Clinical features of congenital muscular torticollis (선천성 근성 사경의 임상적 특징)

  • Purpose : Congenital muscular torticollis (CMT) is a common and benign congenital disorder of the musculoskeletal system in neonates and infants. The pathophysiology is that the sternocleidomastoid muscle (SCM) is shortened on the involved side by fibrosis, leading to ipsilateral tilt and contralateral rotation of the face and chin. In this study, we investigated the clinical features of CMT, the role of ultrasonography (USG) in prediction of prognoses and the clinical significance of early detection and treatment. Methods : Forty seven patients (M:F=31:16) were diagnosed as a CMT between March 2003 and May 2006. We reviewed age at diagnosis, physical findings, USG findings, treatment and therapeutic outcome from their medical records. Results : The median age at diagnosis was 90 days (18 days-9 years, 7 months) and the right side of neck was affected in more patients (right : left=26:21). Of 24 patients with a palpable neck mass, 21 had USG; 19 cases showed sternocleidomastoid tumor (SMT). In cases with no neck mass, USG was performed in 11 patients; seven had postural torticollis (POST), three had SMT and one had muscular torticollis (MT). Among 40 patients with follow-up, 36 had total resolution. There was negative correlation between the age at diagnosis and the recovery time, whereas the final outcome was not correlated with USG findings. However, the patients without positive findings in USG had earlier resolution (1 month vs 2.6 months, P=0.0008). The patients with SMT had earlier diagnosis and excellent outcomes. The patients with MT were delayed to diagnosis and had the longest time to resolve. Lastly, the patients with POST had delayed diagnoses, but they had excellent outcomes. Conclusion : Since the patients with delayed diagnoses, in despite of benign courses, may take a long time to resolve and rarely need surgical treatment, it is important to diagnose and treat early. This study showed that USG findings of the SCM may be used as predictive factors.

Effects of skin temperature change, cold pain and muscle activity by Cold Air Application type on the induced delayed onset muscle soreness (지연성 근육통 유발 후 냉기 적용 방법이 피부온도의 변화, 냉각 통과 근육 기능에 미치는 효과)

  • The purpose of this study was to after induced delayed onset muscle soreness, the purpose of this study is to present the effective way on skin temperature changes and cold pain for 14 subjects during to applied with only Cold-jet stream and Cold-jet stream with infra red. 14 healthy men and women who delayed onset muscle soreness eccentric contractions induced by exercise and then the biceps alone was applied to the Cold-jet stream. We measured the time that skin temperature fall from room temperature to $10^{\circ}C$(first period), the time rewarmed from $10^{\circ}C$ to $20^{\circ}C$(second period), the time fall again to $10^{\circ}C$(third period) and the time rewarmed again to $20^{\circ}C$(fourth period). Cold-jet stream with infrared combination therapy was performed with the same method. Results of this study were 1st and 2nd experimental cooling experiments in the to fall to $10^{\circ}C$ Cold-jet stream with infrared combination therapy than in the Cold-jet stream was longer(p<.05). At second period, It took longer in Cold-jet stream with infrared rewarmed than Cold-jet stream to rewarm skin(p<.05). Cold-jet stream with infrared combination therapy than Cold-jet stream had less incidence of cold pain(p<.05). Thickness of biceps brachii were found significant difference related measurment each group was consistent. In this study, Cold-jet stream with infrared combination therapy more effective than Cold-jet stream in reduced cold pain and lowering skin temperature. Clinical and Laboratory Features of Korean Mucopolysaccharidoses (MPSs) (한국 뮤코 다당체 침착증 환자에 대한 임상적 고찰)

  • Purpose : The mucopolysaccharidoses (MPSs) are a heterogeneous group of lysosomal storage disorders. They are caused by a deficiency of the enzymes involved in the degradation of glycosaminoglycans. Early recognition is important because recombinant enzyme replacement therapy is now available for MPS. We studied the clinical characteristics of 80 MPS children with the object of determining the epidemiological, clinical and radiological features in Korean MPS children. Methods : Diagnosis of MPS was confirmed by skin fibroblast enzyme analysis in 80 patients between February 1995 and December 2004. Charts were retrospectively reviewed for clinical and radiological findings, as well as for intelligence and speech evaluations. Results : Hunter syndrome (MPS type II) was the most prevalent type, appearing in 51/80 cases (64 %), followed by Sanfilippo syndrome (MPS III-18%), Hurler syndrome (MPS I-15%), and Morquio syndrome (MPS IV-4%). The average age at diagnosis was 5.5 years (range 1 to 20), and the male-to-female ratio was 4.7 : 1. Typical radiographic changes were observed in 45/54 cases (83%). Mitral regurgitation was the most common cardiac defect. Moderate to profound mental retardation and hearing loss were present in 14/35 cases (56%) and 33/38 cases (82%), respectively. Four MPS II patients had bone marrow transplantation, with mixed outcomes. Five MPS I patients are currently on enzyme replacement therapy. Conclusion : Our study showed a high proportion of MPS II cases (64%), which may represent population variability. By studying the clinical features of these patients, we hope to alert pediatricians of the warning signs of MPS.

The Immediate Effects of Pectoralis Minor Self-stretching Exercise on Muscle Length and Acromio-humeral Distance in Normal Adults (작은가슴근 자가 신장운동이 정상 성인의 작은가슴근 길이와 봉우리-위팔 거리에 미치는 즉각적인 효과)

  • Shoulder joint pain is a very common musculoskeletal disorder, in which about 20 percent of adults experience it more than once in their lives. In this study, an objective assessment was made that subacromial space was increased by pectoralis minor stretching exercise, which has a positive effect on impingement syndrome and shoulder pain. Also, This study examined the effects on pectoralis minor length(PML) and acromio-humeral distance(AHD) following pectoralis minor self-stretching exercise in normal adults. In this study, a total of 30 subjects were recruited and randomly divided into a self-stretching exercise group(n=15) and manual stretching exercise group(n=15) to compare the immediate effects of treatment. Studies have shown that both groups have significantly increased both PML and AHD within the group(p<.05). There was no statistically significant difference in the comparison between the groups(p>.05). The results suggest that self-stretching exercise can be recommended as a treatment method for impingement syndrome and prevention of shoulder disease.

The Case Report of Posterior Headache Caused by Traffic Accident Treated with Musculoskeletal Ultrasound-guided Acupotomy Therapy (교통사고 후 발생한 후두통의 근골격계 초음파를 이용한 침도치료 임상증례)

  • Objectives : The objective of this case report was to observe the effect of musculoskeletal ultrasound-guided acupotomy therapy on posterior headache caused by traffic accident. Methods : Musculoskeletal ultrasound-guided acupotomy therapy was performed to two patients whose brain MRI or CT results were normal, but posterior headache did not improve with general eastern medical treatment. Results : One patient's VAS(visual analogue scale) of posterior headache was decreased for the first time after ultrasound-guided acupotomy therapy, and steadily reduced. The other patient's VAS was also decreased for the first time after musculoskeletal ultrasound-guided acupotomy therapy. Two patient's Korean HIT-6(Korean headache impact test-6) scores were decreased after one month. Conclusions : If general eastern medical treatment had little effect on traffic accident induced posterior headache, musculoskeletal ultrasound-guided acupotomy therapy can be applied.

Correlation of Clinical Outcome and Cuff Integrity after Open Repair in Large and Massive Rotator Cuff Tears (대형 및 광범위 회전근 개 파열의 개방적 봉합술 후 임상적 결과와 회전근 개 연속성(integrity)의 상관 관계)

  • Purpose: To evaluate the relationship between the clinical outcome and the cuff integrity following open repair in large and massive rotator cuff tears using ultrasonography as an imaging modality. Material and Methods: From November 2001 to April 2005, 17 cases(16 patients) who had open repair of tear measuring more than 3cm were assessed with minimal follow-up of 12 months in this study. 6 cases had a large tear and 11 cases a massive tear. There were 6 men and 11 women with a mean age of 52 years at surgery (range, 33 to 72 years). The evaluation consisted of the preoperative and postoperative shoulder scores according to UCLA shoulder scoring system and Visual analogue scale (VAS). Ultrasonography was performed by a experienced musculoskeletal physician at a minimum of 12 months postoperatively to evaluate the postoperative cuff integrity. Results: Retear were detected in four of seventeen cases. Regardless of the presence of recurrent tear, 14 cases had UCLA score of more than 29 points (good grade). All 17 had an improvement in the functional score, which increased from an average of 15.1 to 31.2 points. All cases showed pain relief and five cases had no pain. Sixteen cases except one had the range of motion of forward elevation above $90^{\circ}$. Conclusion: Open rotator cuff repair in large and massive tears showed low retear rate. At a minimum of twelve months follow-up, all cases had improvement on UCLA score, pain relief, increased range of motion of the shoulder regardless of retear. And the correlation between recurrent tear and function score was not statistically significant.

The Effects of Speed Variations in Treadmill Training on Thickness of Lower Extremity Muscles (속도가 다른 트레드밀 훈련이 다리 근두께에 미치는 영향)

  • The purpose of this study was to determine the effects of speed variations in treadmill training on the thickness of lower extremity muscles. A total of 36 university students were divided into three groups: MVTG (n=12), HVTG (n=12), and ATG (n=12). Subjects in MVTG underwent treadmill training with their own average speed; those in HVTG underwent treadmill training with 130% speed of their own average speed; and those in ATG underwent treadmill training with alteration of speed, between 100% and 130% of their own average speed. Treadmill training was performed for 60 minutes a day, three times per week, for a total of 6 weeks. Ultrasonography was used to compare the muscle thickness between rectus femoris, vastus lateralis, gastrocnemius, and tibialis anterior. The result was as follows: The rectus femoris, vastus lateralis, and gastrocnemius were significantly increased after the training period, and they also were statistically significant in interaction. Moreover, tibialis anterior was also significantly increased. Therefore, compared to the average-speed treadmill training, speed variations had a greater effect on thickening the lower extremity muscles. In the near future, we will conduct a study applying the findings from this study in a rehabilitation program for patients with gait disturbance due to nervous or musculoskeletal system disease.

Diagnostic Approach to a Soft Tissue Mass (연부조직 종양의 진단적 접근)

  • Soft tissue masses of the extremities and torso are a common problem encountered by orthopaedic surgeons. Although these soft tissue masses are often benign, orthopaedic surgeons need to recognize the key features differentiating benign and malignant masses. An understanding of the epidemiology and clinical presentation of soft tissue masses is needed to develop a practical approach for evaluation and surgical management. Size and depth are the two most important factors on which triage decisions should be based. In a differential diagnosis of a tumor, it is important to know the characteristics of the soft tissue mass through detailed history taking and physical examinations before the diagnostic procedures. A variety of imaging studies, such as simple radiography, ultrasound, magnetic resonance imaging, positron emission tomography, computed tomography, bone scan, and angiography can be used to diagnose tumors. Know the ledge of advantages and disadvantages of each imaging study is essential for confirming the characteristics of the tumor that can be observed in the image. In particular, ultrasonography is convenient because it can be performed easily in an outpatient clinic and its cost is lower than other image studies. On the other hand, the accuracy of the test is affected by the skill of the examiner. A biopsy should be performed to confirm the tumor and be performed after all imaging studies have been done but before the final treatment of soft tissue tumors. When a biopsy is to be performed, careful attention to detail with respect to multidisciplinary coordination beforehand, cautious execution of the procedure to minimize complications, and expedient follow-up and referral to a musculoskeletal oncologist when appropriate, are essential.

Clinical Analysis of Ectopic Kidney in Children (소아 이소성 신장의 임상적 고찰)

  • Purpose : When the mature kidney fails to reach its norml location in the renal fossa, the condition is known as ectopic kidney. Presenting symtoms can be various and it generally depend on the associated anomaly. Beside urologic anomalies such as hydronephrosis and vesicoureteral reflux, various anomalous vascular net work, skeletal anomaly or genital anomaly can be observed in this condition. Methods : Sixteen children with ectopic kidney was studied retrospectively to analyse initial presentation, accompanied anomaly and prognosis. Results : 56% of the children were accompanied with other urologic anomalies such as true incontinence and vesicoureteral reflux that required surgical treatment. 31% of children were either diagnosed incidentally during evaluation of other non-urologic disease or during follow-up evaluation of abnormal antenatal renal sonogram. Conclusion : Ectopic kidney can be often misdiagnosed as tumorous condition or as a surgical condition depend on the abnormal location of the kidney. Careful evaluation using abdominal sonogram, DMSA, VCUG and abdominal CT scan should be performed in order to search for associated anomalous condition and for proper management.

Multicenter clinical study on birth weight and associated anomalies of single umbilical artery (단일 배꼽 동맥 환아의 출생체중과 동반기형에 관한 다기관 연구)

  • Purpose : To compare birth weight between infants with a single umbilical artery (SUA) and normal infants, investigate the associated anomalies of infants with SUA and isolated SUA (no abnormality of external appearance on birth, except SUA), and determine the prognosis of infants with isolated SUA. Methods : Live-born infants with SUA (n=59) detected by physical examination from among 15,193 live births in seven university hospitals in Korea between January 1, 2004, to August 1, 2007, were reviewed retrospectively, with 236 normal infants serving as the control group. Results : A statistical difference was observed between the groups in birth weight and in vitro fertilization. The incidence of infants with SUA was 0.37%. Congenital malformations were observed in 21 infants with cardiovascular (n=15, 25.4%), gastrointestinal (n=2, 3.4%), genitourinary (n=9, 15.3%), neuromusculoskeletal (n=6, 10.2%), central nervous system (n=1, 1.7%), chromosomal (n=1, 1.7%), and other (n=3, 5.1%) abnormalities. There were 49 (83.1%) infants with isolated SUA in this study population; among them, the associated congenital malformations were cardiovascular (n=6, 12.2%) and genitourinary (n=6, 12.2%) abnormalities. Two infants with cyanotic heart disease were operated and four infants with acyanotic heart disease showed improvements without any treatment. Six infants with genitourinary abnormalities on renal ultrasound had mild hydronephrosis without further consequences. Conclusion : The incidence of structural abnormalities in the cardiovascular and genitourinary systems is high and the genitourinary anomalies associated with isolated SUA have relatively good prognosis.
