  • Eruptive disturbance of the permanent lower first molar is an uncommon condition caused by physical barriers on the eruption path or failure of the eruptive mechanism. Once eruptive disturbance of the permanent lower first molar is diagnosed. treatment should be started as soon as possible to establish a normal eruption pathway and to avoid any detrimental effects on the developing occlusion A case of primary retention of the mandibular first permanent molar treated with operculectomy and forced eruption which showed good treatment results and stability is described

  • As the dental pulp is encased with a rigid, noncompliant shell, changes in pulpal blood flow or vascular tissue pressure can have serious implication for the health of pulp. Numerous studies have demonstrated that orthodontic force application may influence both blood flow and cellular metabolism, leading degenerative and/or inflammatory responses in the dental pulp. The aim of this case report is to present a case about tooth with chronic periapical abscess which showed normal vital responses. Excessive orthodontic force is thought to be the prime cause of partial pulp necrosis. Owing to remaining vital tissue, wrong dianosis can be made, and tooth falsely diagnosed as vital may be left untreated, causing the necrotic tissue to destroy the supporting tissuses. Clinician should be able to utilize various diagnostic tools for the precise diagnosis, and be aware of the endodontic-orthodontic inter-relationship.

An Convergence Study on the Characteristics of the Dental Arch Development According to the Causes of Short Stature (저신장의 원인에 따른 치열궁 발육의 특성에 대한 융합연구)

  • This study is aimed to establish the direction of orthodontic treatment by analyzing the differences in the dental arch development due to the cause of short stature. Dental diagnostic tests were conducted on patients who were diagnosed with short stature. Idiopathic short statured children were classified through the paired sampling based on the age and gender of a short statured children with growth hormone shortage. Control groups were classified using same method as above, after selecting candidates with an arch length of less than 3mm and malocclusion. In conclusion, short statured children with growth hormone shortage or idiopathic had the higher rate of crowding and the small value of overbite compared to normal children. Therefore orthodontic treatment for short statured children needs treatment plan included evaluation for Arch length discrepancy to treat a crowding early. This study will provide important data for successful orthodontic treatment according to the characteristics of dental occlusion of short statured children.

Correlation between self-esteem and stress after orthodontic treatment in the adolescents (청소년의 치아교정치료 진단군과 비진단군의 자아존중감 및 스트레스간의 상관관계)

  • Objectives : The purpose of the study was to investigate the correlation between self-esteem and stress after orthodontic treatment in the adolescents. Methods : A self-reported questionnaires was filled out by 347 high school students from September 23 to October 11, 2013. Data were analyzed using descriptive statistics, $x^2$-test, Fisher's exact test, t-test and Pearson correlation coefficient by SPSS 20.0 program. Results : Among 347 students, 25.4% of the students experienced orthodontic treatment. Significant differences existed in gender, satisfaction with dental appearance, uncomfortable level of biting and chewing, recognition of orthodontic treatment in orthodontic treatment group and non-treatment group. Self esteem was lower in orthodontic treatment group and stress was higher in orthodontic group. Self esteem had a negative correlation with stress. Conclusions : There existed the close relationship between self-esteem, stress, and dental malocclusion in the adolescents.

  • Clinical therapy that combines full-mouth rehabilitation with immediate implantation and orthognathic surgery poses a challenge to prosthodontists. This clinical report describes a multidisciplinary approach to the diagnosis and treatment of a patient presenting with skeletal discrepancy and rampant caries. The results thus achieved indicate that full-mouth rehabilitation by fixed immediate and early loading implantation accompanied by orthognathic surgery can be a predictable and effective treatment procedure.

  • Etiologies and traditional treatment modalities regarding anterior open-bite were discussed to elucidate the advantages and disadvantages. And an emphasis was placed on the understanding of the true nature of anterior open-bite. Most anterior open-bite malocclusions can be treated with a high degree of sucess and stability without surgical intervention with the congnizance of the anatomy, the physiology and the dynamics of orthodontic mechanotherapy. In this regard, a Multiloop Edgewise Arch Wire (MEAW) Technic, which has been developed during the past twenty years by Dr. Kim, was introduced. Three cases with anterior open-bite malocclusion were presented to demonstrate the mechanotherapy.

  • A device called incisor inclination indicator to control the axial inclinations of the incisors in the diagnostic setup is introduced. It is used to control the retraction of the maxillary and mandibular incisors. In this article, we describe the use of the incisor inclination indicator to prescribe adequate torque into the anterior lingual brackets and evaluate the results of treatment in a bimaxillary dentoalveolar protrusion case that underwent orthodontic treatment with the pretorqued anterior lingual brackets. Retraction of the maxillary and mandibular incisors was achieved with careful control of the axial inclination. It is indicated that the use of the incisor inclination indicator is an effective adjunctive laboratory procedure for anterior torque control during retraction in lingual orthodontic treatment.

  • This paper describes the case of a 50-year-old female with a Class II malocclusion who presented with severe bimaxillary protrusion and generalized alveolar bone loss due to adult periodontitis. The treatment plan consisted of extracting both upper and lower first premolars and periodontal treatment. Anterior segmental osteotomy(ASO) of the mandible and upper anterior segment retraction using compression osteogenesis after peri-segmental corticotomy(Speedy orthodontics) was performed. Correct overbite and overjet, facial balance, and improvement of lip protrusion were obtained. However, a slight root resorption tendency was observed on the lower anterior dentition. The active treatment period was 9 months and the results were stable for 27 months after debonding. This new type of treatment mechanics can be an effective alternative to orthognathic surgery.

Analysis of Characteristics of Medical Accidents and Disputes in Orthodontic Area (교정과영역의 의료사고 및 분쟁의 성격분석)

  • As people are more concerned about their health and medical care, there have been an increasing number of medical disputes due to increased medical demand. In order to prevent and provide solution to currently surging medical accidents and disputes related to orthodontic treatment, in July 1998, the Korean Association Of Orthdontists surveyed 2,200 members of Korean Association of orthodontists on 30 items to recognize the pattern of medical accidents and prevent them. The survey was about accident-related items including personal profiles of members and patients who have undergone medical accidents or disputes, the cause and solution to the accidents, cautions related to members orthodontic treatment, and medical recording and archival. Based on the survey result, we analyzed characteristics of medical accidents and disputes in orthodontic area. It is more important to predict and prevent possible medical accidents or disputes based on current situation than to solve them after disputes occur. For this, we should not be negligent in raising treatment proficiency level based on patient-doctor trust and in obtaining new medical information. We should also provide medical environment where Patients themselves can decide whether to get treatment after they are offered detailed explanation on diagnosis, treatment procedure, complication, and possible hazard. We should take caution when treating patients and pay attention to charting and maintenance, which is the most fundamental, as well. Also at the Korean Association of orthodontists level, it is desired to provide education program on prevention and solution to medical accidents and disputes, and actions and organizations that can help when accidents and disputes occur.

Characteristics of Orthodontic Patients in Department of Pediatric Dentistry, Chonnam National University Dental Hospital (전남대학교 치과병원 소아치과를 내원한 교정환자의 분포양상에 대한 연구)

  • Interest in orthodontic treatment has increased. Consequently, the percentage of the orthodontic treatment in pediatric dentistry has also increased. Given this background, the purpose of this study was to analyze the characteristics and the trends of pediatric orthodontic patients of Chonnam National University Dental hospital. A total of 670 patients (349 male, 321 female) diagnosed with orthodontic problems during the period from January 1st, 2004 to December 31st, 2014, were analyzed. The number of pediatric orthodontic patients was high in January, February, July and August. When it comes to age, the percentage of the patients who were eighty-years old accounted for the largest age group with 19.6%, followed by seven-year, nine-year, ten-year, eleven-year, and six-year old age groups. Skeletal class 1 accounted for 48.1% of the total cases, followed by class 2 (28.7%) and class 3 (23.3%). Mesofacial type accounted for 65% of the total cases, followed by dolichofacial type (19.3%) and brachyfacial type (15.8%). The period of the first phase orthodontic treatment was steadily shortened from 30.4 months in 2004 to 11.5 months in 2013. The result of this study is expected to provide information of the pediatric orthodontic patients.
