The Influence of Parental Behavior on Ego Resilience of Korean Middle School Student (부모의 양육 행동이 중학생의 자아탄력성에 미치는 영향)

  • Objectives : Parental behavior is related to personality development and ego resilience in the childhood. The objective of this study was to identify the influence of parental behavior on ego resilience in Korean middle school student. Methods : Subjects were selected based on stratified multi-stage cluster sampling in Korea youth panel study 2013(Boy : N=1,075, Girl : N=1,033). We used Parental behavior inventory(PBI) to estimate parental behavior and the Ego resilience scale to estimate ego resilience. The data were statistically analyzed using a Pearson correlation analysis and regression analysis with the statistical package for the social sciences(SPSS). We considered differences to be significant when p<0.05. Results : A regression analysis showed that rational explanation, affection, Interest and inconsistency of the parental behavior domains influence ego resilience. Also rational explanation, affection and Interest of the parental behavior domains showed a significant positive correlation with ego resilience(r=0.24, r=0.31, r=0.22, p<0.01). In contrast to early childhood studies, inconsistency showed no significant correlation. Conclusions : Adolescents who had taken more rational explanation, interest and affection from their parents were more likely to have higher ego resilience. However, inconsistency of parental behavior showed no correlation with ego resilience of adolescents, which means that they are affected by several other factors than parental behavior. This study would be a basic research that could be a help to psychosocial approach in pediatric psychiatry.

Review of Psychiatric Adolescent Inpatient with Dermatologic Consultations (청소년 정신과 입원 환자들의 피부과 자문 의뢰에 관한 행태 분석 및 고찰)

  • Objective:To review the patterns of the dermatologic consultations of psychiatric adolescent inpatient and to explore the relationship between the dermatologic disorders and psychiatric disorders. Methods:We retrospectively studied the data from 22 cases referred by psychiatric adolescent for a dermatologic consultation over 10 years in Daegu Catholic University Medical Center and compared with the data from 108 cases referred by the other department adolescent patients. Results:The mean age of patients was 15.9. The male to female ratio was 1:1.44. The most common psychiatric and dermatologic disorder was major depressive disorder and acne, respectively. The most frequent reason for consultation was to ask for dermatologic disease or condition(54.5%) followed by to perform cosmetic procedure of patients need(40.9%) and to perform dermatologic test(4.6%). Conclusions:More than just a cosmetic disfigurement, dermatologic disorders are associated with a variety of psychopathologic problems that can affect the patient. Increased understanding of biopsychosocial approaches and liaison among psychiatrists and dermatologists could be beneficial.

Symptomatic and Clinical Profiles Across Motoric Subtypes in Delirium (섬망의 운동성 아형에 따른 증상과 임상적 특징)

  • Objectives : To investigate clinical and symptomatic differences among motoric subtypes of delirium. Methods : A total of 256 patients referred to psychiatric consultation services for delirium due to general medical condition were assessed retrospectively. Motoric subtypes were determined according to Lipowski's criteria for hyperactive, hypoactive and mixed subtypes. All patients were evaluated according to Delirium Rating Scale-Revised-98(DRS-98-R) by trained psychiatrists to obtain symptomatic profiles of delirium. Results : Hyperactive subtype were 50.8%(n=130), mixed 46.1%(n=118) and hypoactive 3.1%(n=8). Hyperactive patients were younger than mixed subtype($69.62{\pm}13.976$ vs. $73.97{\pm}11.569$, p=0.022) and received antipsychotics to manage symptoms of delirium more frequently(83.8% vs. 57.6%, p<0.001). Hyperactive patients had higher DRS-R-98 scores on both noncognitive($7.14{\pm}3.543$ for hyperactive, $5.62{\pm}3.279$ for mixed subtype) and cognitive subscales($10.00{\pm}3.574$ for hyperactive, $6.38{\pm}2.875$ for hypoactive, $7.43{\pm}3.771$ for mixed subtype, p<0.001). Conclusions : We demonstrated that clinical and symptomatic profiles were different across motoric subtypes in delirium. Diagnostic and therapeutic approach should be made differently according to motoric subtypes of delirium and special attention is needed not to underestimate or delay treatment in specific motoric subtype of delirium.

Effect of Paeoniae Radix Alba on a thioacetamide induced liver fibrosis mice model (Thioacetamide로 유발된 간섬유증 동물 모델에서 백작약이 미치는 효능)

  • This study investigated the anti-fibrotic and antioxidant effects of Paeonia Radix Alba water extract (PR) on thioacetamide (TAA)-induced liver fibrosis in a mouse model and its underlying mechanisms. Liver fibrosis was induced by intraperitoneal injection of TAA (three times a week) for 8 weeks. Furthermore, silymarin (50 mg/kg body weight) and PR (200 mg/kg body weight) were administered for 8 weeks. PR treatment downregulated aspartate aminotransferase (AST), alanine aminotransferase (ALT), ammonia, and myeloperoxidase levels. Moreover, PR treatment downregulated NOX2 and p47phox and upregulated antioxidant enzymes by activating the Nrf2/Keap1 signaling pathway. Furthermore, PR inhibited the factors associated with fibrosis, such as α-SMA and collagen I. AMPK/SIRT1 was upregulated by PR treatment. Overall, these results suggest that PR attenuates liver fibrosis by regulating the Nrf2/Keap1 and AMPK/SIRT1/NF-κB signaling pathways through the inhibition of oxidative stress. Hence, PR has potential as a remedy for preventing and treating liver fibrosis.

Accuracy of Automatic Cephalometric Analysis Programs on Lateral Cephalograms of Preadolescent Children (소아 환자 대상의 자동 계측점 식별 프로그램의 정확성 평가)

  • The aim of this study was to evaluate the accuracy of 3 different automatic landmark identification programs on lateral cephalgrams and the clinical acceptability in pediatric dentistry. Sixty digital cephalometric radiographs of 7 to 12 years old healthy children were randomly selected. Fourteen landmarks were chosen for assessment and the mean of 3 measurements of each landmark by a single examiner was defined as the baseline landmarks. The mean difference between an automatically identified landmark and the baseline landmark was measured for each landmark on each image. The total mean difference of 3 automatic programs compared to the baseline landmarks were 2.53 ± 1.63 mm. Errors among 3 programs were not significantly different for 12 of 14 landmarks except Orbitale and Gonion. The automatic landmark identification programs showed significant higher mean detection errors than the manual method. The programs couldn't be used as the 1st tool to replace human examiners. But considering short consuming time, these results indicate that all 3 programs have sufficient validity to be used in pediatric dental clinic.

A Case Report of Hyperhidrosis with Sympathicotonia Detected by Iris Diagnosis (홍채로 진단한 교감항진 국소다한증 치험례)

  • Objective: The aim of this case study was to describe a case of iris diagnosis of primary hyperhidrosis and the use of Korean medicine. Methods: A patient with symptoms of hyperhidrosis was diagnosed as having Taeeumin after assessment using the four basic Korean diagnostic methods. Iris diagnosis was used for further examination. The images obtained showed a remarkably defined collarette and increased nerve rings, which suggested an overactive sympathetic nerve system. Under the diagnosis of Taeeum, a Korean herbal medicine was prescribed with additional herbs to help alleviate the hyperactivity of the sympathetic nervous system. Results: The patient had been receiving treatment for hyperhidrosis for >30 years, with various medical attempts to relieve her symptoms, which were ineffective. She showed signs of improvement from day 4 into the treatment, and 80% of her symptoms were improved after completing a 6-week treatment course. Conclusion: The herbal medicine prescribed to the patient proved effective for reducing her chronic symptoms that had been unresponsive to previous medical treatments.

The Prevalence and Characteristics of Pre-eruptive Intracoronal Radiolucencies in Children and Adolescents (소아·청소년에서 맹출 전 치관 내 방사선 투과상의 유병률과 특징)

  • The purpose of this study was to investigate the prevalence and characteristics of pre-eruptive intracoronal radiolucencies (PEIR) from panoramic radiographs in Korean children and adolescents. This study examined panoramic radiographs of 3,000 patients aged between 5 and 14 years old who visited ten dental hospitals in Korea. The age and gender of the patients, the tooth type, the number of intracoronal radiolucent lesions, and the location and size of the lesions were recorded. The overall prevalence of patients with PEIR was 2.5%. The difference in the presence of PEIR between both genders was not significant. Within each tooth type, the mandibular first molar showed highest prevalence of PEIR (29.6%). The central part of the crown was the most frequently observed location of PEIR (56.8%). The size of the PEIR lesions was mostly limited to less than one-third of the thickness of coronal dentin.

Colorimetric Analysis of Preformed Zirconia Anterior Crowns for Esthetic Restoration (심미수복용 기성 지르코니아 크라운의 색조 평가)

  • The purpose of this study was to identify which combination of zirconia crowns and cements is most similar in color to the maxillary primary incisors by varying the color of zirconia crowns, crown thickness, and shade of cements. Prefabricated zirconia crowns in 3 shades and crowns fabricated using 6 types of zirconia blocks were used in this study. These were filled with A2-shade or translucent-shade resin cement and the $L^*$, $a^*$, and $b^*$ values were calculated using a spectrophotometer. The color differences between the natural teeth and the zirconia crowns were assessed. The shade of the final restoration was more similar to that of the natural teeth using A2-shade than translucent-shade resin cement. Application of A2-shade cement to a 0.5-mm-thick crown fabricated from a smile series 2 zirconia block resulted in the color most similar to that of the natural teeth. A2-shade resin cement is recommended for zirconia crown restoration in anterior primary teeth compared to TR-shade resin cement for more esthetic restoration. Since restorations with Nu-smile zirconia crowns were not esthetically favorable in terms of shade, improvement of the shade characteristics of the product or development of a new kind of zirconia crown is required.

A Study on the Febrile Illness in Autumn among Farmers in Gyeongju-si - Scrub typhus, Leptospirosis, HFRS - (경주시 일부 농업인의 가을철 발열성 질환 실태조사 - 쯔쯔가무시증, 렙토스피라증, 신증후군출혈열 -)

  • Objectives: This study aimed to assess the status of Scrub typhus, Leptospirosis and HFRS(hemorrhagic fever with renal syndrome) among farmers with febrile illness. Methods: We involved a total of 841 farmers who had febrile illness(508 and 333 village residents were, respectively, three and four district of Gyeongju city) selected during autumn of 2014 and 2015. Data were collected by survey questionnaires and blood sample examination. Results: Serum response rate for Scrub typhus and Leptospirosis was 1.5% each and 1.9% for HFRS. Serum response rate for Scrub typhus was significantly higher for tick-bite cases(38.5%), while Leptospirosis was significantly higher for those who were in fruit-planting work(23.1%). Similarly, serum response rate was significantly higher for HFRS who were working in venyl green house work(25.0%). Conclusions: Government authority should develop effective and efficient preventive strategies to create awareness of infectious diseases among farmers. Extending information, education and communication be reached to farmers that could change their perception and help early diagnosis and treatment and reduce the disease burden and its complication.

Color Distribution Analysis of Maxillary Primary Central Incisors using a Spectrophotometer (분광광도계를 이용한 상악 유중절치의 부위에 따른 색조 분포 연구)

  • The objective of this study was to analyze the color distribution of the maxillary primary central incisors and the relationship between the color of each tooth region by dividing tooth into incisal, middle and cervical region using a spectrophotometer. The subjects were 105 sound maxillary primary central incisors from 3 to 6 years old children. Color measurements were performed by one dentist using same protocol. CIE $L^*$, $a^*$, $b^*$, $C^*$, $h^*$ data and the best matched shades with coverage error were obtained for each region and whole labial surface. There was a significant difference in CIE $L^*$, $a^*$, $b^*$ according to regions and the difference in color between each region was more than the clinical acceptance. By correlating the relationship between the color of each region with $L^*$, $a^*$, $b^*$ values, it is possible to extrapolate the color of other region with one color or to be used as data for making restorative materials and prostheses that are more aesthetically pleasing for primary teeth.
