  • 자궁경관 임신은 최근에 인공유산의 증가와 인공 보조 생식술(assisted reproductive technology) 등에 의하여 증가되는 양상을 보인다. 과거에는 자궁경관 임신은 대량의 무통성 질출혈로 진단을 내리고 치료 방법은 천자궁적출술이 유일한 수단이었으나, 지금은 질식 초음파의 개발로 자궁경관 임신을 조기에 진단하고 자궁 보존적 방법으로 치료하여 차후 임신을 기대할 수 있게 되었다. 그 치료 방법 중 하나로 Methotrexate(MTX)를 주사하여 성공적으로 치유된 많은 보고들이 있었다. 본 저자들은 임신 8주에 자궁경관 임신으로 진단된 환자에서 전신적 MTX 투여와 복식 초음파 관찰 하에 양막강내 MTX 투여 병합 요법으로 치료한 1례를 보고하고 치료과정에서 자궁경부 병변의 지연 관해에 대해서 문헌 고찰과 함께 보고하는 바이다.

A Computer Code DEUKER for $D_2$O Scattering cross Section

  • Based on the Butler scattering kernel for D$_2$O, a computer code DEUKEB has been developed to compute the scattering laws, differential scattering cross sections and total scattering cross sections. Interference scattering between ally two atoms of a D$_2$O molecule is important in resolving the distribution of scattered neutrons in thermal energy region. Energy-transfer scattering cross sections are, therefore, studied in the various incident neutron energies. This study may be put in practice to utilize the kernel in determining the neutron spectrum in a reactor system. The study also shows that the scattering process in D$_2$O is somewhat different from that in $H_2O$.

Radiation Therapy of Head and Neck Cancer with CO-60 HDR Transcatheteric Irradiation (고선량율 강내 조사를 이용한 두경부암의 방사선 치료)

  • The basic strategy of irradiation is to deliver a dose to the cancer that is high enough to make cancer cells incapable of reproduction, while keeping the doses to the various healthy tissues below tolerable levels. In order to improve local control and survival, as a boost therapy after external radiotherapy, high dose rate transcatheteric irradiation using remote control afterloading system(RALSTRON-20B) was used for twelve patients with head and neck cancers. Present results showed complete remission of cancer in 9 out of 12 patients without treatment related complications. Although this procedure is easy to operate, well trained skillful hand is essential for good results. Furthermore our experience suggested that meticulous treatment planning should be developed for better results.

  • Hypertrophic osteoarthropathy(HOA) is a systemic disorder characterized by proliferative periostitis of long bone and polysynovitis. Clubbing of the fingers and toes usually accompanies HOA. This syndrome is most frequently with lung cancer. Although disappearance of HOA with removal of the tumor is well known, very few typical cases are reported in Korea We report case of lung cancer with HOA, which was dramatically relieved after the surgical resection of the tumor.

  • Usefulness of an optical fiber was demonstrated by detecting the generation of self-excited combustion oscillations. OH chemiluminescence intensity detected by the optical fiber showed mostly excellent agreement with those obtained by high speed CCD camera measurements when combustion oscillations were strong. Symptoms of self-excited combustion oscillation were also studied in order to predict the onset of combustion oscillation before it proceeded to a catastrophic failure. For the purpose, we have found and proposed unique measures to tell the onset of self-excited combustion oscillations based on the careful statistics of fluctuating properties in flames, such as pressure or omission of OH radicals.

  • During communication between wireless users, disconnection may occur during the handover process. The high handover latency during the process of handover degrades the service quality of the wireless communications. This problem becomes more crucial if the operation network is transmitting real-time multimedia applications. This paper studies the hanover procedure of mobile IPv6 and investigates various factors affecting the handover latency through mobile IPv6 testbed and evaluates the various parameters for reducing the handover latency.

  • Recent research shows that latent directions can be used to image process towards certain attributes. However, controlling the generation process of generative model is very difficult. Though the latent directions are used to image process for certain attributes, many restrictions are required to enhance the attributes received the latent vectors according to certain text and prompts and other attributes largely unaffected. This study presents a generative model having certain restriction to the latent vectors for image generation and manipulation. The suggested method requires only few minutes per manipulation, and the simulation results through Tensorflow Variational Auto-encoder show the effectiveness of the suggested approach with extensive results.

  • Purpose: Current results of autologous stem cell transplantation (SCT) suggest that this procedure may prolong disease free survival In patients with acute myeloid leukemia (AML). Autologous SCT is increasingly used as treatment for AML in first remission. The aim of this study was to evaluate the outcome of autologous SCT for patients with AML in first remission treated by autologous SCT using cytarabine, melphalan and total body irradiation (TBI) as the conditioning regimen. Materials and Methods: Between January 1995 and December 1999, 29 patients with AML in first remission underwent autologous SCT. The median age of patients was 33 years (range, 16 to 47). The conditioning regimen consisted of cytarabine ($3.0\;gm/m^2$ for 3 days), melphalan ($100\;gm/m^2$ for 1 day) and TBI (total 1000 cGy in five fractions over 3 days). Results: The median follow up was 40 months with a range of 3 to 58 months. The 4-year cumulative probability of disease free survival was 69.0%, and median survival was 41.5 months. The 4-year relapse rate was 27.6%. The factor Influencing disease free survival and relapse rate was the French-American-British (FAB) classification ($M_3$ group vs. other groups; p=0.048, p=0.043). One patient died from treatment-related toxicity. Conclusion:: Although the small number of patients does not allow us to draw any firm conclusion, our results were encouraging and suggest that the association of cytarabine, melphalan and TBI as a conditioning regimen for autologous SCT for AML on first remission appears to be safe and effective.

Primary Radiotherapy of Oropharyngeal Carcinoma : Experience in Korea Cancer Center Hospital (1980. 1-1986. 12) (구강인두 종양의 방사선 치료 성적)

  • Sixty-six patients with squamous cell carcinoma of the oropharynx were treated with radiation therapy and retrospectively analyzed to evaluate the treatment result in the Department of Therapeutic Radiology, Korea Cancer Center Hospital between January 1980 and December 1986. There were 42 patients with carcinoma of the tonsil including the fossa and pillar, 9 patients with carcinoma of the base of tongue,12 patients with carcinoma of the soft palate, and 3 patients with carcinoma of the posterior and lateral pharyngeal walls. Considering all oropharyngeal sites of involvement together, response rates for T1, T2, T3, and T4 were 80%, 77%, and 40%, respectively, with a overall response rate of 70%. The response rate for N1, N2, and N3 were 69%, 63%, and 40%, respectively, with the overall regional response rate of 70%. In lower T status, undifferentiated carcinoma and primary tumor arising from the soft palate, higher response rates were obtained. The S year overall and disease-free survival rate were 56%, 55%, respectively. A better prognosis was obtained in early T stage (T1+T2) (p<0.01) and in patients without tumor extension into adjacent structures in carcinomas arising from tonsillar area (p<0.01). Through this study we suggest that, in terms of anatomical and functional preservation, radiation therapy seems to be an effective method for the primary treatment of patients with oropharyngeal carcinoma.

Effects of Cryosurgery in Primary Lung Cancer (원발성 폐암에서 냉동수술의 치료 효과)

  • Background: Cryosurgery has been used to treat primary malignant pulmonary tumors at our institute since November 2004. In this study we analyzed our treatment results and complication rates. Material and Method: A retrospective study using medical charts and imaging data was conducted involving 17 patients with a total of 17 malignant pulmonary tumors who were treated between November 2004 and March 2007. Fourteen patients were males and 3 were females. The median age of the patients was 64 years (range, $54{\sim}77$ years). The average size of the tumors was 48.8mm (range, $36{\sim}111mm$) in diameter. The patients were followed with chest CT scans 7 days, 1 month, 3 months, and 6 months postoperatively. PET scans were obtained between 6 and 9 months postoperatively. The treatment response was analyzed according to the Response Evaluation Criteria in Solid Tumors (RECIST) criteria. Result: Six months after treatment, 6 tumors (35.3%) showed a complete response, 4 (23.5%) had a partial response, 3 (17.6%) had stable disease, and 4 (23.5%) showed disease progression. In tumors <4 cm in diameter, a complete response was reported in 50% of the tumors. A $x^2$-test showed that in tumors <4 cm in diameter, the p-value for results better than a partial response was 0.034. With respect to procedural complications, there was 1 case of blood-tinged sputum which resolved spontaneously within 1 or 2 days, a spontaneously relieved case of subcutaneous emphysema, and 1 patient with a fever. There were no mortalities and the average hospital stay was 6.3 days. Conclusion: The effects of cryosurgery on primary lung cancer is greatest in patients with small tumors. Considering the facts that cryosurgery is minimally invasive, has a low complication rate, and can be performed repetitively, we believe that it may play an important role in the treatment of high risk lung cancer patients.
