  • The purpose of this study was to analize the initial stress distribution around apex and the alveolar bone of the upper anterior teeth when applying intrusive force by the use of utility arthwire, Burstono 3-piece infusion archwire, and 'J' hook headgear which is usually used in clinital practice. By the use of the polarization plate, initial stresses were analized when 80g and 150g forte applied. The results were as follows. 1. With the utility archwire, moderate levels of stress were evenly distributed on the apical areas of the anterior teeth and concentrated on the apical areas of the first molars. 2. With the Burstone's 3-piece intrusion archwire, moderate levels of stress were evenly distributed on the apical areas of the anterior and posterior teeth. 3. With the 'J' hook headgear, severe levels oi stress were widely distributed on the alveolar bone and apical areas of the upper anterior teeth, and concentrated on the apical area between the central and the lateral incisors. Especially, weak levels of stress appeared along the periodontal ligament space of all teeth.

Effect of Application of ZirLiner® and Blasting Treatments on Shear Bond Strength of Zirconia-Veneered Porcelain Interface (지르코니아-전장도재 계면의 전단결합강도에 대한 ZirLiner® 적용과 분사처리의 영향에 관한 연구)

  • The purpose of this study was to evaluate effect of application of $ZirLiner^{(R)}$ and blasting treatments on shear bond strength of zirconia-veneered porcelain interface. 60 uncolored zirconia plates and 30 colored zirconia plates were fabricated and divided into nine groups of 10 according to blasting treatment such as as-ground, glass blasting and alumina blasting and zirliner application. Veneering porcelains were built up over the center of the treated zirconia ceramic surface using jig and fired according to the manufacturers' instructions. Each specimen was completely embedded in acrylic resin. The specimens were placed in a mounting jig and subjected to shear force by a universal testing machine. Load was applied at a crosshead speed of 0.5㎜/min until failure. Average shear strengths were analyzed with two-way analysis of variance and one-way analysis of variance and the Duncan's post-hoc test. The fracture surfaces of the failed specimens were examined by SEM. The obtained results were as follows: 1. Depending on surface treatment by blasting, the degree of roughness is revealed in the order of Glass-blasted, As-ground, and Alumnia-blasted. The roughness average of uncolored and colored zirconia ceramic were not significantly different from blasting treatments. 2. In uncolored zirconia ceramic, the shear bond strength were not significantly different from blasting treatments. However, the shear bond strength were significantly different from Zirliner application. 3. Used ZirLiner, mean shear bond strength of colored zirconia was lower than uncolored zirconia. Especially, mean shear bond strength of colored zirconia was quite low with alumina-blasting treatment. 4. SEM analysis showed that veneered porcelain failed in zirconia ceramic interface and there was no cohesive failure.

A Comparative Study of Interlocking IM Nailing and LCP Fixation through MIPPO Technique in the Treatment of Distal Metaphyseal Tibial Fracture (경골 원위부 골절 치료에서 최소 침습적 접근법을 통한 잠금 나사 금속판 고정술과 교합성 골수강 내 금속정 고정술의 비교 연구)

  • Purpose: To evaluate and compare the outcome between interlocking IM nailing and LCP fixation in the treatment of distal metaphyseal tibial fracture. Materials and Methods: From January 2000 to December 2007, 17 patient were treated by interlocking IM nail and 13 patient were treated by LCP fixation for distal metaphyseal tibial fracture. Results: According to AO classification, there were 2 type A1 fracture (12%), 6 type A2 fracture (36%), 3 type A3 fracture (18%), 4 type B1 fracture (24%), 1 type B3 fracture (6%), 1 type C1 fracture (6%) in interlocking IM nailing group and 1 type A2 fracture (7.7%), 2 type A3 fracture (15.4%), 3 type B1 fracture (23%), 3 type B2 fracture (23%), 3 type C1 fracture (23%), 1 type C2 fracture (7.7%) in LCP fixation group. The clinical functional outcome (according to AOFAS score) is 75.6 point in IM nailing group and 81.5 point in LCP fixation group. In IM nailing group, 65% of patient showed satisfactory result and In LCP fixation group, 77% of patient showed satisfactory result. Conclusion: There is no difference on clinical results between IM nailing and MIPPO (minimal invasive percutaneous plate osteosynthesis) group in the treatment of distal tibia fracture. But MIPPO group have higher subjective satisfactory score and less complication rate. The weakness of our study is a small case number and limited follow-up and we believe a better designed prospective study will be needed.

Physical properties of a new resin-based root canal sealer in comparison with AH Plus Jet (새로운 레진계 근관실러와 AH Plus Jet의 물성 비교)

  • Purpose: The aim of this study was to assess the physical properties of a novel resin-based endodontic sealer, Any-Seal, in comparison with AH Plus Jet. Materials and Methods: Flow, radiopacity and compressive strength were examined according to ISO 6876/2001. For flow test, 0.05 mL of sealer was placed between glass plate and 100 g weight were applied. Ten minutes after mixing the sealers, the load was removed and the diameters of the compressed sealer discs were measured. For radiopacity, 10 mm diameter and 1 mm thickness sample were fabricated and took radiograph with an aluminum step-wedge and analyzed using imaging program. For compressive strength test, $4mm{\times}6mm$ cylindrical specimen was fabricated and tested after 24 hours and 1 week using Universal testing machine. Results: Both tested sealers were consistent with ISO 6876/2001 in the flow and radiopacity test. The flow values of both sealers were not significantly different (P > 0.05). AH Plus Jet had significantly higher radiopacity (P < 0.05). AH Plus Jet showed higher compressive strength at both time intervals (P < 0.05). Conclusion: Any-Seal showed low compressive strength until after 1 week, so its physical and biological aspect should be evaluated more before clinical use.

Effect of Internal Stress on Cyclic Fatigue Failure in ProTaper Universal (ProTaper Universal에 축적된 내부 응력이 피로 파절에 미치는 영향)

  • The purpose of this study was to evaluate the relation between intentionally induced internal stress and cyclic fatigue failure of ProTaper Universal. ProTaper Universal (Dentsply Maillefer) S1, S2, F1, F2, F3, F4 and F5 (25 mm length) were used in this study. To give the internal stress, the ProTaper Universal were put into the .02 taper Endo-Training-Bloc (Dentsply Maillefer) until auto-stop by torque controlled motor. The rotation speed was 300 rpm and torque value was 1.0 Ncm. ProTaper Universal were grouped by the induced number of internal stress and randomly distributed among one control group and three experimental groups (n=10). The four groups were Stress 0 (control), Stress 1, Stress 2 and Stress 3. These instruments were rotated until separation. For cyclic fatigue measurement, inclined plane was used and time for separation was recorded. It was statistically analyzed using two-way ANOVA and Duncan post-hoc test at 95% confidential level. In all ProTaper Universal, there was statistically significant decrease on time for separation in Stress 3. In F2 and F3, there were statistically significant difference between control group and all experimental groups. And in F4 and F5, Stress 2 and 3 groups showed significantly lower cyclic fatigue resistance from Stress 0 group. In the limitation of this study, cyclic fatigue failure of ProTaper Universal is influenced by accumulated internal stress.

Evaluation of the Radiopacity of Contemporary Luting Cements by Digital Radiography (디지털방사선촬영술을 이용한 합착용 시멘트의 방사선불투과성 평가)

  • This study examined the radiopacity of eight contemporary luting cements by direct digital radiography. Five disc-shaped specimens ($5mm{\times}1mm$) were prepared for each material tested (BisCem, Clearfil SA Luting, Duolink, Maxcem Elite, Multilink Speed, Panavia F 2.0, RelyX Unicem Clicker, V-link). The specimens were radiographed using a Kodak CS 7600 image plate (Carestream Health, Inc., Rochester, NY, USA) and an aluminum step wedge with a range of thicknesses (1.5 to 16.5 mm in 1.5 mm increments) and a 1 mm tooth used as a reference. A dental X-ray machine Kodak 2200 Intraoral X-ray System (Carestream Health, Inc., Rochester, NY, USA), operating at 70 kVp, 4 mA, 0.156 s and a source-to-sample distance of 30 cm, was used. According to international standards, the radiopacity of the specimens was compared with that of an aluminum step wedge using NIH ImageJ software (available at http://rsb.info.nih.gov/ij/).The data was analyzed by ANOVA and a Tukey's post hoc test. Maxcem Elite (5.66) showed the highest radiopacity of all materials, followed in order by Multilink Speed (3.87) and V-link (2.83). The radiopacity of Clearfil SA Luting (1.35), BisCem (1.33), Panavia F 2.0 (1.29) and Duolink (1.10) were between enamel (1.79) and dentin (0.19). RelyX Unicem Clicker (0.71) showed the lowest radiopacity, which was higher than that of dentin. All materials showed a radiopacity above the minimum recommended by the International Organization for Standardization and the American National Standards/American Dental Association with the exception of RelyX Unicem Clicker.

HISTOLOGIC CHANGES IN MANDIBULAR PERIODONTIUM OF THE MONKEY FOLLOWING EXPERIMENTAL EXTRUSION OF ANTERIOR TEETH (실험적 전치 정출시 원숭이 하악 치주 조직의 변화)

  • The purpose of this study was to investigate the histologic changes in mandibular periodontium during overbite closure for openbite treatment by continuous arch wires and anterior vertical elastics. Two female monkey(Macaca nemestrina) with permanent dentition were used. Posterior bite block was fixed to each of their maxillae, which made the animal temporary anterior openbite as well as stabilized the whole maxillary anchorage. In each mandible, all the teeth except the second molars which had been extracted, were prepared for cast crowns. 018 inch Standard brackets were welded on these crowns. After cementation, two types of the $016{\times}022$ inch continuous arch wires, the plain ideal arch to the control animal and the MEAW(multiloop edgewise archwire) to the other experimental one were inserted. Then anterior vertical elastics were applied for two weeks. The overbite depth changes in the monkeys and histologic examinations of the mandibular periodontiums suggested the following conclusions. 1. During two weeks of the experimental period, the overbite increased + 0.3 mm in the control and + 1.3 mm in the experimental one. 2. In both the control and the experimental animal, histologic examinations showed that incisors, canines and first premolars were subject to extrusive force and the rest of posteriors were subject to intrusive one. 3. In periodontiums of the extruded incisors of the experimental one, reorientation of the periodontal fiber structures reflected the direction of force and the alveolar bone surfaces including apical and crestal areas which had been subject to tension, were the front of new bone formation. 4. In periodontiums of the extruded incisors of the experimental one, neither excessive hyalinization nor gross root resorption was observed. 5. Alveolar bone remodeling of anteriors and posteriors was more remarkable in the experimental one than the control.
