A STUDY ON THE RELATIONS OF VARIOUS PARTS OF THE PALATE FOR PRIMARY AND PERMANENT DENTITION (유치열과 영구치열의 구개 각부의 관계에 관한 연구)

  • The purpose of this study was to clarify the palatal arch length, width and height in the primary and permanent dentition. Samples were consisted of normal occlusions both in the primary dentition(50 males and 50 females) and in the permanent dentition(50 males and 50 females). With their upper plaster casts were used and through 3-dimensional laser scanning(3D Scanner, DS4060, LDI, U.S.A.), cloud data, polygonization, section curve and loft surface, fit and horizontal plane were based to measure the palatal arch length, width and height(Surfacer 10.0, Imageware, U.S.A.). T-tests were applied for the statistical analyze of the data. The results were as follows : 1. In the measurement values, the values of the male were higher than those of the female except primary anterior palatal height. There were not only statistically significant differences in anterior palatal width(p<0.05) and posterior palatal width(p<0.01) in primary dentition but palatal width(p<0.05), anterior palatal length(p<0.01), middle and posterior palatal length(p<0.05) in permanent dentition between male and female. 2. In the indices of palate, there were statistically significant differences in height-length index(p<0.05) and width-length index(p<0.01) between male and female in primary dentition. In permanent dentition, there was statistically difference between male and female. 3. In the measurement values, posterior palatal width was increased most greatly. Posterior palatal height, anterior palatal width and anterior palatal length were followed by descending order. On the other hand, anterior palatal height and posterior palatal length were decreased. 4. In the indices of palate, the height-length index, the width-length index and posterior height-width index were increased, but the others were decreased.

  • To evaluate the cephalometric and dental characteristics of obstructive sleep apnea (OSA) patients, 23 OSA patients and 15 control, non-OSA, patients who visited the Sleep Disorder Clinic Center, Keimyung University were investigated. Patients who suffered from apnea-hypopnea episodes over 10times per hour were diagnosed as having OSA after polysomnograph testing, Impressions were taken with alginate. Cephalometric radiographs were taken at maximum intercuspation. The dental cast measurements, including transpalatal width, intercanine width, intermolar width and palatal depth did not differ between the control and OAS groups and did not have a positive correlation with the apnea-hypopnea index (AHI). Upper airway width was statistically narrower than the control group. Upper airway width had a low negative correlation with AHI, but, lower airway width had a low positive correlation, and, the higher the AHI score, the longer the mandibular border to hyoid distance.

The Dental Biofilm Reduction Effect & Control Difficulty Level of University Students through Dental Biofilm Control Program (일부 대학생에서 치면세균막 관리 프로그램에 의한 치면세균막 감소 효과 및 관리 난이도)

  • For the purpose of examining the dental biofilm reduction effect and control difficulty level through a dental biofilm control program in this study, the total of 131 medical records (82.9%) were used for the final analysis upon excluding the records that were not suitable for this study among the records of 158 persons for the medical records of clinical hygiene case reports that were submitted from 2006 to 2011 by students at the Department of Dental Hygiene at N University located in Chungnam region. The result of examining the dental biofilm reduction effect according to the visit number when conducting a dental biofilm control program showed that the dental biofilm index reduced meaningfully as the visit number increased. However, in the case of those that visited for 7 sessions, the level of reduction was not statistically meaningful. For the purpose of comparing the dental biofilm index mean per area of teeth during the final session visit of dental biofilm control program, the area of teeth was classified into labial/buccal surface, lingual/palatal surface and proximal surface, and the dental biofilm index of lingual/palatal surface was the highest with 26.5%. The result of measuring the dental biofilm of maxilla/mandible revealed meaningful differences between the dental biofilm index of maxilla and mandible. The result of analyzing the dental biofilm index of labial/buccal surface, lingual/palatal surface and proximal surface revealed meaingful differences among the dental biofilm index of labial/ buccal surface, lingual/palatal surface and proximal surface.

The determinants of vertical overbite and overbite depth indicator(ODI) (수직피개의 결정요인과 수직피개 심도지수(ODI)의 상호관계)

  • The concept of denture frame, both the vertical and horizontal relationship of the dentitions are ultimately related to a skeletal configuration, leads to postulate that the vertical overbite will be determined by the jaw rotations and anteroposterior jaw relationship. Also, ODI is analyzed to be composed of the determinant factors of overbite such as FMA PPA and FABA. From the geometric analyses of an interrelationship between the ODT and the overbite determinants, the following formula can be induced. ODI norm=$85^{\circ} - 0.5 PMA-(1.08 - 0.01 FMA)(FABA - 81^{\circ})$. This formula indicates that the norm of ODI is not constant value but variable one according to the individual skeletal frames. Through the application of the formula to the various clinical cases, it is proved that the new concept, relativity of the ODI norm, is very diagnostically useful.

Initial changes of dental plaque, gingivitis and decalcification in Korean orthodontic patients with fixed appliance (한국인 고정식 교정 환자의 치태, 치은염 및 탈회의 초기 변화에 관한 연구)

  • Intraoral filled type of orthodontic appliance can cause reversible or irreversible damages such as gingivitis, periodontitis, enamel decalcification, dental caries, root resorption, and pulpal changes. Such adverse effects are brought by increase in dental plaque as well as oral flora. Such an increase causes gingival inflammation and enamel decalcification. The purpose of this study is to get klowledge on initial changes in dental plaque, gingivitis, and enamel decalcification after bonding fixed orthodontic appliances according to time flow, gender, and sides(right/left) of premolar region. For control group, 48 students of dental college, Yonsei university(26 males, 22 females) were chosen; for experimental group, 73 orthodontic patients(36 males, 37 females) who will be treated with fixed appliances were chosen. All the subjects had no systemic disease, juvenile periodontitis and all the females had passed their ,menarche. Tooth brushing instruction was given to all the subjects prior to the experiment. For control group, plaque index, gingival index, and decalcification index were measured twice at 3 weeks interval ; for experimental group, the same was done prior to, 3, 6, 9 weeks after bonding fixed appliances. The following results were obtained: 1. In plaque index 3 weeks after placement of appliances, and it showed gradual increase afterwards. 2. In gingival index3 weeks after placement of appliances, and afterwards it showed increase at a faster rate than plaque index. 3. Enamel decalcification began to show between 3 and 6 weeks after bonding fixed appliances. Decalcification index began to increase 6 weeks after appliance placement, but there was no statistical significance. 4. When the comparison was made between two sides of premolar region, the right side showed greater index in plaque and gingival index of experimental group.

Retrospective study on three-dimensional characteristics of mesiodens using CBCT in pediatric dentistry (치과용 Cone-beam CT를 이용한 상악 정중과잉치의 3차원적 특성에 관한 후향적 연구)

  • The purpose of this study was to investigate the three-dimensional characteristics of mesiodens using Cone-beam Computed Tomography(CBCT) and analyze the factors affecting complications and anesthetic methods of extraction. This study evaluated 602 mesiodens of 452 patients who underwent extraction of mesiodens at the department of Pediatric Dentistry in Seoul National University Dental Hospital between 2017 and 2019. The ratio of mesiodens patients over total patient per year was gradually increased over the past 20 years. Mesiodens with labio-palatally horizontal direction while root directing labial were the most common among the mesiodens with horizontal direction. Mesiodens were the most common at the cervical side of the adjacent teeth(37.0%) and mesiodens located in the near-palatal side were observed about 3.83 times higher than the far-palatal side. Most of the mesiodens(82.1%) were in contact with adjacent permanent teeth on all three sides of the CBCT and 46.2% of mesiodens had curved roots. The patient's age, vertical position, presence of complications, and proximity showed a significant difference in the selection of general anesthesia among anesthetic methods. The direction and vertical position of mesiodens had a significant effect on complications. These results provide a better understanding of mesiodens for establishing an accurate diagnosis and treatment plan.

DENTAL TREATMENT FOR A PATIENT WITH NOONAN SYNDROME UNDER GENERAL ANESTHESIA: CASE REPORT (누난 증후군(Noonan syndrome) 환자의 전신마취 하 치과치료 : 증례보고)

  • Noonan syndrome is a relatively common genetic disorder which is autosomal dominant. Prevalence of Noonan syndrome is varying from 0.04% to 0.1%. It is characterized by distinctive facial features, chest deformity, short stature and congenital heart disease. Oral findings in patient with Noonan syndrome include high arched palate, dental malocclusion, articulation difficulties, and micrognathia. The purpose of this case report is to describe dental treatment of a children with Noonan syndrome. 5 year old boy with Noonan syndrome visited to the Seoul National University Dental Hospital for dental treatment. Due to need for close monitoring, concern about seizure and poor cooperation, we planned to perform the dental treatment under general anesthesia. Under general anesthesia, caries treatment was successfully performed and there was no postoperative complications related to general anesthesia. High arched palate was observed which is characteristic in Noonan syndrome.
