Prevalence of Periodontopathogens in Saliva and Plaque of Korean Children and Adolescents (한국 소아·청소년의 타액과 치태 내 치주 병원균 출현율)

  • Early colonization of periodontal pathogens has been related as a risk indicator for the subsequent development of periodontal disease. Such colonization can be easily detected with mediums like saliva and plaque. This study aimed to evaluate the prevalence of the bacteria associated with periodontal disease in saliva and plaque in healthy children and adolescents. The experiment was conducted using 90 samples from subjects consisting of thirty elementary school students, thirty high school students and thirty adults. PCR was used to detect the prevalence and distribution of five periodontal pathogens in the collected saliva and plaque. The detected periodontal pathogens are as follows: A. actinomycetemcomitans, P. gingivalis, T. forsythia, F. nucleatum and P. intermedia. Periodontal pathogens were prevailed in a higher number of adolescents than the number of children. A. actinomycetemcomitans and P. intermedia were detected the most in the adolescents group. T. forsythia and F. nucleatum were detected the most in the children group. The overall result showed that saliva is more a useful medium than supragingival plaque. The detection of high risk periodontal pathogens in children and adolescents without clinical signs of periodontal disease can emphasize the importance of the early diagnosis and preventive approach.

Effect of Dental Treatment Fear and Distrust of Dentists with Dental Caries Experience among Middle and High School Students in Daegu City, Korea (일부 지역 중·고등학생들이 치과치료공포감 및 치과의사불신감이 치아우식증에 미치는 영향)

  • The purpose of this study is to analyze factors impacting on the middle & high school students' dental treatment fear and distrust of dentists with dental caries experience to provide empirical data for the development of students oral health. The subjects of this study consists of 720 middle & high school students in Daegu area. The data were collected from July 1, 2009 to September 30, 2009 by self-administrative questionnaires. Among sub-scales of Dental Fear Survey(DFS), stimulate-response factor was the highest ($3.42{\pm}0.98$), and patient distrust toward dintist factor was the highest score($3.07{\pm}0.912$) among sub-scales of Dentist Distrust Survey(DDS). Mean score of DFS were DDS were higher in female than male student. The students who have higher DFS and DDS score, the also have high DMFT index. The higher DDS, the higher DFS, and it was statistically significant(p<0.001). The results suggest that oral health status of adolescent can improve diminishing dental fear and dentist distrust if patient-specific medical service may be provided.

Comparison of Bioethical Perception between Dental Hygiene Students and Non-Health Students (치위생(학)과와 비보건계 학생들의 생명윤리의식에 관한 비교)

  • This study was practiced to provide basic data which is necessary for ethical education of dental hygiene students by comparing the bioethical perception between dental hygiene students and non-health students. Questionnaire surveys were conducted for 778 students in two universities in Gyeonggi-Do province and analyzed the collected data with SAS 9.2. The findings of the study were as follows : 1. there was no stastistical difference in the degree of bioethical perception between dental hygiene students and non-health students. 2. Students who have family member in medical field were more conscious of bioethics than students who doesn't. According to religion, the protestants were most conscious and no-religions scored the lowest. 3. In case of dental hygiene students, female was more conscious. According to religion, there were statistical differences between the Catholic and Buddhism, the Catholic and No-religion. 4. Dental hygiene students were more strongly opposed to let infant with fatal congenital disease die than non-health students. 5. Dental hygiene students were more strongly opposed to rejecting organ transplant, trade in human organs and using alternative organ of animal. Also, they estimated higher the possibility of medical irrationalities due to trade in human organs. From these results, a curriculum for bioethical education in department of dental hygiene should be developed to form and improve the more desirable bioethical perception of dental hygiene students.

Service Quality of Dental Institutes Measured by SERVQUAL Model and Intention to Re-visit (SERVQUAL 모형으로 측정한 일부 치과의원의 서비스 질과 재이용의도)

  • The main purpose of this study was to identify the service quality of some dental clinics located in Gwangju and find an association between service quality and patient's intention to re-visit the same dental clinic. The search objects were 197 patients from five dental clinics, and data collected by self-administered questionnaire which was composed general characteristics, characteristics relating to visit of clinics, quality of service, patients satisfaction and intentions to re-visit. The used statistical analysis to find the association between quality of service and intention to re-visit was multiple regression analysis. This study looked at relations between quality of dental service, service satisfaction and intentions to visit again, and found that quality of service including six service areas had a statistically significant positive correlations with satisfaction and intentions of re-visit. When this study conducted a multiple regression analysis to identify variables that influence intentions of re-visit, it found that patients revisited the dental clinic only when quality of service was included. In conclusion, quality of service evaluated by patients influenced on their intention of re-visit and especially. Therefore, this study concludes that improvement in satisfaction and intentions of re-visit relies on Accessibility, that is, providing of medical information, consultant telephone, how delivery the dental medical system is. Service quality of dental institutes measured by SERVQUAL model and intention to re-visit.

A Study on Dental Caries Activity Assessment from Saliva of Students of Dentistry College (I) (치과대학 대학생 타액의 치아 우식활성도 평가(I))

  • Composition of oral microbial flora is suggested to play a role in the maintenance of oral health. Among them, Streptococcus mutans plays an essential role in the formation of dental plaque and it is being noticed as incipient infective bacteria of dental caries. The purpose of this study was to prepare the basic data about distribution of Streptococcus mutans detected from saliva by measuring colony density. It is well known that smoking as well as drinking is a factor of dental caries, however there are few investigations about distribution of Streptococcus mutans either smoking or drinking. The materials of this study were collected from students of dentistry college with general characteristics obtained by self-administered questionnaires. Students were asked to gently chew the paraffin to get saliva for 1 minute. Bacitracin disc in the culture medium was activated for 15 minutes, and then sample was incubated in the activated medium at $37^{\circ}C$ incubator for 48 hours. Streptococcus mutans colonization per 1 ml saliva was measured by the number of criteria. The severe levels of dental caries activity were significantly increased in the case of smoking (from 5.6% to 10%) or drinking (form 0% to 7%), respectively. Also, dental caries activity increased according to smoking and drinking periods longer. However, using the SPSS analytical process (Mann-Whitney's U), no significant differences were observed between the smoking and the non-smoking or the drinking and non-drinking, respectively. These results obtained from dental caries activity of saliva suggest that distribution of Streptococcus mutans following smoking or drinking can be used as a significant information for oral health.

A Study on the Working Environment of Dental Hygienists Residing in the Gwangju Area (광주지역 근무기관별 치과위생사의 근무환경에 관한 연구)

  • This study generally was examined and grasped the working environment of dental hygienist working dental hospitals(clinics) located in the Gwangju area. The findings of the research were listed in the following: First, regarding the total work experience of subjects, less than 36 month accounted for the most percentage(39.9%). According to a working place, more than 61 month accounted for 50.0% in general hospitals, less than 36 month for 51.3% in dental hospitals and less than 36 month for 39.9% in dental clinics(p < 0.001). Secondly, regarding working day by working place, 5 days accounted for 95.0% in general hospitals and for 82.1% in dental hospitals and more than 6 days for 97.7% in dental clinics. Regarding a working hour, more than 9 hours accounted for 85.0% in general hospitals, 92.1% in dental hospitals, and 63.2% in dental clinics(p < 0.001). Thirdly, regarding special and differential treatment for long-term workers by working place, giving an allowance accounted for 50.0% in general hospitals, opportunities for holiday, travel and promotion opportunity for 56.4% in dental hospitals, and no special and differential treatment for 56.8% in dental clinics(p < 0.001). Fourthly, regarding monthly net pay associated with the total working years, less than 1.10 million won accounted for 60.6% in less than 36 month, 1.31~1.60 million won for 41.5% in 37~60 month, 1.31~1.60 million won for 42.0% in more than 61 month(p < 0.001).

A Epidemiological Study on the Oral Health of 5 years Children in Seongnam City (성남시 5세 어린이의 구강건강실태 조사)

  • Dental caries is the most prevalent dental issue of childhood. So it is important to control caries form 5 years children and above all, the study of oral health status have been preceding for preventive dental caries. The purpose of this study was to assess the oral health status of 5 years children in seongnam city. The study population consisted of 1035 kindergraten children which had been selected stratified random sampling. The survey is based on WHO, 2000 oral health assessment, which has been modified. From data dt index, ft index, dft index, dt rate, ft rate, percentage of active caries, percentage of pit and fissure sealant, and so on. The findings were as follows: (1) In 5 years children, the experience of dental caries was 73.0% and the percentage with active caries was 49.4%. Dmf index was 4.13 and this observation shows a statistical singnificance. (2) Average number of 5 years children the experience of dental caries surface index was 3.11 and dfts index was 8.91. (3) The experience of dental caries fo the frist molar was 1.9% and the percentage with active caries was 1.1%. Otherwise the percentage of pit and fissure sealant of the first molar was 6.7%. It is show the low percentage for preventive dental caries. (4) Shows percentage of teeth needing treatment for one surface and above two surface was each 27.3%, 34.0%. And percentage of children required single crown and pulp treatment & single crown and tooth extraction was each 3.8%, 4.7%, 3.3%. (5) Soojung-gu and jungwon-gu were having poor oral hygiene as compared to bundang-gu. So it is necessary to establish community oral health program effectively. And for preventive dental caries and early diagnosis and early treatment consider with community characteristic.

Validity and Reliability of the Korean Version of the Index of Dental Anxiety and Fear (한국어판 치과 불안 및 공포 지수의 타당도와 신뢰도)

  • The purpose of this study was to investigate the validity and reliability of the Korean version of a tool used to measure dental anxiety and fear. The Index of Dental Anxiety and Fear ($IDAF-4C^+$) was translated into Korean, and modified and revised to adapt to Korean culture. A survey was conducted among 457 patients in a dental clinic. The validity and reliability were determined using PASW Statistics ver. 18.0 and IBM SPSS AMOS ver. 21.0. Factor analysis showed that Korean version of $IDAF-4C^+$ was composed of three elements: dental anxiety, dental phobia, feared stimulus. The validity of the model was examined by confirmatory factor analysis and satisfied relevant requirements. All elements had convergent validity and discriminant validity exceeding requirements to ensure validity. Cronbach's ${\alpha}$ showed good reliability. In conclusion, the findings of this study demonstrate that the Korean version of $IDAF-4C^+$ has high validity and reliability. Furthermore, it can be used in clinical practice and research to decrease dental anxiety and fear.

The Effect of Heat Therapy on Cutaneous Blood Flow and Skin Temperature at Pre-auricular Region (온열요법이 전이부의 표층부 혈류량과 피부 온도에 미치는 영향)

  • The purpose of this study was to assess the effect of heat therapy on cutaneous blood flow and skin temperature at pre-auricular region. Moist heat therapy and ultrasound therapy were applied to 20 healthy subjects(male: 10, female: 10). Cutaneous blood flow and skin temperature before and after heat therapy were measured with laser doppler flowmetry and thermocouple. The results were as follows ; 1. Cutaneous blood flow and skin temperature were significantly increased after moist heat therapy and ultrasound therapy. 2. In application of moist heat therapy, cutaneous blood flow and skin temperature were more increased and maintained longer than in ultrasound therapy. 3. Before heat therapy, cutaneous blood flow and skin temperature were higher in male. 4. There was no significant gender difference in changes of cutaneous blood flow and skin temperature after heat therapy. In conclusion, both moist heat therapy and ultrasound therapy increased cutaneous blood flow and skin temperature significantly, and moist heat therapy was more effective to increase cutaneous blood flow and skin temperature and to maintain increased cutaneous blood flow and skin temperature. There was no significant gender difference in the effect of heat therapy on cutaneous blood flow and skin temperature.

Oral health status of institutionalized elderly in Korea (노인장기요양시설 입소노인의 구강건강실태와 시사점)

  • Purpose: The institutionalized elderly have placed the most vulnerable state, and the report about the oral health status of the institutionalized elderly was scarce. The aims of the current study were to investigate the oral health status of institutionalized elderly. Materials and methods: The survey of the oral health status was carried out on 487 institutionalized elderly from 4 longterm care facilities of Seoul metropolitan city, Gyunggi province, and Gangwon province in Korea. Results: The prevalence of dental caries, root caries, and dental calculus of the institutionalized elderly was 19.2%, 15.3%, and 23.7%, respectively. The percentage of edentulism among the institutionalized elderly was 26.2%. Those who had poor oral hygiene and tongue coating were 43.9% and 90.5%, respectively. The percentages wearing complete and removable partial denture on maxilla were 12.8% and 3.3%, respectively. The percentages wearing complete and removable partial denture on mandible were 7.8% and 5.6%, respectively. The percentages of institutionalized elderly needing complete and removable partial denture on maxilla were 29.7% and 27.2%, respectively. The percentages of institutionalized elderly needing complete and removable partial denture on mandible were 24.5% and 30.9%, respectively. Conclusion: The oral health stuatus of institutionalized elderly was poor. For the oral health promotion of elderly in longterm care facilities, it is essential to educate nurses and caregivers about the importance of the oral health and oral hygiene method.
