  • The reconstruction of major head and neck defects must be an integral part of the overall cancer treatment plan. The priorities of surgical treatment of head and neck tumor are 1) local tumor control, 2) relief of pain, 3) avoidance of difficult dressing, 4) provision of oral continence, and 5) ability to swallow and manage saliva. The recent advances in reconstructive surgery including the development of musculocutaneous flaps and microvascular free tissue transfer have allowed the surgical restoration of head and neck tumor defects that previously were not possible. These techniques have provided the opportunity to undertake larger, more aggressive resection while at the same time permitting functional rehabilitation. The timing of reconstruction demands on the nature of the resection, the ability of the ablative and reconstructive teams to coordinate efforts, the overall health of the patients, the patient's needs and wishes. So, we report to emphasize current methods for restoring major head and neck tumor defects after tumor ablation, reviewing for the reconstructive operations, postoperative complications, and postoperative sequelae etc, of patients from Jan, 1990 to Dec, 1993.

CLINICAL STUDY OF BISPHOSPHONATE-INDUCED OSTEONECROSIS OF MANDIBULAR AND MAXILLARY BONE (비스포스포네이트로 인한 하악 및 상악골에 발생한 골괴사에 대한 임상적 연구)

  • Bisphosphonates are compounds widely used in the treatment of various metabolic and malignant bone disease. Recently, an association between bisphosphonate use and a rare dental condition termed 'osteonecrosis of the jaw(ONJ)' has been reported. Bisphosphonate-related osteonecrosis of the jaw(BRONJ) is rare, but serious, side effect of bisphosphonate therapy in affected patients. It is characterized by poor wound healing and spontaneous intra-oral soft tissue break down, which lead to exposure of necrotic maxillary and mandibular bone. We reviewed 11 patients of BRONJ visited Ajou University Hospital Dental clinic from May 2007 to November 2008. The management of the patients included cessation of bisphosphonate therapy and various surgical restorative procedures and conservative care there after. Aggressive debridement is contraindicated. A new complication of bisphosphonate therapy administration, osteonecrosis of jaws, seems to be developing. The improved results after cessation of the medication should make clinicians reconsider the merits of the rampant use of bisphosphonates, while further investigation is needed to completely elucidate this complication.

PERIPHERAL OSTEOMA OF THE MANDIBLE BODY : A CASE REPORT AND REVIEW OF THE LITERATURE (하악골체에 발생한 주변성 골종의 증례 보고)

  • Osteoma is a slow growing benign osteogenic lesion characterized by proliferation of compact or cancellous bone. Osteomas may be classified as peripheral, central, or extraskeletal. Central osteoma arise from the endosteum, pheripheral osteoma from the periosteum, and extraskeletal soft tissue osteoma within a muscle. Peripheral osteoma of the mandible is uncommon. They manifest as asymptomatic, fixed tumors of bony-hard consistency that may be sessile or pedunculated. Radiographically, a well circumscribed round or oval radiopaque mass is seen. Here, we report a case of a huge solitary peripheral osteoma of the buccal posterior mandible in a 40-year-old woman who was otherwise in good health. The patient visited at the dental clinic because the lesion makes esthetic problem. Treatment was performed by surgical esthetic recontouring and histological examination. The patient remains free of recurrence after surgical esthetic recontouring operation.

A Preliminary Study on the Fitness of Adjustable Dental Impression Trays (가변형 치과 인상용 트레이의 적합도에 관한 예비연구)

  • This study was designed to investigate the fitness of adjustable dental impression trays. The size and shape of these trays were designed from the results of the dental arch size of Korean adults. Tray samples were made by CAD-CAM working. A hundred dental students(male:50, female:50) were selected for taking irreversible hydrocolloid impression using these trays. The author measured the width and length of impression material on the several measuring points. The results obtained were as follows : 1. Uniform impression material thickness was achieved by controlling the width of the tray using stops and beveled guides. 2. In the upper tray, the impression material thickness was measured to be rather great showing thickness of the labial vestibule 8.3 mm and the midpalatal part 8.6 mm. 3. In the lower tray, length of the impression material of the labial vestibule of first, second premolar contact point was 7.8 mm, and thickness of the lingual part of premolars(1.8 mm) and molars(1.9 mm) showed small values. 4. In the lower tray, the impression material thickness of the buccal shelf area(0.2 mm) and the retromolar pad area(0.6 mm) was measured to be too small.

  • The role of botulinum toxin as a therapeutic agent is expanding rapidly in otolaryngology. Botulinum toxin is a protease that blocks the release of acetylcholine from nerve terminals. Its effects are transient and nondestructive, and largely limited to the area in which it is administered These effects are also graded according to the dose, allowing for individualized treatment of patients and disorders. Botulinum toxin has been used primarily to treat disorders of excessive or inappropriate muscle contraction. In the field of otolaryngology, these include spasmodic dysphonia, oromandibular dystonia, and blepharospasm, vocal tics and stuttering, cricopharyngeal achalasia, various tremors and tics, hemifacial spasm, temporomandibular joint disorders and a number of cosmetic applications. Botulinum toxin treatment has recently begun to show some benefit in the control of pain from migraine and tension headache. It may also prove useful in the control of autonomic dysfunction, as in Frey syndrome, sialorrhea, and rhinorrhea. In over 20 yews of use in humans, botulinum toxin has accumulated a considerable safety record, and in many cases represents relief for thousands of patients unaided by other therapy.

A Case of Lip Reconstruction (구순 재건술의 치험 1증례)

  • The lips play an important role of passage of food as well as articulation and production of intelligible speech. So, the orbicularis oris muscle which composes the lip must act as a sphincter for the right structure. Also, the muscle conforms to the shape of the lips to make words. The surrounding cheek muscles antagonize and synergize to position the orbicularis oris sphincter. It is highly significant from the standpoint of reconstruction that the surrounding muscle can be separated from the orbicularis oris muscle without loss of sphincteric competence as long as the facial nerves and vessels remain intact. The authors performed the reconstruction of a lower lip defect due to human bite and achieved an excellent result from a functional and cosmetic aspect. This report with the literature is herewith submitted.

  • 측두하악장애환자와 정상대조군의 두경부 근육에 있어서 실제 또는 위약 치료에 따른 저출력 레이저의 효과와 측두하악장애환자에서 저출력 레이저의 효과적인 치료기간을 평가하기 위하여, 측두하악장애를 가진 부산대학교 치과대학생 19명과 측두하악장애의 병력이나 증상이 없는 부산대학교 치과대학생 20명을 대상으로 치료전, 치료 2주 및 치료 4주에 구외촉진이 가능한 32 근골격 촉진점의 압력통각역치와 각각의 동통정도와 두개하악지수를 측정하여 다음과 갈은 결론을 얻었다, 1. 치료전 환자군의 압력통각역치는 정상대조군 압력통각역치보다 낮았다(p<0.05), 2. 환자군의 압력통각역치는 유의하게 증가하였으며 (p<0.05) 레이저 조사 환자군의 증가된 압력통각역치는 레이저 모의조사-환자군의 증가된 압력통각역치보다 더 크게 나타났다. 3. 대조군에서는 레이저 조사나 모의 조사에 의해 압력통각역치와 변화가 없었다. 4. 레이저 조사-환자군의 압력통각역치가 안면근육에서는 치료2주부터 유의하게 증가하였고 경부근육에서는 치료4주부터 유의하게 증가하였다(p<0.05) 5. 레이저 조사-환자군에서 동통의 정도와 두개하악지수는 유의하게 감소하였다(p<0.05).

  • The purpose of this research was to evaluate ceramics reinforced with thin metal wires, The results could be applied to evaluate the mechanical properties of in fabricating ceramic crowns and bridges. The wires used in the study were Platinum of 0.025 mm, 0.0508 mm, 0.1 mm and Titanium of 0.127 mm diameters. The ingot used was for staining technique. One or two wires were inserted into the ceramics and the specimens were observed with dental X-rays. The results obtained in this study were as follows: 1.The commonly observed phenomenon was that the wires inserted were pushed away to the peripherally, regardless of types diameter, or numbers of wires. 2.In some Empress specimen the wire was observed to be loosed and cut off.

  • The traffic accident was one of most common cause for the facial bone fracture. When it involved the midfacial structures, the nasal bone fracture was usually shown. If the reduction was not done in time, it would result in facial deformity. Simple case could be corrected by simple rhinoplasty. However, severe cases would require more invasive technique. We used triangular osteotomy included the nasal bones, the vomer, and the medial wall of maxilla for the correction of post-traumatic nasal deformity and reported the result with the review of literatures.

Full mouth rehabilitation using zirconia crown in severe worn dentition: a case report (마모가 심한 환자에서 지르코니아 크라운을 이용하여 전악 수복한 증례)

  • The progressive attrition of teeth is a normal process by aging. However, excessive tooth wear with decreased vertical dimension of occlusion and collapse of occlusal plane may cause pathologic pulpal condition, occlusal disharmony and functional disorders. In this case, a patient with severely worn dentition was treated. Diagnostic wax-up was performed at the increased vertical dimension. After evaluation of provisional restorations for 12 weeks, final restorations were fabricated with zirconia crown and routine clinical assessments were made. Esthetically and functionally satisfactory results were obtained.
