Purpose: We wished to evaluate the clinical usefulness of ultrasound, when loose body is palpated and is shown radiographically in knee. Material and Methods: Twenty-five knees of twenty-two patients with loose body in knee radiographically from March, 2007 to July, 2009 were involved in this study, there were 8 males and 14 females, there were 11 right knees, 8 left knees and 3 both knees, the average age was 56.3 years (range, 32~76 years). The location of loose bodies were 13 cases at suprapatella pouch, 1 case at infrapatella anterior compartment and 11 cases at posterior compartment. In loose bodies of suprapatella pouch, we examined the mobility of loose bodies longitudinally with ultrasound placed at superior patella pole while flexing and extending the knee. In loose bodies of infrapatella anterior and posterior compartment, we examined the mobility of loose bodies longitudinally and transversely with ultrasound while flexing and extending the knee. Results: All cases of suprapatella pouch and infrapatella anterior compartment were diagnosed to movable loose bodies and were removed surgically, all cases of posterior compartment were diagnosed to immovable loose bodies and were not removed. Conclusion: When we determined the surgical removal according to mobility of loose body in knee, ultrasound may be helpful clinically.

The purpose of this study was to investigate whether medial gastrocnemius ultrasound imaging of the Delayed Onset Muscle Soreness (DOMS) has the possibilities as a measurement method. This study was conducted from April 21th 2011 to April 30th 2011. Thirty-five healthy subjects were included based on the absence of regular physical activity, and no history of recent trauma, musculoskeletal pathology, cardiovascular disease or drug intake. All subjects induced DOMS through climbing for 5 hours and we measured the visual analogue scale (VAS), creatine kinase (CK) and maximal voluntary isometric contraction (MVlC) of ankle plantar flexor prior to DOMS and at 24, 48 and 72 hours post DOMS and these measurements were compared with pennation angle of medial gastrocnemius measured by ultrasound imaging. Results of this study were as following. VAS, CK, and MVIC of ankle plantar flexor were found significant difference related measurement period (p<0.05) and pennation angle of medial gastrocnemius were found significant difference related measurement period (p<0.05). Furthermore, we confirmed that the flow of change between variables related measurment period was consistent. Through this study, we think that measuring the changes in pennation angle of medial gastrocnemius over time using ultrasound imaging will be able to be used as a new method measuring DOMS.
Background: Fibrostenosis of large airway due to tuberculosis is one of the most perplexing clinical problems not only because it can lead to respiratory failure but also because of difficulty in the management. No one technique, such as balloon dilatation or insertion of self expandable metallic stent, has proved totally satisfactory in the management of fibrostenosis. We evaluated the effect of laser therapy in patient with severe fibrostenosis due to tuberculosis. Method: We classified the fibrostenosis to three types by bronchoscopic finding - the diaphragm type: stenosed by fibrous diaphragm, sparing the tracheobronchial wall, the collapse type: stenosed by collapse of the wall due to destruction of the cartilage, and the combined type: stenosed by nonspecific inflammatory scar tissue within internal lumen with collapse of the wall. We have treated 10 patients complaining dyspnea due to with severe fibrostenosis of the diaphargm or the combined type using a neodymiumyttrium aluminum garnet(Nd-Y AG) laser through a flexible bronchoscopy. Results: Eight of the 10 cases improved after laser therapy and maintained during a follow up period of average 31.9 months. All of the cases undergoing laser therapy showed no serious complication to need the therapy. Conclusion: The results of our present study indicate that the Nd-YAG laser therapy is an effective and safe method for the management of selective tuberculous fibrostenosis.
Purpose: The purpose of this study was to make a report on the clinical prognosis of post traumatic lateral and posterolateral instability of the knee after LCL augmentation and popliteal tenodesis. Materials and Methods: The assessment was made among 21 cases who underwent augmentation of lateral collateral ligament(LCL) and popliteal tenodesis with allograft or artificial ligament (synthetic polyester, ABC ligament, Surgicraft, U.K.) at this Medical Center during the period from July 1996 to July 2003 and whose follow-up period was longer than one year. The authors recorded and analysed the physical findings (external rotation recurvatum test & posterolateral drawer test), stress roentgenograms(preoperative and postoperative) and Lysholm score. Results: The lateral and posterolateral instability of the knee were improved in 20 cases postoperatively, acccording to the clinical test and stress roentgenograms . Average Lysholm score was 52.5 preoperatively and 86.7 postoperatively .Conclusion: Our study found the surgery of lateral and posterolateral instability of the knee with augmentation of LCL and popliteal tenodesis using allograft or artiflcial ligament is simple technique. Taking these results into consideration, it seems to be one of effective methods of treatment.
Background: Analysis of cells in bronchoalveolar lavage(BAL) fluid had been used to predict the histologic changes of the bronchioles and alveoli in patients with interstitial lung diseases(ILD). Definitive diagnosis can be a1so made in some cases of ILD, such as histiocytosis. However, there are a few data of the cellular components in BAL fluid in normal Korean individuals and in patients with ILD. In order to evaluate the role of the cellular analysis of BAL fluid in prediction of alveolitis and differential diagnosis among ILDs, we compared the cellular components in BAL fluid from 50 normal individuals and 86 ILD patients. Method: BAL was performed by instillation and retrievement of normal saline with fiberoptic bronchoscopy. The cell number was counted by Hemocytometer. Differential count was done up to 500 cells on slides prepared by Diff-Quik stain and non-specific esterase stain. We compared the recovery rate(RR), cell numbers(CN), and percentages of each cellular components(CP). Results: The results were as follows: 1) There was no difference in RR, CN and CP between the normal smoker group and normal non-smoker group. 2) Total cell numbers recoverd in BAL fluid increased in collagen vascular diseases(CVD), hypersensitivity pneumonitis(HP), idiopathic pulmonary fibrosis(IPF), and miliary tuberculosis(Mil TBC) groups. 3) The percentage of lymphocytes increased in HP, IPF and Mil TBC groups. Macrophage percentages increased in HP, IPF, and Mil TBC groups. Neutrophil percentages were increased in CVD, HP, IPF and Mil TBC groups. Eosinophil percentages were increased in HP, IPF and Mil TBC groups. The numbers of each cells showed same findings as the percentages did. Conclusion: The analysis of cellular components of BAL fluid can predict the presence of alveolitis in many cases of ILDs. However, It was not helpful in differential diagnosis among ILDs.
Purpose : We have evaluated the outcome of the operative treatment of PCL rupture using the achilles tendon allograft. Materials and Methods : A retrospective study was completed for 30-PCL reconstruction cases, using the achilles tendon allograft from september 1996 to march 1998. There was an average follow up of 24 months, with range of 12 to 50 months. Results : The active range of motion was improved postoperatively. The Lysholm Knee Score was improved from a mean of 54 points preoperatively to 87 points postoerative 18 months. The "Cybex 340 isometric test" revealed satisfactory results compared with autograft. Conclusions : Use of allograft for recostruction of the PCL is an attractive option because it precludes the harvesting of autogenous tissue in a knee that is already at risk for patellofemoral and tibiofemoral osteoarthrosis.

Purpose: The purpose of this study is to analyze the merits and demerits of double bundle reconstruction and achieve improvements hereafter, by comparing the results of double bundle and single bundle reconstruction using tibialis anterior allograft. Materials and Methods: Twenty seven patients were divided to undergo either double bundle(n=14) or single bundle(n=13) reconstruction with tibialis anterior allograft tendon. The evaluation methods were AP laxity with KT-2000 arthrometer, isokinetic knee strength measurements, pivot-shift test, IKDC subjective score, Lysholm knee score, Tegner activity score, radiographic evaluations with postoperative MRI, and second look arthroscopy. Results: Lysholm knee score and Tegner activity score were significantly better in double bundle reconstruction. In pivot-shift test, single bundle reconstruction was evaluated as grade 0 in 10 of the knees, grade 1 in 1, and grade 2 in 2. Double bundle reconstruction was evaluated as grade 0 in 13, and grade 2 in 1. In second look arthroscopy, single bundle was evaluated as excellent in 6 of the knees, fair in 7, anteromedial bundle of double bundle reconstruction was excellent in 13 and fair in 1, and posterolateral bundle was excellent in 4, fair in 9, and poor in 1. There were no significant differences in other evaluations. Conclusion: Favorable outcome may be expected with double bundle reconstruction of ACL. However there are still need for improvement in terms of reconstruction technique and rehabilitation protocol to reduce PL bundle injury.

This study used silicon that is similar to the density of the tissue of the human body to compensate for the uneven areas that are in contact with air in order to reduce susceptible artifacts. The subjects of the study were 16 normal people and the areas of the human body in which there are a lot of uneven areas with complicated structure and a lot of susceptible artifacts were formed since the surface area that comes into contact with the air is large were the areas that were chosen to be examined. A 3.0T superconducting magnetic resonance device was used as the test equipment and SPIR images that are sensitive to magnetic differences were obtained as sagittal planes on a line that extended the metatarsal and the phalanges, including the middle of the longitudinal arc and the 5 distal phalanxes. The method of analysis was to reduce the susceptibility between the tissue and the air to discover the reduction of susceptible artifacts by comparing the SNR and CNR before and after applying silicon. A statistical analysis was utilized for the sample matching T examination. The results of the study revealed that the susceptible artifacts were reduced in the images of the uneven areas that were compensated and applied with silicon. The SNR increased in significant amount in correlation from $3.91{\pm}1.33$ before application to $21.69{\pm}4.52$ after application and the CNR decreased in significant amount in correlation from $28.97{\pm}8.20$ before application to $4.88{\pm}2.14$. In conclusion, this study did not affect the voxel but it was an innovative method of improvement that compensated for the fundamental issue of the difference in susceptibility between the air and the body. The application is simple and the study has great significance in that it proposed a method to reduce susceptible artifacts in a low cost and highly efficient manner.
Purpose: The purpose of this prospective study is to compare the results of arthroscopic reconstruction of ACL using autologous hamstring tendon (Group 1) and autologous bone patella tendon bone (Group 2). Materials and Methods: From Jan. 2004 to Dec. 2007, fifty patients were analyzed in this study. A single surgeon performed the ACL reconstruction with autologous hamstring tendon (25 patients) and autologous bone patella tendon bone (25 patients) alternatively each other. The mean follow up period of two groups was 38 months (range 25~58 months). We evaluated the result of Lachman test, Pivot shift test as a physical examination and Lysholm score, Tagner activity scale as patients' satisfaction and functional status and Telometer for anterior instability of each groups at the time of final follow up. Results: In final results, negative or mild positive findings on Lachman test were 24 out of 25 cases (equally) in each groups.) Twenty two cases in the Group 1 and 21 cases in the Group 2 were negative on Pivot shift test. Lysholm scores mark 94.6 points in group 1 and 92.3 points in group 2 at the final follow up and Tegner activity scales mark 8.5 points (Group 1) and 8.1 points (Group 2) at the time of last follow up and there were no differences between the two groups statistically. The numbers of patients who have less than 5 mm of anterior translation of tibia under telometer at 20 degrees of knee flexion are 24 cases in group 1 and 23 cases in group 2. Conclusion: The overall improvements of clinical scores (Lysholm scores and Tegner Activity scales) of the group 1 were a little bit superior to those of the group 2, but there were no significant statistical difference between two groups.

We propose a 3D modeling method for surface inspection of non-linear moving object. The laser lines reflect the surface curvature. We can acquire 3D surface information by analyzing projected laser lines on object. ill this paper, we use multi-line laser to make use of robust of single stripe method and high speed of single frame. Binarization and channel edge extraction method were used for robust laser line extraction. A new labeling method was used for laser line labeling. We acquired sink information between each 3D reconstructed frame by feature point matching, and registered each frame to one whole image. We verified the superiority of proposed method by applying it to container damage inspection system.
