New Approaches to the Control of Pathogenic Oral Bacteria (바이오필름을 생성하는 병원성 구강 세균을 제어하는 새로운 접근법)

  • In the oral cavity, there are hundreds of microbial species that exist as planktonic cells or are incorporated into biofilms. The accumulation and proliferation of pathogenic bacteria in the oral biofilm can lead to caries and periodontitis, which are typical oral diseases. The oral bacteria in the biofilm not only can resist environmental stress inside the oral cavity, but also have a 1,000 times higher resistance to antibiotics than planktonic cells by genes exchange through the interaction between cells in the oral biofilm. Therefore, if the formation of oral biofilm is suppressed or removed, oral diseases caused by bacterial infection can be more effectively prevented or treated. In particular, since oral biofilms have the characteristic of forming a biofilm by gathering several bacteria, quorum sensing, a signaling system between cells, can be a target for controlling the oral biofilm. In addition, a method of inhibiting biofilm formation by using arginine, an alkali-producing substrate of oral bacteria, is used to convert the distribution of oral microorganisms into an environment similar to that of healthy teeth or inhibit the secretion of glucosyltransferase by S. mutans to inhibit the formation of non-soluble glucans. It can be a target to control oral biofilm. This method of inhibiting or removing the oral biofilm formation rather than inducing the death of pathogenic bacteria in the oral cavity will be a new strategy that can selectively prevent or therapeutic avenues for oral diseases including dental caries.

  • 저자는 Standardized foil technque으로 Aminefluoride를 함유한 MJ-148 구강세척제를 사용하여 치석형성을 관찰한 바 다음과 같은 결과를 얻었다. MJ-148은 대조군에 비하여 49%의 치석형성 억제 효과를 나타냈으며 이는 significant level 0.001이하이므로 치석형성 억제를 위한 구강 세척제로 사용될 수 있다.

Effects of antibacterial mouth rinses on multiple oral biofilms model (구강세정제가 다중 구강 바이오필름 모델에 미치는 영향)

  • Objectives: To confirm the antibacterial effects of each mouth rinse on multiple oral biofilms in vitro. Methods: The antibacterial effects of different mouth rinses were examined by ATP and counted colony forming units (CFU). Preformed oral biofilms on saliva coated hydroxyapatite (sHA) disks were treated with essential oil and saline; then, the multiple oral biofilms were observed by Scanning electron microscope (SEM). RNA sequencing analysis was performed on total RNA isolated from old biofilms of P. intermedia ATCC 49046. Results: In the CFU measured result compared to controls, preformed multiple oral biofilms were reduced from a low of 39.0% to 95.7% (p<0.05). The size of bacterial cells changed after treatment with the essential oil, and some of the cells ruptured into small pieces of cell debris. Gene expression in P. intermedia ATCC 49046 significantly altered in RNA transcribed and protein translated genes after exposure to essential oil. Conclusions: Mouth rinse solutions with different ingredients had different antibacterial effects and may alter surface structure and gene expression as determined by RNA sequencing.

A Study on Adolescents' Misconceptions about Oral Health Knowledge in Some Areas (일부지역 청소년의 구강건강지식 오개념에 관한 연구)

  • This study was conducted to provide basic data in establishing a method for helping adolescents form the concept of correct oral health knowledge, by searching for their misconceptions about oral health knowledge and figuring out relevant factors. Results showed that they had appropriate knowledge on a relatively large number of questions regarding the level of oral health knowledge, but they also had misconceptions about the toothbrush grip, how to use medicines for gum diseases, and the project of fluorine concentration adjustment in water. It was shown that the score of oral health knowledge turned out to be 10.78 on average as a whole and the mean difference was within the error range. As for the correlations of misconceptions about oral health knowledge by group, it was noted that there were no differences in gender and education level, but those who completed oral health education had more appropriate concepts than those who did not (p < 0.001). In terms of the paths that they obtained oral health knowledge, the Internet and mass media (35.4%) had the highest rate and school (9.6%) had the lowest rate. With regards to the level of oral health knowledge, school had 10.56, which was the lowest score. In this regard, in order for adolescents to form adequate oral health knowledge, the following should be done. therefore, it would be necessary to activate school oral health education so as for adolescents to exercise adequate oral health behavior and to train many oral health educators so as for them to be provided with more educational opportunities.

Comparison of biofilm on titanium and zirconia surfaces: in vivo study (생체 내 티타늄과 지르코니아 표면에서 형성시킨 치면세균막의 비교)

  • Purpose: This study was conducted to compare in vivo biofilm formation on titanium surface and zirconia surface. Materials and methods: For biofilm formation on titanium and zirconia in oral cavity, after producing oral appliances using acrylic resin and orthodontic wire tailored to 9 subjects, we made titanium and zirconia specimens ($6mm{\times}6mm{\times}2mm$), fixed them on oral appliances and maintained them in oral cavity of test subjects for 24 and 72 hours. Test subjects who have equipped two pairs of specimens maintained oral hygiene not by using toothpaste but only by tooth brushing. After 24 and 72 hours, we removed and observed specimens through scanning electron microscopy (SEM). Results: Biofilm formation showed large deviation depending on individuals. For formation comparison between titanium and zirconia for 24 hours, zirconia showed less biofilm formation than titanium. Biofilm formation showed large deviation depending on individuals. As for formation comparison between zirconia and titanium, the degree of biofilm formation in zirconia was less than it was in titanium after a lapse of 24 hours. The result of biofilm formation in 72 hours trial show that zirconia has an inclination to formate less biofilm than it was in titanium. Conclusion: Based on the above results, we can conclude that early biofilm formation in oral cavity was influenced by difference of abutment materials.

RELATIONSHIP OF THE BACTERIA INHIBITING PLAQUE FORMATION AND THE NUMBER OF ORAL BACTERIA (치태형성 억제세균과 구강내 세균수와의 관계)

  • The inhibition degree of the isolated bacteria on plaque formation of Streptococcus mutans, and the effect of these bacterial genus on the concentration of total bacteria in saliva were assessed with the following. The effectiveness of the isolated bacteria on the inhibition of plaque formation was assessed culturing Streptococcus mutans in the beaker with orthodontic wires. The mean weight of plaque produced on a wire was 152mg in the culture of Streptococcus mutans only, whereas being reduced to 4mg, 78mg, or 72mg in the combined culture of Streptococcus mutans and Enterococcus durans, Lactobacillus acidophilus, or Streptococcus oralis. The colony forming units (CFU) of Streptococcus mutans were $3.6{\times}10^8$ per ml in the culture of Streptococcus mutans, only, wheras being $1.4{\times}10^6,\;5.6{\times}10^6,\;or\;3.8{\times}10^6$ per ml in the combined culture of Streptococcus mutans and Enterococcus durans, Lactobacillus acidophilus, or Streptococcus oralis. When saliva from children was inoculated on brain heart infusion agar, the colony forming units of bacteria were $4.8{\times}10^6\;to\;1.3{\times}10^9$ per ml of saliva. The concentration of Enterococcus, Lactobacillus, or Streptococcus inhibiting Streptococcus mutans in saliva was not proportioned to that of total bacteria replicated on brain heart infusion agar. These results indicate that the isolated bacteria inhibited the replication of Streptococcus mutans, resulting into inhibiting the formation of plaque, but the concentration of Enterococcus, Lactobacillus, or Streptococcus inhibiting Streptococcus mutans, in saliva might not affect the total bacterial concentration of saliva.

구강보건교육의 중요성과 활성화방안

  • 구강보건은 상병에 이환 되어 있지 않고, 정신 작용과 사회생활에 장애가 되지 않는 구강조직 및 장기의 상태이고, 이러한 구강건강을 보전 내지 증진시키려는 계속적인 노력과정이 구강보건이라고 정의할 수 있다. 한편 교육에 대한 정의는 시대나 사회에 따라 다르고, 보는 사람의 입장이나 가치관에 따라 다양하다. 그러나 대체적으로 교육은 인간의 행동을 계획적으로 바람직한 방향으로 변화시키고자하는 인간형성의 작용이라 말할 수 있겠다. 즉 목적 달성과정이라고 할 수 있는 것이다.

Inhibition of Plaque Formation on the Titanium Surface by Anti-bacterial Varnish (항균제 처리한 titanium 표면의 치태형성 억제효과)

  • 매식된 인공치아의 성공을 위해서는 적절한 교합과 수동적 적합성을 갖는 보철물의 제작과 구강내 노출 직후부터의 세균성 치태조절이 요구된다. 본 연구는 전처리(passivation과 tridodecyl - methyl - ammonium chloride(TDMAC) 처리)가 다른 타이타늄 표 면에 chlorhexidine varnish와 테트라사이클린 을 도포시 약제의 방출역학을 알아보고 구강내 치태형성의 억제정도를 평가하기 위하여 시행 되었다. 이를 위해 방출용액으로 인산완충액 성분의 인조타액을 1일${\sim}$1개월간 매일 교환하여 약제농도를 측정하고 타이타늄 박막에 잔류한 약제 활성을 측정하였으며 항균제 도포한 타이타늄 원판을 부착한 장치를 구강내 위치시킨 1일${\sim}$3주 후 원판을 제거하여 주사전자현미경으로 세균 부착상을 관찰하였다. 테트라사이클린은 TDMAC 처리된 표면에서 $10{\sim}18$일까지 유효농도로 방출되었고 표면의 유효 항균 활성은 $3{\sim}4$주간 유지되었으며, chlorhexidine varnish 도포 시에는 TDMAC 전처리시 초기에 $3{\sim}7$일 간 증가한 유효 항균 활성을 방출하여 매식지대치 등에 이러한 항균제도포 시 매식치 주위환경에 항균활성 공급원으로 작용할 수 있음을 보였다. 주사현미경적 관찰시 모든 타이타늄 표면에서 구강내 위치 30분 후에는 세균이 부착되어 있지 않고 타액 단백질 성분에서 유래한 것으로 보이는 피막물질이 표면을 부분 또는 전면에 걸쳐 덮고 있었다. 구강내 노출 2시간 후 항균제 미도포 표본들에는 약간의 구균이 단층으로, $1{\sim}3$일 후에는 부분적으로 두꺼운 세균층을 형성하였고 7일 후에는 표면전체에 걸쳐 세균층이 덮여있었으며 주로 구균과 약간의 간균이 주종을 이루었다. 항균제 도포시 구강내 노출 1주일 이전까지는 미도포군에 비해 치태형성이 지연되는 경향을 보였지만 2주 이후에는 세균 수나 치태형성 양상이 유사하였다. 이 연구로부터 항균제 도포시 1주일 이전의 초기 치태형성을 감소시킬 수 있음을 알 수 있었으며 이러한 연구결과는 타이타늄 임프란트 지대치 표면에 항균제의 도포가 임상적으로 유용할 수 있음을 시사하였다.

  • Ipriflavone(IP)은 골흡수 억제효과에 골형성 촉진효과를 지니는 약물로 보고되어 왔다. 이러한 IP의 특성때문에 골의 치유를 촉진시키기 위한 약물로서 구강외과 영역에서 쓰일 수도 있으리라 생각되었다. 이에 저자는 그동안 보고되어온 IP의 골 형성 촉진효과가 실제로 나타나는지를 확인하고 또한 어떤 농도에서 나타나는지를 알아보기위해 IP를 서로다른 농도로 하여 골세포주(MC3T3-El cell line)의 배지에 넣은후, 골 형성의 지표로 쓰일수있는 collagen합성정도를 보고자 하였으며 이 자료를 앞으로서 in vivo 동물실험 연구의 기초자료로 사용코자 하였다. 본 연구에서 IP의 골형성 촉진효과를 collagen 합성정도를 측정을 통해 확인하였고, 특히 IP이 $10^{-7}M$농도일때 현저한 collagen합성의 증가를 관찰 하였으며 앞으로의 동물실험등을 통해 구강외과 영역에서의 사용가능성에 대해 좀더 연구해 보고자 한다.
