.53mm{\pm}1.96$), Clindamycin($18.73mm{\pm}0.96$) was smaller than former orderly, and That to Ampicillin ($7.87mm{\pm}0.83$) was smallest. 3) In Actinomyces viscosus, the diameter of inhibition zone to Erythromycin($28.73mm{\pm}1.22$) was largest, those to Ampicillin($21.73mm{\pm}1.03$), Clindamycin($21.33mm{\pm}1.59$) was similarly next order, that to Vancomycin($19.00mm{\pm}1.96$) was smaller than Clindamycin, and that to Tetracycline($11.93mm{\pm}0.70$) was smallest. 4) In Fusobacterium nucleatum, the diameter of inhibition zone to Ampicillin($31.07mm{\pm}1.91$) was largest, that to Erythromycin($28.87mm{\pm}0.92$), Clindamycin($20.47mm{\pm}1.51$), Vancomycin ($16.73mm{\pm}0.96$), Tetracycline ($12.13mm{\pm}1.06$) are smaller than former orderly. 2. The results of root canal cements and pastes(Table 3) 1) The external diameter of tube is 4mm, so 4mm of the inhibition zone diameter means non-susceptable. Prevotella intermedia (old Bacteroides intermedius) was non-susceptable to Calcium hydroxide powder+D.D.W., Calcium hydroxide paste(pulpdent Tempcanal), and Actinomyces viscosus was non-susceptable to Zinc phosphate cement, Calcium hydroxide powder + D.D.W., Calcium hydroxide paste(pulpdent Tempcanal). 2) In Prevotella intermedia (old Bacteroides intermedius), the diameter of inhibition zone to Zinc oxide eugenol cement($13.67mm{\pm}3.30$) was largest, that to Vitapex($9.20mm{\pm}2.96$), Zinc phosphate cement($6.13mm{\pm}2.07$) was smaller than former. 3) In Actinomyces viscosus, the diameter of inhibition zone to Zinc oxide eugenol cement($17.40mm{\pm}5.20$) was largest and that to Vitapex($8.80mm{\pm}1.70$) was next order. 4) In Fusobacterium nucleatum, the diameter of inhibition zone to Vitapex($42.33mm{\pm}17.2$) was largest and those to Calcium hydroxide paste(Pulpdent Tempcanal)($14.47mm{\pm}3.72$) and Zinc oxide eugenol cement($8.93mm{\pm}2.71$), Zinc phosphate cement($8.20mm{\pm}2.27$), Calcium hydroxide powder+D.D.W.($5.53mm{\pm}2.10$)was next orderly. And then In Zinc oxide eugenol cement and Zinc phosphate cement group, two of fifteen samples showed no inhibition zone, in Calcium hydroxide powder + D.D.W. group, 8 of 15 samples showed no inhibition zone.

  • In this study we measured the amount of light energy that was projected through the tooth material and analyzed the degree of polymerization by measuring the surface hardness of composites. For polymerization, Optilux 501 (Demetron, USA) with two types of light guide was used: a 12 mm diameter light guide with 840 nW/$cm^2$ light intensity and a 7 mm diameter turbo light guide with 1100 nW/$cm^2$. Specimens were divided into three groups according to thickness of penetrating tooth (1 mm, 2 mm, 0 mm). Each group was further divided into four subgroups according to type of light guide and curing time (20 seconds, 40 seconds). Vickers' hardness was measured by using a microhardness tester. In 0 mm and 1 mm penetrating tooth group, which were polymerized by a turbo light guide for 40 seconds, showed the highest hardness values. The specimens from 2 mm penetrating tooth group, which were polymerized for 20 seconds, demonstrated the lowest hardness regardless of the types of light guides (p < 0.05). The results of this study suggest that, when projecting tooth material over a specified thickness, the increase of polymerization will be limited even if light intensity or curing time is increased.
