  • It is important to investigate the growth aspects of craniofacial skeleton in the orthodontics. The purpose of this study is to find the factors, which have a great influence on the growth aspects of craniofacial skeleton, and to evalute the control values of each factor in order to construct orthodontics diagnostic and planning.

GROWTH CHANCE IN THE LIPS OF THE ADOLESCENCE (from 8 to 16 years old) (청소년기 (8세에서 16세) 구순부 성장변화에 관한 누년적 연구)

  • This study was undertaken to assess the effect of growth on the lips. Not only does lip growth influence the stability of such orthodontic treatment; it also directly influence facial profile, in which the lips have an important part. An understanding of the growth of lips is thus central to a consideration of profile change in orthodontics. By analyzing the serial lateral cephalograms of 15 male and 15 female of 8 years old to 16 yaers old who have normal occlusion. The result of this study were summerized as follows; 1. The largest growth increments in the length of the lips was mod age of 14 in both sexes. 2. The thickness of lips showed lager value for the male than that of the female in the most age group. The lagest growth increments at A point was occured age of 14, while Ls, Li, B point decreased after the age of 10-11. 3. The largest increase in the interval between crest of lower lip and edge of upper incisors was occured between ages 9 and 11 in males. The interval decreased slightly from 8 to 16 years in females. 4. The nasolabial angle decreased slightly from 8 to 16 years in both sexes. 5. The mentolabial angle showed large variation.

Alopecia : An unexpected effect of orthodontic treatment (교정치료시 병발된 탈모증)

  • A case is described, where an adolescent boy developed alopecia areata and alopecia totalis during the course of routine orthodontic treatment for the resolution of a dentoalveolar Class II division 1 malocclusion. The orthodontic treatment lasted 22 months, with a successful outcome. However, within eight months of the onset of treatment the patient lost all his hair Exhaustive medical tests and differential diagnosis determined that the etiolgy of the patient's alopecia was psychological stress evoked by the orthodontic treatment. Numerous reports suggest that psychological stress can cause alopecia by affecting the immune system. Therefore, it appears reasonable to assume that in the case of this patient, alopecia had resulted from stress effects on the immune system, leading to autoimmune disease-like conditions in tissues surrounding the scalp hair follicles. The alopecia condition was successfully reversed by daily oral and topical applications of vitamin D. It is concluded that the immune system plays a pivotal role in tissue remodeling around the teeth and elsewhere in the body, and that any conditions capable of affecting this system may cause unfavorable outcomes, such as alopecia.

A Study on the Skeletal and Profile Change after Using the Activator in Class II Malocclusion (II급 부정 교합자의 Activator 치료 후 골격 및 안모 변화에 관한 연구)

  • To establish the diagnosis and treatment plan for skeletal Class II malocclusion, patient's skeletal morphology, prognosis as well as the treatment effect is one of the important factor to consider. Therefore, the present study classified analyzed the difference between initial(T1) and after use of activator(T2), and after finish of direct multi-bonding system treatment(T3) for Class II malocclusion during growth period according to the treatment result(effective body length) and morphology of vertical skeletal type. The experimental group was classified into two groups(1 group, 2 group) according to the effective body length change between before and after use of activator, showed good treatment effect of activator for patient with small mandible and large differential between maxilla and mandible, and short anterior facial height. And the difference between 1 and 2 group in the experimental group before treatment(T1) disappeared in the finished treatment(T3). But in contrast, the initial difference of T1 stage between a and b group in the control group did not disappear in the finished treatment(T3). In short, experimental group's treatment effect was much better than contrast group and the treatment effect was maintained and got stable results at comparison experimental group with contrast group. Through this study, we can find activator's treatment effect and stable retention of that in growing Class II malocclusion patients. By estimate of activator treatment effect through these results, we can establish the correct diagnosis and treatment plan for adolescent Class II malocclusion estimate of activator treatment effect and lead the ideal facial growth pattern.

Factors that influence treatment duration for patients with palatally impacted maxillary canines (구개측으로 매복된 상악견치의 교정치료기간에 영향을 미치는 요소)

  • Objective: The aim of this study was to determine the relationship between the status of a unilateral palatally impacted maxillary canine (as seen on the panoramic radiograph) and the orthodontic treatment duration. Methods: A total of 36 subjects were chosen (8 males and 28 females, $13.7{\pm}2.5$ years). All patients had undergone orthodontic traction of the impacted canine after a closed flap surgery. The position of impacted canine on the panoramic radiograph was traced and calculated with regard to the treatment duration. Results: The canine overlap over the lateral incisor (COGr) had significant statistical difference between the short-term and the long-term treatment duration groups (divided by average treatment time of 21 months) (p<0.05). Multiple regression indicated that the variables of significance (with treatment duration as the dependent variable) were the canine vertical height from the occlusal plane $(HCV,\;r^2=0.115;\;p<0.05)$ and the mandibular anterior width $(MnDW,\;r^2=0.142;\;p<0.05)$. The treatment duration, the canine angle to the incisor midline (CA), and HCV decreased from primary dentition (under 12 yrs) to permanent dentition (15 yrs). Howerer, these increased again at the adolescent ages (over 16 yrs). Conclusion: These findings suggested that orthodontic treatment of a palatally impacted canine would show good prognosis at an age of early permanent dentition with the canine showing smaller CA and HCV.

Orthodontic Treatment of a Child with Short Root Anomaly : a Case Report (Short root anomaly (SRA) 환아의 교정적 처치 증례)

  • Short root anomaly (SRA) is very rare, but can be problematic for physicians because patients with SRA are more vulnerable to root resorption with orthodontic forces. During the mixed dentition period, it may be difficult to diagnose generalized SRA. This article reports the treatment of an orthodontic patient with SRA at the early mixed dentition stage. Despite local tooth loss, a relatively favorable outcome was obtained without excessive root resorption. Ultimately, orthodontic therapy is possible for patients with generalized SRA, but precautions should be taken to avoid complications, such as tooth loss or root resorption.

A STUDY ON THE PRE-ERUPTIVE POSITIONAL CHANCE OF THE LOWER THIRD MOLAR FOLLOWING ORTHODONTIC TREATMENT (교정치료에 따른 미맹출 하악 제3대구치의 위치 변화에 관한 연구)

  • Most of orthodontic cases are treated with extraction of certain teeth, which influence the pre-eruptive movement of the lower third molar The purpose of this study was to evaluate the positional change of lower third molar following orthodontic treatment. Pre- and post-treatment pantomograms of 163 orthodontic patients (77 nonextraction group, 78 first premolar- extraction group, 8 second molar- extraction group) were analyzed in terms of the mesiodistal and buccolingual angles of lower third molar. The results were as follows. 1. The change of the mesiodistal angle of lower third molar by orthodontic treatment was significant in second molar-extraction group. 2. The mesiodistal angle of lower third molar in pre-treatment was significantly correlated to the mesiodistal angle in post-treatment and/or the change of the mesiodistal angle by treatment. 3. The change of the buccolingual angle of lower third molar by orthodontic treatment was significant in non -extraction group or first premolar-extrction group. 4. The change of the buccolingual angle of lower third molar by treatment was significantly correlated to the mesiodistal angle in post-treatment, the change of the mesiodistal angle by treatment, the buccolingual angle in pre-treatment or the buccolingual angle in post-treatment.

Microimplant mandibular advancement (MiMA) therapy for the treatment of snoring and obstructive sleep apnea (OSA) (코골이 및 수면 무호흡 치료를 위한 마이크로 임프란트를 이용한 하악골 전진술)

  • This case report describes the treatment of a 66 year old adult patient with a diagnosis of severe obstructive sleep apnea who was intolerant of nasal continuous positive airway pressure (nCPAP) treatment and oral appliance therapy. An alternative treatment of snoring and obstructive sleep apnea (OSA) with 2 orthodontic microimplants anchored to the mandible providing skeletal anchorage for mandibular advancement was implemented. After a 2 week healing period, a custom designed facemask provided extraoral anchorage to which the microimplants were connected to for titratable mandibular advancement. Microimplant Mandibular Advancement (MiMA) therapy resulted in resolution of the symptoms of severe OSA with a reduction of the apnea-hypopnea index (AHI), snoring and OSA symptoms.

  • The purpose of this case report is to provide the information of the treatment of the impacted-displaced teeth associated with the disease in the jaw. A 10-year-old boy presented with a large radiolucent lesion accompanying the displacement of the second premolar and first molar in the left mandibular body area. The lesion was diagnosed as unicystic ameloblastoma. As a surgical procedure for the present case, marsupialization was executed. For the eruption of displaced impacted teeth, no orthodontic intervention was done for second premolar while a helical spring was used for the forced eruption of first molar. Goof occlusion was established by simple orthodontic intervention.

  • The authors treated a case of bimaxillary dentoalveolar protrusion corrected by anterior segmental osteotomies: As presurgical treatment & process, closing of the upper anterior spacing with fixed appliances was worked out, cephalometric predicition & model surgery was done, and fabrication of intra-arch acrylic resin splints were made. It was not necessary to make an intermaxillary fixation with wire after surgery. Only intramaxillary fixation of anterior retracted mobile segment with resin plate was required for 8 weeks. After surgical treatment, leveling of the upper and lower arch with rectangular arch wire were accomplished as the Intrusion of anterior teeth proceeded. We will continue to improve the class II molar relationship by using class II elastics and have a good occlusiion through the orthodontic treatment.
