Review of the Recent Clinical Study Trends on Korean Medicine Treatment for Vaginitis during Pregnancy (임신 중 질염에 대한 한의학적 치료 최근 연구 동향 분석)

  • Objectives: The purpose of this study is to review the recent clinical study trends on Korean Medicine treatment for vaginitis during pregnancy. Methods: We searched articles published from electric databases including Oasis, KISS, Pubmed, and CNKI. The period was set from 2012 to 2021. Searched keywords were "임신 중 질염", "한약", "Vaginitis during pregnancy", "Herbal medicine", "妊娠合并阴道炎", "中药". Results: 34 articles were finally selected. Except 1 case series study, there were 33 RCTs, 1 article of which used both herbal decoction and herbal fumigation for treatment group (TG) ; 18 employed herbal fumigation only, and 14 took both herbal fumigation and western medicine. The total effective rate or cure rate in TGs, regardless of treatment methods, was higher than that of control group in all RCT studies. Conclusions: Our review found that Korean medicine alone or combined with western medicine is more effective for alleviating symptoms of vaginitis during pregnancy than the control group.

An experimental study for the development of prescription on atopic dermatitis (아토피 피부염 외치치료제 처방 개발을 위한 실험적 연구)

  • Objectives : Atotang was composed of 10 kinds of traditional medicinal herb. This research was performed to examine biological effects of Atotang for the development of prescription on atopic dermatitis. Methods : Atotang was extracted with 80% EtOH. Free radical scavenging assay has tested for anti-oxidative activity as well as the contents of total polyphenol. We observed the production of ROS, nitric oxide(NO) and the inflammatory cytokines such as interleukin-1beta(IL-${\beta}$), IL-6, tumor necrosis factor-alpha(TNF-${\alpha}$), Prostaglandin E2($PGE_2$) in Raw 264.7 cells stimulated by LPS. We used Disc diffusion method to investigate antibacterial activity on Candida albicans, Staphylococcus aureus and Staphylococcus epidermis. Result : Content of total phenolic compound of Atotang was 36.3 mg/g ext. DPPH and ABTS scavenging activities were 77% and 46% at 200 ug/ml respectively, showing dose-dependent increase. The amounts of ROS and NO in RAW 264.7 cells were decreased by 30% and 19% at 200 ug/ml, respectively, showing dose-dependent decrease. The prodcution of IL-1beta, IL-6 and TNF-alpha in RAW 264.7 cells were decreased dose-dependently by 81%, 67%, and 20% at 200 ug/ml, respectively. Atotang was reduced LPS-stimulated production of $PGE_2$ by 33%. Atotang on C. albicans, S. aureus and S. epidermis was selected by a disc diffusion method and inhibition effect of the Atotang on the growth of S. epidermis was the greatest. Conclusion : The results indicated that Atotang showed biological activities showing anti-oxidant, anti-inflammatory and antibacterial effects. Based on these results, it is concluded that Atotang can be applied to the prescription on atopic dermatitis.

Factors Affecting Depression of Korean Physicians (한국 의사들의 우울과 이에 영향을 미치는 요인 분석)

  • Objectives This study aims to assess the status quo of depression among Korean physicians and identify stressors and psychiatric assets related to it. Methods The questionnaire was designed to assess depression, stressors, burnout and psychiatric assets. 343 physicians were included in the analyses. Results Physician depression in Korea was found to be related to several daily life stressors and occupational stressors. It was also related to higher burnout and lower psychological assets. Physicians who reported workplace problems, familial problems, and mannerism were at higher risk of depression while who reported passion (in psychiatric assets) were at lower risk of depression. Conclusions This study identified factors affecting physician depression in Korea. Further research would benefit physicians and their patients by identifying and testing various, including personal and organizational, intervention methods.

Qualitative Study on the Experience of Patients with Facial Palsy Symptoms Treated with Korean Medicine after Western Treatment (안면마비 증상을 호소하는 환자의 양방 치료 후 한방 치료 경험에 대한 질적 연구)

  • Objectives : The purpose of this study was to explore the experience of patients with facial palsy symptoms treated with Korean medicine treatment(KMT) after western treatment. Methods : We conducted an individual in-depth interview with 10 participants who had experience in both western and KMT for facial palsy symptoms. This qualitative study adopted the grounded theory of Strauss and Corbin, and collected data was analyzed by using coding(open-axial-selective) that was performed to create paradigm model, which can explain this study's main theme. Results : The central phenomenon of this study was 'choosing Korean medicine treatment after western treatment preceded', and the core category was derived as 'selecting process of KMT after western medical treatment and improvement plan of Korean medicine treatment'. Conclusions : For patients with facial palsy, KMT was perceived as an essential treatment rather than an alternative, and recognized as the main treatment. Patients didn't settle for western treatment which was in progress, actively selected KMT and suggested the specific plan for improvement of KMT. Through this, we should be considered the direction of the KMT for facial palsy symptom which is the common disease of KMT.

A Survey on Korean Medicine Doctor' Opinion and Treatment for Developing Korean Medicine Clinical Practice Guideline for Premenstrual Syndrome (월경전증후군 한의표준임상진료지침 개발을 위한 한의사의 의견과 치료에 대한 실태조사)

  • Objectives: This study was aimed to figure out korean medicine doctors' opinion and treatment of Korean medicine clinical practice guidelines (CPG) for premenstrual syndrome (PMS). Methods: We conducted a questionnaire survey targeting korean medicine doctors belonging to the Korean Association of Oriental Medicine by e-mail and text and analyzed the answers. Results: 1. The average monthly number of patients visiting the respondents' clinics for PMS was 2.7, the main age group was 20-30s (80.8%) and the treatments the patients given before were mostly western treatments such as painkiller and combined oral contraceptives. 2. The treatment period for PMS was mostly less than 3 months (69%), and the frequency of acupuncture treatment was the most twice per week (41.1%). 3. The most effective herbal medicine for affective symptoms was Soyo-san, and for physical symptoms was Gyejibokryung-hwan. The most commonly used acupuncture point was 三陰交 (SP6). 4. Other opinions of the clinicians about Korean medicine clinical practice guideline of PMS were the need for promotion of PMS, convenience of taking herbal medicines and price competitiveness. Conclusions: We figured out korean medicine doctors' opinion of PMS CPG, clinical diagnosis and treatment.

Clinical Research Trends of Gut Microbiome for Respiratory Diseases (호흡기질환에 대한 장내 미생물의 임상 연구 동향)

  • Objectives: This study aimed to review the clinical research of the gut microbiome for respiratory diseases to assist the design of trials for respiratory diseases by regulating the gut microbiome with herbal medicine later. Methods: We searched three international databases (PubMed, CENTRAL and EMBASE) to investigate randomized controlled trials (RCTs) of the gut microbiome for respiratory diseases. The selected trials were analyzed by study design, subject diseases, inclusion/exclusion criteria, sample size, study period, intervention group, control group, outcome measures, and study results. Results: A total of 25 studies were included and published from 1994 to 2021 mostly in Europe and Asia. Subject diseases were many in the order of respiratory tract infection, cystic fibrosis, allergy, and so on. As outcome measures, the gut microbiome in a fecal sample was analyzed by 16S rRNA sequencing analysis method, and symptom assessment tools related each disease were used. Major intervention drugs were probiotics and the results were mostly improved in the composition and diversity of the gut microbiome. Conclusion: Clinical studies of the gut microbiome for respiratory diseases have confirmed various effects and this review provides basic data for a well-designed clinical study for respiratory diseases by regulating the gut microbiome with herbal medicine.

Comparative Analysis of Course Satisfaction and Student Assessment Results in Redesigned Problem-Based Learning (문제기반학습 교육과정 개편에 따른 과정 만족도 및 학생평가 결과 비교 분석 연구)

  • The purpose of this study was to redesign a problem-based learning (PBL) curriculum and compare the differences between the previous and redesigned PBL based on the results of course satisfaction and student assessments. The PBL was redesigned using curriculum design guidelines (including revisions of curriculum objectives, learning components, learning environments, and assessment methods) that were developed based on previous studies and evaluation results. A comparative study was employed using course satisfaction surveys from the previous and redesigned curricula, and a total of 45 students participated. We also compared student assessment results from concept mapping, learning issue reports, modified essay questions, and reflection journals. We identified four key findings. First, we explored the possibility that the redesigned PBL could be implemented by student facilitators without professors as tutors. Second, the redesigned PBL fostered group dynamics that facilitated developing communication skills and collaborative learning through small-group discussions. Third, the new learning elements added in the redesigned PBL made a meaningful contribution to enhancing students' clinical reasoning based on hypothetico-deductive reasoning. Fourth, concept maps in redesigned PBL contained more complex and various nodes and connections, and the levels of the nodes were more appropriate. The implications of this study can provide meaningful preliminary information for redesigning PBL curricula for medical students to develop their essential competencies through PBL.

An Analysis of Clinical Research Trends on Interventions of Korean Medicine for Ovarian Cysts (난소 낭종의 한의학적 치료에 대한 국내 임상연구 동향 분석)

  • Objectives: The purpose of this study is to review the clinical research trends of ovarian cysts and to recognize the efficacy of Korean medicine intervention. Methods: Based on four domestic databases, including Korean studies Information Service System (KISS), Oriental Medicine Advanced Searching Integrated System (OASIS), Korean Medical Database (KMbase) and Research Information Sharing Service (RISS), we analyzed the case reports using Korean medicine intervention, which include acupuncture, moxibustion and herbal medicine. Data retrieval was carried out from May 12th, to 18th, 2022, and a total of 9 papers were included. Results: All papers were published in Korea and they contain seventeen case reports in total. The most frequently used intervention was herbal medicine, especially Gyejibokryeong-hwan-gami (桂枝茯苓丸加味), Guichulpajing-tang-gagam (歸朮破癥湯加減), Guibiondam-tang-gami (歸脾溫膽湯加味). Most cases reported statistically significant results on using Korean-medicine intervention. Also, there was no serious side effect of Korean medicine. Conclusions: In this study, we investigated the efficacy of Korean medicine intervention as an adjuvant therapy for ovarian cyst patients and research trends on ovarian cysts. Further studies are needed to supplement the safety and the evaluation of Korean medicine. However, the results should be taken cautiously as more clinical studies are needed.

Investigation of Clinical Research of Acupuncture Intervention in Assisted Reproductive Technology in Pubmed (보조생식술에 있어서 침의 중재에 대한 임상연구 동향 - Pubmed를 중심으로 -)

  • Purpose: The purpose of this study is to examine acupuncture intervention suggested in the papers of assisted reproductive technology(ART, hereafter) in medicine journals and to establish the further direction. Methods: The papers published between 1996 and 2009 in Pubmed were reviewed. Results: 1. A variety of randomized controlled trials and case series were found in Pubmed concerning influence of acupuncture in ART. It was investigated into improvement of pregnancy rate, analgesic effect during oocyte retrieval, change of serum hormone, and reduction of anxiety. 2. It was on clinical pregnancy rate that main focus of the investigation was placed and the improvement of pregnancy rate differed from studies. 3. For the analgesic effect during oocyte retrieval, the electro acupuncture was mainly used and the studies reported the reduced pain rate, tiredness, and nausea. 4. Acupuncture was reported as significant in improving the state of anxiety during ART. Conclusion: It would be necessary to further study the period of acupuncture intervention, the selection of acupoint through well-designed study, and establish standard of evidence of them.

A Research of the Needling Depth for Gaining Deqi and Quality of Those Sensation - In Chung-wan(CV12) and Kwan-wan(CV4) Loci, Often Used in Ob&Gy Treatment (부인과(婦人科)에서 상용하는 중완혈(中脘穴)과 관원혈(關元穴)의 득기감(得氣感)을 느끼는 자침(刺鍼) 깊이와 득기감(得氣感) 발현 양상에 대한 연구)

  • Purpose: This research is to investigate the needling Depth for Deqi based onone's age, weight, and the level of obesity and the different Deqi sensation in Chung-wan and Kwan-wan. Methods: We took a research on Deqi for the 123 patients who visited a local Korean Medicine clinic at random. The variants were the age, the weight, thetotal fat rate, the waist-hip rate, BMI, the weight size, and the Sasang constitution. Result: Older patients need more depth of Deqi in Chung-wan and Kwan-wan compared to the younger ones. The higher the level of obesity(weight, total body fat rate, the waist-hip rate, BMI) goes up, the more Deqi in Chung-wan and Kwan-wan goes deeply. As the level of obesity goes down in the order of Taeeum-in, Soyang-in, Soeum-in, the depth of Deqi also decreases in Chung-wan and Kwan-wan. Deqi in Chung-wan is the feeling of pressure around the acu-point, and Deqi in Kwan-wan is the feeling that it spreads out to the upper, lower, the right, and the left side of the acu-point. Conclusion: In case of Needling Depth for Deqi on Chung-wan and Kwan-wan, that increases or decreases as the age, the level of obesity(the physique), and the Sasang constitution changes.
